Well, of course, in the background, which is hugely important has been fantastic and really impressive work by scientists and doctors, both to kind of work on the treatment protocols to find various medicines and to do research on vaccines. So that's all that's also entering in the background and it is beginning to have an effect on fatality rates, but there's a lot more to come on that. And what are the improvements on the medical side specifically. Well quite a lot of it's kind of really basic medicine. So when people first presented with what was thought to be essentially a respiratory illness, the focus was on the lungs people were put on ventilators in very large numbers vary eilly and that that wasn't always very good for them and didn't help them as much as was needed. Now it's realized this is a disease that attacks many organs in the body has stroke is a big problem that kids can suffer the heart damage and doctors a much better at keeping an eye on fluid levels on oxygen levels on treating the symptoms that might be harming various organs. So just in sheer sort of management of the disease that being big improvements. And then, of course, the medicines of and POPs one of the most important decks and methods zone is a is a very cheap steroid that can be bought and supplied in large quantities in that has for people who need oxygen and to need ventilation can lead to substantial declines in mortality of twenty to thirty percent. And the hope is that over the coming months, there'll be more medicines and. Vaccines which will bring that fatality rate down still more. and. What's your sort of overarching take on the the public health response. For the Public Health Response has been just much much more variable for a number of complex reasons. I think one problem political problem has been some differences on on what the priorities should be. Some countries have a ticket only favored keeping the economy open and Sweden's often held up as an example there possibly an idealized example because in fact, actually there were restrictions others have tried to keep the death rate down to zero by having a blanket. On the economy I personally think the both of those extremes are wrong. Sweden's actually had quite high death rate and quite a big blow to its economy compared with the neighboring countries, Finland Norway. Denmark. He's done some worse on both those measures. New Zealand has had a stunningly successful. Ability to keep the deaths right down but it's paid a very high price because it's shot the the whole country down and if you look at a country like Taiwan been even more successful on deaths and paid a much much smaller price in terms of the economy. And I think that that's because the countries that have been prepared to make trade offs and have had good public health can succeed in both reducing fatality rates and in keeping bits of the economy open. But what does that look like in detail? How should that that trade off actually be made you think? I think are two or three key components, and the first of those is a really granular testing and tracing operation. The second thing that's important is being sensible about the trade offs and then the third thing that's important is how to communicate this. I think people are quite goods in an emergency at taking difficult steps to change their lives but it's very hard to sustain and this will need to be sustained already need to stay for nine months but this'll go on for a long time yet before supplies all vaccines are plentiful enough for everybody to being vaccinated and even then the vaccines might not work that well or they're affect might be temporary so we're going to be living with this disease for a long time yet. I mean a lot of this advice was was starting to become clear even even months ago, and why do you think it is at different countries responses continue to be so divergent when the the best practice seem. To, be converging. I think that's a hard question to answer I. Think part of it was the enormous relief when countries have had a bad first wave came out of lockdown at the end of the spring, and in the summer. And there was a sense of cough people just thought thank goodness and governments having got through that desperate first phase of the disease themselves were relieved are never quite caught up. I mean the countries that have done this really really well, and I I think Taiwan is absolutely top of the class here have been good from the very beginning. and You perhaps feel at some other countries catching up in other places. In the United States, I think has to stand ups an example of this, the poor communication and the changes in rules reflect the politicization of the disease. There are too many people who have a stake in it working one way or another, and it's it's a tragedy. I think that the Centers for Disease Control and prevention which went into this pandemic as perhaps the most highly regarded public health body in the world has suffered from errors and poor leadership and denigration from the White House. That's that's just done nothing tool for the overrule effort. Are you hopeful are you more hopeful about the the post vaccine era and the experience today has made us all. Yes I think people, and I think people learn and learn more slowly than than baps they ought to but they do learn and you know I say the scientific response to this disease has been incredible and and that's something to feel cheerful about is just the even the best scientific response will take longer than people might like I did that time many people will suffer?

