"Travelers and experts of all kinds to candidly and unapologetically discuss any and everything about travel. My guest this week is the wonderful doctor. Ariel Polonca Dr Paul Anka Welcome. Thank you for joining me She's an orthopedic surgeon. And why don't you tell people what exactly it is you do medically share so? I'm Dole Fellowship trained in both trauma surgery and specializing in the lower extremity. And I'm currently working in a private practice in San Diego wonderful and nothing said here should be taken as medical advice. If you have any kind of concerns consult your own physician that being said you are quite the world traveler. You've been to a number of very interesting places that I haven't been namely I guess let's jump into the Middle East right because that's a hot topic right now. So I believe into Iraq and Kuwait. I did so what brought you there. How was that? I know it's so fascinating well I try to do to humanitarian trips. A year and usually those are based in Central America Haiti But one of my colleagues at Stanford. That's where I used to practice. He had known that I do these trips frequently and they needed someone that specialized in ankle and foot reconstruction so he came up to me during one of our lecture days and said Hey Palanca will you come to Iraq with me. I said Yeah and he goes what. Yeah because I didn't expect you to say yes like what I'm saying. Yes so give some comes up to you and ask you to go to Iraq. You say yes was. This was two years ago. Yeah very recently. Very recently And what was the experience like it was? It was incredible. I had no idea what to expect. I pretty much brought everything I could think of I didn't know if I was GONNA have to cover my head. I didn't know what my role there was going to be But ultimately we flew into Kuwait It's a little safer to do. So but it was a big pain in the butt. Actually I would not do it that way again. Red Tape Ever I. It took us a full day to cross the border. Well Full Day and we would get stopped at one checkpoint. They would get us out of the CAR. They would search everything. Go through all of your papers all your documentations and then you would actually see the next checkpoint. You could see it. You could yell at it and we would go to that next checkpoint and they would do it all over again. There's probably seventeen times before we finally crossed the border but once we were in Iraq we were traveling with someone who is sort of like royalty there and I mean we just bypass customs and they took us and service mint tea in the back totally like Kinda undercover And then asked for medical advice which we gave them and it was kind of quid pro quo handshake thing and once where we are in Iraq. It was fine but that was a huge struggle to get over there. Do find being a physician that it makes things easier sort of because you know you're kind of given a priority like oh well. These are doctors so they're here to help like they kind of guy. I think definitely once we were in Iraq. That's how it happened. The understand that we're there were there on our own dime like we're not making any money. We're just there to help you get a lot of maybe some special treatment pretty much wide woman traveling to Iraq. They assume you're military or some sort of politician They were very surprised to hear that I was getting surgeon But they I was treated phenomenally there by everybody and I like I said it was a little. I really didn't know what to expect. But ultimately in Iraq they actually have more female physicians that they do male physicians really but they don't have any female surgeons think they have. They've had one in the twenty five years and she's a plastic surgeon. Gosh Oh It was I was able to go to medical school one day and actually talk to them and they were segregated. So the men's side the MED students in the Women Med students out on the other side And afterwards one of the girls found me on facebook and just said you know. I didn't know it was possible for women to be surgeons. And that's what I WANNA do and that was really neat. Wow that's awesome. Yeah what about Actual like staffing or the under staffed as well. Because I know that's the trips. I did with the medical people that was a big problem was like we don't have enough nurses. We don't have enough doctors. I didn't run into that. I think they actually have plenty of doctors And they're they're operating rooms are very well. Run I think that there's also just a little difference in you know they didn't check my credentials once which it's crazy all other countries that have gone to. It's it's a really rigorous you know they make you show that you had to get my undergraduate transcripts before they would even let me set foot in the hospital all your vaccinations. All of these things and I walked in. No-one had me introduce myself. Taking bad you can. I could be anyone I scrubbed and did a surgery. Wow it was but they looked at the person that I was coming in with. And he's very well known and regarded and he's like the chief of the hospital so I walked in with him. They just kind of shook my hand and that was that. But is that kind of refreshing as a private practice person. And you know the technicalities you have to go through America to just be able to just do your job yes someone. I can't tell you I mean when this last trip that I went. I went to Kenya and the amount of paperwork that they asked for me was insane. I've never had to get so many things together in order to have permission to operate there. And there's a part of you that saying you know I'm using my vacation time my own money to do this and to come here and to try to help and you're making it really difficult for me to do so so that was some point. I think that we have a medical license. And you don't have any malpractice suits against you in your functioning operating surgeon in a country. Why doesn't translate? Where did you think of Kenya? Was that your first son there. It was my first time in kidney. Yeah it'd been didn't is my first time in southern Africa or Central Africa and I really liked it. I was in CA- job which is a small town. It's about eight thousand elevation feet And it's an hour and a half out of Nairobi But it's a small town. Lots of ex pats there to hospitals one orthopedic. Surgery residency. Actually and then they specialize in pediatric Orthopedic Surgery Surgery in one of the hospitals and one of the hospitals was big public hospital More what I was expecting it. You know three or four patients per room pretty limited resources. They are understaffed The rounds were a little chaotic seeing. How many people people just getting shipped in? And that was a little. I don't exactly know what the word I am trying to use here. But ultimately it was very similar to other Third World countries that I had been to whereas they also had a private hospital That is funded by Americans cures international and that was a very small thirty two bed hospital but it was very clean very organized muscle prize money. Yes because of the private money more civilized because they simply have more people per patient as opposed to this hospital where you know the patients maybe got care that day. Maybe they were seeing that. Maybe not also it's all cash pay so you can have a patient in there. That has a Tibia fracture as a femur fracture have something going on and until they raise the money they stay there until their family members can get enough money for them to actually have the surgery. What kind of injuries and things were you seeing is? There's a lot of automobile accidents and things todd tons of motorcycle. Accidents is probably the number one trauma for sure in the peace hospitals. Mainly congenital and that was what was really nice about them forming that new pediatric hospital there. Because they didn't have really anywhere to go. There were a couple of places in Nairobi but a lot of people will bring their children who have birth defects or things like that to that hospital a lot of infection too and that was the big thing lot of infected fractures that weren't treated appropriately. Initially compound fractures that get infected in their big deals and hard to treat. And it's just really sad to know that probably could have been prevented if they were simply treated correctly cleanliness and lack of clean water and lack of money. You know if they don't have enough money to get the surgery done in the in the first place because it need immediate surgery with Antibiotics Anna. Clean out and stabilization. And if they don't have the money to do so like there's no insurance there you know it's not like there there is insurance. I shouldn't say that they do have an insurance policy. But that's usually not. Most people do not have insurance there and it's not like they're going to do the surgery like they would do here in the United States and then have you pay afterwards. That's not how it works. You pay or you. Don't get the surgery. Yeah that's a bit frightening that that's you know. So much of the world has sort of systems like that right so you really sort of despite all our problems with healthcare and whatever in the US you know you can't compare to a place like Eastern Africa. Let's go back to to Haiti. Is that somewhere? I've never been and you've been there many times. I think I've been there five or six times five or so. What initially drove you to Haiti. And what over those periods of time like have you seen any kind of change? Or how would you describe it there? Yeah I went I right. After the earthquake I went with a Stanford Group and even though We went as a medical team. We did very little medically because it was just such a disaster. It was really just triaging shelter. Food WATER CLEAN UP THAT. That earthquake was devastating. All hands asked absolutely So we didn't really you know I didn't do any surgeries I didn't do. We gave up some antibiotics and medicine. wrap some wounds things like that but it was mainly just helping Just relief efforts. I then went back five years later and to be honest it was really sad to be. How little had changed The roads had been mainly repaired and some of the buildings had been taken care of but really it seemed very similar to when I was there five years ago. A little less rebel eighty very very poor countries a very poor country with a lot of corruption. The police the political scene is just is almost terrifying You know the last time I was there actually. Five days later..

