Right? That's a good question, right? It's um I was just handed it to it took a look at it seems like pretty big news about Mike Madigan tersely worded statement. A reading from this year besieged Illinois House Speaker Mike Madigan said he was suspending his reelection campaign. Telling Democratic rivals it was their turn, make the next move. Quote. This is not a withdrawal, Mike Madigan stated. I suspended my campaign for speaker. As I said many times in the past. I've always put the best interests of the House Democratic Caucus and our members first. House Democratic Caucus cannot work to find someone other than me. To get the 60 votes needed for speaker. Hmm. Who will that be? It was clear. I'm told that in that closed door meeting. He didn't have the votes he needed. So he's moving on. But what happens next? I don't know. Maybe you look at that as a good thing for 2021. I wonder who is the person who fills those shoes. Is it a Mike Madigan? Ally? Is it someone fresh Someone new. You marry the most? Well, you know the thing that you're totally not comfortable doing now that once you're doing it, you're gonna know that. Hey, it that the worst of it's behind us just going to like a bar and meeting some friends. Just going out that dinner with my girlfriends or something, You know? I mean, I'm tired of going to heaven hanging with my husband. No, no offense, But my company we're keeping is getting a little off. But I'm saying I'm gonna agree with them with them every night. It's like I just like to, you know, Go see my girl friends and hang out. Have a drink, right? Right, I you know to me. I feel like there's so much I want to do. Traveling overseas. Re taking that trip to Japan that we had put off in the spring of last year was a big family trip. We're super excited. That's big, but I think the vision in my head that makes me smile and makes me feel like we turned a corner is sitting at a Cubs game at Wrigley. Like a gorgeous day packs stands. You know the sounds of the ball park roar of the crowd, Seventh inning stretch the beer man, all that stuff just to sit there. And not feel uncomfortable. Just actually high five. The person next to you, right, Right. Touch him. Touch somebody on it. You just want to hug people, you know to say hi and Some people are starting to hug again, though I've noticed Yeah, I've sort of noticed that two holidays. There is even a might not Nana, who lives with us part time she She had a few folks and you know, she's she's a hugger. She wasn't doing it all it all the time. But she did that. Um and it and it feels good. You know, it just it feels like we're avoiding it right And that that there'll be a time in the near future when it does feel good again. Um, it's not right now. And then what happened is we vote that way through a card, And the reason for that is that I could do a transaction in 60 seconds across the world. I can buy an orange and bring it back the next day and all but they cashed. So I think we need to go to the individual cards and then we need to be able to say, But for example, I don't care how I don't care what you want to add. Vote or not, I would tell you, but I really don't care. So I think the first thing you should be able to the virgin of the old your vote you can and when you do There's a receipt of that history to get sent to you one way or another, and they would get if there's harvesting. If we continue to do it, which I do not think is correct nor right. They should be registered people because the voting process is more than most. This is one of the most foundational constructs of our society. You know, they say that my polling place I go there every year. They're expecting to go there again. And I'm not sure on election night if they wait till November, 3rd that that's going to be their pope. Polling place. Williams get out early. I mean again. I don't know why people waiting, Steve. I mean, I gotta do They own their own places. Change polling places for Ah! You know, for Election day They do change that They're not the same places early Election. Early Voting is not the same as when you do the Jets on the way. I know how proud I know. There's definitely more places for Early voting, but but but they're changing. Did they say that Because it's all listed. If you get the ballot, it's listed every single polling places listed for early voting Election day voting. It's all listed in about the well in the end the practice. For sure, and people should look at that. Well, well ahead of November 3rd because again, I don't know, because several people said to be all we're going to go to the elementary school. "I mean, look, the kids in my my family, they all they all sort of they they love the Tic tac, and they seem highly like influenced by everything going on there. They're all learning the same dances and doing the same. It's like a new language. I've got to tell you. But no adults understand. And they like it that way, Although some adults I think are going on Tic tac. It's still largely a kid thing. Hey, can I get you to dress Halloween real quickly? We're going to talk about it coming up on the show a little bit. But thesis he came out and said, Hey, you know what? We are not going to cancel Halloween after all kids, but we've got to do it safely and we knew sort of that message was coming out. We heard something similar from the CDC. Um, you know, just kind of what you're thinking about this, Dr. Steve, what you think about Halloween? I mean, I think I think it was any holiday where you're wearing a mask already. I mean G with, you know, I think I think there's got to be some normalcy you if possible. There's gotta be some normalcy injected into kids lives and Halloween is one of the qualities where I think things could be done safely Wife because everyone will wear masks. And if the trip retreaters as well as the candy givers are both wearing masks and just, you know, they drop the candy into the bucket, You know, hand sanitizer social distancing. You don't congregate like don't like kids congregate. Grab some candy and move on. That kid's go anyway, right? It's the perfect client. It's the grab and go. You don't want to sit and talk. You want to grab and go on to the next one. So you know if everybody keeps moving, you know what's great. I think an important point, by the way, because I will always remember this as the kid for adults