"You're listening to the foreign desk with me. Andrew Miller still with me is Joseph. Woo Foreign Minister of Taiwan. Let's get back to Taiwan's peculiar relationship with China. You've personally been dealing with it for a very long time. We met in Taipei in two thousand and five. When you were mainland affairs minister I remember asking you about the future and you replied the destroying China would probably not be a one hundred percent. Blessing photogr warm but the chaotic. China would be a disaster now at the time you earned most Taiwanese politicians. I spoke to thought that the strange limbo that has become the status quo was probably actually the best option. Do you think that's still the case? Yes he's still the best option for us and it serves the best interest of all parties concerned. The earlier part of your question is whether China's relations with Taiwan is better off or worse off than when I was serving in this council. I can tell you that is probably not as bad as what he was. At that time the closer relations was terrible at that time. And I think You came to one not long. After China issue that anti-secession law the cross-strait relations work in a terrible situation and we gradually improved over the years even though it gets worsened in the last few years. But it's still not to the degree when we sold the across the situation in two thousand four two thousand five in two thousand six and etc but what we see in China is that China is becoming much stronger. Either in the military sense or in the economic sins and China's objective remains the same. They want to take Taiwan over peaceful possible by force if necessary and I think they are gearing up their military activities. They're all Taiwan. They have several major air exercises and surface exercises around Taiwan aimed at intimidating Taiwan people. So we sense that there is strongly and I think China use also much stronger economically and diplomatically so they tried to suppress our international participation. They tried to take over our diplomatic allies and etc and they tried to impose the one China principle on international community so even though the closer relations seems to be better than during the period of time when I was serving in the men and Affairs Council. But it's not that good and I think a strong China may not be a blessing for Taiwan as I say. China will continue to threaten Taiwan militarily diplomatically. But if China is going into a chaotic situation what we are very concerned about is that China may want to find. Scapegoats to divert is domestic attention so the current period of time when the Chinese economy is slowing down when the international criticism against China is becoming very strong. We are concerned that China may want to fight Taiwan the best scapegoats that he can have and as a result we are being dealing with China in very cautious way and I say that a little bit earlier under current kind of circumstances not only title. Why should try to maintain a status quo? Which prevented across a situation from going chaotic in. I think the international community should also take interest in helping Taiwan helping China to maintain the status quo across the Taiwan Strait. It's clear enough. We're seeing it already. That China is attempting at some level to use its own handling if the covert nineteen pandemic as the lever of diplomatic outreach. Is there any element of Taiwan trying to do the same? Do you see an opportunity here to score points off China whether it's inviting. Us correspondence which have been expelled from China to set up in Taiwan or taking in more Hong Kong dissidents like the bookseller lamb winky. And we've also seen Taiwan as China is donating face masks and other gear to Japan the Philippines in Europe. I'm sure those are all gratefully received but is a complete altruism on your part or are you sensing an opportunity here to promote Taiwan. Then me try to clarify. We try to do things as you indicated out of our goodwill to other countries or two individuals who are being persecuted or being damaged by the virus outbreak. We think it is the right thing Taiwan to be able to do that. We are not doing it because of China. If you look at the American officials description of Taiwan they described Taiwan asset democratic success. Story a reliable partner in a forceful good in the world in. We think that we deserve that description. Because our democracy is very successful we want to share. Time wants experience of democratisation with other countries. We want to support those people who are fighting for their own freedom fighting for democracy. So there's there's no hope of winning back some of the the diplomatic partners. Beijing has prized off in recent years because obviously countries have to make that choice about whether they have a a full relationship with either Taiwan or the People's Republic in the last few years. Is You know. Obviously China has prized off El Salvador. Kiribati the Solomon Islands. And a few others. Think you're down to just fifteen full diplomatic partners now. Yes we have fifteen diplomatic populace right now. We want to maintain the diplomatic relations with our diplomatic allies. And if there's any other country want to recognize Taiwan people naturally we welcome that but we are not going to engage in diplomatic competition with China. We will try to safeguard nevertheless our remaining diplomatic allies. And we'll we'll try to show to the international community that the way Taiwan is helping those diplomatic allies can be seeing us a Taiwan model. We help their agricultural production. We help the Job Training. We help in the medical assistance and we tried to make their lives better. Because what I'm wondering is if you get a sense as a foreign minister that anything about what the world is now having to deal with is is putting other crises into some sort of perspective or Div feel like that. Diplomacy is carrying on as usual while everything else is happening. Why I think the world is very different place right now. For example in some European countries or even the major metropolitan areas of the United States. The medical system is almost collapsed. And it's heartbreaking to see that in some countries. The medical doctors need to choose among patients who to save an who gave up. And it's very heartbreaking for everybody who sees that news and if you look at the economic impact. I think is even greater. Economic production seems to be not only slowing down in. Some occasions is stopped. The consumption is also slowing down. Tourism is slowing down and under this kind of situation. We also need to look at the way that economic production model as being handled whether it is a viable to put all the critical or strategic material production in the same country in. I think many countries are actually looking at these. You right now and we're also looking at the. Who itself where the W. H. O? Is Actually helping international community fighting against Kobe? Nineteen or whether this any new model in dealing with whether we are going to produce the medicine or vaccines fast enough under the. Who is still not certain in therefore this a lot for us to examine what the current diplomacy has been doing. And what I would suggest is that we need to engage in international cooperation Indian with current epidemic or future epidemic. He was determined to retake China to go back and reunify as though