States who contracted the virus through unknown origins both of those patients are from the bay area Solano and Santa Clara counties joining me now is Dr Charles Chu an infectious disease expert and professor at UC San Francisco doctor to thank you for being with us and it's a very busy time for you Dr sure there's a lot of fear in the community right now how much concern should people have what level is appropriate I think we should be worried we we definitely should be concerned because we are starting to see some cases of of the virus circulating in the community and by that I mean cases of infection where we don't know where it came from we don't know how this one individual ones from Solano county became infected and we also do not know how the second case in in Santa Clara county became infected as well so I think we should be worried we should be but at the same time we should also be quite vigilant and really be able to keep in touch with the with the guidance from our public health agencies and manage medical authorities which is to make sure you wash your hands to make sure you don't touch your face after that what else would you recommend for the recognition would not be too old to wear masks for instance not at this time yet we still do not see definitive evidence that we have community transmission we have lots and lots of cases that are being that that that are circulating in the community but the other thing would be to get your influenza vaccination because it's still the fact that influence is far more common than than getting an infection from this virus and so if you get your influenza vaccine vaccination that's just to protect you from that it's not going to help you with this new virus that yes yeah the influx of the influenza vaccine doesn't protect you against this virus but it does protect you against influenza and influenza this year has already been responsible for thousands of deaths so it it's it's just to put things into proportion your risk of actually getting influenza is still far greater than your risk of getting infected with this fires on their website the centers for disease control says quote the potential public health threat posed by code nineteen the formal name for this virus is very high both globally and to the United States where do you think we are in the trajectory of this virus how do you expect it to evolve well well right now we're actually seeing more cases of the infection from the virus outside of China it used to be that the vast majority of cases were in China but I I think as of yesterday there was a turning point in that we're now seeing evidence of transmission say of the virus in other countries say in South Korea and Japan in Italy and Iran so far it appears that at least the United States we still have very few cases under fifty I believe and out of which nearly all of the cases we're able to track meaning that they're either returning travelers from China that were able to follow do contact tracing on as well as potential individuals who come back because are quarantined these two cases though are the first cases of community acquired transmission where we did not we did not know where they got infected from and do you expect to see more of those will this become a seasonal virus I expect I I believe that that's certainly a possibility one thing that might happen and that may be due due to the characteristics of this virus that roughly about the the majority of people roughly about eighty to eighty five percent of people infected they have mild symptoms that have no symptoms at all it's only the small fraction of say ten to fifteen percent of of of individuals affected who develop more severe disease which is pneumonia so I I do feel that there may be there is certainly the possibility and the CDC has warned of this there's certainly the possibility that we may see ongoing community transmission and more more cases of this virus in the community let's turn now to talking about testing and then therapies so first on the testing yesterday governor Gavin Newsom held a press conference is that the state had only received a few hundred test kits from the CDC and that we needed more can you tell us about the testing protocol and how many more kids do we need well we we we probably will end up needing hundreds to thousands of cats and the reason is that we we need to we desperately need kits that can be done locally one big problem which has been increasing in eight states at all testing had to be sent to a centralized laboratory the CDC and as a result you would have to wait to ask our doctors would have to wait days in some cases longer than you know twenty four forty eight hours two to three days before you get a result back so what's really needed are diagnostic tests that could be done but locally by either the county public health laboratory or by the hospital and I think that the CDC is making efforts on moving that to expect testing to be local and this is part of your work you were on with us short time ago to talk about creating a rapid diagnostic tool yes how is that going so that's going well we we've developed as a point of care diagnostic where input so we can actually diagnosis infection get a nasal swab get some mucus from patient and be able to diagnose an infection within thirty minutes so structurally this this test works the the main challenges right now or is finding a commercial partner where we could that potentially put this test get FDA approval for this test and be able to deploy it so that could be used to to help doctors make diagnoses and if that happens how long would it be until we could see it in action well it's it's my hope that we really need to because of the public health urgency of this thread that we really need to have better diagnostic test so it's not just my efforts but really kind of the efforts as a scientific community as a clinical community we "Several more unexplained cases of the new coronavirus have appeared on the west coast it's raised concerns that covered nineteen is spreading in ways that are hard to control their new cases in Washington state Oregon and California and the disease continues around the globe the World Health Organization now reports cases in at least fifty six countries join now with the latest by NPR science correspondent Richard Harris Richard thanks for being with us sure good morning Scott first please bring us up to date on these new cases in the U. S. yeah well it's been quite a week it earlier this week there was a case reported in Solano county California that's sort of midway between Sacramento and San Francisco and that's raising concerns because that woman had never really come in contact with anybody who they knew had been exposed to the corona virus she hadn't travelled to China she's now in the hospital and people are really concerned about that the CDC sent like ten additional investigators out there to figure that out but I just as