'There's No Quick Fix For COVID-19,' Cautions Pennsylvania Secretary Of Health
"Going to talk more now about how states are preparing to distribute those vaccines as they become available, and to do that, we're joined by Dr Rachel Levine. She is the Pennsylvania health secretary and also president of the Association of State Health Officials. Dr Levine welcome 12 Things considered. Thank you very much. I'm very pleased to be here. Will you walk us through the logistics for when Pennsylvania first receives its initial shipments of vaccines? I mean, where does it go? How do you keep it cold? What do you actually have to do? Sure. So we're all waiting the meeting of the FDA on December 10th. And then they will consider all of the data of submitted by the Fizer Corporation for the first vaccine. When they issue their emergency use authorization or you away, then operation Warp speed will distribute the vaccine. To hospitals in Pennsylvania and throughout the nation just to get specific is the federal government or or fives or corporation, sending it to the state of Pennsylvania to distribute it to hospitals. I I asked, because this vaccine has to be kept incredibly cold. It's not just like you can send a bottle of aspirin, right? So you are correct. This is an ultra cold chain vaccine that has to be kept at minus 80 C or or Celsius. So it will be operation works speed that will be sending it to the hospitals that we designate now will depend, of course about how much we're going to get, and it comes in trays of 975 bottles. So these have to be hospitals that can Deal with the cold chain and can deal with that amount of vaccine to then administer. Now, the CDC recommends that health care workers and residents of long term care facilities should get first access to these vaccines. That's a large number of people have you decided whether you're going to start With older healthcare workers or people with underlying conditions are only doctors and nurses who see covert patients. I mean, how are you going to prioritize within the priority groups? Sure. So we're going to be developing a decision tree for hospitals to use and we'll be finishing that this week. I want to point out that the distribution for the nursing home and other long term kitchen facilities Goes in a different way. So operation works speed working with Walgreens and CVS will be receiving the vaccine with those two pharmaceutical companies, and then they will be working to administer the vaccine in nursing homes and other long term care facilities. Do you have a sense of how many doses The first shipment is likely to include and how that compares to the number of people in this top priority. First group, so we don't exactly know how many doses we're going to get in the first shipment. So you know, we had lots of discussions with Operation Warp speed, you know, will be looking to attack the end of next week or certainly when they're going to be sent out to know exactly how many doses we're going to get in the first week. And then the expectation our weekly shipments to the hospitals and to Walgreens and CVS for those distributions. I gotta ask a lot of depending here on the ability of operation Warp speed to carry this out effectively And with Corona virus testing, the federal government kind of fell down. I mean, There weren't enough tests. Some of the tests were not accurate. Initially. How confident are you in the ability of this Trump Administration program to get this very difficult job done? We have heard directly from General Purna. You know, we have confidence in them, but I'm sure it will be a significant logistical challenge. This has to occur throughout the United States, all at the same time to all of the states, the territories. And then some specific large cities. And are you also concerned about the smooth handoff from one administration to another? I mean, if the distribution depends on Operation Warp speed, which is a Trump administration program will the Trump Administration only has so many more weeks in office. Now, the Department of Defense under General Karna, you know, will still be there in terms of continuity, But in terms of the representatives from helping human services and the administration, we would like to think that they'll be robust conversation. Nation and communication as the administration transitions. Obviously, that has not happened as much as it should have yet, But we're hoping that all those communications will be going on forthcoming. Just to get back to the scale of the logistical challenge here. This is a vaccine requires two doses weeks apart. Do you have the infrastructure to track and time who has had a first dose when yes, we do. But that is a information technology challenge in terms of making sure that we get the right vaccine into the right person at the right time. And then not only the visor vaccine, but the Madonna vaccine that also is a to dose vaccine. We've heard so many states talk about the extreme budget pressure that they are under and this is an expensive undertaking. Do you have the money? You need to do it? And if not, do you think the federal government is ready to provide it well, it will be essential for the federal government to provide more funding to the States territories in cities that will be tasked with ministering the vaccine. Operation works speed cost billions and billions for the development of the vaccine on Lee $340 million has been allocated for the next part of the mission, the distribution and the administration so clearly states and territories and cities. They're going to need more funding from the federal government to finish and accomplish this mission. These issues you're talking about could occupy you for more than 40 hours a week every week, and you are at the same time dealing with a spike in Corona virus cases and having to contact trace people who may have come in contact with exposed individuals, overcrowded hospitals. How are you juggling all of this at once? Well again. This is the public health challenge of a lifetime. I don't think any of us have seen a 40 hour week. You know, in our memories I stand corrected three ways to deal with the pandemic. You can work on containment that involved the testing and contact tracing very hard to contain a virus that has spread this far. For example, Today we are reporting 11 over 11,000 new cases. In Pennsylvania. It's impossible to contact that many people, so we have to prioritize. We prioritize to congregate, setting such as nursing homes and