he kept refusing to accept a two-state solution at the UN where Mouse People's Republic of China would receive the Security Council seat and Taiwan's Republic of China government but stay in the UN in other words mouse. And in less than a week after that dope to Ted Ross was asked in a briefing by CNN whether or not attacks from the likes of Donald Trump harmed the credibility of the WHO and himself at a time when they specifically need this credibility all over the world and he didn't mention that is all he then came out this quite remarkable claim that he's been the target of a racist online campaign for the past three months and that is coming from Taiwan. Where are those attacks coming from than if indeed there are attacks the thing is you look online? It's the Internet. There is unsavory stuff everywhere and of course. China has this fifty cent army which is a group of Internet users who are actually paid by governments local governments often to spread information that is beneficial to the Chinese state. And why fifty cents. It's very cheap. And so the resulting product you get from. That is also often largely ineffectual. So here we have this case. We've got this terrible disease. It's killed way over one hundred thousand people worldwide. Aren't there more important things that we should all be focused on right now? Well not for an authoritarian communist government. Perception is reality and since the the PR spin of this is being very integral to China's campaign In combating the virus being very very sensitive about it being linked in any way to Wuhan or to China which in in subways is absolutely understandable Samson. What's the biggest lesson we can take away from? Taiwan's handling of the disease. Well I think number one full the United States and Western Europe now. They had weeks if not months to prepare for this and they didn't learn the lessons from what Asia's democracies and China in certain cases did to contain this virus. I'm actually listening a while ago to your fascinating conversation with the rich Howard from the Howard Institute and what he said about the importance of finding local solutions to work for local communities and empowering and inspiring people from all walks of life to participate in the solutions that affect them and that really resonated with me. Because you can see that in so many aspects of Taiwan's fight against this disease while the government absolutely has played its role with early intervention and clear communication and high level expertise it's attitude to openness has been absolutely key in bringing people on side and more importantly allowing them to take an active role in fighting this crisis one of the worst aspects of crisis. Is that feeling of being imported. The you can't do anything about it. You've just gotTa sit there. Wait for it to be over. And that creates all kinds of anxieties and people but at Taiwan's case because it was an open transparent and it has this long tradition of civic what we call civic tech where people at the public largely try to adapt solutions from the technology via in the public. Good not just profit but in the public good and Taiwan citizens have been able to get together take the day to the government has provided them and turn into something practical and useful. Jim Has a recommendation Richard. I've been reading one of those books that you hear about your whole life. But you've never actually read a book called Pilgrim at Tinker Creek. Which is a classic of mental literature by Annie Dillard and it's about a year. She spent living in remote part of Virginia in the Blue Ridge Mountains and basically just taking walks around her little house and sit on the banks of a creek and observing nature and thinking about the universe. And it's a kind of thing that a guy like you would think might find sort of annoying and I just absolutely love it. I'm just blown away by her writing in it's funny and it's deep but also at a time like this. It's so nice just to read something about the joy of life and the planted in nature and not to be thinking about everything else. That's going on around us. So I couldn't recommend this more pilgrim at Tinker Creek. I found the Taiwan example inspiring. Here we have a a feisty democracy right off the coast of China with only four hundred cases of Corona Virus. And just six deaths as of the middle of this week It's also a country where people largely trust the government and seemed to be okay with the kind of technology on contact tracing and surveillance That has been needed to help. Taiwan through this health emergency one wonders though whether the US which is so much more culturally and racially diverse Would be a place where this would work as well One other thought about a Taiwan is that it does have a single payer universal healthcare system and perhaps that is really helped it a combat this corona virus although There are many other countries with single. Payer universal healthcare systems that have not done nearly as well But to that point a friend of our family whose Mexican American and lives in New York was telling me just this past few days that his uncle died of corona virus and had been reluctant to see a doctor because he was worried about the cost of treatment. I think that when the worst of this pandemic is over there's going to be a renewed and perhaps even stronger debate over health care accessibility and cost. There's no question that the reluctant people get tasked. Not just the shortage of test but the obstacles that were put in people's way and cost is a major one are something that we're really going to have to look at. So I'm I'm with you on that. I may not agree with you ultimately about the solutions. But there's a real problem here and that's the debate we're GONNA have to come back to in future episodes but right now. I think one of the big lessons from Taiwan is the benefit of being an outsider. They're not part of the UN. They're not part of the IMF they're very close to China and don't trust China so they were extremely sceptical about everything. They heard coming from China about corona virus. I suspect that if China said. Oh it's all under control at Taiwan health officials were probably already zipping up. There has matt suits you know but I think there's a lesson here for all of us is don't just go along with the crowd if something if there's subtle signs or growing signs of a problem there's a lot to be said for for stepping outside the consensus. Yeah it's not a member of the World Health Organization and tried to warn the world but the message didn't get through properly this whole story with the world. Health Organization is blowing up right now with trump now threatening to de fund the American contribution in the World Health Organization which I think is is misguided. The leadership aside. They do a lot of very important work all over the world. I'm sure all the medical people at who are are pretty upset with what's going on. But the fact is the leaders of the organization have allowed themselves to be dragged into this bizarre political battle where they won't even acknowledge the existence of Taiwan practically they. They've tried to help China with their propaganda that they did such a great job of controlling corona virus. Clearly all you have to do is look around the world and see that patently untrue and so they have brought great distrust upon down upon organization that needs trust.

