A Conversation With Dr. Suzanne Koven on Black Women in Medicine

"Early on. I believe that displaying medical knowledge the more obscure the better would make me worthy. That belief was useful spur to learning but ultimately provided only superficial comfort during my first year clinical skills course and colleges asked me to identify a rash mycosis fungoides i blurted out since it was one of the few rashes whose name i knew and the only one associated with cancer. My answer turned out to be correct causing three jaws to drop it once the oncologist the patients and my own but the glow of validation lasted barely the rest of the day a little further on and training. I thought that competence met knowing how to do things i eagerly performed lumbar punctures and inserted central lines and applied for specialty training in gastroenterology a field. In which. I had little interest thinking that i could endoscope my way to self confidence my first few years in practice. I was sure that being a good doctor meant curing people. I felt buoyed by every cleared. Chest x-ray every normalized blood pressure. Unfortunately the converse was also true. I took cancer recurrences personally when the emergency room page to alert me that one of my patients had arrived there. I assumed that some error on my part must have precipitated the crisis now lead in clinical career. I understand that. I've been neither so weak nor so powerful sometimes even after i studied my hardest and tried my best. People got sick and died. Anyway how. I wish i could spare you years of self-flagellation and transport you directly to the state of humility.

"Now, Jeff, Have you been getting tested for Koven? Yeah. In fact, I have, Richard I just In fact, just this 15 minutes ago, picked up my third kit from vault. I did the first one over the holiday, which As a little bit of time because of FedEx, etcetera, But yeah, I ordered mine yesterday morning at 10 30. I picked it up just 15 minutes ago and I gotta wait 30 minutes after I finished my lunch. And then I get the dialing to zoom. So that's 11 aspect that if people don't have zoom, they need to go somewhere where they can get access to resume because Basically go into a virtual waiting room. And then the supervisor looks at your photo ID confirms you are who you are, and that your kid has the right number on it, And then they just hang out while you Elevate into Ah, into a test tube. Uh, you twist the end off and you you put the re agent in there and shake it up and put it back in the pre labeled box and Take it back to the quick sin or the UPS store. Drop it off and within 48 hours. You have an answer. Now you've done this before. This is my third time. Yeah, my wife and I both have done it every week. We She has a small shop at El Museo down there at the the winter market, So we're exposed every weekend to people strangers. There was a way of just making sure we're not pushing it around. We, uh we've been doing it since we heard about it, and it's just it's breathe. So a smart they didna goes. I've done some research on that. And There is under there frequently asked questions. There is a question is my didna collected during this test, and the answer is no. The only purpose of the test is to dirt determine whether you are infected. Now that's their answer. I haven't been able to get any more information about that question. But you're you know you're smart enough time. I'm just not going to be skeptical. You know, we need to get on with this thing like I'm encouraging All of my friends like, you know, all of my friends Just just do this. If we all do this, we can get that positivity rate down and we can start to move into released yellow. So all right, you know, don't you agree that the logic is sound? I believe the logic is completely sound. I wish that we'd had a louder sort of amplification of that idea that message. I mean, the governor give her credit. She always tried to do this. I mean, you go to her Web site is where you can find. You know, a link to the vault thing, and she she they've always been encouraging. Healthy people to get tested. And I've had done to swab tests and I've done one swab test in Pecos. If you want to do the swab test, go to Pecos. I mean, they waited literally five minutes to get a swab and I had my answers in three days, so Um, you know, it's out there for everybody. They just need to accept that. It's not invasive. It's just mean the home test is a piece of cake. You get a nice person, somewhere in my last one with some woman in a cabin in Maine was doing my my supervision. So you brought that up. I forgot about that part. It does have to be supervised. Have to watch you do it. Yeah, because they want to make sure that you're doing it correctly. You basically have to conjure up enough saliva, and it's not instantaneous. Unless you're a drooling, you know, but, you know, And then he got a tap out so that they don't want the foamy stuff they want. Sorry, folks. If you're eating your burrito, be prepare for 10 minutes, but they're not gonna be looking at you while you do it, Richard. All right, so, but you have to wait a half an hour after you eat. It says at least half an hour before after you eat, drink or smoke Take given an hour, you know, drink drink, I would say drink, you know, be well hydrated. I had of doing the test that will probably help the drooling process. So Yeah, but But don't be drinking tequila. All right. Appreciate it, man. Thank you. I'm gonna be smelling my Santa Fe pale ale while I wait for us tonight so In the meantime, I'll be listening for your results. So all right, All right. All right. There you go. There's somebody who has been doing the the vault health. Spit in a test tube home testing. It is free state in Mexico as a deal with vault. Live here in New Mexico. Make sure you tell him you can't submit it until you tell me living to Mexico on then it's free. There you go. Eyewitness account. It does work. The fine print because Jeff Read it. I didn't read it because I have done it yet. It says No, we're not collecting your didna. We just want to do your saliva. Get back to you as quickly as we can. So there you go. That is cool. So I will definitely be doing it and I will do it on a regular basis. I mean, why not? Gonna help the city's Santa Fe County Santa Fe. Go from red to yellow to green. I will do My part. I'm a good patriotic Santa Fe resident. For two for 12 60..