other long term care facilities, Correctional institutions, schools, etcetera, Then we have mitigation. Basic mitigation sets his mass and hand washing and social distancing. And then in Pennsylvania, you know we had to have a stay at home advisory. And you know, we have really tried to recommend that people avoid not only large gatherings but small gatherings. And then they'll be the vaccine those of the tools and the public health to box that we have to work with. When you're dealing with community spread on the level that we are seeing now, where there is so much of this disease and so much transmission of it. Do you have to change your approach? Donald trump saying one person coming in from china and then saying it was fifteen. And then saying we're gonna be fine in the fall. Will the fall is here and looking at this at the lines of people waiting to get a test. It's worse now than ever before. Just as donald trump was warned. Then if the president were issuing that same mandate should we have a national testing plan or has the boat sailed on that already so the current administration has actively discouraged. Testing has said oh. We're testing too much. That is definitely not what you're going to hear under the new administration if anything you're going to see a massive ramping up of testing and really trying especially in communities of color that have been really hard hit to make sure we have much better equity in access. But who's who's responsible for that. Is that this sort of patchwork we've got now of some state some cities. Some urgent cares or is it the federal government saying okay. We're doing widespread testing. Yeah this is the place where you're going to see. The federal government really stepping in one of the places where they could have a real impact is invoking the defense production act that would mean the federal government could compel manufacturers to crank up production. President trump has done this to a limited degree but the biden team wants to use the law. More broadly both with respect to personal protective equipment as well as scaling up production of test kits. So i i really do think we need a national plan here and that will include testing Not just people who have symptoms but people who have no symptoms expanding testing and encouraging mask. Mandates are things president-elect biden could do once. He takes office in january. That's two months from now. Normally an incoming administration would be getting information from the outgoing one. But selene says that's not happening. How much does president. Trump's refusal to concede and this sort of official transition being stuck in limbo. How much does that get in your way. Well that's a huge problem. We know historically that transitions are real period of vulnerability for the country in terms of national security. And that's whether that's a a foreign threat a terrorist threat or a threat like the corona virus and so that really could very well translate into americans dying unnecessarily to give you some examples of the kind of information we want and we can't get company gloves. do we have. How many syringes. Where are we in negotiating contracts with pharmaceutical companies with the logistics of delivery. Where are we in terms of developing databases to track who's gotten vaccinated who needs to be vaccinated. I mean those are things that are in a wait and boring nuts and bolts kind of stuff. But that's the stuff that you need to be able to plan a big response like this in terms of scaling affects nations even if the to really promising looking ones pfizer moderna if they are skilled up distribution. Is this huge challenge. How are you all thinking about vaccine distribution and getting it into the communities that that need it most so why you have to scale up. The this is where really moving forward with. The transition is really going to be important because we need to have a better understanding. What's happening on the inside with the pharmaceutical company capacity. And then once you've manufactured these doses how how do you distribute them in wear and some of that will depend on the characteristics of the vaccines. So you know the the pfizer vaccine needs to be deep-frozen minus seventy celsius. Well most family doctors. I don't know that any family doctors have the capacity to store a vaccine at that temperature. a lot of community hospitals will not have that kind of capacity either. And so what you're really looking at is having probably big academic hospital centers major health systems. Maybe pharmaceutical retail chains having central depots. Where they can do that kind of storage. But that means it's going to be harder to deliver that vaccine in particular to rural areas. So my guess is you're going to see the pfizer vaccine really targeted to urban areas. That have the capacity to deal with what we call that cold chain of freezing whereas the modern vaccine which is Which has less stringent temperature requirements in his relatively stable for a month at higher temperatures. You're going to see them journal. Vaccine probably prioritized for rural areas. And as you know a couple other vaccines emerged from the pipeline. You'll see further and further sort of targeting based on how easy it is to get those vaccines to those parts of the country. How effective those particular vaccines are for. Certain patient populations you know he might have pfizer here new york city and you might have maduna in rural wyoming listening to you. Describe the kind of logistics around that. I keep thinking about communities that have been particularly hard hit when we are talking about black and brown communities indigenous communities Ease there. I guess a painful chance that given the kind of medical infrastructure that some of these vaccines require that they're going to be doubly hit in the vaccine rollout process. Yeah this is something that we're really attentive to an it is a tricky one because there are issues a vaccine skepticism and hesitancy in some of these communities for good reason. And if you say we're going to prioritize communities of color for vaccination there will be a contingent of people who will say. Oh so we're going to be guinea pigs for this new vaccine so you have to address those very real concerns even while you are trying to prioritize because the fact is these are communities whether it's the navajo in the southwest or the south bronx here in new york city is their communities have been hit really hard and continue even