"A guest offers you one piece of advice. Ideally something you can try right now if you want. My name is Z Nicholson. I guess you could say I'm sort of a comedian first improviser or writer, actor, yeah, that's, I guess that's me. I think sometimes the thing that creative people struggle with is finding structure to their creative work, and I'm a big proponent of that. I was a track athlete in college and I realized that I sort of like approach my creative writing and acting almost in a similar way that I did to doing track and field, which was like kind of a little bit every day always and, you know, I know there are many talented people out there that I think. Unfortunately, sort of like lack the again, I just kind of personal structure for lack of a better word to frame it that kind of results in a lot of projects that have finished and I think if you're kind of used to being like, I always show up and I do even like 45 minutes or an hour every day, like it helps you sort of push through that inevitable valley of despair where you're like, this sucks, this will never get done. I hate this thing that I'm working on. Just kind of like having that built in consistency is really key to getting things done. For folks who haven't imposed that kind of structure on themselves yet, like what's a good place to start? Yeah. So I'm a big proponent of taking classes. I love taking classes, even I've been writing professionally for a few years now, but I will still take writing classes if I'm, you know, working on a movie idea that I kind of want to get off the ground, I'll take a movie writing workshop and inevitably that sort of structure of like, okay, I've painted whatever little amount of money to take this online class like that's gonna force me to sort of like put in a few hours and kind of get the project jump started. And I don't know for me at least like, you know, doing high school and college, I think I got used to that. Kind of academic structure. And I think, yeah, a lot of people get to the point in their careers and whatever career they do, whether they're like, I'm some level of expert in this. I don't need to continue to learn about this, but I think you can always learn about it and also forcing yourself to go learn about it is just building in that structure for yourself. So that's a big one that I find that I still do. I think also people get a little bit caught up in I think like space is really important to especially in the pandemic, I was finding that it was challenging to have my room also be the place where I work and eat and watch Netflix and then trying to write in here. Sometimes is really hard. And so what I would do is just take my computer and go sit at a park bench and yeah lo and behold inevitably the second you just shift your energetic kind of like spatial relation like things will start to flow. Big part of that also is like leaving your phone in the car is really good too. So yeah, if I'm kind of trying to get something going, I'll be like, all right, let me just go do 45 minutes or an hour. Leave my phone in the car. If nothing comes, that's fine, but yeah, you'd be surprised how quickly removing those distractions and switching your space will really tend to make things flow. So no spoilers dibs to everyone But by the way the carolina panthers play on thursday night football this week and the dallas cowboys play on monday night football this week so the panthers literally have the longest possible man arrest outside of coming off of their bye versus the cowboys coming off the shortest amount of possible. We're actually seeing that developed right before our eyes we kind of knew already drafting the kid but the fact that he's so fast on the edge he really makes the quarterback step up immediately and if that interior can get there. There's gonna pose a lot of problems now. I will say this the only issue. I have with leaving mike. Parsons on defensive. Line is that. We're going to get jalen smith again and a lot of jalen smith now again. I've been probably the most critical about this guy over anybody else on this roster but and some of it's justified but i would say i would say most of it's justified okay. Most of us justified. I would say if you could continue to play passable. Football like make the tackles when he's they're getting the coverage when he has to be play decent linebacker then you can. You can mess around with mike. parsons neal. He can kind of move around too. But if you wanna have mike parsons like exceed and really start succeeding on the defensive line. You gotta you gotta rely on everybody behind him and that starts with jalen smith so it worries me as good as michael. Parsons look and as good as he could probably look against philli. And who's going to spy jalen hurts. You know what. Oh six when he was nineteen and according to some records molly was eighteen but according to court testimony she may have actually been sixteen years old when she got married but needless to say the time of the homicide. She was married to zeke austin and she was seven months pregnant at the time. Now like i said zeke are defended in this case. According to the published newspapers account of the homicide zeke was no stranger to run ins with law enforcement which is kind of odd because there was no to quote the tuesday morning edition of the asheville citizen newspaper. Zeke austin is a man well known to the officers and one often placed in jail. Two years ago he cut a man named will atkins as they were passing on the street and for some time there was no clue as to why he had caused the injury. Come back to haunt this particular young man. So we're going back to march of nineteen seven near the northern buckingham county area of north carolina. Sa- community at that time that was known as martel village which was situated at the bottom of elk mountain road in today's town of witton north carolina. 'There's No Quick Fix For COVID-19,' Cautions Pennsylvania Secretary Of Health
