Interesting comment with Diane Sawyer, their Matthew Perry, the Friends star Chandler, of course, and you know, did you follow his personal life, all those years he was on alcohol or pills while they were doing the show. He said when I was chubby, I was on alcohol when I was thin, I was on pills, but that was his life, and about that monster doing one arm push ups waiting for him. He said this, when asked how people will know he's not doing well again, don't you wonder that about the John Mulaney's in Matthew Perry's of the world. People you know and like and champion, but you don't know. He said, if I say I'm just going to chill alone at home tonight, I'm not doing well and part two if I ever say, I'm cured. You know, I'm not doing well. Matthew quince the director of the Chicagoland for is the director of rosecrans for Chicagoland. And this is his business. Hey Matthew, you're on WGN. Thanks for giving us a couple of minutes of your time. How are you? Get your welcome, John. Good morning. So we had to be able to. I guess none of this surprises you. That is, you know that you must know. I'm asking, is this typical? Is this the life of somebody addicted to something like Vicodin? It can be, certainly, I think, given his position, I think he was, he was probably here accessibility, maybe more so than people in the general population might not have. Well, how unusual is 55 Vicodin a day? That's another fact that's just jumped out of this story. That sounds extraordinary to me, does it to you? No. That's the crazy thing about this is that I've heard many stories along those lines. That if people can find it, they'll they're tolerance will build to such a point that they're able because I think a lot of people think they hear that and they think how did they not just drop dead? As soon as they're taking that many in a day. Well, yeah. But really, it really speaks to just the power of tolerance. And him drinking a whole handle of vodka in one day, I think a lot of people would say, how is how is this body even able to do that? But I think over time, if you listen to him talk, you can hear him talk about how physically he just started to break down because of that. Yeah, well, it's sort of see it too, and the pictures we see now, but it's an interesting point you make, the tolerance, that is. I would imagine that if you took a Vicodin or two, you'd be kind of high for half a day, maybe four would be all you'd need. But I guess he built up such tolerance that four wasn't going to do it, huh? Correct. If we've ever been prescribed for a surgery, a hernia surgery or other types of surgeries, a lot of tooth getting pulled will get prescribed them. And often it's taking one that's taking two. But it can really get addictive very quickly and build that tolerance. I guess you know this too. I'm just reading from the wiki on it in 2020 Vicodin was the 16th most commonly prescribed medication in America, 16th most prescribed medicine more than 30 million prescriptions who knows how many pills. It sold under the brand Vicodin or Norco, those are essentially the same chemical compound, and it's not available in all countries in the United Kingdom, for instance, they won't prescribe it. It's not available there. What should we know about that drug? Like, I presume it has legitimate uses, right, Matthew? Absolutely, but it's really meant to be more of ideally it's really meant to be more of an acute pain tolerance management piece. For somebody that maybe had a surgery where for the first day for days, there is just an extreme level of pain. It's really just meant to kind of make that more manageable and then transition to something like advil or Tylenol. But I think and I think if I'm not mistaken, I think part of his story was that I was just kind of reviewing some of that and I think you got to that jet ski accident. And you hear that so many times. If somebody has those predisposition, they have the genetic risks for addiction or they're already maybe developing the problem with alcohol. And then you get prescribed this medication. I think a lot of people aren't aware of what that can quickly turn into. It can really just accelerate that addiction piece. He was drinking every day at 18 first started drinking at the age of 14 and described his first pill, he said, as crystal clear as if it were yesterday, it was like he said warm, honey, entering my veins, does do some people react differently to a Vicodin than others, some people go, oh God, I got to have this forever. And other people might appreciate its effect, but they're not going to fall into addiction. Is there any way that you all know who's who before they give the pills? I think a lot of it is just similar to any drug. It's looking at the profile of their family history of addiction. Their own substance use with drug like alcohol if they had problems with alcohol, they had problems with many other drug. He drank in his teenage years, so that was kind of that age of first use. All of those risk parts of that risk profile are going to determine whether somebody is going to potentially have that type of reaction to it or to become dependent on it like that. Do you have any advice for people who are prescribed Vicodin? What would you tell them like? Okay, you've had surgery or in an accident, take Vicodin. What else should you? Tell them. Generally speaking, don't get it refilled. Unless it's unless it's more of a chronic pain condition. And even in those situations, I would really encourage people to look at other means of managing pain instead of those type of opioid medications just because they can become I know for me personally I had a hernia surgery and I've told this story a number of times where I think it was maybe just three or four days I was taking Norco as prescribed and then I decided you know what? I'm going to go ahead and stop this because I feel like the pain manageable and I remember when I started I literally thought to myself oh my gosh I'm starting to get the flu. I had never experienced something like that before and put two and two together and realized that that was kind of the mild beginnings of that with those withdrawal. It just shows you how quickly that withdrawal in that physical addiction can come on so quickly if you don't if you don't really try to wean off it as quickly as possible even if the pain is there to really try to as quickly as possible. We not to something else that's going to be less have less of that addiction potential. 