Good Habits for Weight Loss

"Dishing up nutrition. If you're just tuning in this morning I'm Shelby Olsen. I'm a licensed nutritionist an in studio with me as clear Leah Klein. With a tongue twister! Clear no Leah Klein Schroeder. Who is a registered and licensed Dietitian? Let's face it. We are in the midst of some of the most stressful times we have had in our country. Over the last several decades so. When we think about stress as nutritionists and Dietitians, we are looking at stress so differently. We understand that stress can affect our sleep. Stress can affect our cravings, and really stress can affect all aspects of our physical, our mental and our emotional help health. You know what I'm working with clients and they're struggling with sleep. You know they ask me shelby. What if I took some sleeping pills with that be affects. Would that kind of get me over? The Hump and the real answer is. Really coming back to your nutrition and your lifestyle habits so. If you are just joining us our topic for today is senator centered around this idea of good habits for weight loss now as Tristesse Dietitians. We can help you with a plan specifically to help you with weight loss, but we can also help to make sure that you're getting a good night of sleep. We can stop those cravings, and we can really put together an individualized plan to help improve your physical, your mental and your emotional health now because of the covid nineteen. Restrictions and some of the stay at home orders. We have pivoted. We are doing all of our counseling via zoom video chats or phone. If you have questions or if you're interested in learning more about that, give us a call at six, five, one, six, nine, nine, three, four, three eight, or you can go to our website at weight and wellness dot com to schedule that now before we went to break, I just wanted to highlight one of the things that Leah said. With Our Name Nutritional weight and wellness, even though the weight comes first. We are so focused on wellness and I. Remind my clients and my class participants this day in and day out. When you have total health and wellness, the weight loss comes. So as nutritionists Dietitians, we are wanting to problem solve. Any of those lifestyle or nutrition habits to help you have that wellness and we are not going to. Put together a plan that. Would sacrifice you feeling crummy day in and day out just to see the number on the scale move. Though, even though we're nutritional, weight and wellness, we want to be very clear that. We're looking at you as a whole person and we wanna make sure that that wellness is coming into play. So before we went to break, we were talking more specifically about all. Our first habit that we shared of eating a balanced breakfast and Leah I know you talk about eating a balanced breakfast in your sleep. I think we all could. But you know we're looking at the component of some animal protein. Some colorful carbohydrates like some berries or some spinach, or maybe even having some sweet potato, and having some natural fat like butter or Almond butter, or having a half of Avocado, or even having some canned coconut milk. Having that balance of the three is something we talk about day in and day out right right and I know that you love the research. So can you tell us a little bit more about what the research says about a balanced breakfast? Yes, absolutely, we have to talk about the research i. mean a lot of what we do here is based on the research. So what if they? What is research found about? Balanced breakfast so? There's numerous studies out there. That show a beneficial effect of eating a higher protein breakfast like protein, just for a second no protein as shelby mention and we do. We are big proponents of those animal based proteins, things like eggs and chicken and fish and beef and Turkey and pork. But a high protein breakfast has a wonderful effect on our appetites. Balancing out our hormones and keeping energy levels stable throughout the day and especially in teenagers. Yeah I know. Maybe you can relate shelby I know. As when I was a teenager, breakfast was not my favorite thing, I was I was definitely not a morning. person so I needed something like super easy, and I didn't I know. I