"Going to talk more now about how states are preparing to distribute those vaccines as they become available, and to do that, we're joined by Dr Rachel Levine. She is the Pennsylvania health secretary and also president of the Association of State Health Officials. Dr Levine welcome 12 Things considered. Thank you very much. I'm very pleased to be here. Will you walk us through the logistics for when Pennsylvania first receives its initial shipments of vaccines? I mean, where does it go? How do you keep it cold? What do you actually have to do? Sure. So we're all waiting the meeting of the FDA on December 10th. And then they will consider all of the data of submitted by the Fizer Corporation for the first vaccine. When they issue their emergency use authorization or you away, then operation Warp speed will distribute the vaccine. To hospitals in Pennsylvania and throughout the nation just to get specific is the federal government or or fives or corporation, sending it to the state of Pennsylvania to distribute it to hospitals. I I asked, because this vaccine has to be kept incredibly cold. It's not just like you can send a bottle of aspirin, right? So you are correct. This is an ultra cold chain vaccine that has to be kept at minus 80 C or or Celsius. So it will be operation works speed that will be sending it to the hospitals that we designate now will depend, of course about how much we're going to get, and it comes in trays of 975 bottles. So these have to be hospitals that can Deal with the cold chain and can deal with that amount of vaccine to then administer. Now, the CDC recommends that health care workers and residents of long term care facilities should get first access to these vaccines. That's a large number of people have you decided whether you're going to start With older healthcare workers or people with underlying conditions are only doctors and nurses who see covert patients. First group, so we don't exactly know how many doses we're going to get in the first shipment. So you know, we had lots of discussions with Operation Warp speed, you know, will be looking to attack the end of next week or certainly when they're going to be sent out to know exactly how many doses we're going to get in the first week. And then the expectation our weekly shipments to the hospitals and to Walgreens and CVS for those distributions. I gotta ask a lot of depending here on the ability of operation Warp speed to carry this out effectively And with Corona virus testing, the federal government kind of fell down. I mean, There weren't enough tests. Some of the tests were not accurate. Initially. How confident are you in the ability of this Trump Administration program to get this very difficult job done? We have heard directly from General Purna. You know, we have confidence in them, but I'm sure it will be a significant logistical challenge. This has to occur throughout the United States, all at the same time to all of the states, the territories. And then some specific large cities. And are you also concerned about the smooth handoff from one administration to another? I mean, if the distribution depends on Operation Warp speed, which is a Trump administration program will the Trump Administration only has so many more weeks in office. Now, the Department of Defense under General Karna, you know, will still be there in terms of continuity, But in terms of the representatives from helping human services and the administration, we would like to think that they'll be robust conversation. Nation and communication as the administration transitions. Obviously, that has not happened as much as it should have yet, But we're hoping that all those communications will be going on forthcoming. Do you have the infrastructure to track and time who has had a first dose when yes, we do. But that is a information technology challenge in terms of making sure that we get the right vaccine into the right person at the right time. And then not only the visor vaccine, but the Madonna vaccine that also is a to dose vaccine. We've heard so many states talk about the extreme budget pressure that they are under and this is an expensive undertaking. Do you have the money? You need to do it? And if not, do you think the federal government is ready to provide it well, it will be essential for the federal government to provide more funding to the States territories in cities that will be tasked with ministering the vaccine. Operation works speed cost billions and billions for the development of the vaccine on Lee $340 million has been allocated for the next part of the mission, the distribution and the administration so clearly states and territories and cities. They're going to need more funding from the federal government to finish and accomplish this mission. These issues you're talking about could occupy you for more than 40 hours a week every week, and you are at the same time dealing with a spike in Corona virus cases and having to contact trace people who may have come in contact with exposed individuals, overcrowded hospitals. How are you juggling all of this at once? Well again. This is the public health challenge of a lifetime. I don't think any of us have seen a 40 hour week. You know, in our memories I stand corrected three ways to deal with the pandemic. You can work on containment that involved the testing and contact tracing very hard to contain a virus that has spread this far. For example, Today we are reporting 11 over 11,000 new cases. In Pennsylvania. It's impossible to contact that many people, so we have to prioritize. We prioritize to congregate, setting such as nursing homes and other long term care facilities, Correctional institutions, schools, etcetera, Then we have mitigation. Basic mitigation sets his mass and hand washing and social distancing. And then in Pennsylvania, you know we had to have a stay at home advisory. And you know, we have really tried to recommend that people avoid not only large gatherings but small gatherings. And then they'll be the vaccine those of the tools and the public health to box that we have to work with. When you're dealing with community spread on the level that we are seeing now, where there is so much of this disease and so much transmission of it. Do you have to change your approach? I mean it, Zeke