"And that's what popped up all around the valley. But Langa parks are these small mobile home parks, twelve trailers or less that are owned by Phil workers because in nineteen ninety two, there was a Bill passed by Senator put uncle, Richard Polanco, that incentivized field workers to own their own parks, somewhere legitimate. Well, run offices and others were somewhat substandard. And so the health department of riverside canny went in and did a lot of regulations on on those. Palumbo park folks. So that's what we're facing today as a water district is trying to get quality water to those Palanca parks that have popped up all around the valley. He didn't really quite answer the question. He definitely did not really quite answer the question. What he's saying is they tried. That's what he's saying. They were in charge of creating housing situations for them, and then it got to difficult to meet all the regulations. Yeah, which made me think so. It was too hard for you to make sure that they had clean running water that they had a safe place to be that there was basic amenities to his credit. What he is saying is is not that he said that there were little things that they were getting infractions for for which the cost was too big. So he's, I don't. I don't think he saying we didn't want to provide water and clean place to stay or anything like that. I think that he's saying that the state made it too difficult for them to do that. Does. To mobile home park. And so now there are no more labor camps, but there are these parks and so I went to go check out so you walk them about homework. If they some as this, I'm into the water. "Are you have declared unequivocally that kcp is not manna from heaven he's not manna from again now let me throw this this qualifier in this my way and we're off if you're just tuning it rob palanca when he announced that they were signing a kcp he described him as like manna from heaven all right if if the conspiracy theorists are right in this whole thing of bill simmons who prot who who laid this out a year and a half ago bill semyon throat if the lakers signed kcp to a one year bigmoney deal it's the first step to lebrun coming to the lakers if it turns out that this started the relationship started the daily dialogue between the lakers and clutch sports who represent abhran and lebrun ends up here by you know wipe whatever means then manna from heaven is in a strong enough description as to whether it's while is overly strong no manna from heaven is food that god miraculously gave to the israelites now after the food they had brought with them out of egypt had run out eight let me take a now israelites would have died had god not given the manna from heaven the laker fans are dying now vis would be bigger than manna from heaven what's wrong with you while i don't care about the burial lights this what if we louis drier about israel we do direct no the that not not four centuries ago i'm cared about the people that are here now israelites wouldn't have made it without the mode had no dover modes us would have delivered them the part of the rails he knowing your mirrored knowing your biblical history yeah i mean come on d indeed see what the guy did jisi brigade he parted a see it.