definitely didn't make the best balanced choices at that point, Bolia. My Dad's still my parents listen to the radio. Show and my dad says you forgot to tell about the toaster strudel. Or you've got to tell them how we would buy cereal, and that sort of thing, so yes, I was totally eating high sugar breakfast kind of standard American Diet Duff or Running out the door without anything. Yep Yep Yep same here so parents tell it all sometimes, don't they? So. You have a eating so eating that more high glycemic breakfast so a breakfast. That really has a big impact on blood sugar. Something like that cereal and the skim milk the waffles with the Syrup, a high car breakfast bar. Those are not going to be very satisfying. They don't have a lot of protein in them, and they're gonNA start you on that blood sugar roller coaster throughout the day, and when your blood sugar is on a roller coaster, so is your energy level. Though are those cravings your moods and And Moods Guy and low and high and low exactly everything everything else goes on that that roller coaster, also,

"Krono type is I can actually tell you what the best time of day is for you to nap, but I can also tell you the best time of day for a whole host of other activities, everything from. Best time to have sex. Eat a cheeseburger, ask your boss for a raise like literally your name it all the same time. Want to do all three time the sex, one involve me making a decision. Could could schedule it better that way, the I'm so interested in all of this stuff. I wanna know about like pills. I wanna melatonin I wanna know like all tees stuff. I'll let you hold on letter emulsified in your brain. But I mean like you go, what about sleeping pills, prescription sales, and then most doctors go. Never none of the time, you know, and it's like oh but give me the best one for when, when it could work because I don't like that world where it's like best hangover remedy after New Year's not drinking, right? Okay. Thank you, very fucking much. Could've talked gar store Indian and got the much information idiot. You know, the best way not to get pregnant, though. Okay. What about venereal diseases no sacks? There you go sunburns my act. I'm not I'm done. My work is done. I might think is yes, you're traveling. You're screwed up whatever's going on each life you need a pill. You need to go to bed like, you know, I've found that like Ambien doesn't work well for me. I end up sort of not getting sleepy and kind of waking up in the middle of the night, and then getting into arguments, which is who ate my lemon meringue pie. There's a whole one of. Okay if you're gonna leave the pillow alone, let me just wipe this meringue off my now. I'm not done with you again sugar. Mix brokers house and stole pile. Do the weird phantom eating in the middle of the night of wakeup like it's not it's not a good fit for me, but there are other medications tend to work. And I guess, you'll know when you take them, or is there way to predetermine that, so first of all, this is an excellent question, and it's a question that very few people ask, but more people should be asking. Right. So when we look at the sleep medications that are out there. I let me be very clear. I don't prescribe any medications. I'm PHD not an MD. So I have no dog in this fight there, some sleep medications out there that work, and they work. Really? Really? Well, that's why it's a thirteen billion dollar a year industry. Right. That's why Ambien is one of the most prescribed drugs in the universe. However, here's the issue is ambiance. Good. In the short term, and it's, it's a very clean drug generally speaking, if you take alcohol with it, you have a massive problem on your hands. That's when a lot of those weird behaviors have. Well. Talking about the meringue potty a wash it down. It's not a shoe of all understand you need a melt away. Right. You need gas. So how's the swallow of what are you going to do wash? And you gotta get down there. So here's the deal is Ambien can be or any sleep medication. Let's not just pick on Ambien. Right. Any of these medications can be appropriate if used in the way your doctor prescribes until you. Let's be very straightforward. They're very few people in the universe who've.

