Drug Addiction In America

"Woken to Mentally Yours Metro could ikaes weekly podcast about all things mental health. Today we're talking to Dave. Marlon, he was the CEO of crossroads of Southern Nevada, which was the largest addiction and Rehab Center in the area, the psychotherapist drug and alcohol counselor, and he basically knows everything about addiction and mental health issues in the US and beyond. Making me talking tim today about how the pandemic has been affected addiction issues to get help if you're struggling and how to recognize if you might have a problem with drugs or alcohol. Bruce Dave. Thanks so much for joining us on mental yours and welcome from across the pond. My first question was basically because obviously as I mentioned, we're in London. You're in the US, it such different situation in terms of addiction, mental health, and obviously the pandemic to get started. Could you give kind of a brief overview of the reality of addiction in the US? How serious the problem is that how widespread is a? The Robert Wood Johnson Foundation calls addiction the number one health problem in the US. If we look at the the number of prescription opiates that are consumed in the entire world The United States consumes more than eighty percent of them. We. have. You know we've always had an alcohol problem for a percentage of our population. we we developed enough and phetamine mean and a cocaine problem over the last. Twenty years, and in the last five, six years Oh actually even a little longer. An opiate problem has has become. Our most serious addiction challenge. Kind of the most common addiction issue that you see people coming into your center with. It it's interesting. I've run Iran the largest treatment center in Las. Vegas of. Gene. Years. And now as a private center and they're absolutely opiates or over my last three, four years, they're opiates was the number one drug of choice that clients had presented to solutions recovery without the opiate use disorder their primary. Primary substance. Now I work at an indigent facility in in downtown. Las Vegas where. More than half of our clients are homeless. And what's interesting is with this demographic, there's a much higher methamphetamine use. Would say my number one. Substance of for clients is nothin vitamin with opiates and alcohol running for a close second place. That's really interesting I. Think What was interesting that you said kind of opiates have been coming up over the lost six years because for me, it's felt like the coverage has been really recent like only in the last couple of years, we taught it to the opioid crisis this being a sudden kind of unexpected issue but you're saying it's been building for a long time. It has. Interestingly, fourteen years ago I was running the largest health insurance company in the state. And I remember in my last. My last year or two I remember looking at pharmacy reports and we were all scratching our heads saying what is this Oxycontin and why did it not show up two years ago and now I remember when across the ten million dollar mark at the Insurance Company for monthly use so it really begins began spiking. Thirteen fourteen years ago. It became. Newsworthy in fashionable. Six seven years ago, and now we're a were still squarely in an opiate epidemic.

"And I was like this is this is too much. I can't live by myself. A lot of it was because I I wasn't really productive citizen. Right. I was just this thug kid with with a lot of a lot of bad things in my past trying to move forward. But didn't really believe in myself. I didn't really have. I didn't really have what I didn't believe I had what it took to do anything. Good with. In my life. So I left California, and I came back to Las Vegas, and when I came back to Las Vegas. I had absolutely nothing. House was gone. The money was gone. I literally blew through that two hundred and fifty thousand dollars in about four and a half months. Well, I bought two cars and just lived, you know, I- drugs, this it was it was gone. So when it came back to us, I had absolutely nothing. I located my mom, and I said, well you own me. So I'm going to come live with you for a couple of months until I figure out what I wanna do. And she was living in a trailer park, and I moved in with her, and it didn't take me very long living inside that trailer park to a one day. I was sitting on the porch had somebody come up to me. And they were smoking methamphetamine and he sat down next to me. So c'mon. I was like what does that I had seen it before? But I you know, I had I had seen people doing other things with it. You know? Putting in shooting and things like that. I had actually never saw anybody smoking it. So he was smoking meth, and he goes, oh, yeah. This is math. Try it. I didn't have anything to lose. A why not? And that was the worst thing I could have ever done because method phetamine is one of those drugs that when you do it you're on top of the world. And you know, you hear the stories about how how it just ruins people lives, and it really does. So that's when I started. That's when I got introduced to meth was sitting on that porch that that one day, and from there things got things got even even worse. So I went from smoking meth, obviously to selling meth because I was watching this. And I said, you know, what if I can't get a job? I'm just going to sell this stuff. I already have pretty pretty good experience selling drugs. So I'll be fine. So I started selling methamphetamine, and as I was doing that. It's very interesting to me to think about right now because I don't think I've actually talked about in this way before, but as I was smoking and then selling I started to see like, well, I'm going to sell this stuff. But I'm not making a lot of money doing this. How can I make more money doing this? And for me that was cutting out the middleman, right? So for me that was finding out how I can find the people who manufacture this stuff. And as I started to find those people, I knew cut out the person, I was getting it from and just get it from those guys and. I found a couple people locally that would doing that stuff. And slowly. But surely what I started to do with steal stuff from them. So I was taking the chemicals at it. Needed to make this stuff. Hint one thing led to another. And before you knew it I was involved in manufacturing game. And when you get involved on a level like that, it just turns your whole world upside down because now you're not just involved with drugs, right? You're involved with the manufacturing involved with doing all kinds of of just things that go along with that that the stealing that comes along with that the the lifestyle that you live is just unbelievable because there's really no money, and you can't trust anyone around. You anybody? Nobody at all in that world. Right. They're all cutthroat all out to get for themselves. And they're also most likely for the most part using which doesn't even let them think clearly, right, right? Which unfortunately, it's where a lot of people lose their lives. A lot of people. I was a lot. It's a business in just imagine. Right. If I'm starting to company I can trust. No one in my company. We have a good product..