Is it one day's itinerary so far so do you know roughly. How many days worth of sightseeing? We just did here. I would would paces in one or two days. You don't want to overwhelm south too much with that. So there's tons to doing on as well so you can easily stay for a week the close together because it sounds more than just two days worth so far. There's a lot here. In general in the in the winter months January ish is when we're GONNA find Chinese New Year and you mentioned that it's warm all year round. Is there a best time of year to go to China. The first problem is one that every healthcare system is dealing with healthcare costs. Keep going up and on top of that. There's the wild cost of new niece drugs. But she can't have it so she thought you know what I wrote a book. I write lyrics lyrics all right the government a letter to give the reporter fish. It's a smart way to do that because major incoming incoming occasion I know how do you see her letter got attention. She told us. The government officials came to her home and the President President of Taiwan called her. They say they will try their best. There's a committee that meets to discuss the budget for National Health Insurance Doctors and hospitals and drug companies and government officials. They all come together to talk about things like covering new drugs and no matter how compelling and funny and sick Nick Legia is we have limited financial resources and limited capability. This is Dr Societa Chang. He's a researcher. Her studies healthcare but he's also served on that Budget Committee. We went to meet him at his office National Taiwan University and he told us that the reason that drugs like Lee Jay's aren't covered but right now is that healthcare costs keep rising. The Taiwanese health insurance just can't afford the new staggering costs some treatments in fact he says the National Health Insurance System can't necessarily afford the services. They're covering right now. This is the second big problem one. That's more specific back to Taiwan's healthcare and it's a problem that Societa Chang has been studying since nineteen ninety five when national health insurance. I passed so we published a paper. That's my first. I paper the best so far in that very first paper two decades ago so she has studied the elderly. He wanted to see what happened when you made healthcare cheaper And easier to access before the new system the annual number of physician visits about five six times but after is jammed to like doc tell visit which means the elderly started going to the doctor twice as much once they got healthcare. Good that I don't know I think this is the hospital. We actually wanted to see this for ourselves across the street from societas office. There's a beautiful red brick hospital. It looks more like a New England college building than a medical facility inside. There's a tile atrium here's registration and there's the pact registration area where people take little tickets and then waiting chairs for their number to be called. Israeli is like a DMV wander around every hallway every waiting room. We we see. They're all teeming with people haven't necessarily spent a ton of time in the hospital but this seems like just bustling right most of the time. It's very crowded was lotto patient and walk kind of be like you taking a two in the supermarket. Social Associate tells us that under Taiwan's National Health Insurance Co pays are so low and healthcare is so affordable that people use a lot more of it. That's a problem because it drives up costs us and because it puts a lot of training providers and when we reached out to Taiwan's doctors union the doctors. They put us in touch with agreed people come in for every little thing. The doctors told us it's overwhelming. We're not marvel superheroes one of them complained. Personally I feel that the patients in Taiwan or kind of spoiled by the system share people of their healthcare. So she says but that's because patients are getting too sweet of a deal. I I believe we are too kind to our patient which is not a good thing actually is not. You probably shouldn't run for political office on that message. Acids die right away jokes aside. A lot of people are worried when we talked to the guy who currently runs the national health insurance. He told us that things have to change. And the guy who used to run the national health insurance also told us that something has to be done. He predicted that if nothing nothing changed the system would go bankrupt in ten to fifteen years and our researcher was even less optimistic in two years. Yes ten to fifteen years. No no normal. Wow so why is nobody talking about how to to prevent that. Nobody's talking about because it's election year. Who will take the hot potato? No candidate they're nasty a be right so they were to pretend well. I am not the Knicks breasts and yet I'm going to worry about that later but this is where the really high support number might be good after all even are pessimistic researcher admits that if patients love system this much. They'll fight to keep it they. They will adjust. I believe so one of the doctors. We spoke to said that in the. US patients are in how and the providers he believes are in heaven in Taiwan he said the patients are in heaven and the providers are overwhelmed with too many patients coming in there in. How obviously Ed this is an oversimplification going? Forward Taiwan is going to have to find a balance away to bring everyone down to Earth. That could be done a couple of different ways either are you are willing to pay more as a premium or you're willing to pay more as a copayment. Taiwan might also have to create more situations like clearly jazz where the government tells patients. I'm sorry for the good of everyone. We just can't cover.

