Because i really feel like we really not giving ourselves a good shot by using that birth control priming Two times the goal. This time is to go back go tomorrow. See where i'm at and then try to figure out when my l. h. search is going to be an start. The estrogen patch after that point. And then i guess we would start stems about a week later. And this time i think i'm back on the four fifty of gone all in one. Fifty of men appear every day. But without the loop ron and then later on once we start seeing some developed us start with cetera tied and continued that until trigger. And then stuart retrieval on and right now the plainest do another fresh transfer at day. Five if we get an embryo and freeze any others without doing pta. That would be our second transfer at that point and hopefully it works. Hopefully we get to go to retrieval. The time i guess that's where we're at right now. Hey infertility warrior. During your infertility journey at beat infertility dot o. slash hope now back to the show. I'm curious why do not want to do. Pg ta on your embryos. You mean because. I had a miscarriage. Well so this is my thought process. You know the way that they charge for. Pg ta is if you test one embryo or you. Test like eight embryos. You're paying the same price. There's a couple of contributing factors in my decision. I should say our decision because even on the most informed one of us. My husband i are. We winter a lot of conversations about this because he was kind of more like we should test because he doesn't want to have to deal with the miscarriage again and he doesn't want to have to continue going through these kinds of things while he looks on. So there's contributing factors number one again. I'm about to turn thirty eight and i. it's not necessarily exceptionally likely that. I'm gonna have higher level of abnormal embryos just because of miami. Although i do understand that once your animation gets below like wait for it it can also indicate quality issues. Because you know you're kind of scraping the bottom of the barrel as they say for eggs and not good ones have mostly gone by then but hey we got a good one with our son so seems like it might be okay but because of the way that our insurance coverage works we get three covered transfers so our strategy at this point is to a retrieval dua five-day fresh of what we think is the best and freeze any others. If that transfer doesn't work. Or if i have another miscarriage at that point we will do another retrievable or two or three or however many. My doctor allows us to do up to a certain point on. I mean there is a limit here of time and effort. In many times i would like to do retrievals but i think i would. At least be willing to do to moore retrievals and we would then batch the embrose and then we would unfreeze them. And by the aw incentive for pg t before we do arthur transfer because once we do the third transfer get no more coverage until we do their transfer. We can do retrieval round after a around until we hit our max of one hundred thousand dollars of coverage before we want to do the third transfer we would want to make sure that that was a normal embryo and so that is our strategy but another contributing factor for me for the pg ta especially for the first round. Because we're only getting one. Maybe one egg is what we thought. Mike clinic does not do those. Aic transfers at all and i think that there's some potential with mosaic embryos. I understand why they don't do it. And i understand that it's new the guidance on embryos is not consistent in has been changing. But don't do it there. And in fact i have another friend. Who's going to the same clinic. And i think my impression from her results. She's done a few rounds. Gotten all normal is that they don't tell you. I don't think they even tell you if it's mosaic versus abnormal or. The degree of zeta schism. Go very vocal about getting Reports which she has gotten her reports now that it would be very vocal about getting reports and very very vocal about not destroying his embryos. Until you have one hundred percent looked at them. Because even when i spoke to the genetic counselor at my mfm she you know. And i talked about this question and whether we should do pt before we do another transfer we talked about about. This and i think there is a gray area. There related to Mike clinic does not recognize that. Currently i mean maybe they will in the future but right now they so our thought is. Let's do one more fresh and hope for the best. And if that doesn't work then we're going to do some embryo. Banking or batching and then unfreeze get them tested and then rephrase them and i actually did. Speak to my. I mean i've told my doctor. This is what i want to do. And he hasn't objected. So i guess he's okay with it but my talk to the financial counselor to at our clinic and she did explain that that is something that they do. They're and not happens particularly in situations like mine. Where only getting one embryo. United time and you know. We have limited time to be waiting for the results to come back. So that's our thought process in our strategy. Speaking of insurance we are going to have you back on with your broker. Try to understand investigate. Hey we may have lost our way sexually. We may have lost that part of us trying to conceive in dealing with the stresses of work in dealing with our family and our friends in all of the fires that we've had to put out in our life we've sacrificed pieces of our and perhaps even our intimacy or maybe. We've both had to shift in adapt to each