Now, you got true basically doing what Elvis did the black math. Not basically precisely precisely. Yeah. There's no difference between what Elvis did the black blue singers of the day, and what Drew's do into your bam taking it out. It's going to air it out on the Perry. Coma show this week is gonna hammer those checks head Sullivan and Sullivan. Sorry, Ted, Bob. Yep. Him in him in the kernel taking a right to the Bank. Bam. Hey, I've thought this call just make me think angry at the government. To. I I I was watching Gary played it on my show last week. But I wanted to share it with you. I thought I want to get your take on it shorter version of the clip from the HBO show pod save America where they were talking about sort of the difference between democrat Republican. And it was a nice little snippet Dylan you can find, but you can LOP off and part. John twenty six from Washington. Hey, guys, it's going on. You guys told me out so much of my life and really appreciate it. So I figured I come to you with another question. I guess I'll keep it short. I'm curious what you think my obligation is to do anything in a situation where someone in recovery is taking Adderall prescribed. I've heard you talk about a few times recently. It just it's going to be treacherous ever talked to responser about it. If you can talk to responser girlfriend, girlfriends doing. Yeah. Say that part. Yeah. I thought I heard nice she's been stable for several years on it. Yeah. I mean, there's some things that will. Yeah. Then you got to bring it up because it's a slow slide with that. But it's seen it so many times I can't even tell you with a real real addict, it they should not be taking all long period of time to shouldn't happen. I've seen too many catastrophes. So not to the doctor. But to the sponsor I would start with the sponge because the doctors just don't know for some reason, I don't know how it's the case that they don't understand this. But they don't and they consider it sort of unenlightened in if you don't treat ADD alcoholic, they all have ADD. And of course, they all feel when you put him on a stimulant. And by the way, was she ever stimulant addict. Okay. All right. But I've seen stimulant addicts put on this as well. That's crazy. In for methamphetamine to Dexter phetamine. Oh, that's gonna work out. Fine. Are you being sarcastic? Yes. Okay. Bob. Thanks, john. Good luck. So I was thinking as watching us. Clip and HBO is sort of turned into the arm of the democrat party like they they're just all ever all there's no more neutral or entertainment anymore. They're just running pods America. Which is it's not a good TV show, everyone everyone is skilled, but it takes, you know, Bill Maher's Bill Maher he's been up on stage. Hold the mic for twenty five thirty years. You need you need those Rams, but. At a certain point one of the players was asked to sort of explain offered up the difference between democrat and Republican, and he gave his version of it. They're they're democratic. So they'll give the version of it. And it sort of made you go, okay, actually that. But then I started thinking about it. And I'll I'll tell you play it for played for and that'll they are final trumping president because Democrats are very different. There's an asymmetric politics Democrats care about policy care about power. Okay. But if you're going to that's the Helme what I can if you're going to take half my money in taxes. And then tell me how how I can build on my land or what I can build on my land or what I can't build or if you're going to say to the Jehovah's Witness Florus, we're going to put you out of business if you don't make an arrangement for gay couple. Well, then that is policy. But guess what that means policy is power. It's exact opposite of the truth. It's again that that projection thing again, it's the same exact situation. It's Democrats are always interested in power..

"Healthy lifestyles and helpful void. Substance abuse within our within our communities, especially reaching out to the us. I think is an excellent an excellent focus and program. And it absolutely is. But it's only just the tip of that spirit. I mean, this issue Vanessa, I don't know what you've seen so far as far as Indiana, especially even here in Tennessee with the current opioid crisis going on, you know, from my time, even in the military. You saw a lot of rampant prescription drug abuse for more nations. Servicemembers I mean and not just that is there's certain situations to the over prescription of some of these medications. Contributing to and the fact that you cannot deny medical care. So you have servicemembers right now who are suffering from whether it's some form of trauma or another and pain and stuff like that that are being prescribed these, you know, opiate medications benzodiazepines, you know, a lot of these other kinds of, you know, you know, harmful medications, you know, in phetamine such as at a role and stuff like that. You see these veterans being prescribed that? But we'll most people don't realize an active duty servicemembers servicemember in general, you cannot deny medical treatment. Otherwise that'll further impact you going down the line when it comes to your VA benefits or veterans service claims. Right. Right. Well, like during my time in service, if you if you went to sick, call it was a general expectation that when you left sit call you went to the pharmacy and picked up a prescription. And when you got back to, you know. Your your unit you had to report. Okay. Well, now, I'm I have to take this. And I have to take this for however long, you know, that was just the common common. Norm and expected that you'd go to sit call because something's wrong, and then you're going to get a a pill. That's gonna somehow quote, unquote, magically fix it. Exactly. And and then not on top of that. Even if you didn't get a pill. Sometimes you only prescribe, you know, Tylenol in the glass of water. And in those cases, you know, you see a lot of other servicemembers turning to their friends who have good prescrip-. Well, I wouldn't say good prescriptions. But the heavier duty prescriptions for painkillers or you'll see them turning to alcohol abuse to numb that pain. Which is another further perpetuation of the issues. I think it also comes down to adequate medical care that you are servicemembers need to be provided as. Right. Because if they're only prescribed say Tylenol or not bupropion, and it is something that means further investigation in in a different medication. And then they're like you said, they're running say alcohol or. Trying to get someone else to give them some of their medication that they've got the heavier heavier on the prescription it. You're right it. It's a cycle that. We need to do more stuff that cycle. When you were inactive duty a generation or so apart. So you're experienced maybe a little bit more different than my what was your overall intake of the current crisis. When it was going on with the military or the culture around alcohol these. Well, I'll say it like the I had a friend that was and they're very early twenties that had already been diagnosed with psoriasis of the liver. They graduated from high school. They went today's IT started the military service, you know, right out the gate and by the time that they were in their early twenties had psoriasis deliver from drinking. That soldiers was absolutely turning to the alcohol to cope. Especially with those long hours that we have is well in the military, you know, you either rely on touring caffeine where you turn to alcohol just because you drink so much caffeine through the day. You're getting so little sleep that..