"Well let's play all that a case all us reporters come out and ucla i'm actually still here now uh and we go to shootaround where the thunder is getting ready to play the lakers tonight right so k hi paul george russell westbrook which by the way i want to point out we're still at the men then which is kind of strange because like in the is the only practice named russell westbrook were like tunisia meet the lord byron nation any athlete as ever made you would think they would let him uses own practice court herat's correct it's a little poke at their bruins they're jon now i know they're they're they're eight on station absolutely love haight through you know they they robbed three stores in china how smarter let him years stink in court okay they're worth it nursery around and we all kind of you know ask paul george um you know by what did you make of the camp bring stuff and and he said you know lived there was nothing to it and there no campaign and then he said you know they all the pacers not there was uh there was a bad really there was over the relationship between me and try and draw was inappropriate that he was recorded me or something but there was nothing healthy and be shot talk about fishing and we all kind of coalition at each other and said i never heard anything about pines often this tampering investigation wasn't but investigates them the like they've been conclusion was that rob palanca head had contact with paul george's agent after the lakers they've been warned about stuff so that's what the fine with four that's what they found evidence of um paul george sort of a revealed that there had been a brian shaw had been part of this investigation so you know that unless it's kind of a revelation okay what that means and um he said the pacers thought their relationship constituted tampering a recruiting so i went and made some calls and that there.

"Because they know these don't because they'd rather play with them when their 25 26 so that they know what they're doing at that point like no one's trying to like get in on the ground floor of lonzo ball like now while we to he was good and then i'll go play with yeah nobody wants the mentor had nobody went to deal with the good in the entity of the lonzo ball circuit break i i've read stories about how there's people mobbing in the new york for the game today because he's such a celebrity but he is not a particularly good basketball player right now at least on his level that he's playing his expectations i don't feel abroad like you know where i gotta go i gotta get change lonzo ball diapers even though paul george wants to do that you want to win the title if it you wanna have friends the play with you want to enjoy yourself wrote of we were crammed into a into a media room with the lakers breasts facility on mediaday anyway we're crammed right next to each other the back the room and there was a point where you look back at me with a what the fuck face because rob palanca said basically he won fans to walk away from watching a laker game with the same feeling people got hearing taylor swift for the guitar for the first time or kendrick kendrick lamar wrapping for the first time we made me would you watch this liquor steve do you feel like you're getting that experience.

