"Through the weekend, The National Weather Service says. We'll have some minor cooling early next week. The long range forecast looks mainly dry for Northern California through the end of next week. Forecast high for San Francisco tomorrow. A 61 San Jose should expect a high of 65 tomorrow. 706. This is KQED. This is fresh air. I'm Terry Gross. My guest, Dr. Anthony Fauci, America's top infectious disease expert, has become a hero to many Americans for his scientifically based straight talk about the coronavirus. He became a medical celebrity during the press briefings from President Trump's Coronavirus Task Force, which Dr Fauci served on. He's been the head of Ni at the National Institute of Allergy and Infectious Diseases since 1984 working with seven presidents from Reagan to Biden. None of that stopped Trump from calling Fauci and idiot Retweeting time to fire Fauci and eventually preventing Fauci from being interviewed uncertain TV shows and publications. As you've probably noticed. Dr. Fauci has been unmuzzled by the Biden administration. Dr Fauci and his family have faced death threats from extremists, and he's been the subject of bizarre conspiracy theories about the virus. Much of Dr Fauci is career has been devoted to researching viruses and the immune system. He's made major contributions to the understanding of how HIV effects the immune system. Was instrumental in developing drugs that could prolong the lives of people with HIV and is continuing to conduct research into the body's immune response to HIV. Our interview was recorded last night as part of a WH Y Y zoom event at which Dr Fauci received wh y wise annual lifelong learning award. Wh why? Why is where fresh air is produced. Dr Fauci. Thank you so much for talking with us, and I just want to thank you for everything you've done. I want to start with your new role as President Biden's chief medical advisor. What did he tell you? He wanted you to do. And how does that compare to what? You did it in the Trump administration. Well, it was very clear what President Biden wanted not only of me but of the entire medical team, and it was really Very encouraging because he said it publicly and he said it privately. To us, and that is that science was going to rule. That we were gonna base whatever we do our recommendations or guidelines or what have you Based on sound scientific evidence and sound scientific data. He also says something that was also very encouraging. He said. We're going to make some mistakes along the way. We're going to stumble a bit, and when that happens We're not gonna blame anybody. We're just gonna fix it. And boy was that that refreshing to know that we were gonna let The good scientific method and truth. Glide. Whatever we say whenever we do, and when we don't know something we're going to admit that we don't know it. And try and figure out how we could find out. What the good data is and what the evidence is. So like everybody, I'm alarm to find out now that there's like new mutations of the virus and Was apparently like a hybrid that has like the worst of both worlds. It's more transmissible and also more resistant to vaccines. So how does that change your calibration of the future? I know you want us to keep wearing masks and to keep social distancing in washing our hands and all of that. And two if anything out that up But how does that change your calibration of where we go from here? What The next strategy is Well, I think people need to understand something That's very important that already viruses of which sauce cov two is an RNA virus will mutate and the more the virus replicates. More opportunity you give it to mutate. So when you have so much infection in the community As we have had in the United States over the last few months where you literally have hundreds of thousands of new infections per day we were up to between three and 400,000. Today. We're down now between One in 200,000 today, But we still have you know three or 4000 deaths per day. That means the virus has almost an open playing field to replicate means you give it a knopper to nitty to mutate. So even though this is a challenge We should not be set back by this. We can meet the challenge and you meet the challenge by first getting a handle on the degree of mutations by doing good genomics surveillance number one Number two by doing whatever you can to prevent the replication of the virus by vaccinating as many people as quickly and a sufficiently as you possibly can. And also to double down on the public health measures of uniformed wearing of masks, physical distancing, avoiding congregate settings, particularly indoors. One of the things that we do know is that the vaccines that we have, although they are less effective in preventing disease that when you look at serious disease with hospitalizations and deaths The vaccine's still have a pretty important positive effect, even on the mutants, but we don't get confident about that. We've gotta be able to match future vaccines. And upgrade them to be able to be directed specifically at these troublesome mutants that have evolved is covered now like a forever virus. Like Is it always going to be around and one former or another? Will we always be having to deal with it. You know, Terry, I I don't think we need to make that assumption. That certainly is a possibility that you would have enough. Virus floating around and changing from year to year that you would have to treat it in some respects. The way we treat seasonal influenza where you have to upgrade. The vaccine almost every year. There is a way if done properly to avoid that..