to inspect the candy and make sure it healed on open. Do you remember? We were growing up that that that cycles were putting razor blades and candle? These are the insane things, right? So why do you gotta put that out there, Doctor? See, Because because, you know, because stupid knows no boundaries, and there is no cure, You know? So you know you got it the way to prevent against it. I didn't know that it exists. But I understand you know, expecting candy and look at it, Make sure it's unopened. And you know, of course, you know what houses or large house parties. I mean, common sense stuff. But I think the kid's still have somewhat of Ah, you know, decent Halloween. We don't necessarily have to be locked away in our houses. Everyone knows the kids collect the candy, and then the parents eat it. That's how it works in my household. It's like you know, where did it all go? I don't know. I have no idea. Everyone, including candy givers should wear one leave a light on. This seems to be a thing. A lot of Different suburbs and areas or doing leave the light on her Hang on Halloween sign If you're going to be giving out candy safely, and if you're handing out candy, please socially distance and have hand sanitizer available to trick or treating. Stay on the move. Less congregating means more houses more candy. And ensure there's all treats and no tricks. Please don't reach into Candy bowl's while trick or treating and get candy on Lee at home After you wash your hands, no haunted houses that seemed like a give me I know A lot of folks are saying that that that that's not gonna happen on DH. Then keep your candy crew small because I know a lot of folks like to sit out on the front steps like, you know, and 10 people. Sometimes there's an adult beverage involved. I've noticed, and they give up candy, no loud, known large parties this year, that kind of thing. So those are the rules from the city. And it seems, you know, pretty CDC compliant. They all said this. Go ahead and have fun. But you know, be be careful about how you do it. Right? I I concurred, and they finally put out something that you don't have a truck. "There's just so many things that go into the issue. To die as a result of suicide or substance abuse related deaths, and the suicide rate in the United States has been rising for the last two decades. So it was already a huge problem in 2018 issued its highest level in 1941. And while social isolation protects from contagious virus, you know it puts people at risk for suicide overdoses, alcohol abuse, which we talked about last week, and unemployment matters, because when you have economic livelihood and mixed with the mental and emotional help it There's a potential, you know the everything on a collision course in inspiring get. So it's very, very important that we understand that this problem is man and health ears. Job losses relationship strange, lonely they spent in isolation beetle, all things that have sort of They made this problem for many people 100 times. Worse. And probably at some point is gonna have to be a kind of integration off mental health care more Will Buckley into primary care services better given out by our internist at our positions Because all of these things sort of go together and there's a formula. I believe in Anna and I reach it and we're publishing on it. She was one of sleep. They gotta get patients, leaving 6 to 7 hours a night. When people sleep, you reset their biorhythms taking rhythms, the all the thinking centers of the brain. You keep stress hormones under checked in the absolute first thing that has to happen, and then following that, it's the other stuff that we start talking about getting people symptoms control, and this is where people sort of getting to a problem. What we deal with that we're going to talk about it after the break about the opiate crisis, and how This is also everything is sort of intimately linked together, and one problem sets off a chain reaction off many of the problems. You know, it's for those who have of us. We have someone in our family who has dealt with depression for a while. If you I find just personally that the's the isolation has been augments the issues that much more And you hear about that a lot. You read that these air even more difficult times for people who went into this dealing with some sort of issue like that depression. The isolation as you point out is not necessarily a good thing. Good for the virus, not good for emotional health. But I also wonder about people who have it really sort of been stressed like this in a long time, or dealt with it in this way. What are some of the early signs Doc? That you would say, you know, kind of just point the direction of saying You know what you might need to sort of does something about that. You know, I'm seeing some science here that you're you know, you're you're struggling. All of it. The overt signs are easy people who have changing behavior who isolate who have overwhelming anxiety or panic issues who start having sort of a radical Aaron behavior of many times with family members. They'll know that you know, this is I've seen this before, with no major depressive disorder, bipolar. They mean that behavior before and the triggers go, so a lot of time over the difficulty is in the one that orange and these are the ones that are terrifying. And you know, I don't know, collecting audience and I, You know, I just lost a friend and a key member to do it, and it's a very difficult situation and a lot of times. There are no warning signs. And it is absolutely bear replying because People seem to be doing fine. They're not exhibiting any of the things I just talked about that they're deeply suffering. And how do you get through to those people? HUD, I don't That could be very, very difficult. But that's why it's very important that we check in with our loved ones. And that we talk to people we care about and act difficult question, then have difficult conversations. Because this is a difficult topic. And many people sleeping on the weakness. Hell collecting the 19 eighties and nineties if you took an anti the question Oh, my God. You're crazy, You know, I mean stigma, which just hearing the word. I'm on an anti depressant. Now, you know its exact opposite. It's like what you create. You cannot own anti depressant that shift. The paradigm shifts are huge and that the