"The Rubicon with the budget and you know I think we're you know we're not exactly looking you know some of the estimates have it at 260 billion dollars a year if medicaid were to cover every eligible beneficiary but drill down a little bit and talk about congress you know congress is facing some pressures when it comes to this whether it's their constituents who are dealing with heart disease or whether they themselves are going to slim down and look good they're human too they're going to want to get their hands on these drugs too. What sort of pressures do you see lawmakers facing? Yeah it's a great point you know I've also heard a lot of these folks in congress are on these drugs themselves and you know that certainly plays a role people have constituents but also family members who are dealing with obesity and maybe not just the medical there's all the other disorders that we've discussed but even the mental health issues that go along with that so I think this is a real issue and red or blue I think Congress is facing a lot of pressure so I think there are there are bills up for consideration to amend Medicare's laws I think it will be a careful calculation given the budgetary concerns but I think it's real and I think momentum the does feel to be moving in that direction. Talking about those budgetary concerns as we said at the top the cost of the health care system for the forty percent of American adults who are obese is one hundred seventy three billion dollars a year that's just for the obesity factor that goes along with the type two diabetes in the heart disease and stuff does this cost more than that is there not an economical way to look at I think the offset question is a good one but it's a little bit complex because we can look at it system wide and say oh you know obesity and all the other issues you know cost x but it the savings of preventing those depends on who's paying so it's very possible that commercial insurers and employers will pay for these obesity drugs but because people change jobs they might not have some of these effects until much later in life they're not going to be the ones who save so you know it could be that Medicare ends up seeing a lot of savings if fewer people have all these other issues but they're not the ones who are paying the cost so in that and that will be borne by you know healthy folks with higher premiums so you know I think like we can look at system system wide savings when we look at American health care spending in total but the question is to you know does this make sense depends on who's paying that bill and that changes as people move through the system at different stages in their life you may have just answered this question but I'm going to ask it anyway you think there'd be a time when these drugs will be so commonplace so maybe generic affordable more when it becomes just sort of a standard operating procedure like if you have high cholesterol you take a statin if you are overweight you'll take one of these drugs will it get to that point? I don't want to emphasize under the importance of obesity prevention here you know I think that those principles of healthy eating and exercise need to remain core to prevention and a lot of those things work I think what happens is once you become obese and there's a lot of research going into this right now and a lot of doctors you know we don't have definitive answers as to what cause the of obesity is but there are changes that are happening in the brain of an obese person in terms of understanding signals. So I think that we're moving in the right direction in terms of about thinking taking these drugs and thinking of them as a solution. They're not the end -all be -all. but I do the other really important thing is as these drugs get cheaper which you You know, senile glutide will be up for IRA negotiations as part of the Medicare drug price negotiations in in the coming the years oral pills instead of injectables all these things the market might move to push some of these prices down so that could seriously change the calculus. That's worse than we've seen in something like four centuries so the fact is that the climate crisis is here now. The drought the heat waves that you talked about setting record. Temperatures all across the western united states and even reaching into parts of the midwest. These are the signatures of climate change. And the fact is that the climate crisis is on our doorstep and the question is what are we going to do about it. Are we going to continue to talk about having infrastructure day or infrastructure week for another four years or are we actually going to see congress. Act and pass a bold climate package this summer professor. You said that the biden administration and the senate and house democrats are committed to true climate action. But how do you see this playing out given the clear republican resistance. What what do you think is doable. And an and the stuff. That's not doable. What kind of public pressure needs to come on a come on washington to get it done. Well i think you're right that we need to keep the public pressure up. I noticed that the top of the hour you talked about sunrises marches which have been happening in both california and across the gulf coast. You know there have been lots of actions whether that's against line. Three or four these kinds of infrastructure negotiations that have been trying to raise awareness of lawmakers of just how urgent the climate crisis is and we really need to keep that up and the good news is that just a week or two ago. A group of senators we have over twelve senators. At this point now have said in the line of senator markey no climate no deal meaning that if there is not climate change in the package that moves for this summer. They're not going to move a package for the summer. And i think that's really shaken free in the negotiations in congress because what we're now