"Has has begun to reassemble with magic johnson and rob palanca it's begun to reassemble with lonzo ball and rumors that lebron james wants to come here here's what i worry about now you see you say it's got to be something beyond joe i'm just saying if they're basing their whole complaint on this the case so the question is you you heard that is at tampering please come on come on here's the the part of it the were the part that worries me and magic johnson is new it is job doesn't necessarily annoyed said he went to tampering school but i'm not sure there is a tampering school exactly uh you know if there were some sort of paper trail if for example there were text messages between paul george and magic johnson or magic johnson and paul george's agent or emails or something in writing those of the things that that would constitute real and actual tampering neither magic johnson is much smarter than that he wouldn't put anything in writing well and let me add another element to this the person who broke the story peter vesey does not like magic johnson matter of fact of fence his way round here i did do you have the one where he talked specifically about how magic has smiled his way through the league and gets away with everything up does this sound like somebody that has an ax to grind or not so what i mean he hasn't done anything i mean i i dunno i it just seems to me i want the investigation go quickly because i'm pretty convinced there's nothing there right nah i think the lakers name will be cleared but it's now in the hands of the nba i know this i talked to people today the lakers are cooperating fully there get their handing the nba anything nasty.

"I i i'd be really surprised if anybody but full basing that you know danny ainge obviously eat he's a wildcard y you don't know what he does it and he could make a unilaterally but in the month prior to to the to the the lottery the theme before boston new they're going to happen number one pick what i'm talking to the scouts barrett executives the organisation they were just a few sipping their praise of full they think he is they transcended once every ten years pipe appoint guard so based on that information i think he's going to go number one number two you know at the the lakers better member who's running the lakers on the day to day to magic taught that the top rob palanca is a former agent iraq palanca is you know agents during this process they love to throw shelman the wall qatar and just you know and see what happens at rob pelinka and i think the lakers are are putting out some misdirection out there there they're letting people know though josh jackson barron fox uh the workout alonzo ball wasn't very good i'm not believe in any of it i think they'd like to see if it seemed like sacramento with that not that they might try to move up and make a blockbuster deal somebody else i still believe that what i heard in the early stage that this process that magic johnson loved while the ball but he wants alonzo ball that that still remain the same i think balking to be the number two pex yeah i do too but i think magic's looking at this is a businessman that he's falling in love with the business side of loans he'll what what he'll mean because people will show up to watch him people tune in to watch him i i agree with you on that jerry west to the clippers and we were wondering this prior to the start of the show when jerry was last on the show couple of weeks ago you.

"And i love lebrun but that's what it is now in his second stage of his nba career well what are you know of former nba players played with the warriors briefly colina as a buki reports that paul george has reached out to klay thompson about playing together once clay hits free agency this is what i believe makes way more sense than paul george and lebrun listen look bronze better than clay but you are getting year sixteen lebrun who is rough honor franchise yes you get in the news but if you had paul george and clay the band is growing together and i think is a six seven eight year play its remarkable and i got to be honest with you i like abhran james i've defended lebron james but even i have some lebrun fatigue i think the warriors super team is the new talking point in the nba and i like lebrun but i do think they're becomes a fatigue now that it's very clear that golden state is absolutely definitively younger longer more atletic and better than cleveland outside of the warriors land of the camps landing paul george i think we know what's going to happen next year so this is being reported i believed it was interesting i believe it makes more sense and i don't think magic and rob palanca want to hand over the franchise as they get this rebuilding job for the next decade to lebron james in his sixteen th year on clay in paul feels better to me longterm.

"I like abhran james i've defended lebron james but even i have some lebrun fatigue i think the warriors super team is the new talking point in the nba and i like lebrun but i do think they're becomes a fatigue now that it's very clear that golden state is absolutely definitively younger longer more atletic and better than cleveland outside of the warriors land of the camps landing paul george adding we know what's going to happen next year so this is being reported i believed it was interesting i believe it makes more sense and i don't think magic and rob palanca want a hand over the franchise as they get this rebuilding job for the next decade to lebron james in his sixteen th year adding clay in paul feels better to me longterm remember the lakers are desperate for titles delight the celtics the celtics aren't giving away these draft picks until it just right they're not like cleveland cleveland was desperate to finally win a title boston's not desperate the lakers are desperate tip but the captain john wall a player that i have often been critical love it is being reported that he's trying to recruit paul george to the wizards now i doubt it'll work but he deserves credit here this is something that you don't have to do but in 2017 you should do let me give you an example of something similar my wife and i she as kids my stepped kids.