that story was starting to unfold late yesterday California authorities talked about another presumptive case in San Jose which is at the south end of San Francisco Bay or in San Jose area I should say then health departments in Washington and Oregon chimed in the wee hours a Oregon said there was an adult in Washington County Oregon who fell sick about ten days ago and was tentatively diagnosed just yesterday with the corona virus in there two cases and Washington one woman who traveled in South Korea so that's not too surprising but the other person was a high school student in Snohomish county which you may recall who seems like a long time ago now was the side of the very first case identified in the United States we should all of that seems like a serious new turn of events what's going on it could indeed be quite serious community spread is troublesome to date all the other travelers have been doing pretty well known they've either been travelers to China or Japan on that cruise ship or their spouses health officials contract folks in those circumstances because they can figure out who the got been in touch with them keep following up and make sure those people are keeping track of their own health and so on but when cases start popping up at seemingly random moments that control method gets increasingly problematic so question is is this really the first sign of things going out of control or it is also a possibility that just there is been a long delay in testing and now that states are starting to do testing were almost sensing a burst in new cases and what's behind the holed up in testing here in the US yeah well testers to officially run in Atlanta which is why these are still presumptive tests but the CDC produce some test kit some weeks ago and they sent them out to the states but that didn't work into the state said we're not going to run them until you sort it out she says take a long time to figure out what exactly is been wrong with the kids they originally sent out and are they finally said okay we have the green light you can go ahead and use those cats they sent out a new patches and so on and so that's why we're seeing test now so unfortunately that delayed also means that there's been very little testing over the past several weeks beyond highly suspected cases and so that's why maybe by we're starting to see this out first right now U. S. now has more than sixty cases of the corona virus could you put that into perspective for us well clearly it's nowhere as bad as in China and we've been seeing the last couple of weeks these other areas of outbreak like South Korea Iran and Italy where cases are really rapidly multiplying since most cases here are still pretty well understood a most searched and fact off the diamond princess cruise ship that was in Japan for awhile it's too early to panic clearly but you know it is it is a concern and I think we'll learn a lot more as testing wraps up in the next week or two really what the dimensions of this are here so we keep hearing wash your hands don't touch your face any other steps that people can take yeah well certainly if you're six stay at home and if you are in contact with a sick person or concerned about that keep your distance six suites a good distance think of this is the fluid essentially spreads the same way and and treated appropriately when the CDC says be prepared what does that entail well at this point at its institutions really who need to be kicking into high gear we've been hearing all week from companies about their plans they're canceling work trips there figuring out who can work from home they're putting out their policies one of the biggest challenges I think we're gonna face collectively is individual school districts may decide to close maybe for weeks and weeks and that's a huge issue for parents and also for the employees to figure out so I think we have to start thinking about you know what life would look like if if schools close that's a

S. tested positive for the corona virus with no link whatsoever whatsoever to foreign travel this was a person in northern California how the person got this dreaded respiratory disease they don't know the CDC is saying it's possible it could be an instance of community spread be the first time it happened in the United States there's a first time for everything and they're also leaving the door open that the patient might have been exposed to a return traveler who is infected this is my fear all along as somebody who was just on a plane and in California for a week and got hacked over meaning right behind me then surrounding me on a plane this was my concern is that you're on a plane who knows where that person bounced from one wherever unlikely there but someplace else where they might have brushed against it so the state of California is calling the case its first instance of community transmission first case so to me this is scary that's the scary part we get the floating diseased ship the diamond princess we understand that we understand how people were infected there that makes sense this one doesn't make sense and I think this is pretty scary that what they're saying for example hi John Hopkins center for health security note health security that confirming this one patient there is a good chance that there are other people infected in the country and the virus is circulating on detected so what we need to know is what's the surveillance policy and how many more out there and how do you respond to that The Washington Post did a good story on tracking the spider web an excellent story on tracking the spider web of what could be when it comes to the corona virus just quickly a little more on the Solano county person and this week this was reported to the California department of public health they're being treated at UC Davis Medical Center and of course Solano county is home to Travis Air Force base hundreds of Americans from China were brought in there from that cruise ship and they've been kept in quarantine many have been released so now the health department is tracing people who might have come in contact with that person so that they can figure out one if more people are infected with it but perhaps with who they could have come across so to me this is something to be concerned about I mean it listen there's complications with this there's confusion support communications on the corona virus and mapping it figuring out how figuring out where all of that is so vital no I mentioned Italy so quickly I'll tell you what it's like in Italy Italy meant one of the most beautiful countries ever is that not on your bucket list or was it not on your bucket list hopefully not anytime soon because in Italy boy they're having a really rough time of trying to chase that spider webs down and the reason why is because we keep every time I turn around somebody else I has contracted it so the numbers in Italy just climate and their people are