other in order to make this work and so we hide parts of ourselves from one another and the work is. How do we move beyond our blocks. I you know. I say that you could do that. By being courageous enough to have the much-needed conversations and not just conversations. When you're having sex or after you have intimacy and say. I didn't like that really being able to check in. Make an appointment with each other to like. Hey i just wanna really want to have a conversation about this and not making it. Oh this is talk. So that people get on engine feel like this is like a scary conversation but really just effortlessness of just grabbing t together and being like hey. This is what i'm thinking about. How these are the connections. That i'm making for myself around my own sexuality for a lot of the women in those that identify as women out there. Perhaps you have lost your own sense of desire and closeness to your own body over the last couple of years and you don't know what happened. That is something that you could share with your partner to say yes. Perhaps at one point. In time i was this person and i don't know what happened for you. Take a look in start talking about the stressors that have gotten in the way and perhaps even some self neglect how many times when stress goes up for women. We put our continue to put ourselves last week. Do not always prioritize our own self. Care self care is not just about bubble bath and spot as although they are wonderful. Self care is also about boundaries. It's also about saying no it's also about knowing what to say yes it's also about challenging ourselves picking ourselves up when things don't go the way as planned it's about learning from our mistakes and praising ourselves we did something and push through boundaries that were proud of self care is the committed that we have to ourselves is the commitment that we have to our own the love that we have for our sows. It's the commitment that we have to making ourselves be the best that we can be so that we can show up. It'd be the best that we can be for the people in our lives. It's not just about you know getting to the next checkpoint. South care is really the intention of giving back to ourselves and not in a negative way not just telling ourselves the things that we don't wanna hear telling ourselves things that is right and good about us.

Probably ex ex and some of her cells have Nothing which is the turner cells so some of her cells are now usually women in the situation. Very few eggs. Sometimes they are burners eggs. You know or abnormal eggs which understandable for why she's had miscarriages potentially failed cycles people. Typically who have turner's as well as turner's mosaic art really high risk for a arctic dissection. Which is a problem with the order. Ah during pregnancy. In fact there's a two percent risk of death which can be really dangerous and the american society of reproductive medicine recommends that these patients really consider egg donor educational carrier. Which of course. Nobody wants to do. But it's definitely the safest option because she is mosaic. She certainly may be eligible to sort him before it. And be okay to carry a pregnancy depending on the health of her aaagh recruit. That's in her heart. Usually we do an mr. I am are a like a of her heart and her heart to see sort of how big the arctic route is in there some criteria. We use along working with cardiology and maternal fetal medicine. To see the issue is does she have the ability to have any healthy eggs on her own and could carry a pregnancy. So i think it's a pretty complicated and difficult scenario. Let's talk about my three elements for ivf success and how to evaluate your fertility clinic. This is just my opinion and others might disagree. But i believe the first element crucial to ibm. Success is the lab quality. If your clinic doesn't have a high quality lab. Your chance of success lowers dramatically. How new uncover the quality of a lab asked the right questions. I offer a detailed checklist in my paid coaching programs. But here are a few to get. You started how many procedures does and brielle perform per day. And how many years of training does the embryologist have. What is the labs fresh donor cycle. Success rate and what incubator type desktop versus big box does allow us and what is the oxygen percentage number two in terms of the elements crucial to ivf. Success is the right protocol. I cannot say this enough not everybody is welcome to the exact same protocol. We are all unique individuals. Our bodies are different are hormone levels are different are ages are different are diagnoses are different. So why would we all use the exact same protocol. Although everyone's first retrieval is a bit of a guess unless you stemmed with injectables during your prior cycle that guests shouldn't be the same across all patients even with a specific protocol the antagonists protocol for example dosage levels the number of stim- days that type of trigger etc will all vary between patients. Unfortunately this is one of those. You don't know what you don't know type situations like the lab quality checklist. I have a guy to via protocols. Availability may paid coaching programs. And i walk clients through. There's all the time. But here's my free advice. Ask why as many levels deep as it takes for you to understand your protocol. Here's an example. Why do you recommend this protocol. Why do you recommend these medications specifically. Why do you recommend these dosages. Why do you recommend this trigger shot. When in doubt a why