"So this is actually the the point being that. There's still a lot more rugby not even within TVD. How childhood cancer ranks according to other childhood? Diseases is by Sti Ranking as or STI category as Lisa said and you can see that the ranking just rises as you go oh from low and low middle income which is so low sti was fifteen low middle is nine in terms of the where ranks with all the other potential diseases and and then in the middle and the high middle comes up to forty three. So what I hear you saying is that we think that that's more due to better surveillance better identification of cases and and therefore it's being picked up in the global burden of disease study to be estimated but that's not accounting for the ones that we aren't even seeing aren't even acknowledging the day that they developed cancer and then subsequently passed away from it and we know that those cases are happening and in great numbers because of these previous studies. The simulation studies that you mentioned yet. I think that's a a reasonable substance of the hypothesis as least says hypothesis. And I'll make sure that everybody realizes that a lot of this is integrating across studies ladies and some extrapolation but these are very reasonable conclusions to draw obey stop the totality of data sets. And if you want to refresh yourself on these studies that we are mentioning. This was actually the topic of. I think it's episode twelve off the top of my head where I talked to Jennifer Yay and Zack Ward. Who are at Harvard? They did some simulation studies looking at projected incidence and prevalence of childhood cancer and survival of childhood cancer. In a way that's different than just the normal normal looking at cancer registries way so check out episode twelve. If you want to refresh yourself okay. So that was childhood cancer in terms of where it ranks with other childhood diseases. And then you mentioned that in the whole world childhood cancer ranked number six in terms of daily burden for all different types of cancers. So can you tell us a little bit about how broke out. Sure and so to be clear what we did was take all touted cancers so aol end kidney cancer and liver cancer and everything Put them into one category. Which is this eleven point? Five million dallies that ranked six globally compared to other cancers and these other cancers that that we compare Java cancers to were all cancers of adulthood so in the study qualified that is greater than an equal to twenty years of age up until the end of life and so each each of these other cancers we ranked separately such as Mung cancer liver cancer stomach cancer and that was a reasonable thing to do. We felt because as many of you will know childhood. Cancers are typically treated under one service and adult cancers may be treated under various different services and so from a resource association standpoint. This is a an interesting way to compare the Nevada cancer to other cancers that policymakers or hospital administrators thinking about so when we compare chocolate cancer to all these other cancers by STI category. We did find that in the lowest and Hello Middle Sti settings cancer actually ranked number one in terms of burden compared to any other cancer including lung cancer colorectal breast rest any of them and then as the STI categories rose up until high sti burden or the ranking of childhood cancer decreased for the low and low middle. Sti as you said it was number. One is that primarily due to the years of life lost component of the Dalai calculation. Since I guess kids would have a much greater amount of years of life lost compared to adults who ticket cancers at older ages correct correct and we did find that on a global scale as well with the vast majority of the Dalai components made EPO Years of life lost interesting and the pattern in in the you know with a breakout of STI category buy low and low middle middle and high middle income so the category of childhood cancer reprioritisation. It's higher on the lower end. Is that just I guess. Why does that diverge? So much from the pattern that we saw with how Childhood Cancer Rings Against Other Childhood Diseases Diseases. Yeah it's a good question. I think there are not generally a lot of risk factors that we could intervene on right. So there's not like screening programs or anything that we would would be able to see Eli sort of capture childhood cancer burden and address. I said early on and it's not as far as we know A whole lot of lifestyle risk factors for childhood cancer unlike in many adult cancers where there is an association with either behavioral or Environmental Risk Factors for childhood cancer. There's a strong correlation with population size and many low and low middle STI settings do have higher proportions of the population that our children. And so I think that part of this is due to likely the population structure and some of the the lower sti settings and I think that the important thing to recall is essentially what the Keel was highlighting for comparing haring jotted cancers to other diseases of childhood. Cancer can be something. That's a little bit hard to pick up If there aren't appropriate diagnostic criteria or if people aren't familiar with with cancer diagnosis. And if it's not a relatively rare diagnosis like childhood cancers that can be even harder whereas in the same setting for instance. They're they're very much used to diagnosing meningitis malaria or TV. And so those things will be thought of before childhood cancers and so I think that part of the reason that this transition