"Necessary in order to ameliorate their issue. But oftentimes, folks like this leave. And they go home or try another emergency department and often face another interminable weight. You describe a case of a young woman who came in, maybe with her mom and she had facial injuries. She had been jumped at school. You so you typically have three minutes to talk to somebody. It's never enough time. And you could treat her physical injuries, but it left you frustrated. You want to tell us about that case? Yeah. This patient in particular was assaulted at school, and while she didn't have long-term injuries or anything that needed more than some Tylenol and pain relief, I didn't have a chance to ask if she was safe or being bullied. Or whether or not this was a pattern of violence that we that and this was a central event that we could use to intervene. I just simply didn't have the time to ask those questions. Which is indicative of so many times we are taking care of people where what they're coming to the emergency department with is a physical manifestation of some social trauma or being trapped in a dangerous workplace. Or being trapped in a violent relationship, oftentimes if you sit and ask open ended questions, they're waiting to tell you. But do you have the time to do that? In the book, I engaged our social worker to do some of that work for us. And I'm so thankful that we have social work embedded in the emergency department to pick up that slack when we are unable to. But what if we don't even realize we need to engage the social worker? Because we haven't taken the time to think about it. It's a conundrum we face every day. You know, you say that you typically have three minutes to talk to someone. And this is typically someone who's been waiting a while. And it's their one chance to have the attention of a doctor. And you've got a dozen things in your head about tests you're waiting for on other patients, not to mention your own needs, you may be starved or exhausted. Is it hard to focus on those people and give them the undivided attention, even for the three minutes? When you're working that shift and you're right there at the front of the house and you're trying to sift through as many people as possible. It's extremely challenging. And what you realize is, look, you've got all this training and experience, and you know how to do more. And you want to do more. And you simply can't. And that creates this big, yawning gap between what you'd love to do and what you are doing, that lingers with us after our shift. What would you do if you saw somebody who had been who is living in an unsafe environment and has a small nagging medical issue, but hints at something much deeper that you'd love to pull the string on and learn more about, but you very rarely have the opportunity to do. It nags. The fortunate component is not every shift reflects that challenge. We only work that is one of the many places in the emergency department that we have the opportunity to work. And in those other places, we often have more time to sit down and listen. If you take it, if you aren't shielding yourself from what you might hear, if you are open to it and not fatigued and not scared and not yielding to the barriers that come from protective equipment or social distance and actually just sit down and open yourself and allow patients to open to you. I want to talk a bit about your background here. You write in the book that you're working at this emergency department by choice because you've made it your mission to care for your people. Let's talk about that community and their connections. I mean, you write a lot about the history of de facto segregation in Chicago and redlining and inequities in public education in city services. Tell us a bit about your parents, how they got to Chicago what they did for a living? My father is a physician and my mother is a social worker. And they met on a blind date in Detroit and moved to Chicago in order to start a life. They moved to the south side of Chicago, a community that is full of black folks that moved here during the great train migration. Growing up in a household with those two instilled in me both the importance of investing and being a part of a community, but also the recognition that we're a part of something bigger. We're a part of a tradition that spans a couple of centuries in this country. And in many ways, that led me to see my work as an expression of a social justice agenda. Which meant that taking care of the same population that raised me meant being, in many ways, a community doctor of the old time kind, where you know the people in the community that come in. The references to, you know, when I fell on the corner of 47th in Lake park, that's not simply a coordination on a map. I have experiences on that same corner as well. And can bring into that interaction, you know, a history and experience. After you did your White House fellowship in Washington, that was when you went to work in the private sector, right? You worked for a health insurance company and then a managed care plan. And you described this in the book and in particular a place, I don't know how specific you want to get about this. But in the managed care plan where you really wanted to deliver effective healthcare for needy communities and just the reimbursement streams and the timing by which payments arrived, particularly from governments, state and federal meant that it was just hard to provide the services. And there was a point where we actually had to lay people off. People who did important work, people that health navigators who would help people get through the Affordable Care Act, you know, whistles and trips. I'm just wondering, you know, what conclusions you drew from the experience? I mean, I think most people believe it's an important, it's important for smart public spirited professionals to work in the business. I wonder what conclusions you drew about your experience there and whether that was what you wanted to do. I had the notion that you get what you pay for. And if you start paying for the type of care that patients actually want and need, you would start getting better health outcomes. If you start paying for transportation to get to and from their appointments when that was an obstacle and start paying for the healthy food that's required to keep them healthy. Start paying for the non clinical inputs to their health that extend far beyond medications and surgeries. That we would have better health outcomes, particularly for those who need help the most. And what I confronted was a healthcare system that is working exactly as designed. And when challenged by the deeply ingrained obstacles that are ancient, I struggled just like every other administrator struggles to try.

