The dangerous use of off label prescribing in times of a pandemic

"FDA requires prescription drugs to go through rigorous testing for both safety and efficacy as a result only a small number of drugs that are developed are ever approved for use and then only for narrow indications so take for example. A drug called Gabba Penton. It was approved in the early nineteen nineties as an anti seizure medicine but it didn't work very well. So experts began to promote it for use to treat all sorts of other conditions including pain bipolar disorder attention deficit disorder restless leg syndrome and Alcohol Withdrawal Syndrome. It has not been approved for any of these reasons but doctors prescribe the drug heavily and Gabba. Penton is not the only example even if there is no proof and the FDA has not approved drug for a specific condition doctors can still prescribe it because here in the United States doctors are allowed to prescribe drugs off label in other words for reasons completely different than what they were intended for or for what. They were tested to treat. It stands to reason that Dr should only prescribe pills to treat conditions for which there is good evidence of effectiveness but that is not the case in the United States. These off label prescriptions account for one out of every five prescriptions. That means that twenty percent of all drug prescriptions are for conditions for which the drug was not approved so when the president tells us that despite warnings from the FDA. He's taking the Anti Malaria. Drug Hydroxy cleric win as prevention for COVID nineteen. It raises all kinds of red flags. Currently there is no. Fda approved drug to treat or prevent covid moreover several studies. Most recently in Jama have shown that hydroxy chloroquine has been shown to be in effective intriguing covid in effective. So how wide why? The president's doctor prescribed this drug went. Experts advise against taking the drug will because it was being prescribed off label for a non approved condition and the same is true for thousands of doctors who prescribed this drug for themselves for their families or for patients on the advice of a real estate developer. Who HAPPENS TO BE THE PRESIDENT? I suspect the public thought that the president knew something that the rest of us didn't inside information. Well IT TURNS OUT. He didn't the rest of US. Were reading the medical research and new. The drug was a very little use to those with. Covert the use of off label. Drugs drives up the cost of healthcare and exposes patients to unnecessary risks. Uncertain outcomes. Hydroxy chloroquine can cause abnormal heart rhythms and dangerous rapid heart rates. So doctors who've written these prescriptions without any scientific basis. Now have a moral duty to reach out and inform patients about the risk of medical treatment with hydroxy chloroquine and the lack of any benefit. And we need to rethink the policy of allowing doctors to write prescriptions for drugs that lack any proof of benefit at a very minimum. We should require doctors to inform their patients that they are being prescribed drug that has not been proven effective for their

Quinine: Medicine or Mixer?

"There anything about quinine that makes it worth drinking in the form of tonic water, I know some older people who are convinced that it helps with neurological issues does it have side effects? Or is it fairly benign, even if it's ingested daily quinine is a bitter chemical that's extracted from the bark of the sink Kona tree. It's used in tonic water to impart that characteristic bitter flavor. In fact, I've made my own tonic water using Kona bark that I bought online. My recipe involved boiling this in Kona bark with some other spices like coriander and juniper berries, and then combining that extraction with citrus. Juice grated. Citrus peels and sugar and straining the whole concoction to produce syrup and then to use it. I would add a tablespoon or two of this syrup to a glass and fill it with plain soda water. Jin totally optional now making your own tonic water is fun, especially if you like those sort of do it yourself projects that require a bunch of SO, Tarik ingredients or equip. -ment take a lot of time. Make a big mess, and ultimately produce something that you could have bought at the store for a buck. Of course, my tonic water tasted nothing like store-bought tonic water, some of my friends thought that was a plus others. Frankly, preferred the store bought version the other interesting thing about homemade tonic water is that it's not clear like store bought tonic water, but we're the color of weak tea, but back to his question. What are the medicinal uses of quinine in addition to being used as a flavoring agent quinine from Cinco? Bark can also be used as an effective treatment for malaria Kuala win is a prescription anti malarial medication that contains quinine sulfate from Kona bark, however is more effective drugs. Have been developed quinine is no longer the first line treatment for malaria. It's basically used only in cases where no other drugs are available or in areas that have developed strains of malaria that are resistant to the first line drugs. And it's. Also, sometimes used to treat malaria in pregnant women sin. Kona bark contains another closely related compound called Quinn Adine. It has some of the same medicinal benefits is quinine such as fighting malaria. However, it tends to have more serious side effects. So it's not widely used there is some ongoing research involving Quinn Adine to treat certain rare neurological conditions, but the toxins remains a real issue. Quinine also has gained a reputation as a treatment for restless leg syndrome and nighttime leg cramps and many physicians have prescribed Kuala Quin, which is really for malaria to patients who suffer from leg cramps. However, the FDA has issued a warning against this off label use of quinine containing medications.