"Let's get back to the interview I recorded last night with Dr Anthony Fauci at a WH Y Y Zoom event. Dr. Fauci is now President Biden's chief medical advisor. And served on President Trump's coronavirus Task force. Dr. Fauci has been the director of Nyad, the National Institute of Allergy and Infectious Diseases since 1984. When we left off we were talking about Dr Fauci is work during the early days of the AIDS epidemic. What are some of the things that you learned from the AIDS epidemic that you feel you're applying now to Cove it to the pandemic that we're living in now. Including how experimental drugs are handled. Well, I mean, there are several things that we could discuss. I mean, one of them is the importance Of getting the community involved and dealing with the community and their special needs. Now, you mentioned that in our discussion over the last several minutes about the activist group You have a different type of a situation. Here in the United States, with Colbert. We have a disparity here that is striking and needs to be addressed that if you look at the incidents of infection and the incidents of serious disease, including hospitalization and deaths, Brown and black people suffer disproportionately more. Then whites and the general population. So I think that shines a bright light. On what we probably should have done all along and certainly must do in the future. Is to address those social determinants of health. That actually lead to with the great disparity of suffering in covert 19. Among brown and black people. No. We had the same sort of thing with the disparities of infection in certain demographic groups with HIV. So from an epidemiological standpoint, there were similarities there. We also learned the importance of fundamental basic science in getting solutions. As you know, you probably remember back in the early days. Getting infected with HIV was a virtual death sentence for the overwhelming majority. Almost all of the people with few exceptions for those who were what we call elite. Controllers. But it was the fundamental basic science of targeted drug development that allowed us to develop combinations of drugs, first single drugs and then a couple of the time and then triple and more combinations of drugs. That ultimately completely transformed the lives of people living with HIV to the point where you went from a virtual death sentence to being able to lead essentially a normal life. As well as not infecting anybody else because we know now that something we called treatment as prevention, which means if you treat someone who's living with HIV and suppressed the level of ours Two below detectable. You make it essentially impossible for that person to infect someone else. So we got there through basic science. What about access to therapeutics like one of the things you did for each ivy is that you helped change the process reform the process so that people who were sick and that was a death sentence could get access to certain experimental drug. While the drugs was still being tested. Right s. So are we benefiting? And anyway now from the reforms from the That That's a really great question, Terry because the foundation for the ex emergency use authorization the foundation for that Get a drug out as quickly as you possibly can, based on the fact that the benefit Looks like it was better than the risk. And you didn't have to fully show efficacy yet. Originated way back. During the years of HIV compassionate use. Of a drug Even before you get in emergencies. Authorization originated way back in the days of HIV. Because we didn't have compassionate use to any great extent until we got into the situation with HIV in the early and mid 19 eighties. So there's a very good connection between some of the things that we're doing now, with interventions for covert 19 that actually originated way back when we were doing HIV, and it's very early years. Here's something that I would imagine, really makes your head spin. During the early days of the AIDS epidemic. People were protesting like we demand to have more attention and more therapies like help save our lives. Like science, do something and do it now. And now it's like the protests are Sure you say Mass will save our lives. Sure you say vaccines was there are like we don't want them like you're trying to save our lives. No, thanks. We're not going to do that. So does that Make your head spin? Yeah. Makes my head explode a bit not just been Uh, yeah. I mean, of course, it's very frustrating when you were a scientist, a physician or a health care provider or a public health. Person that when you have interventions That are capable of saving people's lives and because of the divisiveness that I mentioned a little bit ago. That seems to be permeating our society at this present time, Hopefully not the last forever, but it certainly is permeating society now. That you can have, actually, people. Who was looking at things that could conceivably save their lives and are actively pushing back against it. Because they equate it with a political stance or a political statement. That is very frustrating. So I haven't left time for a question that everybody wants an answer to and I don't I don't think there really is one. But I'll ask you anyways. Like when And how does this end or at least get under control so that we could start? Beginning to live our you know our lives again and not feel like we're confined to a bubble. Yeah, I think it's going to be two major step process, Terry. One is within our own country. And the other is globally. In our own country. If we get the overwhelming majority of.

And unless we get the rest of the world.