He successfully appealed for a new trial and was acquitted of all charges. I discussed the corruption ineptitude and basically the overall nefarious -ness of the police and prosecution when it came to his case. I mean seriously guys if there is this case doesn't make your blood boil. I don't even know what to tell you. I don't know what well it is trifling because this case is so complex with multiple ongoing arcs. We are actually going to backtrack a little bit for part three back to rest free first trial. If you recall right before russ's first trial which began on november eighteenth. Twenty thirteen police. Investigators interviewed pam multiple times. Because obviously she was the last person to see. Betsy makes sense. Police were aware of the life insurance policy change and questioned her about it a couple times. She was always immediately defensive and she ended up bringing up her mother. Shirley newman using her almost as defense as to why she had no motive for money. So here's a clip of pam hop from a july of twenty thirteen pretrial interview with police to moscow. My mom got an accident in and home. I really hate to say it wanted mind. My were half a million that i get when she dies. My mom is dementia. Doesn't have the time now. I am ledge person. But if i really wanted money wasn't easier way than trying to combat somebody gets saying shirley newman pam seventy eight year. Old mother died just four months after this interview on october. Thirty first of two thousand thirteen evening. Just two and a half weeks before russ ferreira's first trial. She had fallen off the railing from her third floor. Apartment in an upscale gated assisted living facility. There had been no witnesses and police deemed it an accident. Let's break down what happened. Shirley's husband and pam's father died in the year. Two thousand when she became unable to care for herself pam and her three siblings work together to get surely into the lake view. Thirteen surely had a doctor's appointment pam transported her to the appointment and then surely spent the night at her house. Spending time with pam's family the next day. Roughly five pm pam and shirley arrived back at lake park pam instructed the facility staff not to expect her mom for either dinner or breakfast. She then left the next day on halloween at two thirty pm a housekeeper through surely newman dead. Underneath her third-floor balcony there was damage to the railing and she had fallen to her death after two police investigations. The saint louis county medical examiner's office deemed shirley newman's death as an accident. Shirley had died from blunt trauma to the chest from the fall and the medical examiner also found point eight four micrograms of zoll padam generic ambien in her blood. That's more than eight times. What someone her age and size should be taking but still it was ruled an accident. Even though a structural engineer said that there was absolutely zero away zero way. A woman of shirley newman's weight and height could've damage the six individual pieces of railing. Pam hop was the last person to see shirley alive. Yes she was never interviewed her siblings however had been even though they weren't even local to the area or present and hadn't seen their mother in quite a while in his first trial russ was convicted and sentenced to life in prison. Just a few weeks later. Shirley newman's death and pam's presence was never investigated and never considered moving on to january twenty fourteen as we talked about in part to the saint. "So there's this tiny town in Wisconsin called Mount pleasant populations just twenty, six, thousand people, village and life in Mount. Pleasant is usually is quiet as you'd expect. Their suburb of Racine, home of malted milk and the sight of some of America's largest cabbage farms. But that's the city now presents the suburb place were nothing really happens. Until last year when the village became the site of a completely unprecedented massive international experiment. Truth. Even Mundane. It's going to tell the story this fall. I went to pleasant to meet this woman named Kelly Gallaher. Hi. Halley. commanded she lives in his ivy-covered split. Level House my home is located about. But now she's basically retired. When did you start getting involved in kind of more? I don't I don't know what to call it activism or just Kinda village. What would you call it well? What if there is a name? I think other people have a lot of names for for what I do but. Ones I I probably wouldn't care to repeat the polite name that Kelly's detractors might use for her is busy body. She's an extremely vocal participant and the town's local democracy and her main stage is this place called village. Okay. The hours upon us I would. Village Hall which sits right next to the local white is where the residents of mount pleasant all gather twice a month with their village government to talk about all the local issues that need fixing through weeds this high in the yard of one of the vacation homes this. The previous March when he was running for office, he sent out a press release a basically accusing me of egging his house. Egging his house Yes yes. He filed a police report and suggested in his press release that I had been involved in the attack the political at an actual literal egg attack on his home, which the police report did not verify at all. In fact, it was the the described it as as teenagers and a single egg that was tossed on his driveway was hardly an attack I was not there. I only buy organic eggs which are far too expensive to throw it. Anyone's house let alone someone I don't like it was it was ridiculous. So it's fair to say that my relationship with with him was contentious I think he's a fool and a terrible per village president.