There can be hidden meaning in a medical term

"A name maybe more than it seems Mezin teric added. Nitis sounds awful. It's exactly the type of disease name that might explain why they're going to miss work or not be able to do your chores around the yard or even wash the dishes tonight. This is Dr Michael Wilks with a second opinion. In fact, the term Mezin teric adn Itis is a medical term for swollen abdominal glands, but it really means a small tummy ache. I am constantly reminding my medical students to avoid using technical terms and speak English with their patients. But there are times when a we bid of technical language can be validating, for example, this week Nicole saw me because she'd been sick for forty eight hours with nausea and vomiting. She felt horrible touring those two days she called into work sick whale. Her. Office Astra for doctor's note, a sort of archaic vestige of the past so she needed come in seamy, which meant she needed to get dressed driving her car pay for parking pay for co payment, wait in the waiting room. So that she could now spread the disease to others. Then after seeing me she would leave with nothing other than a doctor's note. And the reassurance that she'd get better soon and my advice to stay well hydrated. Will that isn't exactly all I told her I told her she had Mezin Terek at night and she loved it. She wrote the name down. So she could tell her book group who work colleagues and people at the gym she expected, it would help people who might doubt that. She was sick understand that it was a real thing. The words would give her symptoms legitimacy, and maybe even engender sympathy from others, however, the labeling of minor symptoms as a disease. He's like Hella is for bad breath or restless legs syndrome for periodic twitching or female sexual dysfunction for person who doesn't wanna have sex has some serious downsides and even more worrisome sign that should raise all kinds of red flags is when it disease was developed by drug company to help market a particular product like female sexual dysfunction to market Viagra to women or listerine to market to people with bad breath. And then there are new disease names that serve to medical is what is actually normal for example, we have prediabetes pre hypertension Prediman. Shia. In fact, I hate to tell you this. But we've all got a case of pre death. A name can have a powerful effect on how we see ourselves and manager bodies. Sometimes that means taking our health more seriously. But too often the intent. Ended goal of names is to market and sell pharmaceutical drugs, Nicole left, my office feeling validated. And understood and there were absolutely no prescriptions for any drugs, given just a name of a disease written on a small piece of white paper. This is Dr Michael Wilks with a second opinion.

"You plan ahead. You know, what's coming in? Alternate. You make the right choice. It's hell what I see in my line of work. It's how I see patients be able to reverse disease get off of multiple medications overcome these challenges, chronic health disease. It's by falling a routine, just like this, Cheryl Cheryl was on nine different medications. They diagnosed her with re matured arthritis. They diagnosed her with irritable bowel syndrome. She had sleeplessness shed restless legs syndrome. She had sciatica and Cheryl. Not only did she come into the office. And she was under my care and doctors who are in my office. We all take care of the patients, not only was she ended that care, but she was so disciplined came to all the workshops, every nutritional piece of advice that I would give out at the workshops, which I don't make money on the workshops, we if we have an outside of outside of van I charge just to cover the cost of renting the space. But you know, we don't make money on these workshops, I do because the information's valuable, and I believe people should not only think the way that we think, but also live the way that we live in we see people overcome disease in gained victory over the health by just implementing these things are learning at the workshop. Well, she shows up shell does it every workshop that I've given over the last four years she brings friends just about every time. Her whole family comes to not only the workshops but under care in my office. And sure enough you look at her now, no more. Diagnosis of Reema, Tori, arthritis her joints or no hunger. Achey any more. She's no more irritable bowel syndrome. Her. Sciatic is completely gone. She no longer has restless leg syndrome off of all nine different medications. How does that happen? Well, she showed up to the workshops and implemented. What we talk about in. That's exactly why we hold workshops and seminars for our patients, and we open it up to the community for that very reason because people deserve to live a better life, and they deserve answers. When it comes to the questions they have for their health. That's what we're going to be talking about at resolution reset is the biggest event of the year in the natural health community. It will be February second that is a Saturday from nine to noon. This is our health makeover. So we call it that for a reason because you show up you learn the information, you take great notes, you go home changed in ultimately, a different person. And that is the idea in this is called resolution reset for a reason because it's February second around the time where a lot of times we have great intentions January one but come around to February those..