New Warnings for Common Sleep Meds

"Tom twenty deaths with sleeping pills and not intentional I presume, what's the new warning will. Usually you see these types of. Of black box warnings on cigarettes. But now the FDA requiring some very common top-selling, insomnia. Medications names like Ambien Luneta and sonata to carry its toughest possible warnings right on their labels about very serious health risks. And again, this follows reports of at least twenty deaths associated with those leaping aids warnings, especially important for people who have experienced things like sleep walking eating or driving in their sleep. And the FDA says side effects include overdoses burns, suicide attempts and more even after just one dose of those pills. "I don't always do the trauma piece with someone who has fiber mile to depends on how many sessions I'm gonna get him for. And that's why say you don't wanna just go get rid of the one thing you want and then walk away you can but there's usually layers and other. Better without treating the trauma. Yeah. Well, I guess it'd be the sleep for some cases, I have had people say that they're five amount has gone away. And I don't see them, you know, after that. I I don't know some people do come in for tune-ups, but sleeps another thing. That's I call it a side effect. But it's not really I had a guy that came in. He had rheumatoid arthritis. I said I can't do anything with that. But I can help maybe reduce the pain. And he said, this is crazy. I have lived in my house thirty years. I have had a drawer full of sleeping pills for thirty years. I dumped it out and I ran into him two years later at Chinky. I don't know. And I said, I still sleeping. I'm always curious. He said like a baby cruiser. Yeah. Pain in with when pain perception, what part of the brand new typically working with well with pain perception, it it really depends on the kind of pain. But usually I'm going to start off working with both sides of the brain the left and right hemispheres because there's a circuit that's going on there. That's that's unpleasant for the body. If I'm treating pain, and it doesn't subside when I'm treating both hemispheres, then I know it's emotional. And then I switched the protocol to work on physical relaxation than emotional relaxation. Whether we know about emotional pain that it's insular cortex. That's going crazy. Do you think sense? You particularly at the insular or can you can you isolate that? Fr. I think we are with the electrodes. And by simply showing that one area. So so you're able to reduce some of the activity. Insulin is still the visual thing now or is this now back to the auditory thing. Same thing the visual visual. I got a little confused on the auditory when the auditory visual you're either using auditory with visual or just auditory when using auditory only for alpha beta for four trauma trauma, and is that after you've treated other stuff typically. Yeah. Yeah. You don't wanna hook someone up to that? Unless they're physically and emotionally relaxed. They will become incredibly agitated and very dysregulates. Yeah. Just by trying to go into an office day. I had someone say that they wanted to throw the computer across the room. And I was like he may have moved there too. Well, what it's really doing at least as as how was described to me as it's going through the trauma of file cabinet and putting things in safe place. If if the trauma file cabinets, not ready to be opened. And there's no real sustainability emotionally for no ego strength for it. It's going to have a strong reaction. It's going to get out. You're not invited in here yet. What do you go back to the visual? Yeah. Biofeedback then I go back to all right? Let's work on a little bit more emotional dysregulation in all do some talk therapy to. I mean, I kind of do it all together. I say I think I'm a really good they're pissed, but you you need more than flour to make cake. And so in my facility, we've got a clinical aromatherapist, we've got movement we've got art therapies CBT DBT trauma massage. Even alpha stimulation is is really good. Power picking patients, reach modality. Most people come for an intensive outpatient program, and they do the whole thing they get everything when they come into the center, they get neuro three days a week. They get alpha stem. They get the therapy. Once a week, they do groups. I don't know which was thing. That's working..

"Eight seven seventy C D A L so before I took a sleeping pill. I was always afraid to take sleeping pills. I think what I was younger. We had. I mean, even though I'm that's not my generation. But I remember hearing icons like Marilyn Monroe Elvis having taken sleeping pills. And so when I was younger, I always thought that if you took a sleeping pill. Something back and happen to you. And you would become addicted or whatever. And so for the longest time, I didn't try asleep sleeping medication. Until I was in residency and the Ambien rap Sanofi had given some samples and some of the residents had said. We want our patients talking about with his medication. We should be able to try it and see if it. You know, what what it does? And so I tried a little bit of it in. Wow. I what I did was. I was so freight to take it that I shook a little scraping of some flex from the Ambien. And within I should say, maybe fourteen minutes. I was out like a light. And so I found that to be very very powerful. I found ambiance to be one of the most powerful sleep aids. And then I took a Tylenol pm a few days later to compare the two. And then I was like I see it. Now, I see why people like this Ambien when you take a Tylenol pm. Or a unisom. You may have some drowsiness the next day because of the antihistamine component. But these non hypnotic sedatives, you seem to get up a little quicker more bright eyed, bushy tailed mind, you I only had a couple of flex. But another reason why I was very very nervous about taking sleeping pills. I was always worried that if there was a fire in my house or somebody came in to storm the house like in the movies, not that I have the type of job where I would expect people to be storming the house trying to shoot people up. I'm a doctor, but still I want to be able to jump up out of bed. Same five kids. Maybe it'd be I need to be able to jump up out of bed. What? Fire in my house when I was younger. So I as I'm still kinda traumatized by that go. Yeah. Fire could happen, right? Well, the the reality is as many people out there. We have a different mindset when it comes to sleeping aids. And we have millions upon millions of people now taking medications for sleep not only the unisom or the Tylenol or Advil PM's over the counter, but also an exit valium the benzodiazepines for sleep Ambien. Lou Nesta, these hypnotic or these other these other shows Trump's incentives. Or other medications that just happened to make retired? Well, scientists decided to test rotors okay mic. And they show that half of mice that were given some Xanax valium. Would not wake up to a startling noise. That would basically be equating equitable to a fire alarm.