The vast majority of countries in the world will never let the president of Taiwan visit America is big enough and strong enough that it doesn't care and does let them visit even America has to be unbelievably careful about the kind of the tiny details of what flag flag is flying all the people she's allowed to visit so it's a real kinda vestige of cold war oddity around this otherwise perfectly normal friendly little country simply because China insists that everyone treats them that way and what is that saying when is hoping to accomplish with American visit in theory. This is a visit to some of her allies. Taiwan's still has seventeen countries which recognize Taiwan have full diplomatic relations <hes> they're mostly small very poor countries in the Caribbean central in America. She's going to visit for them this time and the front and the back of that car opinion tool the Americans are allowing her to visit this is a moment when support for Taiwan is very strong both in Congress and with some senior officials in the trump administration so you so only this week the American government announcing over two billion dollars worth of arms sales to Taiwan which always makes China across because America will it doesn't recognize Taiwan as a country is Taiwan security guarantor basically if China were to invade vade Taiwan America should Taiwan hopes be there but side and yet any visit to America by Taiwanese leadership is going to enrage China and has again. Would it would have how they responded so far so it's been relatively sort of boilerplate. Language from the Foreign Ministry and you've seen complaints in tossing in the state media one very nationalist state newspaper suggested that if these weapons which are basically tanks and missiles are delivered Taiwan that may be China should offer to do bomb them and destroy them unless they're removed that kind of Cuban missile crisis style scenario doesn't seem to be getting play elsewhere these visits they are weirdly serious business. It was back in the nineteen nineties that had an earlier Taiwanese president it was allowed to give a speech at his Alma Mata Cornell University and China launch gigantic missile tests that actually brought to American aircraft carrier battlegroups into the Taiwan Strait so these are these are high stakes but this also comes at a time when mainland China will have been rattled quite a bit by the protests in Hong Kong. I mean what connection do you see between to rest of regions that the mainland takes interest in Hong Kong is a very different case from Taiwan. It is absolutely legally part of China Taiwan is not part of Communist China never has been but what happens in Hong Kong is watched very closely in Taiwan blues protests. Were really a rejection in Hong Kong of the idea that autonomy offered by Beijing under the slogan One Country Two Systems is working very well. Oh that resonates in Taiwan because China's big offer to the Taiwanese is that they can also have one country two systems if they'll agree to come under mainland China's control they could have lots of autonomy so protests involving a million or more people people in the streets of Hong Kong or a dark warning sign to Taiwan that one country two systems might not be all that it's cracked up to be and we've seen that playing out as Taiwan enters election season because Taiwan unlike Hong Kong long unlike mainland China is a proper democracy with big boisterous elections and in this season tying when the incumbent president has actually seen a bit of a boost in her poll numbers because she was pretty tough and outspoken defending the right of Hong in Congress to protest and saying this shows that we can't trust China's offer of autonomy to the extent that her conservative rivals she's going to have to run against for the presidency who are normally very keen on in closer links with China making money by seeking to link the two economies. They actually had to say some unusually tough things about the mainland just to kind of keep themselves in good shape public opinion. Another complicating factor is that America is in this pitched the trade war battle with China surely entertaining the Taiwanese president is going to add tension to that. It is really hard to know what the trump administration's Taiwan policy is. There are a bunch of very senior people around president trump who are lifelong supporters of Taiwan and you can see their influence in his arm sales and probably in this longer visit president trump himself seems not to have a great interest in the fact that Taiwan is democracy. He's famously not very interested in the fact that countries our allies thinks allies as as a burden and so he has been very careful not to use Taiwan as a bargaining chip in these trade talks but it all adds up to the fact that nerves are jangling in Beijing on so many fronts so why should American voters care about Taiwan if they care about China having the right to dominate the whole of the Western Pacific they should certainly care about Taiwan military planners naval admirals they talk about the first island chain especially Japan Taiwan and then down into the Philippines which kind of acts as a kind of barrier that hems in the entire Chinese coastline and for the last several decades the American navy has been able to use that island chain as a barrier to basically Hem China in and to give America free rein.