Here's what I think. I think that I found this with people from allergies in basically autoimmune diseases innocent. And they found was the people that had optimal levels of vitamin d their immune system was much much more balanced importantly because some eighty percent ultra because you're stepping pursuant. And if you don't have enough, you're black instruments systems out of whack you end up with autoimmune diseases you end up with allergies. I mean, I saw patient after patient that had COPD or allergies it much much better because the vitamin d you know, boosted and balanced there. Well, I, I have to tell you we don't have a very good line Virginia, but it sounds pretty extrordinary a ninety nine year old yoga instructor. Carol. Krucoff I I've seen this person on on the internet. I think she's her name is Tara port Portuondo. She seems to be a very lovely woman and yes, you can have yoga instructors of of any of any age. Wonderful. Let's go to line one Terry, Christina in Saint Petersburg, Florida. Hi, Christina. You're on the people's pharmacy. What's up. Good morning. What's what's your story, Christina? I just wanted to express my preseason for the show this morning. I am a yoga instructor down here in Saint Petersburg, and I'm thirty four now. And I've been practicing probably since I was about fourteen and I just you always wanna share things you love. And yoga has always been one of those things that create brings awareness to us. I was awareness to our body, especially, but our mind and heart even more. And I think the more people develop a yoga practice or introduced to yoga. They just start to see a change in them that. You're becoming more aware of maybe better ways to live your life, making better decisions. And like you said, carols through the principles, the Yamazaki and the neon the almost finding the good versus finding all the bad, you just become a better person. Christina much for the call. It almost sounds too good to be true karaoke Krucoff. Well, it. It's very interesting as yoga has become so popular in our culture. It, it's it. I don't want to overstate the benefits. It's a, it's, it's, it's something people need to find for themselves. So it's. It is. It is a transformative practice and it changes to a my my practice now at sixty four is very different than it was in my twenties. My twenties was a much more athletic practice. If I didn't sweat during my practice it, it just didn't feel right. I used to stand on my head. I used to do lots of twisty, pretzel poses, and at sixty four, having dealt with various health issues, I no longer stand on my head. I no longer do end range rotation because I do have low bone density. So my physical posture practice has changed, but I still have a very vigorous practice. I still feel like I'm keeping myself in in very good physical shape from the practice, but also in my meditation practice and my taking the yoga off the map. That is what has really blossomed in this last decade of my life because we put ourselves in challenging postures on the mat. We do these kind of interesting physical movements and and then we're in these these interesting positions, and then the challenge is to breathe and relaxed. A find that place where you're challenging yourself, not straining to notice what's here to notice what emotions might arise. You just tried to balance in you and you and you couldn't. So what does that do to your emotional heart? What's happ so noticing all that you do that on the mat, then you roll up your mat and ideally, Hugh take all those lessons out into the world. And that's to me the magic of yoga, because what we learn on that about how we approach the challenges of the posture practice can inform how we deal with the challenges that we face. Face in our daily life. Carol, we understand that some people use yoga to prepare for surgery. How does that work? Well, I certainly did. I had open heart surgery in January. It'll be eleven years ago. I was born with a by custody a Arctic valve which is a one of the most common valvular abnormalities. My Orrick valve, two leaflets were fused. So over time it became severely narrowed. It was statistics ahead to have it replaced and it had created an aneurysm. So in at fifty four, I was facing open heart surgery, which is quite a major surgery, and my yoga practice was really profoundly helpful because no matter how vulnerable you are and believe me in the hospital in those gowns that never really fit me in their open in the back. And I always feel exposed and cold no matter. What's going on in the outer world, you can still control your inner world, and this is what the yoga practice helps us do. So in preparing for surgery. Certainly in the days before surgery, there's a lot of anxiety and anticipation. So helping to to sleep just keeping my energy positive. And then in the in the surgery itself, my my surgeons were kind enough to let me bring in the time. It was an ipod that I had yoga chanting, and I listen to that as I was put under the anesthetic. And as I was recovering from surgery, even if you're in a hospital bed and you can't really move, you can still breathe. You can still meditate and then in recovery, it's the the teaching that we learn yoga, which is to avoid strain to challenge yourself. So it's, you don't want to be lazy, but you don't wanna be pushy. And in our culture, that's one of the things as yoga teacher that I think I, I really try to emphasize the most because many people come in and they wanted to it right. And they wanna give one hundred ten percent. And they think it's another exercise class where you've got to compete and you gotta look like the person next to and you're pushing yourself in kind of a racing all of that. And I try and my classes to create an environment, welcoming environment. It doesn't matter what anybody else any other Matt is doing or looks like this is your practice and and find that place where you're challenging yourself, but you're not straining. Well, we have an Email from JR in Chapel Hill who says, I've had restless legs syndrome. I entirely life. It was so bad. My legs would spouse them at night in wake my husband up. I finally cured it by doing yoga several times a week. Perhaps the stretching helps to get rid of the lactic acid in the muscles. Yoga has also cured my chronic back end hip pain. So how often does somebody need to practice yoga to get benefit. I would say a short practise daily, and you know, even five to ten minutes, a short practise daily is better than a big long practice now. And then so how much does someone have to practice to get benefit. A daily practice again, even five to ten minutes would be useful. What are some of the other benefits of yoga? Does it have any benefit for the heart? I mean, we don't think of it as a cardiovascular exercise, like vigorous walking or running. What about things like balance fruit sample? So while the it's not aerobic like like running or walking the benefit for the heart may come from the relaxation and the and the stress reduction. The the lowering of the the respiration rate and lowering of the heart rate in terms of literature. And the scientific evidence for yoga is really in its infancy. But over the last decade, there's been a growing body of research looking into yoga. And now if you go to clinical trials dot gov, and you search yoga, there, hundreds and hundreds of studies that are looking at. Yoga as therapeutic modality for everything from cancer to chronic pain. It's just a, there's a huge literature. What the evidence suggests at this time is robust enough to say that yoga can do would be the the back pain in the neck. Pain seemed to be relatively well established some indications for chronic pain and for heart rate hypertension. So it's it's, it's growing and I'm very excited to be a consultant on the study that's being done looking at yoga for two. Very cool. We take a short break, but when we come back, we're going to find out about the importance of breathing. You've already mentioned it a little bit, but we want to get a little bit more like how do you do that? Exactly. There are a lot of different types of yoga. How can you determine the best type free cute a practice? If you've used yoga practice to help with a health condition, we invite you to join the conversation. Our lines are open at eight, four, seven to thirty three sixty six. You can Email us radio at people's pharmacy dot com. That number again eight eight four seven to thirty three sixty six. You're

