Curtis I am a podcast producer at Kirk O' media and I am currently sitting alone in a very empty podcast studio surrounded by hand sanitizer and I'm recording this in an effort to reach out. It's not an easy time right now. We don't know what the day day is going to look like for the next few weeks even months so I'm proposing something. Let's all make something together. If you'll allow me I've just gotten done with about three years of experience with my dad who passed away at the very experienced age of ninety three and I have to say one of the reasons why he lived that long because Dr Steve Saved his life twice. He struggled with kidney failure and heart failure to those of us on the patient side. Like taking care of Dad. The balancing act was overwhelming and while Steve is a very good friend he's also two hours away from where my father lives So once in a while my father had to go to another doctor locally who didn't really understand what he was going through and would make changes and then my father would crash in and we'd have to put them in a car drive them two hours to come up to Steve Hospital. And St would have to save his life again. Your system the concept of telemedicine combined with the absolute need of medical records and perhaps a backup system that prevents even doctors from Prescribing Medicine without considering interactions between them. This is potentially life. Changing life extending. Are you guys tied into electric medical records so that the prescribing doctor can see what else I happen to be taking? Well yes and no we we do. Our providers have access to the electronic medical records of our partner hospitals the unfortunate reality of medicine across the country. We still have multiple different. Emr's even unfortunately than some of the same health system so there's A. There's a huge effort to change that in there are certain vendors that offer more integration. But I guess our hope is that in the next few years we'll start to see better integration across multiple platforms. See what do you have your sights? Oh well we have epic and it works well but there is turner out there and they're still Met Attack I think is still in existence. So that is the issue that is fragmented. We're not all communicating to each other. It's not all necessarily updated from a doctor's office the hospital data. The doctor's offices are not always linked. I have a car. That's completely separate working off to platforms for my local hospital and for my my office and that's because the hospital platform was too complicated for the office. Were not in that position where it's global or certainly national health system communication. Which makes it very difficult. Doesn't that just boggle your minds because there's no difference really between the conversation that you might have with me about what my other symptoms are? What my other ailments are what my other medicines are on a telemedicine system versus being in your office it frustrating to us as consumers that at this point there's not a really easy simple single system that prevents me from making a mistake. And frankly you from making a mistake is part of the downfall. I think of the free enterprise and we obviously know the benefits of free enterprise and the freedoms of America Org Ford's but when you have a national health system and I'm not a strong proponent of national health care but in this regard I am because I agree with you that it should be centralized. There should be a way to know everything that I'm doing real time in my office which then becomes part of the centralized computer database so that when my patient comes in and is not really sure about what medication they're on? I can just go to the computer and say no. This is the medication and with confidence. Knowing that I have the updated medication list that I can then review even if my patient is confused. We don't have that and it's very frustrating. Wouldn't you think that that system would also be a backstop for doctors? Who whatever reason. Don't see something and it tells the Doctor. Now don't prescribe this one because it has an interactive effect with some other medication clue and I think eventually we will have it but yes in spite of the fact that the technology is here. That's the frustration is not like the technology is not available. We obviously have the technology. We meaning the the healthcare system minutes a huge system is not doctors. It's hospitals doctors insurance companies. The National Insurance Company Medicare Medicaid. We're not all working cooperatively yet. In spite of the fact that the technology is right here. Dr Matthew Does your system keep a chart on me like if I mean your system for the next couple of years. Will this end up solving this problem? Because they'll be a chart on your platform so that when a doctor is suddenly talking to me they'll be a kind of a chart that they can look at that. I've earned my way through time on your system to feel a little better that you know everything going on with me. The Nice thing about our charting is that it's fully transparent so when you log into our system you are able to see all of your previous encounters that you've had in telescope health with every provider after I finish a consultation with you. I fell out a note and that will include your exam. Your history my recommendations and any prescriptions. I'm writing you follow a plans in that is sent to you and is kept in our records as well. So it's a fully transparent system you know in healthcare. There's a lot of things that are not readily available to the patient and that's an access issue. chose the software that we're using because we liked that transparency. So you can see exactly whatever any physician rights and we. We've had educate our physicians on that too because a lot of them are used to writing notes Not Understanding fully that the patient is everything and the reality is they should be able to and and that is a simple access issue. I mean if you are putting your trust in a physician or provider you should be able to know everything. They're writing about you and you should be able to access that at any time that you want to. We're certainly paying for it. So that's a nice thing about our system is that it's fully transparent and yet there are times and it's kind of an interesting thing because you want transparency. There are certain psychological issues that you come with patients unrealistic expectations that you want your colleague to know about that. Historically you might able the put in the charts that you can communicate with your colleagues you're going to have to sidebar your colleagues and say you know you have the record. But he's going to be very sensitive about