ordering and wearing the masks almost everywhere so if you're traveling in Italy in anywhere else internationally mask mask mask it up four hundred cases as of an hour ago confirms in Italy and in fact in the hospitals they're trying to scramble as much as they can to get enough beds and quarantine areas to be able to have more people are coming to be quarantined frankly because they keep discovering more people with it and of course they're sanitizing every area they can they're trying to contain it the World Health Organization said that for the first time the virus was spreading faster outside of China where it originated globally more than eighty thousand people in about forty countries have been infected with the coronavirus still the vast majority are in China but Italy hit incredibly hard if we were to look at a map and I would give you a key you know what a key is when you have different colors like let's color in this area this is where the bulk of it is I mean it's Italy is just covered the boot is covered some of the worst infected areas Lombardi right around my lan very close to Venice twelve people died in Italy so the government is taking this serious they're worried they're trying to trace it like I said they're having problems in having enough beds to figure out where they're going to put people schools universities

"The president's during the White House briefing room yesterday to announce a new approach to the virus. Several health officials joined him. C. D. C. Deputy Director and shoot. Okay let's talk about this new case here in California and what the significance of it could be yes so the CDC right after this briefing ended last night reported that someone who in California who doesn't appear to travel to any country where the virus is spreading exposed. Anyone who might have caught the virus overseas had tested positive for the virus. That means this could be the first instance in the United States of what officials call community spread. You know in other words. They caught the virus in this country with no connection to any other country that could have spread here. There's been sixteen reported cases of Corona virus in this country. Far all the other cases have been people card in China or on that cruise ship except for two close relatives of two of those people brought back from China. So this would be very concerning. You know because it's exactly what health officials have been worried might start happening that the virus could start spreading in this country now the CDC says it can't completely rule out that this person might have had some contact with traveler. They don't know about but so far. That doesn't seem to be the case. Okay well then the question on all of our minds of course is what do we know about this person and how the person may have gotten the virus if not through someone abroad? He S O California health officials. Say the person lives in Solano County north of San Francisco and that this person is receiving care at the Ucla Medical Center and according to the hospital. The patient was transferred there from another medical center and doctors suspected the patient might have covert nineteenth. Is Diseased Causes but since the patient didn't meet all the criteria for the corner virus they weren't immediately tested by the CDC. And in the meantime the hospital has treated other patients with farces like this so took all the necessary precautions to protect doctors nurses but after this patient did finally test positive the hospital as a small number of hospital workers to stay home and monitor themselves. Okay so so. This could be the beginning of what's known as community spread in the United States. Although it sounds like there's some uncertainty here but that obviously raises the question of how prepared we are in this country to deal with the corona virus. What what did you feel like you learned from the trump administration when the president and others talked about the approach yesterday? Yes so the president you know. He took a very kind of low key. Approach to this was tried to be very reassuring saying you know. Things are under control But the president announced that Vice President Mike Pence would be taking over in charge of coordinating the nation's response to the outbreak until now Health and Human Services Secretary Acts as our has been chairing the corona virus taskforce. Now he'll stay in that role but trump says he picked pens because of his experience handling healthcare issues especially when he was governor of Indiana and that he will hopefully help coordinate phys across government agencies including like the transportation and education department's NPR health correspondent. You had trump downplaying this and really in a lot of these situations. I mean I kind of think of it as being on a plane and facing turbulence and urine stormy weather. You want the pilot to be calm by. Everything's okay but no but also serious and telling you to take your seats right input safety belts on and make sure everyone does it. You know the president because of his tweeting in his own personal volatility. That makes it difficult. And there's a lot of people who are never going to trust him as their pilot but that's partially probably why. He decided to put Mike Pence there. Because he's such a controlled messenger and for the most part trump during that press conference got out of the way of Public Health Officials. And even swatted down some potential conspiracy theories about whether the CDC was trying to inflate numbers or the risk to hurt his reelection. Okay so maybe. He's putting pence there because people have more confident. I mean it also sounds like pence has experience as governor with healthcare. That may put them in a good position to respond to that. What is what is that experience. Well you know. The thing is though Mike Pence is very controversial. Pick to put here. There was an outbreak in two thousand fifteen of HIV in his home. State among intravenous drug users that is really causing a lot of critics to look at this and say they raised some eyebrows. Wonder if this is the smartest person that put their at one point according to USA. Today the number of people who had contracted HIV in one rural county exceeded the number of people infected with HIV through injection drug use in New York City the entire previous year while the CDC another health officials urged making clean needles available but but state law and pence Moral opposition really initially prevented that two months after the outbreak began. He said he was going to go home. Prey on it two days later he finally issued an executive order to make those clean needles available. So look he's not a scientist and I think that's one thing that a lot of people are nervous about if this does spread and Democrats which would say already critical of the president response here. Definitely they want him to ask for more money from Congress. Trump said fine.