gone from hand to hand combat to fighting an army out. Imagine it has And so the basic public health tool of containment, which we would do for a small measles outbreak or for it outbreak of tuberculosis? That is extremely difficult for public health to do when we're seeing this type of widespread community transmission, and then we have to pivot to more and more mitigation. It's a very unpopular it is made public health officials sometimes unpopular throughout the nation, but they're absolutely essential to try to stop the spread. And then we have the light at the end of the tunnel, which is the vaccine, but there's no quick fix to hope in 19. "Everything that's happening right. Now was warned of six months ago. Nine months ago. I i'll i'll say it now. I was on the phone with zeke emanuel back in march and rem annual back in march. Zeke had been talking to the president. I said what are you saying until it said. It's gonna be really bad mr president. It's going to be really bad. You got to get ready and as bad as it's going to be in the spring it's going to be worse in the fall. That was the warning. Donald j trump guide in march and we heard every doctor tell him the same thing every medical expert. Tell him this same thing and of course you had the press conference where donald trump assured us that this would not come back in the fall. Dr fauci said yes. It's going to come back in. The fall and healthcare systems are going to be pressed remembered all the times. Donald trump said we had the best testing of the world. We've got more testing. You see that picture of those cars lined up around dodger stadium. The best testing in the world not even close because he reviews time and time again to implement the defense production act remember. Donald trump saying one person coming in from china and then saying it was fifteen. And then saying we're gonna be fine in the fall. Will the fall is here and looking at this at the lines of people waiting to get a test. It's worse now than ever before. Just as donald trump was warned. Back in march by zeke emanuel justice warned by anthony algae on live television in a press conference. And so here. We are coming up to two hundred thousand new infections a day. Over two hundred and fifty thousand americans. Dead and that number is skyrocketing a shortage of testing our hospital system at a breaking point. This was not stupidity. This was willful negligence. And it's on donald trump and its own. Everybody that's worked for donald trump. It's on everybody inside the white house and it's on every republican that kept their mouth shut because they didn't want to insult the president. Oh yeah now. They don't want to insult the president while he's trying to actively undermine american democracy this past year they haven't wanted to upset the president because well he was lying about the coronavirus and a lot of stupid people really ignorant people believing. That's why we saw. Actually the governor of iowa mocking a mask mandate a couple of weeks before the election but after the election now suddenly a mask mandate is good science. This is willful negligence and it is. It is party politics at its worse it is. And you don't have to take zeke manuals word for it. We heard it in the presence. Own voice on those tapes from bob woodward. He said he knew how serious it was. He knew it was deadly. He called it the plague but he said he wanted to play it down. And as you point out there are press conferences we can roll out the tape where the president would get up in that white house briefing room and say it's not coming back in the fall. This is the worst of it. He'd move aside. Dr fact would say it's coming back in the fall. This is coming back in the fall so we are right where everyone told us we were going to be the differences between march and now eight months. We haven't done anything to stop what's happening right now. The judgement of history is going to be incredibly harsh as bad as it feels right now when we have some space for this everyone can look back on it. Hopefully we get through this rather than later. The fact that our federal government led by president trump stepped aside and let this virus burn through the country. It's unforgivable and it's happening right now and if you hear the urgency in the at briefing yesterday you hear the urgency from the cdc here the urgency from the doctors who come on our show. Dr oester home yesterday. Said this is the most serious public health a moment we've had in this country right now since one thousand nine hundred eighteen and we did start acting like it

"A big part of what's lean gander is doing this week is being a spokesperson. Getting the biden. Message out about what they need and what they plan to do. The president-elect as talked about appealing to governors to try to enforce mask mandates it. Do you think those would make a difference because there are places like new york. Where you and i both live where those are in place and yet we've got big outbreaks in different communities. Well i think the key is that one how enforceable is a mandate to who is issuing the mandate. Is it somebody that these communities respect. Believe trust and i think that makes a national mandate both enforcement issues as well as the trust shoes very challenging. I do think you know a mandate coming from a governor. For example governor of south dakota that would be taken very differently would be received very differently by people in south dakota. Then if the president were issuing that same mandate should we have a national testing plan or has the boat sailed on that already so the current administration has actively discouraged. Testing has said oh. We're testing too much. That is definitely not what you're going to hear under the new administration if anything you're going to see a massive ramping up of testing and really trying especially in communities of color that have been really hard hit to make sure we have much better equity in access. But who's who's responsible for that. Is that this sort of patchwork we've got now of some state some cities. Some urgent cares or is it the federal