the word that you use those speaking about that sensitivity. Dr Real during COVID. We've had a suspension of hippo requirements. What what's going to happen with that going forward in? How does this affect your platform? I wouldn't call it a suspension of hippo leniency. Maybe leniency. There's there's definitely been a leniency. Towards some of the hippo provision in that was I think with good intention because everybody was trying to help during this time. And you have all these doctors offices at no longer could see their patients in person in a rightfully said. The government said this is probably a time we at all. Al For more innovation in allow folks to be able to find ways to see their people and I'm glad to see that that they relaxed with that. I think that we're all going to have to adjust both on the software side and on the clinician side to make sure we tighten up as things go forward in our own platform. We were lucky because we started in May and kind of worked out many those issues prior to cope so our platform hip compliant. Do you think that this system can end up being kind of a reminder platform as well? Is that part of the system that it kind of reaches out and tells me as a member this is. What's on your schedule and get your button. Go See your doctor. That certainly in the future right now. We're using email to help with foul-up messaging but if you look at some of the other our platform in some of the other platforms out there. I think that everybody's goal was to try to have it help help. It'd be more interactive overtime where it might remind you your questions do things later. But we're definitely at that in the future so as as we bring this thing to a close after ask you all to talk a little bit about how you see the next year you know. Major doctors offices are currently not operating. And certainly don't want to go right now in sit in a waiting room. How are you seeing the next year or so? Play out for doctors operating in the environment that we've got before us and how the consumer should be looking at their medical future. Said the good news. Is I think that doctors offices and hospitals have never been cleaner. That's facts right. There is also to work being done in our hospitals in. I see him. Every health system to protect both the healthcare workers the non clinical staff and the patients in hospitals in in some senses. It may be some of the safest time to go to your doctor's office or hostile because I bet we see reduced rates of cf in Mercer and all these terrible known as we've been seeing because everybody is so in tuned to keeping things clean. There's no way to completely avoid covid but if we can reduce the in the community and your chances of coming across that are much less. I sort of feel like the hospital is safer than your local supermarket now. Not that you shouldn't go to your local supermarket but because we have such. It's on our radar all the time. We're cleaning everything. Segregating those who are possibly you know those who are under investigation or should be under investigation so the hospital is a very safe and I know from our practice. Telemedicine you know. Even in the low tech tally. Medicine that we're doing in our office will be with us for the year but if things remain as they are now where we're still waiting for better treatments and we're still waiting for a technology to catch up with the disease we're going to continue to do a facetime duo from office and we will still use the same sort of technology for patients isolated in room where we want to talk to them. Not just on the phone but face to face without healthcare personnel risk and without a depletion of our personal protective equipment. I think people will get used to this platform and it will actually be embraced long after. Kovic goes away or is under control. Do you think that this is going to affect the urgent care business out and if people ask me are he's so busy in the right now are so busy and volumes have contracted so much because people aren't wanting to go see a provider? I think that what you said at the beginning bill is very important that people shouldn't forgo evaluation for something that they're concerned about Because we're going to see a downfall And we're GONNA see a huge resurgence. I think people who missed an initial window of an emergency or or seeking care and Should have been evaluated. And so I think we'll see we'll see a rebound in both urgent cares any ours a from that how much I I don't know yet And we'll see how that goes over the next six months to year on many people. But there's always going to be conditions that require You know a procedure face to face evaluation so That would be a first place. We'd send a lot of our patients would be something that was a lower acuity place certainly was a less cost to the patient to If you have the you something that needs a basic x ray or Or simple laceration than maybe going to urgent care would be a better And lower cost visit for you. Than to stock for stock ulcers about about stockholders, excuse me. So nineteen twenty six it's formed. It's growing it's growing nearly three decades pass what transpired than to lead to the decision to separate the farm bureau mutual insurance company from the farm bureau organization so in nineteen forty eight. Ohio insurance examiners and those from a couple of states didn't examination and look at look at the organizational structure of the two organizations and made a determination that there was the potential for for some info preists activities. However in the examination, they clearly stated that there was no, no inappropriate activity happening. However that the structure lent itself to that potentially happening. And so they made the recommendations strong recommendation that the two organizations separate neither the Farm Bureau Federation or the insurance company wanted to we argued against it. But ultimately decided that we would follow their examiners recommendation and did separate operations and the name nationwide. Was chosen for very practical reason, it was it was so. It was in nineteen fifty five that we officially changed their name to nationwide. The reason for that was they were looking to it was it was a very aspirational name. And the company was looking to expand across the across the country and just made sense. Like if we're looking to expand if we're we're looking to grow nationwide. Maybe we should take that name. And it wasn't something that we. We were looking to do from the beginning. It wasn't something that we were looking to separate from the from the farm bureau. It was primarily that we were again having this aspirational interests. We are also beginning to operate in a number of states.
