I'm Mary Louise Kelly and I'm Ari Shapiro. People who treat eating disorders say they were flooded with new cases and relapses during the pandemic. Treatment centers are overwhelmed with calls for help, and more young people are being hospitalized. Triggers for eating disorders can include isolation, uncertainty and changes in routines, all of which happened on a massive scale over the last year. Russian Keone Coe is an eating disorder specialist and founder of Empowering You a therapy practice in Maryland. Welcome to all things considered. Thank you, Ari. Thank you so much for having me how much of an increase in demand of you've seen over the last year. Oh, my goodness. It's really been disheartening over the past year. Everybody is full. There is nowhere to refer to and it's been really difficult. And another thing that we're seeing is even when someone finally gets in the treatment there 3 to 4 month wait list to get into an inpatient or residential treatment center. And as people start to get vaccinated, and businesses and schools start to reopen, or the numbers still climbing, or do you feel like the crisis might be starting to subside now? I think they may still climb because now that everybody and always coming out of the woodworks there's going to be more eyes on people and during the pandemic, most the referrals we were getting work for teens because Mom and dad or whomever they're guarding was had eyes on them. Now, as we get out of the pandemic, there's going to be more eyes, friends, extended family colleagues, Um and things are going to change. I think I think that we're just probably at the peak now. Can you talk about warning signs that you advise people to look out for in friends, loved ones or even themselves. Absolutely, you know, One thing I did want to say is that we have to remember. There's a difference between eating disorders and disordered eating. There are people that you know they enjoy working out. But they also work out, you know, to burn calories or to look a certain way or people that are uncertain diets lose weight. That doesn't mean they have an eating disorder. And so I would say, you know, just some of the things we really Want to look out for is has there been a change in their their mood there irritability a change in their interests. They may not want to socialize around food, or if they're at a gathering where there is food, they may not have the food. What we really look for when it comes to the difference between the eating disorder and disordered eating is really an impairment and there are day to day in their functioning. How do you advise people to start that conversation in a sensitive way? That doesn't sound like they are just asking invasive questions about how much a person is or is not eating. Yeah, You know, Ari, I think it really depends on the relationship you have with that person. Um, So if this was a friend, I would and let's say we've you know, gone out to dinner and I've noticed that there has been a really big shift in them. I may just kind of go through the meal. And then maybe a few days later, give them a call and say, Hey, how's it going? You know, I noticed that You seem really down and I don't think we really got to talk too much about like how this past years really impacted everyone, and I think it's important for to try to stay away from talking about food or talking about their body or what they look like. Because we also have to remember that eating disorders come in all different shapes and sizes. I think there's such a huge misconception of what some with an eating sort of make quote unquote look like and if someone is concerned that they themselves may have an eating disorder. You're saying places are overwhelmed. There's no capacity. There are long waiting list. What should someone in that situation do right now? My gosh, The first step might be really to reach out to a therapist that has that specialty. Another great thing is a lot of treatment centers out there. Online and on the phone can do screenings pretty easily just to help you get up by a better idea of where you are, but it again at least the phone screenings. It can take a week or two to even get into those, And so it's like, what are people supposed to do? That solution. The Biden administration is doling out an unprecedented amount of money to states across the country. It's $195 billion, part of the Covid relief law that Democrats in Congress passed back in March. A big fight is already brewing between states and the federal government over how to use that money. NPR's Asma Khalid reports. At the beginning of the pandemic. State officials were really nervous they would run out of money. But turns out many states never took as big of a financial hit as feared, and now they're actually getting in infusion of cash from the federal government. This really is a historic opportunity for states. Josh Goodman is a state fiscal health policy expert with the Pew Charitable Trusts. States don't generally have this amount of flexible money to work with. In many states, the funds from the American rescue plan are equal to about 10% of what they spent last year. Gene Sperling is the White House point person on rolling out covid aid money. One of the top lessons from the great recession was that when we went into recovery, state, local governments were still starved and cutting back. On teachers and spending and vital services. The Sperling was in the Obama administration, He says they learned lessons from 2000 and nine and want to make sure states have extra funds to help people who got hit the hardest. But there are limits on how exactly states can use this money. For example, if a state has a budget surplus, they cannot use it on general infrastructure. Think roads and bridges. Here's Josh Goodman again for states that didn't have revenue losses. It's only water sewer in broadband infrastructure, the three allowable categories and there's another sticky issue. There's this very broad, expansive language. It says that it can't be used either directly or indirectly to facilitate a tax cut. Jared Wall Zack is with the Tax Foundation, a group that favors more conservative tax policies. But what does indirectly mean? That's a really tough question. It's a question the Biden administration tried to answer the other week, It said States can cut taxes if they can prove they used other funding sources, not the extra federal covid money to finance those tax cuts.