government saying okay. We're doing widespread testing. Yeah this is the place where you're going to see. The federal government really stepping in one of the places where they could have a real impact is invoking the defense production act that would mean the federal government could compel manufacturers to crank up production. President trump has done this to a limited degree but the biden team wants to use the law. More broadly both with respect to personal protective equipment as well as scaling up production of test kits. So i i really do think we need a national plan here and that will include testing Not just people who have symptoms but people who have no symptoms expanding testing and encouraging mask. Mandates are things president-elect biden could do once. He takes office in january. That's two months from now. Normally an incoming administration would be getting information from the outgoing one. But selene says that's not happening. How much does president. Trump's refusal to concede and this sort of official transition being stuck in limbo. How much does that get in your way. Well that's a huge problem. We know historically that transitions are real period of vulnerability for the country in terms of national security. And that's whether that's a a foreign threat a terrorist threat or a threat like the corona virus and so that really could very well translate into americans dying unnecessarily to give you some examples of the kind of information we want and we can't get company gloves. do we have. How many syringes. Where are we in negotiating contracts with pharmaceutical companies with the logistics of delivery. Where are we in terms of developing databases to track who's gotten vaccinated who needs to be vaccinated. I mean those are things that are in a wait and boring nuts and bolts kind of stuff. But that's the stuff that you need to be able to plan a big response like this in terms of scaling affects nations even if the to really promising looking ones pfizer moderna if they are skilled up distribution. Is this huge challenge. How are you all thinking about vaccine distribution and getting it into the communities that that need it most so why you have to scale up. The this is where really moving forward with. The transition is really going to be important because we need to have a better understanding. What's happening on the inside with the pharmaceutical company capacity. And then once you've manufactured these doses how how do you distribute them in wear and some of that will depend on the characteristics of the vaccines. So you know the the pfizer vaccine needs to be deep-frozen minus seventy celsius. Well most family doctors. I don't know that any family doctors have the capacity to store a vaccine at that temperature. a lot of community hospitals will not have that kind of capacity either. And so what you're really looking at is having probably big academic hospital centers major health systems. Maybe pharmaceutical retail chains having central depots. Where they can do that kind of storage. But that means it's going to be harder to deliver that vaccine in particular to rural areas. So my guess is you're going to see the pfizer vaccine really targeted to urban areas. That have the capacity to deal with what we call that cold chain of freezing whereas the modern vaccine which is Which has less stringent temperature requirements in his relatively stable for a month at higher temperatures. You're going to see them journal. Vaccine probably prioritized for rural areas. And as you know a couple other vaccines emerged from the pipeline. You'll see further and further sort of targeting based on how easy it is to get those vaccines to those parts of the country. How effective those particular vaccines are for. Certain patient populations you know he might have pfizer here new york city and you might have maduna in rural wyoming listening to you. Describe the kind of logistics around that. I keep thinking about communities that have been particularly hard hit when we are talking about black and brown communities indigenous communities Ease there. I guess a painful chance that given the kind of medical infrastructure that some of these vaccines require that they're going to be doubly hit in the vaccine rollout process. Yeah this is something that we're really attentive to an it is a tricky one because there are issues a vaccine skepticism and hesitancy in some of these communities for good reason. And if you say we're going to prioritize communities of color for vaccination there will be a contingent of people who will say. Oh so we're going to be guinea pigs for this new vaccine so you have to address those very real concerns even while you are trying to prioritize because the fact is these are communities whether it's the navajo in the southwest or the south bronx here in new york city is their communities have been hit really hard and continue even now to be. Hit harder So we really do need to prioritize them if we're going to maximize the impact of the vaccines. We've been talking about trust. And i have to ask you about one of your colleagues on the advisory board Zeke emanuel who quite famously wrote an essay. saying he'd like to die at seventy five and that made a lot of people in the disability community really angry. How do you get their trust. I noticed the president-elect mentioned them specifically in his speech how do you their trust with him as a part of this group. Well i don't really want to speak to to zeke in particular but in terms of the disability community ron clean and i actually co hosted a podcast together. He had to step away for biden campaign. Were going to be the chief of staff. He's going to be the chief of staff for for President-elect biden and ron and i interviewed early on during the pandemic rebecca coakley. Who's with the center for american progress. Who is a disability rights. Activist really did listen to her concerns. Were really very much attentive to the needs of that community as well so we are taking notes. We are reaching out to people from the disability community to better understand what policies they need.