"Anything it doesn't matter if it's weight loss or heart disease or diabetes whatever it is when you're struggling with anything like that remember that the choices that you make each and every day that's what matters more than just anything else let's go the phones and talk to john hi john calling him out rls smut mother has a restless leg syndrome and i wanted to know what the best way to treat that would be a she's seventy nine years old rls restless leg syndrome is something that's actually pretty common but it comes down to our minerals so the minerals that we take each and every day if we don't get enough of them so yeah things like calcium magnesium boron the different minerals if we don't have enough of the minerals coming in that's what becomes the greatest challenge to us so we have to have that coming in to really make the impact so i would encourage you to do with restless leg is to get a blood test on primary care physician take a look at it and see exactly what's going on with those numbers so for example with something like this leg if she's not if she doesn't have enough vitamin d then she's not going to be absorbing her other minerals like magnesium and calcium she won't be able to utilize them as much so getting the vitamin d level up is going to be the first key and vitamin d of course you can bring those numbers pretty easy just take to supplement the also wanna take vitamin k two with that so if there's vitamin d three and the k to you want to blend those together we've got a blended formula like that at the website if you go to inshapenetwork dot com you can see it there but yeah it's it's it's extremely important to put those two together restless leg is typically a mineral imbalance in the body none of the medications do work it's a great mandate but it comes down to balancing the body up more than triple.