"Is a mink outbreaks of Koven 19 over 5400 make have died in Taylor County, Wisconsin. Denmark is currently Exterminating the mink population, and Wisconsin is seeing Their own outbreak amongst me, mink of Koven that is being spread between people and mink. I asked you all to speak to this kind of recent development in Wisconsin a little bit beforehand, would you? You know, starting with you, Amalia, Would you speak to what? You all? Put about the mink on dear pandemic. How how You all have have kind of tried to try to discuss that with the community thus far. Yeah, So there is this breaking story about Mick, Mick and Denmark and it's It's one of these things that's like it. It's AH, It's really scary. It's a really scary story about a new mutation in the virus and Danish Mick. And here in Wisconsin. We do have a lot of mink and So I can see this becoming an emerging story here. So what We know First of all, I'll just start by saying I'm I am not a make expert in anyway, but I'll tell you what I know about this situation. Scientists in Denmark are watching. What is this new mutation and monitoring for it? Breaking out beyond just the few people that it's affected So far? We know that for whatever reason, mink and spirits and other members of the same animal family are particularly susceptible to covert 19, and they could be infected by humans, and then they can also transmit their infection. Back to humans, which is as far as I know, unique among the animal cases of cove in 19, and we've seen so far. I don't think we observed cancer dogs. You know, we haven't cats and dogs can get covered 19, but we have and but it's pretty rare and it seems like they don't They don't transmit it back to humans again, so but but makes on the other hand have And so this is something to watch for. I think it's not time to panic about the mink population or a possible mutation of covert 19 That's stemming from from the infection makes, but it's certainly something to watch for me. Is the good time to panic. When one has it like Cannick time. Get worried. This is not a drill. We are in a horrible situation. I mean, make our fourth largest export in Wisconsin, Our fourth largest export has cove at 19. When do we get to be really living in terror? Because Wait. You know, I feel I feel like folks are a little casual way. Don't want anyone to feel terror. You know, I think so Unhelpful, actually. All right. That's one of the things that our mission is. Your pandemic is to try and help people navigate that information in a way that is reasonable so we can make rational choices because fear and panic are not helpful Responses from our tagline. Stay safe. Stay sane rate. We are facing information overwhelmed we are facing News headlines like Oh, my gosh, we found, you know, covert on frozen chicken wings. I need to get rid of all of my, you know, frozen food. We You know this is a lot on a daily basis for people to digest in tow, like, sift through like exactly what you're saying, Allie. Is this the moment to panic, right? Um, I do. You know, I think the situation of Cove it is is grim right now in Wisconsin, and I won't sugarcoat that. But I think the same tools that we have had available to us. We can all Assess, you know, in our daily life like where can I make adjustments along these five facets of space mask air restrict in time for me. That's a really grounding tactic. I can say if I'm faced with, like, Should I do this? Should I not? I can think about how I can modify it a cross those lines. You know, or I can just kind of make the decision like this isn't a great time to be out doing things that aren't essential, you know? And that in some ways can be grounding as well. Because it takes the decision making out of it for you. You know, I was just so you know, I think panic as a response is really unhelpful. But I do think that the situation in Wisconsin is currently Catastrophic and heading for works, and it is time to change whatever behaviors you can change like it is time to to increase our own personal projections and restrictions. And so I don't think there's any reason to panic because we know what to dio. But it is time to do it. So distinguishing between panic and taking it seriously right. It looks different T panic then to take this seriously. It looks different to panic than to take practical steps to mitigate the risk of spreading cove at 19. If you are just tuning in, you're listening to a public affair on W R T 89.9 Madison. My name is Ali Muldrow. And today we are talking about how to keep our families. Community is how to keep each other safe on the during the latest covert 19 spike in Wisconsin, we are joined today by Dr Malia Jones and Dr Amanda Simon. And I think, you know I was struggling when I was preparing to come up with a question that is positive and hopeful on DH, you know, gives us gives us something to look forward to this holiday season. I think part of the dynamic that's really stressful is we seem to be caught between the spike and the re open. There's been this tremendous push to reopen since we originally shutdown going all the way back to April hearing remarks like we're going to reopen by Easter or kind of the He's you know these these dates thrown out that weren't necessarily rooted in our ability to contain Cove at From your expert perspective. When should we be talking about reopening? What? What should folks who are in the process of reopening be doing right now? Um and Amanda will start with you. And then finally, I'd like to hear your answer as well. I mean, I think the reality is we are open and and that has been the