You know, part of physical. Therapy is getting some pain relief. And and your therapist is going to work on some gentle, stretching they might use some different modalities such as electrical stimulation or ultrasound to get the circulation going to relieve some of that muscle pain or spasm, and the ones some of that is improved them. We can start working on some strengths and a good physical therapist just understands about how to work with you one on one to sort out the options and stretching and strengthening to help you get some pain relief as as well as prevent things from flaring up, and they are really experts at working with you. The physical therapy spine specialists of the ones who can work with you directly to to work on your posture ergonomics and the types of exercises to take the pressure off those injured areas so that you're able to stay active because one of the problems with back pain is that if you're not doing anything. The muscles are at your core section are for example are getting weaker. And if those muscles are getting weaker, then you're more likely to get hurt if you're lifting or bending or going through your day. So what we try to do is keep people gently active, but at the same time, we want to address those painful situations and okay? So let's say and this happens quite often Alva patient who comes in. They're not so good. Well, they do help pain somewhat. But what they're really good at is reducing swelling by reducing swelling often injured structure, whether it's swollen joints are swollen disk that is going to help reduce some of the pain. So you can move forward and make some gains and and range of motion when you're exercising. But what's important to understand is that we want to avoid medicines that are simply hiding pain other words medicines like morphine or vicodin or code on those medicines Percocet, for example, those are just strong painkillers that aren't doing anything for the inflammation. But they do block. The signals the pain the sensation of pain. So those medicines are very important. You know, situations like cancer, cancer pain, fractures, major surgeries. Okay. Sometimes you gotta have a painkiller. But a lot of times for back pain. You know, I want. I want to be able to feel you know, what's going on when I'm exercising. And I tell my patients, you you don't want to be taking a pain co that could be masking the symptoms. So that you are doing more damage on the other hand, there are conditions were, you know, we need to treat the pain more aggressively. And I think what's interesting is that there's been a real shift, you know, painkillers, and and I'll be talking with Dr miss Schiavo, which in the coming weeks a lot about this. You know, painkillers are our international news these days because you know, there's a real problem with addictive painkillers. Opioids and the reason is these medicines. They're a powerful, but they need to be used properly. And and there is a lot of abuse going on. So part of what we try to do. Here premier medical is help people understand that, you know, painkillers pain. We're talking about a symptom and painkillers are really just treating a symptom. So. When you see your doctor, and you're you're getting a careful exam, and you're having discussion and proper diagnostic studies, you're looking for answers to get the healing process going. Sometimes surgery makes sense to but we're focusing on the non surgical options because we want to figure out okay. If we can put if we can put medication into place, for example, where there's a bulging disc or herniated disk, you know, make sense. Okay. Let's try and put a smaller concentrate amount of for example, a little bit of cortisone your swollen disk pinched nerve to relieve the swelling. So that a patient is able to exercise move a little better. So they can do their rehab to strengthen the muscles gently, you know, it's important to understand that those shots. Those shots are really not the fix. They're not the magic bullet. Those shots are really just a tool. There are steppingstone to get your body tuned up to the point where you're able to do some rehab get your strength back because those injections are not gonna do the, you know, they're not gonna do the strengthening in the rehab for you there. There are a tool to help you recover and the cortisone injections that we use because cortisone is a great. Medicine for reducing swelling. It it also has a lot of other side effects. So so what's important about cortisone, which is also known as steroid or prednisone or glucocorticoid? So these medicines. This this family of Madison's is great information. But when they shut down all those cells and reactions in your body that are causing swelling. They also shut down a lot of those good proteins. They shut down a lot of the good pathways and mechanisms that help your body heal. So if we're going to have to use a cortisone shot, we need is a very strategically. And sometimes, you know, we do have people take things like prednisone steroid tablet to reduce swelling. But my preference is to be able to use a much smaller amount. So when you take