Biden’s first 100 days: What to expect on Covid-19, the economy and immigration

"Over the weekend ron claimed biden's incoming chief of staff south this memo that went into some detail. About what these first steps are actually going to be in practice. So in that memo the first ten days of biden's term he intends to extend the pause on student. Loan payments has been in place during the pandemic good extend moratorium on evictions as well rejoin the paris climate agreement. That's one of the first steps on climate change and rescind trump's travel ban on a group of predominantly muslim countries. Then there's also something clean is calling quote hundred masking challenge which i guess if they played their cards right on a branding level could be kinda like the ice bucket. Challenge in terms of the ice bucket challenge. Were selling themselves short. And i don't think that we should until they start in all seriousness. This one at least at first seems a little bit more symbolic it's going to include an executive action that basically codifies what we have now massing will be required on federal property and interstate travel all in all. His team is talking about a dozen or so executive actions on inauguration. Day alone yeah. Pretty busy day And then what's gonna come. After that. They have a lot of stuff planned. So second day biden is planning to sign a number of executive actions on cove nineteen specifically aimed at reopening schools and businesses safely and then expanding the nation's testing capacity day three is going to feature biden directing cabinet agencies to quote take immediate action to deliver economic relief to working families bearing the brunt of this crisis sound. Good not entirely clear what it means the stage probably find that i. They're hearing about it. Yes yes and then we could likely see more on climate change and immigration as well on the executive fronts and there are also reports of by team is going to propose a massive immigration bill. As soon as this week that would get sent to congress so yes just a few small things to get started on here but the main takeaway at least from the start is that they are going to be certain things within the realm of by direct control. And they're going to be others. That might more to statements of purpose. That would later require congress to actually get the ball rolling and pass something that dynamic is going to be extremely important as his term starts and they try to hit the ground running right and the cova. Bill is definitely going to rely on congress. So let's quickly touch on the vaccine plan. That biden laid out just last friday as part of that. Yeah so the top line goal that he had is one hundred million shots in the first one hundred days as we heard at the top of the show from his cdc director and that number didn't really sound quite as ambitious at the end of last year. And we thought that we'd be quite a lot farther along than we are right now but as it stands at the time of record we're looking at about ten to twelve million people that have been vaccinated in the us. Even with about thirty million doses. That have been made available and remember not so long ago we were talking about twenty million doses. Actually getting into arms getting administered by the end of last year so needless to say we are quite behind. It's gotta get picked up pretty quick so her yes so has biden thing that he can do this while he wants to use female in the national guard to build more vaccine clinics across the country with a goal of having around one hundred federally supported centers by the end of his first month the also talked about encouraging. All states to broaden their current eligibility to frontline workers and anyone older than sixty five which some states have already done but the idea there is to not have any doses going to waste by also talked about using the defense production act to ramp up actual vaccine supplies and then also make doses more available at local pharmacies. All very solid stuff but the issue. I think they and others are seeing is there are still just two vaccines with limited supply right now. So we're going to have to see how doable this all is over the next couple of

"For 60 bucks a month. That's pretty good. Yeah, you know, we we do have to set realistic expectations because we are available. 24 7 like if you have an issue at seven PM calling for my pinkeye, it's 7 30. Well, I won't answer the phone, then you know, we have patients that send us messages at 8 P.m., because that's what They remember that they need their thyroid medicine refilled so they'll send me a text. I'll read it. I'm not responding to it, but I'll get to it the next morning. You know, I've been doing this on this practice for over a year and a half, and we have well over 400 patients that we take care of, which is much better than my insurance based 3000 that I used to be responsible for, so I actually know people's names. It's pretty great. But in that year and a half, I've had less than five phone calls after hours, and that includes begins. You know, it's It's nice that we have that time to actually talk to them like humans, Monica, You know, I see a lot of patients that come Tow us that I consider to be underinsured. They save and scrap and research and then pay for these insurance plans that are like terrible $10,000 deductibles and all sorts of stuff 100 bucks a month, and they're still scared to even use it. Those are the people that I think are ideally suited for it. I think when you have kids, it's nice to have a direct primary care relationship. But I also think that if you have if you're older, and you're sicker that it's also nice testament is going to be able to spend 30 or 60 minutes, making sure you're on the right. Meds, and it almost seems like really well worth it. So for my own parents who don't live here, I've actually purchased for them a direct primary care relationship because I want them to be able to get taken care of and not worry about whether the doctor takes Medicare this or that of the other. Sure, so we actually have quite a few Medicare patients. I've opted out of Medicare so that we can infect still build them. But it's for that exact reason they are just done being seen for three minutes. It can only bring up one, maybe two issues that they're having. And especially for some of those patients. It's hard for them to get to the office. And so they appreciate the fact that their appointments with me are never less than 30 minutes. Never never, especially for my patients that I no need that extra time I make him for 60 minutes because I'm not doing favor service and I don't have toe see 30 patients today I'm able to take that time with them. So we have a lot of families because we have the moms who are like my kids pulling on his ear. Do I call the pediatrician? Wait on hold, hopefully get an appointment, too. Morrow only to wait in a waiting room for 30. Plus minutes that's surrounded by gross kids for pediatrician to tell me. Oh, Yep, Nope. Your kids find and then charge me What 130 bucks for that visit, or they can text me say, Hey, Jonny's pulling on his ear. Can I stop in? Sure. Absolutely. I actually looked at a kid's ears while he was sleeping in his car seat the other day. Why get him out? How is a DPC model different than say, I've I p medical books. So the difference Dream Director McCarron Concierge medicine is Thekla per month. Esso in concierge medicine. You're usually looking at 1000 to $1200 per month for access to that provider. Typically, concierge medicine will also charge her insurance when they get paid or not. I don't think they really care because they're getting most of their money the other way, but they are going to be the ones that will answer the phone at 9 P.m.. They typically have about 150 patients that they're responsible for. And so it's an even more niche sort of thing. But people just can't afford that, right. It's a zoo. Little affect possible, Monica. Thank you so much for joining us today we're running. The time on this particular segment. We're going to bring you back for another one to talk about lifestyle medicine. If people want to make an appointment or meet you in person, how did they get older?
