I mean if you guys ever want I have so many recommendations like we stayed in some amazing hotels It is really touristy and very like it literally felt like the Brunch. We did today like I thought I was on Habit Kinney. Okay in the downtown booed. Do you think it's because used to what I find is if you stay at the nicest places you're going to be treated like a tourist. I'm talking about the town was like wall to wall shopping now. And you're like Oh but I also thought that was the best part of Bali like Redid. Chengdu and like the beach towns. I don't know like people love them but for us we were like I wish restated into booed in the jungle and it just felt like any generic touristy beach town. I think he'd be an yeah. And you're like all right. Everyone's tourists there. Where at least in the kind of jungle areas It felt more authentic. Can you just sort of had to like pick and choose where you wanted to go? But it it's amazing. 'cause they have the infrastructure so like you can do the wild jungle getaway but still like Oh. I forgot my toothbrush. I can buy one. You know like yeah you can do. That's convenient sort of a mixed review. No I loved loved Bali like I want to go to different places in engineering. That's what I was GONNA say like. You think that. Yeah just did Allie. In a way that you're like I might not have done. I might not do it the same way again. Oh Yoga's everywhere no like more yoga right down the street in you know what I mean like. Oh I did more yoga last week that I did hear in Bali. Yeah it's just everyone's just thinking about it. "Travelers and experts of all kinds to candidly and unapologetically discuss any and everything about travel. My guest this week is the wonderful doctor. Ariel Polonca Dr Paul Anka Welcome. Thank you for joining me She's an orthopedic surgeon. And why don't you tell people what exactly it is you do medically share so? I'm Dole Fellowship trained in both trauma surgery and specializing in the lower extremity. And I'm currently working in a private practice in San Diego wonderful and nothing said here should be taken as medical advice. If you have any kind of concerns consult your own physician that being said you are quite the world traveler. You've been to a number of very interesting places that I haven't been namely I guess let's jump into the Middle East right because that's a hot topic right now. So I believe into Iraq and Kuwait. I did so what brought you there. How was that? I know it's so fascinating well I try to do to humanitarian trips. A year and usually those are based in Central America Haiti But one of my colleagues at Stanford. That's where I used to practice. He had known that I do these trips frequently and they needed someone that specialized in ankle and foot reconstruction so he came up to me during one of our lecture days and said Hey Palanca will you come to Iraq with me. I said Yeah and he goes what. Yeah because I didn't expect you to say yes like what I'm saying. Yes so give some comes up to you and ask you to go to Iraq. You say yes was. This was two years ago. Yeah very recently. Very recently And what was the experience like it was? It was incredible. I had no idea what to expect. I pretty much brought everything I could think of I didn't know if I was GONNA have to cover my head. I didn't know what my role there was going to be But ultimately we flew into Kuwait It's a little safer to do. So but it was a big pain in the butt. Actually I would not do it that way again. Red Tape Ever I. It took us a full day to cross the border. Well Full Day and we would get stopped at one checkpoint. They would get us out of the CAR. They would search everything. Go through all of your papers all your documentations and then you would actually see the next checkpoint. You could see it. You could yell at it and we would go to that next checkpoint and they would do it all over again. There's probably seventeen times before we finally crossed the border but once we were in Iraq we were traveling with someone who is sort of like royalty there and I mean we just bypass customs and they took us and service mint tea in the back totally like Kinda undercover And then asked for medical advice which we gave them and it was kind of quid pro quo handshake thing and once where we are in Iraq. It was fine but that was a huge struggle to get over there. Do find being a physician that it makes things easier sort of because you know you're kind of given a priority like oh well. These are doctors so they're here to help like they kind of guy. I think definitely once we were in Iraq. That's how it happened. The understand that we're there were there on our own dime like we're not making any money. We're just there to help you get a lot of maybe some special treatment pretty much wide woman traveling to Iraq. They assume you're military or some sort of politician They were very surprised to hear that I was getting surgeon But they I was treated phenomenally there by everybody and I like I said it was a little. I really didn't know what to expect. But ultimately in Iraq they actually have more female physicians that they do male physicians really but they don't have any female surgeons think they have. They've had one in the twenty five years and she's a plastic surgeon. Gosh Oh It was I was able to go to medical school one day and actually talk to them and they were segregated. So the men's side the MED students in the Women Med students out on the other side And afterwards one of the girls found me on facebook and just said you know. I didn't know it was possible for women to be surgeons. And that's what I WANNA do and that was really neat. Wow that's awesome. Yeah what about Actual like staffing or the under staffed as well. Because I know that's the trips. I did with the medical people that was a big problem was like we don't have enough nurses. We don't have enough doctors. I didn't run into that. I think they actually have plenty of doctors And they're they're operating rooms are very well. Run I think that