"To follow. Us on Twitter adopter Dalia and on Facebook Dr Dalia show so Brad rights thank you for. Your segment on sleeping pills and. Dementia he went I been on. Sleeping pills now for years he said it started with, Tylenol pm then straight Benadryl then Ambien then Lou Nesta sonata zanex then clot open and. Now nothing seems to work, but, I, feel. Fine the, next day So a lot of people. Were a little shocked that there was. The study that came out saying that sleeping pills, for put you at. Risk for Alzheimer's And we had tons of studies. That have talked about early death you know even a Arctic I think there was a section relationship to sleeping pills and so we're finding that sleeping pills They not be all that safe and what's a, little frightening to many others is that A lack of sleep is dangerous so they think if. They use it a sleeping pill that's actually doing something productive, and positive for the body and then when you hear that you'll be having side effects Then that's not Now, you're like well what? Do I do do I try to go? With not sleeping, so what Brad I would recommend I when he says he started, with Tylenol pm And then what straits Benadryl most tolerant PM's are Advil pm, they, have Benadryl diphenhydramine that's? The antihistamine that makes you sleepy so a? Lot of these, sleeping aids have some form of an antihistamine that makes you sleepy Right problem is as. You, get really groggy the next day. When ambient came out. These non seductive hypnotic we call them. They were amazing because you would wake up and be ready to go if anybody's taking Ambien One of the biggest marketing angles that. It has that the the the high points or the highlights is that when you wake up the next, day you're up, no groggy feeling and people like the idea of not having a medicine. That's still under system They need to be able to get, up and go and it also says a fast action the problem is that you get tolerant to it Excuse me the Ambien new take the more you need a maximum doses ten milligrams there's the CR they had to do an extended release or a controlled release. Version Ambien used to work so quickly that people would. Wake up after three or four hours. And then be up in the middle of night And good to go and they're like oil. Isn't it I need this to work full six. To seven, hours so CR, came out where there was a kind of a, dual. Coding so you had the outer coating which is like, half of the. Medication then after you metabolize, down into. The core then you have the other half of the medication so CR allows you to sleep the full night but again it was expensive but it wasn't generic like the regular Ambien Lewis generic and so it took a I wasn't as. Popular as, Abia luminescence sonata, the same class of Ambien Xanax those are different benzodiazepines. Those are also very addictive If you start taking them every day and then you stop abruptly you could seize so you know my. Recommendation for someone like Brad is. To make sure he sees a medical provider. And has has some. Somebody oversee his slow withdrawal Because it has connotations to it but. If you're on. A benzodiazepine every, night we don't want you to quit cold Turkey now let's say you weren't on benzodiazepine let's say you take an Ambien every night What do, you do, how do you get off of, it well you're not gonna go through withdrawal symptoms like, the benzodiazepines with Ambien or Benadryl, you won't go through withdrawal symptoms you're probably not gonna die, if you you know go one night without taking it but in order to get off the sleeping pill. Habit it's also mind not just chemical so, you need to get the idea out of. Your, head they you will not be. Able to sleep unless you took a medication I really think we need some placebo pills Where I mean the thing is with placebo you. Have to think you're. Taking a sleeping, pill it's a. Dummy pill you take it you fall asleep And. You didn't really, take a medication I would recommend if anybody was going through, a quarter call rehab of sleeping pills That, they without knowing you know either the doctor or the. Medical provider whoever's doing this gives them something that, is kind of a dummy pill maybe one night they have a regular pill another I'd ever w And, then we throw the dummy pill that they're not actually using. The regular pharmaceutical sleep then we have to work on the cognitive aspect of it of you gotta stop