"And I said, Well, if I had the ability to implement mitigation measures, I would implement the mask in public rule, which is something that I've been very consistent about, and frankly, something that President Trump's coronavirus task force that I meet with on a weekly basis was also recommending. On Brantley. It's the only thing and we've talked about this many times in your show that allows businesses all businesses to be open at all times. So from my perspective, you know, having a mask in public rule eyes, the least restrictive thing that you can do with the most protection and it reduced cases by 90%. Which is what I also said in the interview. I mean, of course, you know, obviously you know how the announcers are. You sort of want Oh, Try to beat you and and they said, you know, Well, well, why aren't you able to do it so well, You know, the governor's order prevents me from doing it, which is a very factual statement. I didn't mean it as a criticism. It was just a fact. And then he said, Well, you know, have you spoken to the governor of our nation? We'll try to reach out to government have been able to reach him. So then, of course, you know, they sort of play with that as well. So I mean, if you if you watch the interview It was certainly not meant as a criticism. It was just sort of actually responding to questions, But of course, you know the spin the spin doctors that get you know, interviews. They do what with it, whatever they want. And and frankly what? I what I've stressed to the government, which I think is far more important than a mask in public is I've asked him And he hasn't done this and you know again. I'm not trying to criticism that you're just Fox. You know, I've asked him to pre empt. You know the county's curfew because I don't feel and I think that's a real bad business killer and something that hurts restaurants and And people in our community, You know, that's something that I really think we should look at. Because you know if you're being irresponsible at 11 30, you shouldn't be open. If you're being a responsible at 8 30 shouldn't be open. But if you're being responsible at 11 45, you shouldn't be forced to close at 12. If you're being responsible, like 12 30, you shouldn't be forced to close. And you know, we have a lot of you know, Miami, Brian, You know, we have a lot of restaurants that you know people eat late in our city, and it's sad that you know so many servers and waiters. Um, you know, are forced O have to go home and and not earn as much as they could, because he's curfews are in place that are hurting our businesses. And so I really wish that the governor would Rather than preempt our ability to, you know, not put a mask in public order pre empt. You know the county's curfew that would be, I think much, much better for for small businesses. There's wars. We take a look at what you're talking about. And by the way, I would agree with you. I think the best government is that which is closest and most responsive to you. So I think your request are are very, you know, measured in legitimate. The only thing about the mass deal was I mean, that really wasn't heavily enforced. Even when you have the ability to find was it Oh, yeah, we did. We absolutely did. And, frankly, you know, one of the things that I was trying to communicate to the governor for a while now is not only did we enforce it, but it worked. You know, we saw a 90% from Remember the summer spike. We had 2300 covert patients in our hospital or hospital systems were completely overrun. We were at crisis care. We're converting regular badge into ice you but thank God, we're not anywhere near that. Now, But you know, that's you know that the mask in public well, from my perspective was the most effective tool that we used to get the numbers down. We got it from about 3500. Down to 350 a day. I mean, we're still at about 2600 infect new infections. Today we were down a 350 a day without closing one business. Incredibly, if I'm wrong, I mean, there's nothing that precludes law enforcement from going to somebody in the city that isn't wearing a mask and saying, Hey, we'd like you to wear a mask. They just can't find somebody right? Yeah, And you know when there's nothing that precludes someone from saying, Hey, you should wear your seat belt. You know what I mean? But you can give you a ticket. I mean, I think you know, enforcement or or stop our stop sign, or or, you know, or red light. Right. And you know we don't We don't just tell the police officers him. You guys are sort of like, you know, traffic cop second just, you know, go up to people and say, Hey, we really would love for you not to run that red light next time, Okay? We really would love for you not to run that Stop saying I mean, you could kill somebody. If you wanna stop site or red, Like, you know, there has to be some sort of accountability and there has to be some sort of consequences. I mean, we're not little kids, You know if we do things that endanger others, Um, you know, we have to pay a certain consequence. Otherwise, there's no disincentive for us to do it again. And so you know, I look again. It's something that we could Debate about for a while, but the numbers are very clear. I'd be happy to go in person to your show and show you the graphs of how we got the numbers down from 350,350 were right now 2600 and we have about 950 hospitalizations, which frankly, is much better than we were in the summer, Thankfully, but it's still far greater than what we should be. Now again, I agree generally with where you're coming from, And I think the governor has had a lot of sound policy That's resulted in us having a better than average economy well, actually having on a population adjusted basis, fewer cases than even states like California that have had some relative coming measures that being said some of his, I think, inconsistent policies