now to be. Hit harder So we really do need to prioritize them if we're going to maximize the impact of the vaccines. We've been talking about trust. And i have to ask you about one of your colleagues on the advisory board Zeke emanuel who quite famously wrote an essay. saying he'd like to die at seventy five and that made a lot of people in the disability community really angry. How do you get their trust. I noticed the president-elect mentioned them specifically in his speech how do you their trust with him as a part of this group. Well i don't really want to speak to to zeke in particular but in terms of the disability community ron clean and i actually co hosted a podcast together. He had to step away for biden campaign. Were going to be the chief of staff. He's going to be the chief of staff for for President-elect biden and ron and i interviewed early on during the pandemic rebecca coakley. He of course, the former White House health policy advisor under President Obama he's now vice provost of global initiatives at the university, of Pennsylvania, and he's currently an informal adviser to the Joe Biden campaign and the Covid nineteen and Vaccine Recommendations. By the way he just co authored a research letter for the Journal of the American Medical Association comparing us, covert fatalities to those of some larger OECD countries to talk about that in just a moment. But Dr Manual let's start with this news from J. and J. Does this concern you? Well, any time? There's a serious adverse event it has to concern you but I think as. J. F. O. explained. You know you have to let the process evolve you have to actually examine carefully what the? Situation is the adverse event is related to the vaccine or placebo. Related to something else a pre existing condition, and so those are exactly the kind of questions at. Researchers will look at and try to uncover in the next few hours. And then we'll find out more. If it's in the you know arm with of the vaccine it does raise serious questions because you only have a few thousand people in the study like this one adverse event is serious especially when you're considering a vaccine that you're going to roll out to tens, hundreds of millions of people maybe even billions of people. So you know. That's the that's the ultimate concern, and this is actually standard process. For every research study you get a serious adverse event, you investigate it, it happens all the time. It's just the world's not always watching. So closely to see what the developers are excited worth pointing out that the the CFO of Johnson and Johnson also pointed out, they don't know any of these answers because they have turned it all over to independent investigators, and that is what kind of beefs up the credibility issues around those. The CFO himself has no idea even though this was thirty six hours ago whether this was in a placebo patients someone who actually received the vaccine will continue. That they've been carefully collaborating with the NIH on this trial and you know that I also think should give the American public. Some reassurance at this is going to be done thoroughly and to the highest scientific standards might detrimental. Let's talk about that paper that you wrote. You found that the United States did have higher death rates from Cova. I think the big question is was that something that was taking place very early on in the pandemic and that has improved since then and what have you found? Tell us a little bit about your study. So what we did is to take the United States mortality from it. Compare it to eighteen. Countries a high income countries like Japan and Australia Canada and Jeremy and also the countries that were hit very hard at the start Italy Spain, France and other. European countries. And we looked at the whole period and compared to most countries We actually the United States has done poorly even if you include the early phase but if you exclude the early phase March and April when every many countries were overwhelmed especially places like Italy and Spain. And you exclude them and then look say may after countries have experienced, got their arms around how to manage this of virus. I turns out the United States at extraordinarily bad even compared to places like Italy a we had from. May Tenth. To today roughly ninety thousand more deaths and we should have had we followed Italy scores ninety thousand Americans who died needlessly. As I have pointed out before Italy didn't have anything special or different in terms of Treatment Vaccines Diagnostics compared to the United States, what they have is better public health. Implementation of the public health measures, and that actually is a could could have saved tens of thousands of lives in the United States. and. We can see that when we compare our experience to those of other countries. What are you talking about in terms of reactions? You mean people wearing masks, you mean contact tracing do you mean testing that's put out? How much this you think falls on the healthcare system. As a fault and how much of it rely falls on public policy reaction to it. How much falls on just citizens following the rules well. It's all of it, but it's mainly the public health response in public policy. It really is implementing those public health measures, countrywide with fidelity. And then slowly reopening. So you do have to have social distancing you do have to have a trying not to go indoors. You do have to have having crowds less than twenty. You do have to have wearing face masks doing hand hygiene and focusing your testing and contact tracing capacity first of all, building it up, which we never did successfully in this country, and then focusing it on hot spots because we know this virus breaks out. A super spreading events it's not the usual person to person to person you know eighty to ninety percent of people will not pass this to anyone. Ten percent to twenty percent of people 'cause eighty percent of the infection. So you have to be able to identify them and quickly suppressed that we never built up that capacity the federal government under president trump punted it to the states and then states did very different. Things had Florida right now as rapidly opened up restaurants and many other things and a lot of us are expecting super spreading situations in Florida. We've seen. Places that kind of ignored? This up and down the Midwest saying Oh it's not here. Now having very high rates of cases we've had thirty one states that are going