"Well, if you don't have them then what does that mean for treatment for you and others with the disorder? He's pretty pool so seventy eighty years, and people become patching up this narcolepsy condition by just feeding people. Narcolepsy stimulants, Iran, phetamine type drugs and these work to some extent in some people, but for some people don't work it so and for those who do work, it's not perfect. So they just slightly holding you together. What's interesting and again, touching that paddocks that I mentioned is the now within lost ten years said there's a new drug and it's a massive sedative. It's it's chemically related to j.j bay, which is known for being abused as date rape drug. It's that strong sedative this. This stuff is cooled, sodium oxalate, and it essentially just put to sleep. So you're treating narcolepsy. With a massive settting which seems very weird, but it's for those people who take and and it works. It can be life changing. It's dangerous thing after take it very carefully as to be very well controlled. And but by getting some deep sleep, lost your daytime functioning to the cast plex tightened up. So there's all makes it very clear that narcolepsy is not just about lack of sleep at it. There's a disorder in the brain. There's that your sleep isn't being regulated properly that it's, it's a chemical imbalance in your brain. Yeah, there's no other way of the candidates brain damage. "The time when p pd testing was coming in full throttle they were blood tests urine test and then detailing to my career they actually introduced the fact that they're not going there they're going to test for in phetamine and i've said this from the day they said that they were gonna start busting guys rent fettah means that that was going to be when you started to see production go down from guys and the the the longer careers are going to be a little bit shorter these guys aren't going to be able to play as long you saw a lot of guys in the eighties and nineties playing one hundred sixty two games schedules and the dirty little secret is is that guys took impediments to last that long throughout the course of a season to honor their contracts and once you took that away now you're starting to see guys play one hundred you're superstar players play one hundred and forty to one hundred fifty games but yes i agree with you that that has changed the game definitely ryan jeff beck and alexi i think you have a strong take about how analytics are are positively or negatively affecting basketball especially the nba well they are they're really hurting the nba and what happens is you have to understand the money there's so much money in winning and losing in tanking in prospects and the people who own the nba these owners their business owners and business owners when it gets down to the to the fibers of who you are what are you get down to you do what you know best and when i'm developing a real estate project or a company analytics are everything if you can't show me numbers i'm not going to put my money behind it and they're taking that same theorem into basketball in it's essentially ruining the sport because why you see tanking or you see a guy shot fifty percent from the three the last year or you look at the last year's champs and go well golden state wanted with three point shooting we'll guess what they got steph clay in katie the three best shooters arguably of all time that we've ever one the best of all time and then as a trio the best shooting ten we've ever ever seen in sports that's why they can play small ball that's why they can play.

"Yeah and i think that they could tell it was doing something but again this identity question of how much is me how much is the drug and how much is it really helping it seems like the study that's talked about in the film and which study was that this is dr martha farris study at u penn it was done in the two thousands and what's amazing is that one of the effects of adderall which is an phetamine mixed phetamine is that it makes you feel like you are doing better but the idea that these are smart pills or that their cognitive enhancers is a little bit misguided you could come away with this film with kind of mixed feelings on the one hand a lot of people don't like to think that they need something that they need a drug to ace attest or finish a work project on the other it isn't clear what long term harm these pills due to most people you know unless you have a a serious medical condition they're rarely deadly i'm like things like opioids what harm do you see here i think the harm that is maybe most urgent that applies to the most people is what the film focuses on which is a little bit more of the identity questions of what is lost if we are all in an adderall world what does it mean to feel like you need it to succeed from my reporting that seems like the most widespread applicable sort of risk and and question that needed to be discussed more in the open director alison claiming her new documentary take your pills is out tomorrow on netflix thank you.