challenge. It has not. Worked to maintain levels of covert 19 that are manageable through the typical, you know, test contact trees isolate in quarantine methods that we have available to us. Through the public health infrastructure. And so I think the question is not When do we re open? It's you know, how do we be? How do we be moderate? And how do we modify Get back to some normalcy? It doesn't have to be a dichotomy between open and closed, you know, or public health and the economy. We have tio kind of find a middle ground. If these strategic about the venues and Places that we socially interact in the ways that we can employ strategies that mitigate risk in our own household and one were engaged in the community and that it's you know, we have keeps saying it's the same message that we have been trying to express for months. Because I think you know we were heading into winter. We're heading. We have some more months ahead of us. We have some reason for optimism. We have some good news on the vaccine front that came through this week, You know, but we still have some time to Goto work through the rest of the you know, data being collected from these from vaccine trials. The eventual implementation of administering that vaccine at the population level is probably we're looking at sometime mid next year, you know, so I think from AA Coping perspective. You know, we should be thinking about what are we going to do? Heading into the winter to to keep keep going? We are going to have to keep going. And we are gonna have to figure out ways to modify what we do to reduce risk for ourselves and our communities. You know, something that we've talked about in dear pandemic is bubbling with another, you know? Family. But that means you have to, like set those ground rules. You have to commit to really only becoming like a household, you know, and and sort of setting expectations that you know we're going to interact as a household. This is going to give us a little bit of social interaction over the winter. But we are, you know, not going, Tio go to bars restaurants. We're not going to visit extended family. We're not goingto You know, have our kids having playdates right? And so it's time to strategize right? It's time to kind of think. What can I do to manage the next few months until were sort of back into warmer weather and the vaccine's on the horizon? T make it through these, You know this next period where the best thing we can do is limit our social interactions in order to starve this infection of new people to infect. Yeah, I see. You know, I see a lot of pandemic fatigue and I don't know over these last couple of weeks. I have I think they get stalled in Wisconsin..

"Positive. The Koven 19 pandemic could lead to a shift in the political landscape in the U. S Senate in which party controls the chamber. Come November. I asked USA Today Washington bureau chief Susan Page on Skype. How the pandemic factors into the possible shift. If we had talked on January 1st, we would have said hard to see how the Democrats get All the seats they need to get a net gain of three or four to control the Senate. Now that looks entirely possible. We've seen the prospects for nine of the Republican senators who are running for re election get worse since January on Ly to of the Democratic held seats are competitive, and this is because In large part because of covert 19. For one thing, Koven, 19 has hurt President Trump and we know that Senate candidates really depend on the top of the ticket, and there are also some particular races in which Cove in 19 has played. A big role in a way that has hurt the Republican incumbent. Let's talk about a couple of those. What are people focused on here? Which states Aaron play? You know, you look at Montana that was a state that's a red state. Steve Bullock, the governor decided to ride it was still the Republican incumbent senator still ahead, Steve Daines. Then Coburn, 19 happens and the governor Gets all this attention as governors have gotten during this pandemic, and he gets credit for doing a pretty good job. And now that race is tied, or you look at North Carolina now we knew that was going to be a close race. Tom, tell us The Republican incumbent. Democrats are now making an issue of the fact that when he was the speaker of the State House, he blocked Medicaid expansion. And at this moment, with so many Americans losing their health care coverage, that is an issue that Democrats think hurts. Tom, tell us So talk a little bit about President Trump's leadership on this issue as a factor in the outcome. What are you hearing? What are people saying about where this is going to take the Senate? You know if you want to look at how much the presidential race affects Senate races look at what happened in 2016 in 2016 every Senate race was won. By the candidate of the same party that carried it in the presidential race. A perfect match. It matters a lot how the top of the ticket does, And we know from national polling from statewide polling that President Trump has seen his approval rating decline and is losing in a Siri's of matchups with Joe Biden both nationally and in some of these key states. How important is money during this time of the pandemic very important and will no more midnight tonight NBC reports to do for the second quarter. We've seen some of the Democratic reports. They're pretty good. We haven't seen some of the Republican reports and we think that's because they're not going to be as good as the Democrats. Money matters when you need TV ads because you can't do big campaign rally. Use a today Washington bureau.