up and not down which is a very worrisome situation going into the fall and we're going to move inside. It's going to much easier to pass this virus along. And a lot of us are seriously worried about the consequences hazy I'm trying to do whether that was apples to apple. So when in May in the United. States. We got we got a later start right in Europe and by May they're already seeing. Progress over in Europe did you adjust I'm not sure whether you adjusted for that because we were right at the height. Close to it in May still and and they were on the downside in. Europe is. Standardized that in a way or wrong on that or or because we were going to have a lot more. A lot more deaths in in May than they would because they they were they got it a lot earlier there. Then I have a follow up question to. That is a super sophisticated question and you're one hundred percent, right we started a week or two later than European countries like Italy France But if you make that adjustment, it makes a slight difference not. Difference Yeah. So it's not it's not. Our peak was in May and their peak was in the end of March early April. In fact, our peak was earlier in April. and. By, by May tenth, we should have gotten our arms around and then also in our paper, we look at June seven and subsequently, and even if you look at June seven, we have tens of thousands of more debts that other countries like again the Netherlands France Spain. Italy. So we've done poorly, and by the way our data collection goes through mid September. So we've done poorly even with the August blip because of the. Spring summer vacations in many European countries where we know people like Silvio Berlusconi in Italy partied and ignored all the recommendations and got cove it So we have done badly even after even if you include the fact that we got this about a week or two later compared to other countries that was a very good question typically on the high level statisticians or clinical researchers ask that kind of. I don't with. Feeling so good about myself. All of a sudden the problems we've had here the number of cases the number of that's a number of hospitalizations was not inevitable. It was a result of bad public health measures being implemented or not implemented as the case may be and you can see this repeatedly states seeming seeming to learn nothing like Florida rushing to open up restaurants bars when we should be slowly slowly be opening. Detrimental very quickly. It I guess we do have time for one more question how how much of an impact do you think a Americans healthiness versus other countries have how do we rate just in terms of underlying co morbidity is that might be there well, it's a mixed bag for one thing our population skews younger than most European countries, they have a much older population more people over sixty five. And we know that older people tend to die from this disease unfortunately we have more co morbidity in terms of diabetes in terms of obesity, but they have more Komo British in terms of lung problems because they have higher smoking rates in the United States Net Nan. It probably comes out in the wash but we're going to have to do some more. Rigorous Studies of the CO morbidity situation and the age distribution of the population. But I don't think it's GonNa be tens of thousands of deaths. It might be a few thousand maybe even ten thousand but the overwhelming response effect that about half of our deaths or unnecessary that's not gonNA come out because of small differences in co morbidity between our countries and their country. "I grew up going to this court watching my dad argue in this court running across the courthouse launda file pacing working lawyers but ad was a lawyer arguing the paul brown federal courthouse there and i just got the phone with a couple of lawyers and chairman specifically when there's a prosecutor at the county level there for a long time bob jarvis and he said i think there's a very good likelihood zeke elliott winds this temporary restraining order on friday which would mean if you're keeping score that not only will be playing week one he'd play well into week whatever may be nine ten eleven of the nfl season possibly the entire two thousand seventeen nfl season now this is why the uh not not to get too bogged down in legalese but the fundamental question is was the process fundamentally unfair that's it and win care roberts the lead investigator questioned tiffany thompson's credibility suggested zero games and there's at least very good questions about whether or not that information was heard listen to given credence followed on the net breeze the question was it fundamentally unfair and judge amos ms antonts hearing that asked the nfl on three separate occasions isn't this fundamentally unfair how is that not unfair on three separate occasions now judges can give head fakes judges can give cover they can act tough to one side to award it the other way they can do a lot of things with the feedback i'm getting at least from lawyers who were in that courtroom is they would expect they would bet that on friday zeke gets this tro.

"It's just one of those situations this incredibly unfortunate i'm not surprised at the arbitrate upheld a decision because the arbitrator his job howard henderson's job wasn't to define guilt or innocence on a pot of ezekiel elliott as the as the lawyers on zeke you'll elliott's side are trying to proclaim his responsibility and job was to basically decide whether rajic goodell in the league office acted arbitrarily a capricious on in in a in a certain fashion as it pertains to the suspension in the bottom line is that according to the laws of the c b a which the players association agreed to roger goodell was perfectly within his right to make this decision and there was no other real decision that the arbitrator could make so the judges will come on and it will become a federal issue and we'll hear from the courts i think all of that is predictable and they'll lambaste the nba the nfl they'll talk about roger goodell and how he stepped beyond the bounds and all this other stuff but in the end they will be just like almost everyone else they will they will relent and submit to the collective bargaining agreement because if they don't they will open a pandora's box with everybody will get to flood the court with these cases and they are not going to allow that to happen zeke he'll elliott will serve six games no matter what the hell he does a you could put money on that.