is being seen when comparing cancers compared to other diseases of childhood. Is this in this case. Finding artifact meeting there is an artificial probably under diagnosis or misdiagnosis of cancer. Childhood whereas awesome when you're comparing cancers in children to other cancers and adults. They're sort of on a more comparible level so to speak of diagnostics asks at that makes sense. Yeah that makes sense one other thing from this table. This figure six that looking at right now. Is that compared to adults childhood cancer. Has This really dramatic decrease in terms of the Dalai ranking compared to adult cancers when you move from middle to high I middle sti ranking and then into highest ranking so in high middle compared to other cancers it's ten and then highest ranking just falls off a cliff to twenty two meaning that the Dalai burden seems to both well would it be that the daily burden from childhood cancer decreases when we get to high income countries or that the burden from other cancer's adult cancers increases or is it a combination of the two why's that so divergent question and the important thing to highlight to my can't specifically say which did you you make up. What proportion of that but I would hypothesize that you know because we are rankings? Everything imperative or relative that that the other diseases of adult are increasing in burden on if there is an association with lifestyle risk factors for some of them or exposures on the environment that come with development and so- comparatively childhood cancer declines. And I think the other part of that is likely. The population structure changes is that in many Settings at the population structure is more heavily weighted to children than in highest settings. I would add actually see. This is a really important signing that we had just because it was kind of like a Eureka moment for us. If you look at and figure one in the paper you'll actually see like like the figure for adults and then you'll see children or children on top of the adults and it's almost a little bit like mirror images of one another. And that was the first inkling that we got that I think we says describing that the risk factors are likely contribution the fact that children are being cured in high income countries and we mentioned this in the a discussion of the paper and this is actually one of the major planned areas for improvement. which is that the way late effects are handled in the global burden of disease? Study currently for children who are diagnosed and subsequently cured of cancer is limited and so that potentially also could play to a potential underestimate of the disease burden relative to others. I'm I'm just because we're not taking into account all of the long term complications that we know that survivors experience. It's fascinating so the last thing I want to cover just specifically in the paper as our time is coming to a close. Is this other super super interesting figures. Who figure for it's kind of a complicated figure but in essence since it gives a ranking by country for each of the individual diagnostic categories of childhood cancer that you looked at and to me this figure really makes clear why dailies are good way to try to help set priorities because because it gives you essentially a distribution of which cancers are responsible for the greatest burden of disease in a given country take for example Indonesia if you look at the category of cancer that you said causes the highest number of dallies? It's leukemia's leukemia is not otherwise specified. Broke out leukemia's a few ways so that was number. One in terms of the delays caused number two was brain tumors and number. Three three was acute lymphoblastic leukemia. So the more common leukemia that we think about in the United States so a health system planner could look at these numbers and say okay if we have certain resources this is where should we direct resources. Should we direct them to try to improve the care of leukemia's or a brain tumors but then you have to think about how treatable these diseases are and what resources are available to direct in those directions. So it's a little more complex than just looking at the numbers but it gives you a spread of what is more common in a certain country so then if you look down at the disease distribution in Mali you see that so non-hodgkin's lymphoma is actually the highest identifiable burden of disease in that country and and then leukemia is actually down at number seven. So then you have to ask yourself why is that is the different trend. Because we're not identifying leukemia cases or is it because for whatever reason we have more non-hodgkin's lymphoma and we should divert resources to try to tackle that type of cancer so something like that where you can compare what is going on in your country and where maybe the most minimal places to to reroute resources. Is that a fair way to look at that or my awesome were. I think that's a very reasonable way to look at got it and I think you are correct that it's not as simple as sort of just looking at the ranking. You have to sort of put it together. With with knowledge to so for instance me is lymphoma being Hodgkin Lymphoma being so high comparatively Is that due to Birkett For instance or or is it due to the fact that were missing diagnoses of leukaemia in settings like molly and I think it could be both and I do want to mention to that. Leukemia separated out into three different categories. Here so I think the ones that you were mentioning were specifically acute lymphoblastic leukemia.