question never fails the third and final crucial element for success is the best dr for you. Remember how you. And i are not the same that means you and i might not both do well with the same doctor. I routinely pair my coaching clients with doctors through a rather complicated process that i developed myself. But i'm about to talk about a simpler version of that. It won't help you find the right doctor but it will help you evaluate your current one. Don't misunderstand me though a doctor can be good the best even but not the best for you. Get out a sheet of paper and pen. Or because i'm going to have you write a few things down but i it's important to be brutally honest with yourself. Remember you are the customer and you deserve nothing short of excellent care so the first area that we are going to look at is overall clinic operations. I want you to write down the overhaul clinic operations. What do you like most. What do you like the least then. On a scale of one to ten with one being poor ten being excellent rate the overall clinic operations circle that number for reference later the second aspect is clinic atmosphere. Hope now back to the show. Let's dive into our main topic for the day. I actually didn't even know what to call it in my notes because her covering so many things but at the end of the day i think it really comes down to recurrent implantation failure and loss. Let's just call it that. We have a lot of different topics to go through. But i wanna start at the very beginning of the process and talk about reimplantation. There's a lot of confusion surrounding the structure of the endometrial before implantation. Can you tell us a little bit about the facts. There has so obviously helping the body be healthy is important for implentation things in perspective so the enemy trim is the lining of the uterus. We talk a lot about it because it's always being measured with to see a nice thick what we call try. Lamb inner lining where. We see almost like an oreo cookie. Where it's like three stripes but uterus has made of three layers the sarosa outside covering layer the meiomi trim which is the muscle layer of the uterus. That gives it that power to push baby out and then the end dmitri which is where when you have your period. That's that's what comes out. An emmy tree is composed of several things. Mainly glands that sort of lined. Dmitri i'm and they mature as the cycle goes on and they become branched tubular and the goal is to reach down to the muscle layer. And this happens because of changes in our hormones throughout the cycle if we think about a menstrual cycle hormones. The beginning of the cycle after a period have low levels of estrogen as the follicle grows the estrogen levels. Grow than you ovulate. And you start producing cra gesture own and progesterone changes the proteins. Being expressed in aligning and top layer with the functionalities is the layer. That sheds each cycle and the bottom layer is what we call the us and this is sort of the regenerative layer that grows every cycle and sort of helps grow that functionality. And we think about sort of the full phase or the time between your period and the time of violation where the enemy trim grows thicker and thicker the glands mature the vessels mature getting ready a potential implantation ovulation happens in between ovulation in the next period. This is what we call blue as we often hear about fees defects and stuff like that. That's where this comes in and there is what we call. As soon as progesterone is being expressed there are proteins that are turned on and those proteins are turn on in a very specific order and we have what we call the window of implantation. The window of receptivity. And it's a little unclear. Exactly how long this window is. Most people think it's about twelve to twenty-four hours where the uterus is sort of sticky and it usually happens sort of around six to nine days after ovulation depending on the person so that is a little bit about sort of the dimitrios in general and the structure around the lightning. You know not much of our all of our bodies changing all the time but obviously the uterine lining is one that regenerates itself holy every twenty eight days. It's pretty amazing. What about the question of thickness is there really an ideal or at least minimum thickness and data needs to be an order to accept an embryo. Not really bought what we like to see most of the data points to eight millimeters. There's not really a whole data and if it could be too thick but usually sort of eight to fifteen millimeters that range now. Some people have had whatever it is procedures. Pregnancy is other things that have damaged the base. Alice layer and it cannot grow thick or sometimes people just for whatever reason have never had any procedures can't grow a thick lining and aligning chambi thin and still receptive for an embryo. It just has a little bit of a lower chance of success. And when they did sort of some of the initial studies of embryo transfers in they measured the linings the sort of linings above some of the study said seven. Some of the study said e. We're going to be more successful. But it's not just about the thickness. It's about sort of the pattern that we see the ultrasound sort of that. Try lampeter pattern. And it's about the biochemical changes hormonal changes microscopic changes that are happening to the lining. Making sure those are happening in the correct way. Is it possible for the end. Dmitri to breakdown prior to an embryo transfer and therefore it's no longer in ideal environment.