there's also just a little difference in you know they didn't check my credentials once which it's crazy all other countries that have gone to. It's it's a really rigorous you know they make you show that you had to get my undergraduate transcripts before they would even let me set foot in the hospital all your vaccinations. All of these things and I walked in. No-one had me introduce myself. Taking bad you can. I could be anyone I scrubbed and did a surgery. Wow it was but they looked at the person that I was coming in with. And he's very well known and regarded and he's like the chief of the hospital so I walked in with him. They just kind of shook my hand and that was that. But is that kind of refreshing as a private practice person. And you know the technicalities you have to go through America to just be able to just do your job yes someone. I can't tell you I mean when this last trip that I went. I went to Kenya and the amount of paperwork that they asked for me was insane. I've never had to get so many things together in order to have permission to operate there. And there's a part of you that saying you know I'm using my vacation time my own money to do this and to come here and to try to help and you're making it really difficult for me to do so so that was some point. I think that we have a medical license. And you don't have any malpractice suits against you in your functioning operating surgeon in a country. Why doesn't translate? Where did you think of Kenya? Was that your first son there. It was my first time in kidney. Yeah it'd been didn't is my first time in southern Africa or Central Africa and I really liked it. I was in CA- job which is a small town. It's about eight thousand elevation feet And it's an hour and a half out of Nairobi But it's a small town. Lots of ex pats there to hospitals one orthopedic. Surgery residency. Actually and then they specialize in pediatric Orthopedic Surgery Surgery in one of the hospitals and one of the hospitals was big public hospital More what I was expecting it. You know three or four patients per room pretty limited resources. They are understaffed The rounds were a little chaotic seeing. How many people people just getting shipped in? And that was a little. I don't exactly know what the word I am trying to use here. But ultimately it was very similar to other Third World countries that I had been to whereas they also had a private hospital That is funded by Americans cures international and that was a very small thirty two bed hospital but it was very clean very organized muscle prize money. Yes because of the private money more civilized because they simply have more people per patient as opposed to this hospital where you know the patients maybe got care that day. Maybe they were seeing that. Maybe not also it's all cash pay so you can have a patient in there. That has a Tibia fracture as a femur fracture have something going on and until they raise the money they stay there until their family members can get enough money for them to actually have the surgery. What kind of injuries and things were you seeing is? There's a lot of automobile accidents and things todd tons of motorcycle. Accidents is probably the number one trauma for sure in the peace hospitals. Mainly congenital and that was what was really nice about them forming that new pediatric hospital there. Because they didn't have really anywhere to go. There were a couple of places in Nairobi but a lot of people will bring their children who have birth defects or things like that to that hospital a lot of infection too and that was the big thing lot of infected fractures that weren't treated appropriately. Initially compound fractures that get infected in their big deals and hard to treat. And it's just really sad to know that probably could have been prevented if they were simply treated correctly cleanliness and lack of clean water and lack of money. You know if they don't have enough money to get the surgery done in the in the first place because it need immediate surgery with Antibiotics Anna. Clean out and stabilization. And if they don't have the money to do so like there's no insurance there you know it's not like there there is insurance. I shouldn't say that they do have an insurance policy. But that's usually not. Most people do not have insurance there and it's not like they're going to do the surgery like they would do here in the United States and then have you pay afterwards. That's not how it works. You pay or you. Don't get the surgery. Yeah that's a bit frightening that that's you know. So much of the world has sort of systems like that right so you really sort of despite all our problems with healthcare and whatever in the US you know you can't compare to a place like Eastern Africa. Let's go back to to Haiti. Is that somewhere? I've never been and you've been there many times. I think I've been there five or six times five or so. What initially drove you to Haiti. And what over those periods of time like have you seen any kind of change? Or how would you describe it there? Yeah I went I right. After the earthquake I went with a Stanford Group and even though We went as a medical team. We did very little medically because it was just such a disaster. It was really just triaging shelter. Food WATER CLEAN UP THAT. That earthquake was devastating. All hands asked absolutely So we didn't really you know I didn't do any surgeries I didn't do. We gave up some antibiotics and medicine. wrap some wounds things like that but it was mainly just helping Just relief efforts. I then went back five years later and to be honest it was really sad to be. How little had changed The roads had been mainly repaired and some of the buildings had been taken care of but really it seemed very similar to when I was there five years ago. A little less rebel eighty very very poor countries a very poor country with a lot of corruption. The police the political scene is just is almost terrifying You know the last time I was there actually. Five days later..