So this is a nineteen Ninety-three movie based on a book by Jean shepherd, his semi fictional, anecdotes from his one thousand nine hundred sixty six book in God, we trust all others pay cash and tonight, we are so pleased to welcome to one and only Scott Farkas. So you just saw with ralphie's retribution here ralphie's rage. I think that scene is called and his name is ward. And he's going to talk to us all about a Christmas story and what he's doing now. Zac, look at that safe. Yeah. That's that's that's an ugly kid. No. It's a talk on it. So neat to see you now. I mean, is it surreal to have been Scott Farkas? Yeah. Yeah. It was it was a lot of fun to do at the time. And and then what it turned into over the years is just something that you can never expect to occur. Yeah. But did they come a point where because of when a Christmas story took off that you were like? I sorta got I've got a following nonstop. I'm Scott Farkas, man. Yeah. To degree definitely opportunities where people were excited to work with Scott Farkas and opening doors for me that I don't think I would have gotten into otherwise. But it was accumulation of my will my work over the last gosh importing nine years old now. And I started working when I was ten so the last thirty nine years. Yeah. Definitely changed the conversation when darker people because they had a relationship with me as a child, and they met me as a grownup, and so they had these great fond memories of their Christmas. And watching me get the stuffing beat out of me. And can we just pause and say step Farkas is now forty nine years old? Oh boy. So gosh, it's just so you're doing the thing. We were you're explaining to me yesterday. We tried a little bit about what you're doing now with this global sports finance exchange, all sports market dot com. It sounds amazing. And we're going to hear about that. I got gotta go back and ask you what what was it like auditioning for the role? And I'm sure at the time, you didn't know that it would take off as it is that you'd always be Scott Farkas that would be the thing on your Twitter. And when he saw us together, he said, you get his lines, and he gets yours there. Because you know, he was scary too. Though, you guys make a great tag team. Yeah. Right. Do you guys still meet up and catch up at some of those autograph things have appearances of people wanting to booking guys in? Yeah. We do charity fundraisers across the country around this time of year. But we're very close friends. I mean, he's somebody I've known now for. Thirty five years a lot of people, you know, for that period of time. So he's like he's like brother me, where's this like Scottish warts you played flick and the other guys from this from the movie, there's someone that we shared a very rare experience together. So that's a very close bond. I have to say flick flick. That's what they say in the movie when he when he's missing from the classroom. Sorry. I'm too much of an expert. Okay. So, but you also they want you when they do these these twenty four hour things they have you guys, and we have a picture of you and your little what kind of hat was that the Coon tail or whatever we have a shot of you wearing that as an adult, which I have to tip my hat to you for that. Thank you. Yeah. That's actually that one is a is a hat made out of recycled for so a friend of mine Toronto she had found old for hats. Yeah. That's the thing. I did for GPS. How to make the perfect snowball? And the ad is just beautiful. It's made from recycled I so antique furs that were over at the Salvation Army and so forth friend of mine. Toronto took them in recycled them and turn them into hats. And that one had the face on it, which kinda creek most people out. But. Suited me. That's a good sport. It has to just have fun every every year this time. Yeah. It's really cool because you experience is not only with the parents and the grandparents but also in the children. So it's one film that crosses all generations and raise a family together. And it doesn't matter. Ethnicity, religion or background is they they share that moment where they all come together and watch a Christmas story and eat too, much, food and laugh and smile, I sure we've all had all this. We've all had. Okay. It was a period piece, but we've all had dad Matt at the furnace. Yeah. The playground bully the neighborhood dogs ruining Christmas dinner and having to go to Chinese or wherever and it just resonates. And so just thanks to all you guys. "I've said it before and it's like, you know, sometimes nudity can become boring if somebody's making a movie for ninety minutes at some point, you no longer get titillated by. You know what I'm saying? It's like it just becomes part of the background. And by the time we're seeing children mowed down even if I was being offended. I think I would have run out of gas with my offense at this point. And if that's the game, they're playing constantly upping the stakes at what could offend somebody. Well, you're done when you mowed down a bunch of kids, it's also worth pointing out. They don't just kill kids. Is a guy in a wheelchair to they have a running gag that this corrupt cop is using a guy wheelchair that we think you might be a vet or something like that, but he uses him to panhandle for him and then collects the change at the end of the day and stuff them in his garage. I think he's speeding opiates in like an IV bag. Maybe there's some joke there if you're willing to laugh about our current treatment of veterans, but it's a lot of the same kind of joke. What I'm going to point out is I don't think is funny, but sometimes you guys have told me, oh, it's very funny. We have reviewed movies where you do not tire of this stuff deadpool and I just think that it's not that much different