"For the most part. You can predict who's gonna get. Oh s but there are a lot of cases that you can't predict it comes on suddenly out of the blue and it's your body's reaction to the human koranic a gannett atropine so the h. Cg that gets administer jarring cycles and occasionally you get a patient that you just can't predict and it's really scary. It's also could be very useful for patients who have some sort of tumor or some kind of cancer where administering exogenous genotropin to them in. Hormones is not a good idea right so if you have a tumor that's responding to hormones. You definitely don't wanna take more hormones into your body to get preserve your fertility before your cancer treatment and then the third thing is just for the safety and comfort of our patients. Doing stimulations stem cycle after stem cycle. All those injections all those medications in here body like what if we could reduce all of that and just do that on immature eggs in the lab and grow them to maturity in the lab so the physician could go in in theory and extract a lot of eggs that are immature and then we can do the final maturation steps in the lab. That's always have medical procedures. Start off is that. I developed for people who desperately need it but then it ends up benefiting everybody a lot of times in the end even people who could undergo a stimulation cycle but i mean look at the discomfort associated with that right so how much pain and suffering could we solve by doing. Ibm in the lab. It's significant so. I would say for on the research side. It's going really well. But how far off are we from putting that into actual clinical practice. Were pretty far off from that. It's gonna take years you know. You're talking about a training. A whole new generation of physicians and embryologist changes in drugs that are available and even just the approval process for those drugs are for the use of those drugs can take a long long time. "That says whether you like those tissues feebles views for training or for research and it's always so when patients make that choice to allow us to use their materials after. They're done building their family. It is so important for the training of mbala. Just we would not really be able to train junior mbala just without having those tissues to work on so that is always just such a blessing to us when when patients do decide to make that choice. And there's also some investigations that we still would like to undertake in the lab that we need human tissues and human embryos to work on so whenever you can feel good about discarding in choosing that option to allow those embryos fused for research. That is always so good for us in so good for the industry. One of the other options is something called compassionate transfer. That's where the lab will actually be embryos as if they're going to be transferred to a receptive uterus but instead of being a receptive or prepared uterus. The embryos are passed into unprepared uterus where they would not be able to implant in the uterine lining and so that can be an interesting option. One other thing that i've seen and i think it's a little bit of fraying on Ivf patients there are some services that say that they will do a ritual with your embryos straws and discard them in a special way. And you know for me. I think that that would be the worst option. Not only would you be paying for this service. You go into the lab at actually asked to have those embryos yourself into your own ritual most labs i know are more than willing to give the embryos straws to the to the perspective parents. An article was published in human reproduction in november. Twenty twenty outlining the top future. Infertility related research priorities. We are only going to be talking about one of them today. So this is part and one of a pretty long series but before we dive into the specifics of today's priority. Can you tell us about the study design methods and limitations. Yes so this is such a fantastic study in. It's a broad survey of a lot of different stakeholders that have specific concerns about infertility and there are many fundamental questions that still need to be investigated and so the question is for for countries in general is how do you allocate research funding to these questions. What are the questions. What are the most important things and really. How do you improve. Care for people with fertility problems so this was a huge study in it was survey it was also a systematic review of the clinical practices and systematic reviews that have been published over the past year. And like you said there were diverse stakeholders and they were in all across all different categories. So male infertility female infertility unexplained infertility assisted assisted reproduction ethics and access in organization of patient. Care in this particular study. There were three hundred eighty participants from forty different countries. There were four hundred and thirty two potential research questions that or submitted and ultimately what the survey does is sort of can help to guide our priorities where we put the research funding and highlight different areas that have been overlooked in the past including the emotional psychological impacts of infertility but really overall the goal is all about improving access to fertility treatments. And really just helping to organize. The you know. Future research agenda across multiple different countries. As i mentioned we are going to be talking about. Just the first research priority. Today i think this is going to generate a lot of interest though because a lot of us have this problem so this one is what are the causes of implantation failure. And i know you wanna talk about the placenta. I yeah and so. It's kinda need that our listener questions. Today were about the embryo in the placenta because the causes of implantation failure will relate to both you need both a healthy embryo and healthy uterine environment or a healthy womb. So just want a review like a couple of thoughts. That i had when i was preparing for this about the placenta in about human pregnancy and about the womb itself in in humans but i think one of the most riskiest moments in any human pregnancy is when the fertilized egg.