seeing is that majority leader schumer is saying okay. We can have to track process. We can continue along with this bipartisan idea. That's been going on for several months now and we can finally start the budget reconciliation process for the broader infrastructure. Package that senator. Sanders is helping to lead so. I think that we're starting to see this to track process developed but the fundamental thing. That's part of this process is not climate deal. We have to have a bold climate package. That's happening through the budget. Reconciliation process passed this summer. Well the president has already a sharply reduced his an initial proposal on infrastructure. What's in the bipartisan policy. Package now what's been excluded so far. Well we don't actually know. A lot of what is in the bipartisan policy package. There was a two pager that came out a couple of days ago and it said that there would be about six hundred billion dollars in new spending so things like overwhelmingly roads and bridges and sort of that kind of infrastructure. There was some more hopeful things though because previous republican proposals have included for example zero dollars for the power grid while this proposal included about seventy three billion dollars for the power grid. There's also significant investments in public transit. So there are decent ideas in this. Bipartisan approach but it is not a substitute for a climate bill. At the scale that's necessary and there's also been questions raised about some of the pay fors for this republican bill for example they've been talking about putting taxes on electric vehicles. The exact opposite thing one should be doing right now and senator sanders in particular has said that. He's not interested in a proposal. That does that nor is president biden. Who campaigned on saying that. He would not raise taxes for anybody making four hundred thousand dollars or less so the pay fors in the bipartisan. Approach are really lacking right. Now they include things like re purposing cova bills. I sorry colin which probably need to be spent on kovin. That's worse than we've seen in something like four centuries so the fact is that the climate crisis is here now. The drought the heat waves that you talked about setting record. Temperatures all across the western united states and really even reaching into parts of the midwest. These are the signatures of climate change. And the fact is that the climate crisis is on our doorstep and the question is what are we going to do about it. Are we going to continue to talk about having infrastructure day or infrastructure week for another four years or are we actually going to see congress. Act and pass a bold climate package this summer professor. You said that the biden administration and the senate and house democrats are committed to true climate action. But how do you see this playing out given the clear republican resistance. What what do you think is doable. And an and the stuff. That's not doable. What kind of public pressure needs to come on a come on washington to get it done. Well i think you're right that we need to keep the public pressure up. I noticed that the top of the hour you talked about sunrises marches which have been happening in both california and across the gulf coast. You know there have been lots of actions whether that's against line. Three or four these kinds of infrastructure negotiations that have been trying to raise awareness of lawmakers of just how urgent the climate crisis is and we really need to keep that up and the good news is that just a week or two ago. A group of senators we have over twelve senators. At this point now have said in the line of senator markey no climate no deal meaning that if there is not climate change in the package that moves for this summer. They're not going to move a package for the summer. And i think that's really shaken free in the negotiations in congress because what we're now seeing is that majority leader schumer is saying okay. We can have to track process. We can continue along with this bipartisan idea. That's been going on for several months now and we can finally start the budget reconciliation process for the broader infrastructure. Package that senator. Sanders is helping to lead so. I think that we're starting to see this to track process developed but the fundamental thing. That's part of this process is not climate deal. We have to have a bold climate package. That's happening through the budget. Reconciliation process passed this summer. Well the president has already a sharply reduced his an initial proposal on infrastructure. What's in the bipartisan policy. Package now what's been excluded so far. Well we don't actually know. A lot of what is in the bipartisan policy package. There was a two pager that came out a couple of days ago and it said that there would be about six hundred billion dollars in new spending so things like overwhelmingly roads and bridges and sort of that kind of infrastructure. There was some more hopeful things though because previous republican proposals have included for example zero dollars for the power grid while this proposal included about seventy three billion dollars for the power grid. There's also significant investments in public transit. So there are decent ideas in this. Bipartisan approach but it is not a substitute for a climate bill. At the scale that's necessary and there's also been questions raised about some of the pay fors for this republican bill for example they've been talking about putting taxes on electric vehicles. The exact opposite thing one should be doing right now and senator sanders in particular has said that. He's not interested in a proposal. That does that nor is president biden. Who campaigned on saying that. He would not raise taxes for anybody making four hundred thousand dollars or less so the pay fors in the bipartisan. Approach are really lacking right.
