They're not nearly as communist hamstring injury. So it's, but it's it's an injury that to me would be characteristic for an offensive tackle. Because one of the things that an offensive. Tackle has to learn and execute and practice every day and coaches are always afterwards. What they call the kick step, which is when the ball is snapped the guys on the outside. And you watch them, you know, it's all in the footwork, and they have to take that first step back, and it's kind of a kick step where they they set up a brace themselves for the past protect and that would put that would put immediate strain on the quad. I would assume. Yeah. Just a quick step into, you know, just start have to brace the body for impacts promotion, you know, for run blocking getting that good push off. And you know for pass blocking bracing for impact. All of those things would put almost instant strain on that area. So you know, certainly can be problematic, especially if it doesn't settle down in a quick manner. So he's going to try to start. But. It's the kind of thing that can just happen again. And again, and again, and then he's gotta leave. Yeah. It's difficult with these kind of soft tissue injuries to to gauge you know, how serious it is. 'cause it's really. There is not a good way on a on a study in MRI or an x-ray just figure out how quickly these things are going to get better. How quickly they're going to become not symptomatic, essentially, you know, you can get an MRI is useful. If it's your injury where there's actual a lot of physical damage to the muscle. But if it's just a symptom thing, you know, how that how badly does it hurt when it's used that's different from one person to the next, and there's really nothing you can use objectively to figure out when when the time is when the optimal time to return. It's going to be less question for me. Doctor is there's is there anything that they can do in a preventive kind of way the trainers before the game. I mean, it doesn't seem to me like the kind of thing that you could brace or tape necessarily the way you could say a knee or something like that and says a quad thing, I don't know that you'd be able to do anything in terms of sending a guy out on the field with some kind of protection that would make it less likely that he could re-injure it. "Women architecturally the bones are. Different. The pelvis of a female athlete is going to be wider. And because of that it puts more tension on the muscles that guide the kneecap. So a lot of times we can balance or get that kneecap to track a little more than two the middle by strengthening the right muscle groups. So it can be very painful, we see athletes sometimes there are exercises that can resolve it. Sometimes there's taping that a good physical therapist can teach an athlete to do that will help track the patella cited over a little bit. And then if it's really not helping there are injections, we can do because if the underside of the patella bruised, or there's some type of inflammation, sometimes a little injection can help make that go down. And I think in the gash see I know we try to avoid surgeries. But in the worst case scenario, there are surgeries that they can do to try and release that. But you know, my whole thing is that if you're gonna go to the operating room, it's gotta be last resort because you never know how things turn out. But there are a lot of non surgical options for pain from things like Patel ephemeral syndrome. Right. Yeah. And you know, one of the things I like to also talk about you. Because sometimes when you you send someone to physical therapy for this. They started in they get the taping. The the originally they'll start to say, well, wait, I'm trying to understand this. And I again, I'm a visual person. I like to describe when people look down at their leg. I say, you know, most people heard the name quadriceps quadriceps means four, and there are four bellies or four different muscles that are coming across could verge, and you know, sometimes over time to we'll get part of that quadricep muscle. We get a little more stronger than the other side and you'll get a little bit of the tracking change that's happening for multiple conditions. A lot of times it's prior injuries. It's an injury on the opposite side. And so you're going to get a good physical therapist is going to be able to identify that. And then start taking through a longer term process of getting all those muscles to work together and balancing in sync. So that so that your. You're kind of walking equally and straight on both sides. Because that's the other issue is both sides have to work in in unison. There has to be a relationship with them because you're like a lot of times, you'll get a hip injury or something or low back injury on one side. And then next thing, you know, you're starting to get the knee on the non injured side hurting you because you're walking differently. So a lot of times it does take a little longer to do to get this together. But that's where good physical therapist will help create the balance. So here we have a couple we got. So I guess we're on a shy caller day today. So I have to to callers off air that I want to answer some some questions and the first one is relating to what we're talking about. And the caller wanted to know what are the best exercises for a knee. And I think I'm gonna go ahead. And and try to answer that one which is it depends on what's causing the problems. So it's really hard to tell someone what is the best exercise because as this show is covering today, and we'll continue to cover. There can be things that are very complex and dealing with tears or or things that are wearing down versus patellar ephemeral. We just described as exactly what the right types of exercises will be something to will cause you to to get better. So I think that for that caller. It's so broad pain is the symptom you have, but the cause and the treatment can be very very very different that having been said, though, I think that we've all heard about you know, and football season's here. Right. So we've all heard about NFL players getting ACL tears ACL ligament. That stabilizes a need keeps your thighbone from sliding forward on top. Of your shinbone, and ligaments like a thick band. And actually it turns out your hamstrings. The muscles on the backyard thi- are a stabiliser the knee and even elite level college division. One athletes can help avoid surgery for that ACL ligament by keeping their hamstring strong. So in arthritis, you know, by doing certain exercises to strengthen the quadriceps muscle. You're talking about muscle groups in the front of the high as well as in the back of the fight with low impact. Cardiovascular like bicycle bicycle but doing the right type of pedal stroke to strengthen your quadriceps and your hamstrings. So it's a great question that are Shaikh are shy. Surgery what step one and when do we take it absolutely so we've coined a term called pre, hab pre basically means that you're getting some form of therapy before you even decide to have these. Surgeries and this is a decision that you can