putting something in your mouth, the fall, asleep this idea that you need and, your body will, fall asleep the one thing you can count on is. Your body will not just stay awake for months. At a time It may initially stay step for days you may not be able to sleep I mean we're talking you know initially you may have such bad, insomnia that your brain is will not fall. Asleep until it gets up. Medication but eventually you will falsely on. Your own your body has to sleep So if you do do your quote unquote detox again not if you've been on benzodiazepines you really. Need to, be Dr, supervised or, medical providers supervised but what a lot of people have to do is pick a week where they don't have, to go to work or they have to. Take the kids to school Where there may be on vacation And they just cold Turkey it Again not if you're on benzodiazepines and then you just and then eventually you will fall asleep and. Then get yourself into a cycle? So. How do these medications work okay so we know that the Benadryl diphenhydramine or some of. These aids histories that we use it's a. Side effect that they cause drowsiness so as well as other side effects that they have like sometimes they, could, interfere. With your urinating confusion delirium etc and we don't recommend, anybody over sixty five take diphenhydramine, but you can't get a tolerance especially if you take it for a couple. Of weeks according to web day and so these, things just as, a side effect they cost sleepiness but you're selective, gamma amino view. Teric acid patients you're Gabba medications these are the ones that work on, the Gabba receptors in, the brain the gap of receptors so. Controller level of alertness and relaxation so these selective Gavin medications target only. A certain type of Gaba receptor, the one dedicated to promoting sleep the more rapid. Onset Were selective in their action less. Prone to side effects but again, still have addiction potential They are though, relatively safe in terms of. Not be like the benzodiazepines but you can't have memory, disturbances you can't have behavioral. Changes some, people even have hallucinations Or some people, even sleepwalk then you have something, called the sleep wake cycle. Modifiers like reserve and stuff These are ones that app, directly on the sleep wake cycle or circadian rhythm. Sleep wake cycle is controlled by brain called the hypothalamus and so this remelted odd or. The rose around it finds specifically to melatonin receptors in.

"An example Unfortunately the current sat or classes of sleeping. Pills that we have do not produce naturalistic, sleep so they are all a broad set of chemicals that we call the sedative hypnotic and sedation. Is not sleep it's different it doesn't give? You the restorative natural benefits of sleep Another concern with sleeping. Pills is that they have been linked to a higher risk of death and cancer and I think this evidence. His Papp's. Not made its way clearly out to the public yet No we don't currently know. Whether that evidence is simply correlation versus causal we don't simply know if people who are taking sleeping. Pills also people who are, more likely to die faster death or more likely to suffer from. Cancer but it could very well be that those sleeping pills do 'cause higher likelihood of death, and cancer that data is currently. Unclear but I think the public needs to be informed about that evidence it also needs. To understand that you don't necessarily need to look to sleeping pills, to help particularly if you're suffering from insomnia there is a non pharmacological, approach that is, called cognitive behavioral therapy. For insomnia or CBT I and you work with a therapist lasts a couple of weeks and it is. Just as affected a sleeping pills in the short time but even better it lasts wants you Finish work with a therapist unlike. Sleeping, pills where I once you stop them not only do you tend. To go back to the bad sleep that you are having. Before but worse you tend to have what's, called rebound insomnia we asleep is even worse so. I wish I could say that the current sleeping medications right now produced beautiful naturalist sleep and I'm. Not a tall against medication, please please don't think that so it's simply that at this stage. We just don't have that complex medication that can replicate the complex process that is sleep Matthew, Walker speaking to Terry gross last.