along the lines of what you're talking about. I agree with you or you're coming from just one to kind of bet all these different issues now I did understand. They've got something potentially real quick. Sure, quick that I agree with what your prior comment. I think You know, there's a benefit to some of the decisions that he's made. It don't want to highlight that. You know, there are things that have benefited our city tremendously. I think part of this renaissance that we're seeing from New Yorkers and people from California fleeing those those States toe sort of run away from socialistic government is a big byproduct of the fact that we've been open throughout this period. So I do want to commend him for that. I don't want to make it sound like I'm just You know, you know, Listen, public officials. We don't always agree on everything. I think the most important thing is for us to be respectful of each other on certainly don't go out of our way to be critical unnecessarily. And that certainly wasn't trying to do that. But you know how like you said, you know, we said, you know how the media sometimes will spend it. And I appreciate you coming on to talk about it. I understand if you don't have the time do you happen to have maybe four minutes or so to toe? Hang on, come back with us. Yes, sir. You got to be great. "Think it's a joke. Sense of humor will add this anonymous producer in the morning. My wife is a roller coaster. Engineer for universal studios orlando. Who by the way. We need to hang out with roller coaster engineer school. Nice but she's getting asked to go help finish their new park in beijing. Set to open this summer Human resources gave her the rundown on what it takes to get into china right now and says i'll work backwards. When you get to china you have to quarantine for two weeks in. What is essentially a chinese motel six with hall monitors. You can't leave your room where they shouted you in chinese. They bring you food every meal. You can't outside and you can barely do laundry. But our anonymous producer continues. What i found more interesting is what you need to do before you go want you to take both tests the pcr and the antibody test you cannot test positive on either. So he says seem strange. They wouldn't want people with the antibodies but maybe they're being overly cautious but here it comes. If you have had either the pfizer or moderna vaccine. You are not allowed entry into china. Hello what's that about that. Needs to be reported on jess. If you've had just the vaccine visor or donor. You're not allowed to enter the enter the country that is sketch. Well that needs to be confirmed. But i would say He said it's chinese. Know that these vaccines are just. Don't do anything or they make it weird or perhaps they are worried that if you come in and you've had the r. m. m. aren a vaccine which is what these are Perhaps they're worried that if you catch the the next variant flying around china that you might die because that is what. The biggest problem was where the animal trials of these particular types of quote vaccines that were skipped over. Think of the minx. Yes oh not only think of the minx. Word is out now Let's see if i could find this news story There is now news. Coming from super. Shocker shocker from the pharmaceutical industries that you probably might have to think about getting your pets vaccinated against covid nineteen because you know they can get they can died. They can pass it on to you. Oh yeah the coming for your pets. Will you said that sometime back. Also it's a great market. There's going to be no subsidy so you wanna be wanna have your your pet vaccinated for covid nineteen is. You're gonna have to pay you pay big way. Thinks a couple of hundred bucks a shot to say no why to fun. There's other clues of weird collaborations. I don't see. I'm not quite sure what this is about to have some ideas. This is the well health safety and they are. They want they are. They're promoting to become the brand that the building. You're entering is safe from covid germs. And they've got a celebrity lineup of people promoting this This this seal of approval Starts each one of them. Got a few pictures or something. Well it's starts with lady gaga. We all wanna feel safe in the places we spend our time. Restaurants theaters stores alltel stadiums. Offices banks childcare centers and schools. I'm dr richard carmona. This is the wealth health safety seal from the international well building institute the global authority on healthy buildings. When you see the world health safety seal it. Means health and safety measures have met rigorous scientific standards so look for the well health safety seal out side and more confident going inside. Deniro scam i mean. Do you think that they're really in on the money side or is this may be something for venues that they're involved in because they heard wolfgang puck and a bunch of restaurants. Yeah yeah well that makes sense and deniro. i guess for the movie theaters. Okay deniro a bunch of restaurants to property and stuff in new york so they got no shares. They were on their knees begging to be. Please give me the seal. So i can open up and god got needs to go back on the road doing concert. Yeah it's the open me up seal of approval me right. You are so right now. Let's get back to the american run down. Because i mentioned a newsletter that democracy now is one of the outliers. That doesn't seem to have a clue about what's going on here about. Let's back off on the scary reporting at least newsom knows what's going on. He's been red in but she but david hasn't been reading. Yeah so we still get these death reports and it's always worse than the next day you know. It's just. She can't whatever she does when she reports is always the worst. I mean when she does climate reporting still hottest day hottest tuesday and the history after