a medicine like apprentice zone Talbot by mouth, it's gonna get that looted in your system. It's getting to lead in your blood. It's going everywhere. So you know, when you take a steroid talent by mouth, it's going. Affect your blood sugars are going to bump up. It can affect your immune system. It can also affect the the thickness of the bones causing osteoporosis. So we'd rather use a smaller amount of cortisone and use some type of image guidance. For example, x Ray or ultrasound to very carefully deliver a much smaller amount of that cortisone to the area of injury rather than taking a pill by mouth and having to blitz your whole system and then only getting a tiny bit because he gets saluting your blood having only a tiny bit get to that injured part of your back. So people often ask about these types of injections, you know, do these injections hurt. Well, yeah, it's it's an injection. No one wants to have to get an injection done. But you know, these days, you know, this is two thousand eighteen this is not this is not the nineteen seventies or something where a lot of injections were done. Just by your doctor would going and swipe it alcohol Pat across your back and just stick a needle in there. We we like to use digital. Imaging. So we use x Ray guidance to help place, spinal injections in the studies, very clearly show that if you know, you're doing a spinal injection about forty percent of the time. If you're not using x Ray, there's a forty percent chance that you know, we're not going to be in the right spot. So it's really important to remember. If you're gonna get an injection steroid injection, your back, your doctor should be doing it using some type of specialized equipment, it could be ultrasound. It could be Ray guidance. Now, there are exceptions to this. You know, we're talking about for herniated, discs bulging disc, when women are going to have a baby you can't be using x rays in there. You can't be shutting xrays. So that is one situation where a spinal procedure has to be done, very, very gently and delicately and doctors have to go by field. There's you just that's the only way to do it. But. Shifting gears and talking instead about know herniated disk, a bulging disc, a pinched nerve to do those types of procedures the standard of care is to use some type of image guidance to make sure that medicine is being delivered very precisely. And you know, we have a saying when it comes to cortisone shots in in medicine in that close only counts in hand grenades. Horseshoes and cortisone, and the reason is cortisone spreads. So I can't tell you how many times I've seen patients in the clinic. And they say, you know, I see him for back pain or sciatica are pinched nerve. And they say dockyard had an injection in the emergency room. Well, you know, it turns out like I could bat in the emergency room. They probably just had an injection into the the botox or in the lower back the doctor the nurse. Probably just gave him an injection in the muscle. Just kind of blindly, which means no image guidance was used and remember we just talked about that cortisone. Spreads. So if you give someone a big enough dose of cortisol on whether it's a cortisone tablet, whether it's a shot in the in the muscles in the Barak the lower back in the muscle. It's going to spread. It's gonna spread. So sometimes a cortisone shot. You know, it it doesn't matter. You know, if you're going in the muscle, you be enough dose doesn't really matter and in the emergency room where they're just trying to cheat people quickly and make sure that they're not gonna die. They'll do types of treatments like that just to make sure people get some pain relief quickly. But you know, when you see a specialist, and you're getting something done. We are when it comes to spine. We are very carefully using those medications rather than blitzing you or, you know, giving it a Smeg a dose of cortisone just kind of blindly wherever it needs to go. We wanna be using a smaller concentrated dose and very precisely and carefully placing it as close as we can safely next to that urinated a disc or in that swollen joint or by that compress nerve. And and not just for pain relief. Not just to get the swelling off that structure, but a lot of times that can help. Play a diagnostic role it can be a therapeutic treatment because it's going to get you some relief. But it's also hopefully going to help your doctor pinpoint okay? Is this really what's causing the pain because if we give you a tablet, and it's spreading throughout your body. We might not be able to differentiate us much between, you know, certain structures as opposed to if we specifically put the medication and alumni coming close to a particular nerve or a disc that can help us, you know, figure out what is causing the problem. And again that helps us custom tailor your treatment to your specific problem. So the injections are an important part of it. There's newer medicines now to which are different from the typical painkillers, you know, things like gala Penton or Lyrica, you know, how we use. Medicines are also focused on what type of pain so certain.
