"There's lots of parallels Are during the lockout season on everything kind of nba's all in turmoil in it's obviously the right now the whole world into more of course with with the covid nineteen pandemic. So what was it like kind of making such a life. Changing decision like that into into going orlando. I'm during a time like that and obviously finding your way in the nba. It it made it a little more difficult to actually break into the nba. Because it was a lockout going on. I wanted to. I guess my first choice was to go to new york and on the league office there but at that point you know it was. It was a little more difficult because it was a lockout but there was an opening in london. And you can see like hey it's basketball adjacent because it was more of a business role as opposed you know on a hard core basketball role but then again you know. That was the opportunity that was presented to me And i absolutely took it and it wasn't exactly what i thought it would be doing kind of like working in business and basketball but it was certainly closer to what i wanted to do but then again. I didn't necessarily know exactly what i wanted to do. But i was like. Hey it's basketball. At least so i'm going to give it a shot and i think it's it's it's smart of you to draw there right now because it is a little more difficult than get some emails every once in a while from from from young bride super super intelligent kids trying to basically asking how the how to break into the industry. And it's it's really hard because there's a pandemic going on um in you know and gives out to stay at home and you know. Sometimes they can't even go to classes right. So i mean it's it's even more difficult to break into sports at it's already difficult but during a pandemic even more difficult i guess my biggest advice and i guess i i followed my own advice when i was twenty..

"Are they. Do they have microchips in the real sense of the word that work in the same fashion. Of course they do technology for of course they do. They've got smart dust that this undetectable and i'll show you that in a moment so the idea that we're going to pretend like this you know the idea is they're playing with the word microchip just like they play with the word like when we talked about the dna altering discussion of the vaccine like. Are we really going to say you know. By definition. This means that you can't if you say alters it's wrong i don't care what you want to call it before what this thing does is crazy. An alarming it's a genetically modified thing alters your body whether it goes away or not that scares me so just because you think it changes certain way which changes the definition of altering your dna. This is how they play these games. Then it can say all fake news. Even though everything we said was real except for the specific words they think we use to describe it in this case same thing. So here's here's newsweek factcheck. Did the pentagon develop a cova detecting microchip is as as the covid. Nineteen pandemic continues to rock the globe. Scientists are having to contend with conspiracy theories and misinformation related the outbreak in some cases. This results in vaccine hesitancy right. That's all they care about an unwillingness to receive a code vaccine because of distrust and fear right and there's nothing to do with things like to st ski or the constant times. They've tested or light or manipulated. No it's because we're telling them truthful things that they don't like some conspiracy. There's they start. What does what they what. they're saying. He has nothing to do. With the argument. Some conspiracy theorists allege that governments or maybe bill gates are using vaccines way to people microchips. This is only to undermine the argument of what is absolutely real. I mean we're talking about radio by the way that's the tweet. Don't look at the text. that's what they say it's fake. We.

I think all of us by now after a year of witnessing our daily lives are affected by all sorts of social distancing measures and other another measures are maybe thinking or experiencing public space in different ways. And i'm wondering if you can tell us a little bit more about how the pandemic and these the pressure on public spaces also changed the everyday tactics or practices of the refugee youth and as the asylum-seekers perhaps also that are included in your research Yeah so the pandemic is affected. Everybody tremendously and here again. I think there is a lot of similarity so everything around lack of social contact isolation suffering from the fact that there is no activities. I also an asylum seeker centres. There are no activities anymore so people are just in their room doing nothing more specifically. I've noticed a drop in language skills because people speak last dutch nowadays so they don't practice don't have to smoke gophers ation and this has long effect on on the long run as well because not being able to communicate and then you have to just catch up later. The waiting thinkest really horrible people in reception centres beduso people waiting for family unification procedures for example. Everything is being postponed. There are no hearings now returning through this distinction between formal and informal spaces of foreign counter. I'm wondering if you also and this really very much comes from our cities of refuge interest in local authorities. But i'm wondering if you also see. Instances in which local state municipalities are trying to think about the shrinking of public space whether that's formal spaces for encounters that were available to young refugees or informal ones and the long term impact that it will have on their Integration or sets of belonging. If you like amateur are you. Are you asking for a specific initiative if maker for instance I'm just wondering if i understand very much that you're trying to think critically beyond the state. But i think for our listeners who are also interested in the interactions. I think it would be interesting to hear if there are. Also instances in richmond is a patties are really trying to work with these challenges and francis trying to keep certain spaces spaces of unconscious open If you've encountered any such yeah since you know there are many examples. I would say and something that we have come across especially with. Young refugees is the the success of co housing projects so young refugees who have been living with students or other residents in the city they. Yeah they've been so positive about the fact that they've been meeting very easily with dutch people that they really easier access to information that they would be taken to parties that they would meet other people so i think that's a good example for best practice. View like housing initiatives where in a way encounters are a bit forest but at the same time in a housing project where it's also quite normal when you choose for a communal living arrangement that you have interaction with your neighbor so yeah. I think it's especially works..