world. How does tuesday afternoon. So here's her. Cova democracy now desks report. My the world has recorded over a hundred million cases of covid nineteen deaths now top two point one million in the. Us january is already the deadliest month since the start of the pandemic with at least eighty thousand lives. Lost president biden took new steps tuesday to ramp up the rollout of vaccines including the purchase of two hundred million more doses. Half will come. From dern the other half from pfizer biontech weekly vaccine supplies to states and tribes will also be increased officials from the biden administration. Walked back the president's statement earlier this week. That any american who wanted the shot could get it by the spring instead. Saying end of summer is a more likely timeline. O-man i i have a couple of clips on that if i kind of wanted to go before we get into specifics. Because there's a mask thing that's full of crap is a vaccine discrepancy that it's all going on the e. or they're suing not producing enough. Well they're telling not just modern. I think it's uh wasn't during pfizer. They're telling pfizer. Don't send anything to the uk even the ordered early. We need him. And you're producing here in brussels so we're not gonna let let the those those traders who ran away from us we're not gonna let them have it to trying to subvert the warring factions now between countries. But the thing that that struck me. Is you have everyone jockeying for position. Everybody working on maybe their legacy. Maybe their next job as they're out the door. The medical leads were out in force Just trying to show how virtuous they are and how right they are and how they defended the country. And i'm not sure how it ties into the president biden's administration but do have a few clips. It's interesting to see how everyone's jockeying and it starts Well fao she of course would be the top guy key figures from the start of the pandemic or now opening up about president. Trump's coronavirus task force and a candid interview with the new york times. Dr anthony fauci says despite clashing with trump. he never considered resigning. Dr fauci tells the times. I always felt that if i did walk away. The skunk at the picnic would no longer be at the picnic. Adding he couldn't resign because someone had to push back against the nonsense being spouted from the administration. He's spiking the ball. Like i gotta make sure everybody knows what i burkes is out now. Burks a said she was retiring left in shame after she broke her own rules to winter. Is our house over thanksgiving because we to stop the surge. Where did that. Uh.

"That there was no strategy for vaccine Now. That may also mean like they just didn't have a strategy that the Biden administration felt was a strategy. And so therefore they're saying that they lack of strategy there. But what you're hearing from Dr Anthony Fauci, who was on President Trump's coronavirus task forces. Well, there was a strategy and and a plan in place with Trump's God Coronavirus task Force, But it was one that was not very well articulated. It was one that you didn't have to start from scratch. Because technically, we know the vaccine got out right. We know that there are vaccines not only in arms but also in places that are going to be dispersing it into arm. So there was a least a starting point. So the starting from scratch part doesn't really make sense but going on into distribution and what was gonna happen next. That's where there was a big catch. That's where Dr Fauci says things were kind of sketchy where you had not enough federal government interplay with not enough state government, he said. You don't want to have 100% federal government and you don't want to have 100% State government working the vaccine, he said. You want to have a combination. You wanna have a working there in a synergy between the two And that did not happen so that when in fact vaccines were done, they basically just handed it off to the states and said, Go do what you do, and that's what caused a bunch of trouble. According to Dr Fauci, States did not handle it. Well, All right, so government as Usual. We know death. We know there's a problem because we see story after story. Governor after governor, we can't get enough vaccine. We're running out of vaccine. This isn't working that isn't working. Pick somebody, whoever you want to, and you might actually hit the bull's eye, But we know the government as a whole. Has not handled this correctly. That would seemingly be correct. And there's so many different ways you can go about handling it incorrectly that I think that there's more of those than the correct fashion. Interestingly enough for some reason, the state of West Virginia managed to get all of their doses into the people they were supposed to, and began the process of the next level down with very little or no interruption whatsoever. So what they did, we don't know but again. That's the highlight here of the states, Taking their own vaccine and running off to their state with it is you have some states that are completely out. Iowa's almost out. New York City is virtually out at this point of vaccines, plenty other states that still have a bunch of virus on their shelves not being used. And that's where found, she says. You needed to have more leadership from the top on the federal level, guiding offering, help offering some form of financial help if you needed it, but certainly direction Down to the states and in the states interacting with the federal government all the time, and we didn't have that. All right Before we let you go Maurin important than who you think is going to win in F. C. A. S C game's this Sunday. Who do you want the women two games that are being played. That's a good I feel like I am so not a Tom Brady fan that I've got to go for Aaron Rodgers and agree with Packers. However,.