"I'd like to welcome to the show. Zach and Leah from Peregrination Dash Travel Dot com and the peregrination YouTube channel. Who HAVE COME TO TALK TO US About Yunan? China Zach and Leah? Welcome to the show. Thank you so much for having us. We are so excited to be here and I have to get you to explain the term peregrination because it is not a term that I knew but I like it now. That told me what it means. So peregrination is the Latin word for a long or meandering journey. And that's the type of journey we are on and the Pasadena Twenty countries and we are currently in Antarctica so we have actually done a show about McMurdo base in Antarctica. We have never done a show from McMurdo base in Antarctica. Which is where we are doing this show from. But we're not talking about. You'll have to go listen to that show if you want that. So why are we talking about you? Not Well you're not is such a unique section of China that really has not been the main focus of any tourism in China up until the last ten years or so so it's a very unique separate section of the country that has a lot to offer and mainly it has a lot of beautiful scenery that is worth seeing indefinitely worth going to excellent and we have done a show about Yunan before. But it's been at least six eight years since we did a show on that region. Can you put this region on a map for us? So if you are looking at China it's in these southwestern region. China is a very large country as we know It's bordering Vietnam in house. We're learning which one of us the geographer aren't we excellent? And what are you gonNA lay out for us? It's basically a trip from the big city of could make all the way into the mountains and back to Kunming using may differ transportations in seeing things on the ancient tea horse trail as well as things that are brand new in on the innovative side of China will. Let's get to it in somebody till you started in Kunming. We'll start conveying. You'll probably fly in. We had to take a bus in from Vietnam and that worked out pretty well. We walked across the border and took a couple of buses. It took about six hours from there. But this podcast is focused on the two week trip. So you'll probably want to fly into Kunming. Their airport is very large. It's a brand new airport. It's only been there for six years. It's one of the fifty busiest airports in the world. I've no trouble getting air routes all throughout Southeast Asia and it's very accessible for many many different places. I remember a few years back when China went to build a hundred different airports in cities with more than a million population and I don't think I'd heard of any of them. There are a lot of people in China and there's a lot of new airports that they've built in so I'm surprised. It's one of the fifty busiest in the world. But only a little I know could mean itself. It already has six million people. It's a very large city with lots to offer but as we said before it has beautiful scenery even though it's such a sprawling city now they call it the sixty of eternal spring because the temperature is absolutely beautiful all the time and so much you can do in C. inexperienced there too. Is it in the lowlands still? It is six thousand feet level. Okay it is starting to get up there okay. Yeah and technically in the region. It's in the lowlands. I guess you could say because on the western side there's that bridge which it's gigantic as the Himalayas determined easy to get around the city also so but in the last few years. They've been working on the Kunming Rail Transit which just recently they completed most of. I think they're still working on a few sections of it but this makes traveling around the city ridiculously easy and at most. I think we spent one. Us dollar each other trip. So it's very affordable which is definitely a bonus if you're on a budget or if you're not still good so when staying in Kunming we recommend staying closer to the subway line because it makes it so much easier to get around and what's cool is that it has a lake nearby and it's not just it's city by itself and you can expect what you expect from all cities that it has a lot of good food. It has interesting architecture and has an old town has the new sprawling town but it also has cool interesting things like the world's biggest flower market in the southern region called the down man flower market. We took the sub or the metro. They're in it took about twenty minutes and then another ten minute walk but it makes everything so accessible. It's an easy place to travel once you get there the little out of the way to get there but when you get there it's really easy to navigate. And what are you going to recommend that we do while we're in the city itself? Well there's a few things we did which we really really enjoyed. The University district is an interesting place to be. If you've never been to China it's a brand experience for you just because Chinese culture. They're so active. So if you go to a regular park in specifically we're talking about Green Lake Park near the university district they have so many people out dancing in groups doing exercise routines you see families. You are being stopped on the street. Because they've never seen a Westerner. It's an interesting thing to experience. Also the flower market annually. They cut in south four point. One billion stems ridiculous number of flowers. We went there one day. Be should've gone when it was during the peak hours like at three. Am but we got there a little bit later and it was still just absolutely packed with flowers and women cutting flowers. Making flour crowns also really cool river walk which is free and it takes you through the new downtown or the more sprawling city area. The huge high-rises. It's going to the old town. Also it's relatively close to the newer section but you're able to see the rapid changes in architecture in the standard of living that this city has been going through within the past thirty years. It's so evident while this city was probably so small thirty years ago and it has blown up the back they got a huge airport really helps also in the metro and the new high speed rail system. There's a lot going on and it's important also touch on the cuisine. So Union Zine is very interesting. It's it's much different than the other parts of China. It is said to be generalized as more simple and less spicy but there are plenty of restaurants that incorporate this Hunan cuisine I'm making air quotes right now. Because it's hard to generalize what it is because it's it's made up of twenty six mountain tribes different types of cooking in its this huge melting pot of different flavors. Walk around find some street food. Enjoy THE CITY ENJOY. Old Town see the new architecture and enjoy the people around you street food. Is there a particular street food that you found there? The two enjoyed? Yeah so there is an interesting kind of cheese that they loved to preparing cheese isn't really something you think about. When you think of Chinese cuisine God at all yeah exactly. It's like something that even Chinese people are like. This is weird way. You were you eating that. Yeah Yeah exactly. It's a type of goat cheese. It's different than the goat cheese that you and I are more familiar with this ferry leathery sour almost hard textural cheese that they prepare sheets so they let it lay out in the sun and Harden almost taste almost like a pro vallone Interesting tangy flavor to it. They'll put it on sticks in the grill it cover it in a suite date sauce or or sugar over top of it or it can be sauteed in simple also simple flavors like tomatoes or onions interesting base flavor for different toppings Everywhere I mean you'll find it everywhere and also rose pastries you don't think about eating flowers. Most the time but fitting that rose pastries exist in the city of eternal. Spring is the biggest flower market in the country. Also in the same city. So rose pastries in strange leathery goat.