problem you know, it's not cut and dry. But certainly this is the time that we all have to be sort of vigilant, not only a battle typical help. But our mental health eating and sleeping well, staying socially connected limiting news on social media all of these things that potentially trigger a worsening of medical health symptoms. We have to try to either step away from it or shield ourselves to some degree from it because the other side of the mental health crisis when it grips people. It takes them out. Both, you know, physically as well as mentally and emotionally and recognize it and the people around us too. I think, because sometimes you know, people need help. When you said team member, you just lost a team member. You're you're a member of the SWAT team. We should tell people and it was a justice police officer tragically over the weekend. Who had committed suicide. I know you knew him, and I'm sorry about that, and my wishes go out to his family. My sister also knew him indirectly because he's his mother worked at her school. So it Islam. I mean, sad, You know, it's it's It's very sad. TD dot com share got to give us the website for that all right let's turn to some medical questions here for Dr Steve Dr Steve Saltzman is joining us and Dr Steve are how are you today we got a lot to get to cage will die every day what gets you what there are so many questions and today I think the big question is when did this virus actually first arrived in the United States and the reason I'm asking about that is because I saw this study that I think a lot of people have seen and they are at southwest cut other west coast situation where they've identified three individuals who died in February they've done autopsies on them in California and they found that they actually tested positive for covert nineteen they died in February we were a live regionally believe the first case was in Washington after that looks after that so kind of strange yeah we discussed this all back a few weeks ago distinta clear county did autopsies on patients who died at home and they identify these three people like you said with the idea of yours six at the late seventeenth and March six started may news was because it was previously thought again like you said the first U. S. that was a man in Washington state on February twenty ninth you know again at that point he was very limited yeah only patients who had no travel history that who sought medical care for specific symptoms well one all the way back and we're even getting tested at all so again eight one three shows that the limited amount of testing there's a potential undercount of cases and back but the question blended Colonna why is still very perplexing it was an ongoing severe flu season and again like we talked about the symptoms are similar from the flu in corona for many people have it and it wasn't well known at the time that the October November December two all the positions all over and so many of them might even just soon that patient had the flu and didn't even bother checking them back in October November and so you can see combined with the fact that their stay in the country at least twenty five percent of patients are asymptomatic carriers you can sort of see how the picture gets muddy and community surveillance which is going to be a necessity going forward in many places didn't even start to late February so in the end what does that mean yeah it's plausible that uses arrived earlier than was first reported in January yes so I got you know it kind of speaks to what we did talk about a little while ago and a lot of our listeners sort of weighed in on this and said they agreed that they felt as if maybe they had something that seemed a lot like this the fever the cough those types of symptoms that didn't follow the normal lines of of the flu and yet there was no testing for it we didn't believe it was here at that time it it sort of speaks to lease the possibility that this was present in the U. S. earlier than we thought I think I think yes it's possible I don't have possible but he is portable and the antibody cast I'm going to show whether a person has had eight and has since recovered from Colbert nineteen so we did anybody catch will start giving up a lot of information like we talked about yesterday once it crosses over large portions of the country and we really can't you know the N. number that were using the thousands and thousands yeah because that was the other thing we talked about that antibodies study in LA county where it showed that more people have the antibodies were infected at some point perhaps even without knowing it that it seemed to impact a larger part part of the population than they thought like forty times more people according to that one study which I understand they're still reviewing and much more data needs to be counted here right this is why we're trying to get those antibodies studies or tests more widely available so we can tell exactly how much of this is out there yeah exactly they are because if that if those numbers turn out to be true you have to be undercounted huge they were talking about two hundred twenty one thousand four hundred forty two thousand people more may have been inspected that's that's all let you walk by a few there that'll yeah so many so much of this it just feels like we are just learning as we go along and that's that's just the nature of something like this but sweet and also making news today because they resisted the lockdown they kept schools open they kept restaurants open on the cat most businesses open and they believe that because of that they they protected the vulnerable they protected the elderly and they they took a very different approach than most of Europe they believe that they develop herd immunity sooner and that they may re open in a couple of weeks now because of that strategy again no one knew back then what that was going to mean or when you're looking at the what the curves that could have been health officials were making decisions based on best available knowledge but that seems like a very different approach it certainly worked out differently for them well I guess again won the gold the antibody test would tell but you know you put yourself in the position of of making that statement about what her community at what cost you know if I'm hurting unity if you knock