Arthur Caplan, founding head of the Division of medical ethics at and my use. School of Medicine's been with us before Welcome back. Thank you for having me. So let's see if I can set the stage for this. So here we are in the US, we failed miserably at containing the virus. We failed miserably at controlling the virus. We failed miserably at rolling out testing. We failed miserably. A contact tracing And now a vaccine that was invented in Germany was developed here in the U. S by U S company is already going by next week into the arms of countless people in the UK How did that happen? What happened because they have a different style of emergency use approval. They do rolling data every week. Their regulatory body takes a look. We set up meetings with advisory committees. Have them review the data. Then wait a couple more weeks for the FDA to review all the data, and there's a lot of it. We're talking thousands of pages, so they're willing to kind of do it week by week and keep the regulators if you will poised to move where a little more staggered. Trying to kind of do it on our schedule basis with time in between, for the big review. I like the European idea, but I've called for our groups to go faster. I mean, people are dying. As you pointed out, we flubbed just about every aspect of this, so getting the vaccine out as fast as we can to health care workers and nursing home residents is crucial. But I think my cries are falling on deaf ears. We're gonna be waiting another two weeks. Yes, you say we were just talking about meetings now There's too many meetings, actually. But when is our next like Gold Post meeting? There's something that's happening on on the 10th. And then after that, maybe we have another week or two before people actually get shots or another meeting. Yeah, well, the tent will be the big meeting lot of recommendations being made that about emergency use to the FDA. The FDA will take a day or two to think about it. And then I think they'll say OK, and happily, some vaccines already shipping. Uh, there have been shipments in New York State. I think California's got some ready to go. So I'm thinking Maybe five days. Six days after that, December 10th meeting, say December 15 16 should start to see healthcare workers on the front lines getting vaccinated. Why do we have this? Almost madness. Mike and I were talking about it as you pointed out during the break. For meetings. I mean, sometimes there's you know, a meeting can have a good purpose. But in an emergency situation like that, you know if the ship is sinking, you don't want to first have a meeting to decide what you're gonna do with life boats to get on the lifeboats and get off the sinking ship. So meetings. They're killing us. Well, here's why it's trust. So if you look at the polls, there's a lot of folks saying I don't know about this vaccine. I'm not gonna take it member of vaccines only as good as the number of people who accept the vaccine we can have vaccines discovered. But if they stay on the shelves because people reject them, they're no good. Um Even doctors and nurses in some recent polls have shown reluctance to take these new vaccines. They're nervous. They think they've been rushed. We had a situation that Britain didn't have quite is badly, which is we had a president in the White House, pushing, pushing, pushing get these things approved before Election Day. You know, I want to be able to claim that I solved the problem, said Donald Trump, He bred an enormous amount of doubt. Then he went after some of his own experts, so it isn't just the meetings. It's the distrust that Exists here in vaccines in what the federal government is saying about vaccines in what authorities say about vaccines. That's part of what we're overcoming by having these drying assemblies where the FDA says the date is good, and we're signing off. Just some people overcome some of that when they see the health care workers get them. I mean, if all the nurses have taken it, and everything's working out, Okay, and then some of your essential worker neighbors start taking it and it's working out, okay? I think that's a great point. And I think that is exactly what's gonna happen. I think all we see these polls saying, I don't know I'm not sure I don't want to be first. If your grandma is in a nursing home, and you know that many people have died there. If you see all these sports teams getting cut up, unable to play, canceling things because of high positivity rates if you're stuck, isolating and quarantining because things were out of control and health care workers and people who work in nursing homes again. Things and they're having no adverse reactions and it looks like they're getting protection. I think we're gonna shift from meetings and we're gonna shift from distrust to how come I can't get this vaccine, But it's not here in line. Very, very briefly. What happens with all I mean, all these vaccines? I know, because I'm in one of the trials. What happens to all these trials that were designed for two years? That can't go on Canada. It's good point. Thank you for being in the trial. But here's what happens As soon as these things get approved. You were others. They're going to say my in the placebo group, because if I am I want a chance to get this newly emergency authorized vaccine and I want out of the placebo group. Sometimes scientists say, Well, we can't do that. You signed up. We need to get the data. For more than two months or three months. You gotta stay in, but I think that's not gonna happen. There were some very intriguing comments today from the CDC Director Robert Redfield about his prediction Involving the death toll here in the United States. That's the White House has yet to offer many details about how? The Corona Virus Task Force arrived at that estimate that one hundred one thousand two hundred forty thousand Americans would die. From the corona virus but public health officials are cautioning that the eventual number of dead could fall. Well under that figure as Americans are practicing social distancing. We're learning that. A key factor driving the large estimate was a crucial assumption discussed internally by Task Force officials that only fifty percent of Americans would observe the government's stringent social distancing guidelines in reality a much larger number ninety percent are observing those guidelines the director of the Centers for Disease Control. Dr Robert Redfield discussed that possibility. That the number of dead could fall well below estimates during an interview with an Arizona radio station. And here's what he had to say even the models that were a they assumed that only about fifty percent of the American public would pay attention to the recommendations. In fact what we're seeing is a large majority.