than some of those films. Maybe I'm ruining my overall thoughts on this a little bit, but I don't think it's any surprise. The key difference theories Stewart is having somebody that you can relate to on some level to make that work for you. And for me, I very much admit that I like deadpool's a character on the page, but for me, it all comes down to Ryan Reynolds. If you do. Not like Ryan Reynolds, you're gonna hate pool, and that's exactly what I was saying about Zack ward. If I could go with his plate, I hadn't even considered deadpool, but I so went with deadpool because of that romance in the beginning before he goes into the cancer treatment, that romance is what makes me like Wade Wilson, Zack ward has none of it. I blame the script as much as the actor and also Zack ward was hired to play the white cop and he got promoted to lead because they couldn't find anybody to take the lead. So the script caused that this was the best actor they could get. But I think about movies that worked a little bit better like John dies at the end, the charisma of the lead is so important to making a movie break or win. And yeah, that's why deadpool were being. This doesn't as well as dead pool seems to push the envelope in ways that makes me laugh in this doesn't again. It's hard to describe why some humor works and why it doesn't. But here. These are jokes. I had already heard on south park and Ryan Reynolds and deadpool makes it look effortless. And here I feel they're again, they're just straining. They're like constipated with these jokes, and the veins are bulging out of their necks, and they're trying to push it out at as just not making me laugh. It's the difference between the class clown who has impeccable timing can make all class laugh and the kids sitting next to it was flipping burgers at people thinking. That's funny. Yeah, I think you're right. I think this is what I'm getting too with my point now that we're forty five minutes from an end. I need this to come together as a character piece. I need Zack ward and faith to get together as a couple and start mowing people down and me love them for it. Like it's Natural Born Killers or Bonnie and CLYDE. We're rooting for them to take down this awful awful world. They've told us a million times over this is a terrible place that deserves every single bullet. So let's have some fun with that. And I feel like the movie loses focus. We lose track of faith and they go back. Back to the religious complex and it's much more about, yeah, this guy Ritchie and this crunchy doll thing. I'm hating this thing, but his crotch doll has bird-flu and who's going to release it and how all of these mechanics. That's not the focus if they had made it a convincing love story and made it satirical that we're rooting for apocalyptic ending that these to contrive that would be funny, but as is, does ending eight funny..

"We have him going for this job. He goes into this conference room where like the enemies of the CEO their heads are on a stick and do we need all of it. He goes to the welfare office. He will go repeatedly back to his trailer and find out his wife is cheating on him. I know we want to see a lot of people killed, do we need to meet them all because it will take twenty eight minutes before gun lands in dudes hands. I feel like if they're gonna spend this much time, not getting into the action at least start building up. Good reasons why we should feel for dude as seeing him being kept down. On screwed over. But a lot of these situations aren't that it's more played for comedy. That's not landing. And at the end of a lot of these scenes, I just don't feel anything for anybody and just felt like a misstep to start this way and not make dude more empathetic to us. The first half an hour of this is like a really vulgar version of office space. And the biggest thing that this movie has going for it is a stream of recognizable faces. J. k. Simmons as some guy on a street corner yelling about nine eleven victims or be it a guy whose name I don't know Eric of Ari, but whose face I sure as hell. No, from the mummy movies and Stargate as a convenience store owner or. Yeah, this guy doing the job interview, Rick Hoffman. He was one of the bad guys in the two hostile films. So I really like seeing these people. The problem entirely is. Ward. Zack ward is probably a great character actor, but he does not have the charisma or the presence to lead a film. I think of Ron Livingston the guy who did star in office space and think that when postal dude is trying to balance on that broken chair in the middle of the job interview, Ron Livingston could really sell it. But Zach ward, he just is not going to be somebody who I want to see him get the job and get the girl. I'm five with them. I'll say this. He's got the kind of face that can look angelic and devilish. And since you're playing with that in this character, I think it works where I feel sympathetic for him. The circumstance that is contrived is that all of this that's befallen him is really not his fault that we will eventually mind when he gets to doom the denomination of organic monotheism a cult that was created as money-making scam by Dave. We'll find. Find out that uncle, Dave should have gone to jail that this guy went to jail instead and can't get a good job is now stuck in the trailer park while Dave is living it up bedding, beautiful blondes five at a time and having people praise him as the second coming of Jesus. It's just so unfair and my favorite character. My favorite actor in this movie, Dave Foley uncle, Dave. The guy who much like people think about L Ron Hubbard decided the best way to make a lot of money and get a lot of women would be to start a religion, and he even wrote his own bible kind of leg the book of Mormon.