out a large percentage of a vulnerable population that wasn't the right way to go you know yes it's a hero to zero you're only a hero until everything you know goes to hell in a hand basket in your is your so you know you're you're as good as per projections turn out or worse circumstances turn out right now yeah I got yes I mean look at the ideas I would want to be on the decision making side of all this because that that's you you've got to look at data that sometimes isn't as thorough as you'd like it to be and try to make decisions that are best for everybody in terms of all things and those are difficult difficult decisions to make one thing we constantly hear from listeners I feel this way too is I love it for there to be an antibody test that's more widely available I know public health officials in Illinois in Chicago say they want an antibody test out and make it more widely available they're trying to roll it out faster and get more access today they just opened a testing tenth in reserve fuller ten and I've got the address somewhere it was it's on the north side I'll get it for you but it was the basically they started saying Hey if you want to get tested for antibodies you can insurance usually pays for it or else it's two hundred two dollars out of pocket they're accepting people for that zero serological tests is that what it's called you have to be feeling well and no symptoms for fourteen days and they're starting to accept appointments Ashlyn Fullerton that's where it is it's an anti body testing tent the just opened I imagine there's a lot of demand for that yeah you know great idea I just wonder how many cats can they do in a day how how long does it take to run the test really help with the the the handle it's been a continuous problem pull the curtain back and talk to other experts Kathleen walled gotten significantly better depending on your health circumstances health systems no ongoing issues right tell us what the test is Dr Steve I'm gonna get an antibody test is that one of those painful Swabi testers is different which sold the antibody testing the tech to tell whether a person has had and recovered from the cold and it's not been nasal and throat swabs those nasal approach what check does detect viral genetic material this is our blood serum test you can anybody catch the detecting a person's immune response to that buyers so young from Jesus finally took ten to eleven days per person come out and your responses and produce these antibodies so you know that's why you can't use these for acute testing for coronavirus infection this year if you've had instances were covered will be a good guide ultimately about returning to work who actually needs to be further quarantine who can go back to the front lines and continue the fight hopefully you know in a safe and and somewhat protected mad about you openly rejected by taxes is going to do what you're looking we have a great question from a listener here a four seven says Dr I completed breast cancer treatments less than a year ago I can't get mammograms mammograms yet either get chiropractic offices are open other things like that I think I think that that is a real issue right now were people who would normally be going to get some kind of care that does seem important are either not going because they're afraid to be in the hospital or free to overload the health care system or it's just simply not available right now because it's viewed as nonessential I will provide you have for people who may be in a situation you know you look at some things that you normally say Hey wanna go see a doctor about that I mean I could get that checked out and sometimes it's a big deal if you don't she nobody answering this bad per lease my patients as a physician if they have a question I'm available you know that's what I'm here for for them to reach out to you I think overall to the entire audience the important thing to know is that while base this potential overload of the healthcare system is ongoing I would think you wouldn't marry some point many institutions are gonna start opening back up at least for some elective procedures elective surgery all cancer patients who are being followed the patients who who don't necessarily need acute acute care but need sort of chronic are on top of their healthcare and I think a lot of that stuff will somewhat come back to normal again that's gonna be healthcare system to help your system depended to some degree yeah I I I believe that's true people are asking about the antibody test why is it two fifty for fifty dollars I think it's only two and a fifty dollars if you don't want to go through insurance if you don't want to deal with that but I know that most of the stuff should be covered under that that Congress act that is supposed to cover the kind of healthcare costs related to anything covert nineteen I don't I don't know the specifics of that but that's what the release said that I read that it could be two fifty if you don't want to go through insurance I hopefully open up more of those can do we have we have a minute here to here because I know we've been trying to get to this we had questions a lot of questions for you Dr Steve about mask cleaning how do I ensure that I'm cleaning my math correctly off the mask or every time you take off the mask that has to be put into the wash and you gotta clean your hand back this week with the neediest wait I understand people might want to try to clean get to the good doctor Dr Steve Saltzman bring care medical Jeez would you like the we are the world thing to be remade Steve Dr Steve actually I was getting ready for you when I did you would you let yeah you won't you don't wish that believe me you don't want that at all all they do not have to be on can you yes this is the whole different thing but we're happy to have the expertise that we relying on so much during all of this because it's been super helpful I gotta ask you about that you know the big piece of data that was released yesterday presented a very hopeful picture we're constantly hearing about modeling the university of Washington is one of those places that's doing modeling that the federal government Dr Fauci and all of them are using yesterday they said fewer deaths an earlier peak for Illinois this Saturday even could be the early peak and they also said that the health care system