make along with your surgeon or your Arthur pita Dr but. You definitely want to get in for some therapy before you start to consider. Having, one of these replacements and what we know is a lot of. Our patients have great outcomes If they do the pre hab if we're having the surgery done let's talk about. That pre have when does it start we talking about six weeks four weeks two weeks hour and a half. What right so I think pre half start even a year before the Absolutely because, this is, a non what we call a. Non? Pharmacologic. Meaning it's a way that you can decrease taking pain pills Four Yeah, yeah We doing in, pre hab my guess is and I'm, no doctor but my guess is that we're strengthening the muscles around that joint and preparing them to take on a. It's typically very weak in patients who have osteoarthritis general arthritis so if we can get that quad muscle strengthened before the surgery we know that our clients will. Have, better outcomes after meaning they will have decrease pain increase function and just decrease time to return to independence wanna get in their early that muscle up and prepare you for your. Surgery yeah it makes perfect sense, because the people most likely to need near hip replacement probably have been in a lot of pain, and not terribly active for a long, period of time thus they've had their muscles atrophy so we gotta start back over and get back to those teenage. Thighs Close directly exactly in what we know. About some pain is it just stops us from wanting to move, if you're in pain you don't wanna get up you don't. Wanna go to the grocery store as much so we wanna make sure that we're we're just keeping you active in keeping your heart healthy as well because that's. One, of the things that you'll need before surgery is a healthy heart was going to say that's something to keep you off of an operating table is a any kind of an. Issue with your heart that dot. The docs don't want to take on that risk to do they know Blame it all so typically by the way I. Wanted to, ask this question because you'll know the answer and I don't. What's the What is the procedural difference between partial and total joint replacement Right so for a total joint replacement They're basically cutting out both, parts of the bone in your knee and they're putting a brand. New prosthetic me side with the partial we have a lot of new innovative techniques where they, can preserve some of. Your. Own bone some of your own And they. Don't have to take as much of your body parts out when are we going to have. Prosthetic biceps and triceps That's what I need some prosthetic PEX Often by the time. I found sixty Come right out Start over take me back to. About twenty five that'd be. Good right there Part of me wants to think that kind of a partial replacement would require more rehab. Because we're actually trying to get original parts to work with those aftermarket parts but I. Read that it's the total replacements that are going to require the most rehab and. I guess. Because what you were talking about it. Just everything's coming out right Right well it really depends on. The level of. Okay so we we kind of have a grade that we use to determine the severity and the ability that your arthritis is causing you you know if you're in. Pain every single day and you you you've lost function you can't get in and out. Of a car you can't walk more than two to six minutes that's extremely debilitating What most likely require total okay So that's that's kind of the things that we look at with the pre hab and determining if. You're even a candidate. For surgery but that's a decision, you're Arthur doctor with, hips knees both weight bearing joints which wants the toughest to. Rehab Great question Say the knee only because the typically with a total, knee replacement my patients experience a little bit. More. Swelling okay because of the load-bearing versus. The right And I'd have to. See the knee mainly because of the amount of swelling that happens after the surgery and. Our patients ability to manage that ongoing cycle of, pain and swelling I noticed here that your options are rehab as, an inpatient somewhere or rehab at home talk. About. The the ups and downs of both Right so so we have in, a facility an inpatient rehab facility you would stay there on average fourteen twenty days for patients who require more extensive help okay meaning they may not have a family infrastructure where. They have helped with transfers and and walking and getting medication sheet of that nature and. You have to tolerate about three hours a day They're gonna push you aren't they yes. It's it's it's a little bit more aggressive that's an I was gonna, say yes, sometimes if you're sitting around the house and your. Your spouse comes in, and says? Come, on it's, time for rehabs like no wheel. Of fortune Exactly exactly versus versus home are. Outpatient you know that, could, be three to five times a week and then the sessions would be about forty, five minutes, to one hour and that becomes a transportation issue Take you to and from because. Somebody who just got a brand new knee probably isn't going to be. In the mood to drive just So those are floor you know. Our. Patients. Who have, a spouse who can, drive them you know and who's probably retired. "Oh because these tiny little bones they're inside the tendon are incredibly important for mechanical advantage but when they break like kyrie irvings did we have learned a long time ago in orthopedic surgery that it's not enough for us to just put screws in because the force of the muscle is in essence fighting us trying to pull the bones apart the bone will not heal unless the two edges kiss each other they need to be under compression if there's any kind of tension which is the opposite of compression the bones don't touch they will never heal and you'll have fractious that will go on forever and not he'll so when we first started doing patella fractures we learned we could get the two bones to kiss on the compression but guess what happens even you being in a cast or brace you start walking around your firing your quadriceps muscle you start pulling the fracture apart so we came up with a brilliant idea that we learned from the construction industry my dad was a carpenter so this comes in handy and it's called attention bay wire we literally right under the skin lupu wire in a figure eight and we can take the ends of the wire and tighten it so that the front of the bone the more you bend your knee actually squeezes the two bones together so kyrie irving had a patella fracture she had two screws and this tension wire put in which on fortunate unfortunately lives right under the skin well go play basketball for the celtics in the nba you will be rubbing that wire just under the skin gets irritated ended effects his play he's such a trooper like a thomas's and they can play through any like marcel while you can play through anything because the adrenaline and you're a professional athlete but at some point it's enough so they take them to surgery to at least open up the skin and get rid of the irritating wire that's underneath.