"This person rates in from denver. He says i'm fifty nine. She sixty one been married for twenty. Six years were empty nesters. Living in cova times frequency is about two to three times a month. But it's gotten very pretty predictable. Can you help me with sexual initiation. I want to keep things fresh mo. We can try brandon. How do you initiate sex. Oh jeez sometimes. I'm very lazy. And i do the what is it. What do you call it the poco. I know you're there. And the other times i find that i do have to kind of push myself. I know that i enjoy sex. Knew that that. I know that you enjoy sex so i know that if i push myself to initiate that it's going to likely be a win win for all part but how do you initiate so. Yes the puck from behind like especially in the shower lately. Come into mcdonald's tang my towel on that. Just hold my towel. How old do i initiate. You feel like you're putting me on the spot. I mean the pope from behind his goto. What else do i do. You tell me so. I i tend to initiate sex by Touching caressing kinda stroking your body. I think that that's what works. And it's funny. Because as i'm saying what i do. I know that that's what works for me. Like i know that that sensuous touch is what relaxes me and kind of gets me into the head space physically and mentally into sex. Why do that when we're trying. I'm trying to initiate sex with you. Yeah you're very you're definitely physical. I think most people sink physically when they think about sexual initiation you. I guess i want to begin by talking about planting sex seeds. So this is from our latest book the book that i wrote with marla the ultimate guide to seduction and four play. And i make sure. I plug it so that you know that this is marlins work too because succeeds are actually her creation in. She describes them as seductive clues about what is to come in a future sexual experience so they can be planted in the morning. If you wanna get busy at night or you can plant them throughout the week before you meet and you know she says to really consider what type of sex. Your lover likes. So do they like it romantic. Do they like to be filmed. Are they publicly. Experimental and really think about an aerobic activity that you can plan on your own and plant a seed to set the tone so if your partner likes to be spanked. Maybe you're leaving a spanking instrument in their car like a paddle spoon from the kitchen. Or maybe you leave a photo of it in their purse or their briefcase or maybe you leave them a note in their lunch bag and text them a photo of it in your hands. You're the ideas. You're watering the succeed. Kind of leaving additional clues. And i know people aren't really going to work right now but still you have different areas of your home you can use and it. It's interesting 'cause marla reminds us that this whole process of planting succeeds can kind of offer a distraction to reduce stress overall. And help you to kind of weave eroticism throat your day to day interaction so i think the most important thing that i would want to emphasize that initiating sex can't really happen on the spot right. It is really about the way you interact at breakfast for the evening or on monday for

"Infection control concerns. I had any patients for whom prone positioning is not recommended. Yes so there's there's a few Specific patient populations where This may be should be avoided and again a lot of this is being drawn from intensive care experience so patients who have been traumatic injuries to their anterior chest or face should avoid the prone position If there's been a recent anterior thoracic surgery or abdominal surgery should also be avoided or if there are concerns for able spines stable pelvic fracture than the sort of movement manipulation should should be avoided as well practically What we're also seeing. Is that a lot of groups who are conducting. These studies are also choosing not to involve patients who are on continuous cardiac monitoring for example as the the leads are less accessible when somebody's in the prone position And then the other important consideration is Patients who have a decreased level of consciousness. Or maybe aren't able to shift independently and sort of shift around in bed so may get stuck in uncomfortable or unsafe position. without assistance. Finally i think those patients who are spontaneously breathing but have signs of severe respiratory distress and may require imminent emergence into tracheal integration should not be placed in the prone position just for concern of the worsening their other work of breathing and also in a previous life. I was in neath aside and we think if somebody looks like they might imminently need education. You're gonna want them on the back. Exactly and i remember in your article. You also mentioned pregnant patients. So i think that's not necessarily a contra indication because you point to a couple of case reports that say that that prone positioning has been used successfully and pregnant patients folks with covid nineteen and not so. That would be something that you would think about being more logistically difficult but not necessarily a contra indication yeah that's exactly right the it's It's not just in this area. But in many areas of medical research pregnant patients have not been routinely included in trials or or studies in. So there's a there's even less Data available for this patient population however again in intensive care experience prone positioning has been used successfully In in patients who are pregnant including a late in pregnancy and there are case reports coming out now a related to covid nineteen where prone positioning has been successfully used in both intimated in non intimated patients. Who who were pregnant as well so there are. There are some other ways to logistically. Make it a a easier. Might need some more staff on hand for example or something modified mattresses or padding to make it more comfortable for the patients but But from a medical perspective. It's something to consider what evidence exists about the effectiveness of prompt positioning in patients. Who are hypoc sec. In general in general as as we talked about before so for patients who are in the icu. and our intimated with moderate to severe air diaz Prone positioning has been consistently shown through a high quality randomized control trials and also met analyses that to be beneficial to reduce mortality if it's completed for at least twelve hours a day and this has now been widely adopted as standard of care practice for For these patients internationally shifting to you know the patients that were focusing on today. Those were not intimated. Were not critically ill before. The covid nineteen pandemic. This was not a question that many people were looking at There i should say that. All of the data both before and now during the covid. Nineteen pandemic is optional. nature We don't have any randomized data yet. And it really is comprised of case studies case series and mostly coked observational cohort studies. The observational studies at tend to look at a few different parameters. One is the question of whether or not prone positioning improves. Oxygenation for for these patients and Appears to do so again. With the caveat that this is not randomized data there is still conflicting evidence coming out. Not all studies. Do show an improvement in either the oxygen saturation or the pdf a ratio. If it's being calculated and not all studies are showing a sustained benefit in oxygenation either While people are then reverted to the supine position or after Longer durations of trump positioning another area. That's being looked at is whether or not The prone position will actually decrease the need for tracheal intimation. And we're really lacking a lot of data in this area. I think some of that is due to the fact that the decision to intubate a patient is not solely based on their current oxygenation status. But you know other. Medical context is comes into play as well But we don't have a good sense yet about whether or not Prompt positioning will will help decrease intimation rates for these patients. So you've been talking about observational evidence that's available and saying it's not randomized trial evidence at this stage for covid nineteen however there are a number of randomized trials underway. As i understand from your article. Are.