won't be as overloaded as was earlier projected so what would you what are your thoughts when you see something like that with your critical life well the first thing I would say is tell that to Lake Forest in canal you know who are struggling right now it doesn't mean much I suppose but you know let's take a step back and discuss all this modeling because it's very unclear to the general public how this comes about even nothing more at least initially better now than multiple gases it's based on how infectious viruses and more importantly what was the success of our isolation and mitigation strategies once they were implemented an employee review going back several weeks we talk about it there were two big theories around one with the institute for health metrics that spoke about in a best case scenario that they potentially won't be enough all right you bet and then there was that second group which came out with not necessarily this broad range where the first group said with Pekin Illinois at towards the end of April which looks like that's probably what probably why that's happening but this group talked about a worst and best case scenario and in the best yeah and and the worst and best case scenario which does leave was like one foot in the toilet and another on a banana peel but as they are doing this large scale testing you can see how these models are going to become more accurate it's absolutely the lack of large scale testing that hurt these models to begin with which is why he was sort of all over the place now I again you know we have to approach this model with trepidation but I think they're they're basing it on more data now and this will be closer to probably what we would want to see some positive okay well it's good it's good to get positive we have a couple questions from listeners but let me get to this other issue here that I think is is important one when will the stay at home and and I think a lot of people were all asking that we're all trying to be patient and doing everything we can to commit to staying indoors and and riding through the storm and I think a lot of it depends on healthcare answers and the answers from health officials what kinds of things are they looking at now because we don't really have a direct answer here locally every time the questions asked for the state they say that they're they're they're not they're not as specifying anything beyond April thirtieth it's work works set to end April thirtieth but whether it's going to move forward from there or how long ago on we don't know but tell us what kinds of things will will really matter what will they be looking at in order to make that determination right so there's a big group that's gotten together and come up with general guidelines basically run by Dr Scott Gottlieb who is the former FDA commissioner and a bunch of experts and basically the cabbie out of course to what we'll talk about right now is that United States has been open on a state by state basis and it's very clear that New York and Illinois might be very different than say North Dakota and Montana but that being said certain things have to happen in hospitals have to be able to safely treat all patients that require hospitalization basically there needs to be enough beds for patients not just thrown a buyer's patients a second thing is that they need fourteen days of falling numbers of new Colby nineteen chases fourteen is basically a crucial number in this pandemic because at the time that scientists estimate the virus can live dormant without triggering symptoms a third thing that needs to happen is that if they need to be able to test at least every one who have symptoms not just the people with worsening symptoms again we're getting better at that but you still you have to sort of meet specific criteria outside potentially first responders and fog light workers you get tested and so there needs to be a again a much more robust testing system in place and for us you need for me to be able to conduct monitoring of confirmed cases and contacts that's going to be super super important going forward you're right he was doing this when I was in the mitigation phase of the pandemic but once everything got over one they didn't do that you have to be able to monitor people because invariably there are going to be outraged again of the specific virus and so if in fact that happens you have to be able to police this and you need people who were doing this before to be able to trace their so that if somebody said they can grab all the contact and quarantine them before potentially they could set off another massive outbreak so these are some of the things that they're looking for but in the end state all part social isolation is the best way to slow the spread despite going to continue until there's either a vaccination to prevent the infection or to a significant portion of the population I've had the virus and clear basically herd immunity that call back what do you call those two things happen it's never going to quote unquote be completely can paint mmhm okay that helps helps answer a question about this question on hearing that it's happening much much more to men than women it's far more deadly than in terms of the numbers we're seeing two times more deadly for men it's killing two times more men than women why do we think that why you think that is well the children about a robust response to it and again the other problem is potentially that maybe it's hormonal estrogen based these are all potential theories that are floating around not necessarily really known at this time but it's clear and by the way interesting another emerging theory that's coming on is that the body is having a massive reaction to what's going on and that's what's causing people to have all the secondary problems and multi organ system failure so did very interesting data that's starting to get a mask for guarding the public nineteen virus so in the next several months would have a lot more answers question about closing that deceive emergency room workers move remove their clothes as soon as they get home so should we be removing her clothes when we get back from the grocery store is is that being too extreme three no like it's nothing too extreme I I I think in in this current climate especially currently now where we're trying to socially isolate you have to