'cause otherwise I just eat the shit that's here or dude, I eat a ton of sugar, so. This is, this has been this has been good to switch it up. But I've been kind of doing that like cutting it way back on the sugar and the carbs and stuff as much as possible. But I don't know when I don't have the prepared meals. It's kind of harder. Well, then thing I noticed about you is that you eat a ton, but you eat like a little bit throughout the day. Yeah, like a rabbit? Yeah, like a little snack, a little snack little snack. Just a little snacker. Yeah, yeah. That's all I am. But you're saying good shape and stuff, so it feels weird to not be able to exercise though. Yeah. Because have you been doing a lot of exercise before? Yeah. Yeah, a lot more like biking and stuff. I think once I realized that I did have a hernia and that I could no longer ignore that my fucking whole entire right side of my abs was like in pain, bulging out and stuff too. No, I didn't have any like bulging or anything like that. So you didn't see it. Different kind of hernia that I had. Yeah, it's an inguinal. So like, but I definitely had something it definitely is impacting my right side because I would do jiu-jitsu or do a workout and I could just feel like my right lower abs just were like strained or something like that. So I've been switching just to like bike riding and like push ups and like there's body weight stuff. That's because I feel like I kind of, like, slender out almost. It's like my grandma's worst nightmare. She's like slender. You're getting so skinny. Are you skipping meals? Yeah. You're not eating? Yeah. Why'd you pick carps? Well, I don't know, I don't want to say, but no, and I'm not even like that serious. I wasn't really that serious about it. But then I just started doing more cardio. And I've never been really wanted to do a ton of cardio. Yeah. You tried to you tried to go against your body type for a while. You tried to be like a bodybuilder type. You're not really like eating. I'm a protein. Yeah, I'm a skinny guy, and I guess we would call it an ectomorph. And I got it from an ectomorph. That's what a mess and more for us. I'm a messenger. Well, thanks for having me. That was such a lovely voice. I could listen to that all day. Thank you. Michigan accent that she said, challenges would love it. My a's are accentuated. Let's do thing. Why don't we dive in? I wanna hear more about your story and how you got into functional medicine and teaching. Let's start there. Yeah, absolutely pretty, pretty good place to start. Yeah. It's been a wild ride. I constantly with each passing year kind of reflect more and more on this journey that I've been on that really started when I was 17 scrubbing toilets at lifetime fitness and I had navy seal aspirations at that point in my life. So when I turned 18, I went through all the testing and everything and qualified for the navy seal program. I signed a 6 year navy seal contract and to make that long story short, I was about halfway through boot camp for spec ops and they found Ida in Guido hernia. They deemed it a preexisting injury. So I was medically discharged. And I don't think people realize, it's actually not that easy to get into the military in the first place. I mean, you have to have nothing wrong with you, spotless record, no drug use, no injury, no psychological issues, just nothing, you have to be a blank slate. So they'll scrap you if there's really anything wrong with you. And that was the case with me and several of my boot camp buddies. One guy had like a kidney issue and he was peeing blood, so he got kicked out. Wow. And you know, as a young man, I was just a kid trying to figure out life. Like, all right, I just graduated from high school, what am I doing? And I had had these navy seal aspirations all through high school, and that was kind of the sole thing that I was working for. So to have that taken away, you know, kind of against my will, it really felt like an identity crisis in a lot of ways. So I spent about a year trying to get back into the military and you know I was trying to get medical waivers and exemptions so I could get back in and recruiters don't care. That's an impossible paper stack to try to push. So nothing was happening. And I was starting to struggle more and more with my mental health and I think whereas in high school looking back as high school kid you don't have any perception of what is mental illness or depression. You don't have any perspective. But looking back, I probably would say I was like a high functioning depressed young man. And I think that's why I was so hard on myself and why I was pushing myself to do the hardest thing that a man can do, which has become a navy seal. And I think there were some self destructive ideologies that I wasn't really aware of..

They need to know where to cut and how to get the job done. And so you want somebody with that kind of precision is what the top quality that one might look for. And, you know, I had I don't want to gross anybody out, but I had somebody put a scalp in my eyeball Monday. That takes a certain amount of like, I hope he knows what he's doing quality. And that surgery is the most common surgery on the planet, you know that? Really? Yeah. Yeah. Cataract surgery. And it's one that has been given as a gift to less privileged people around the planet because this is something that as people are aging to get cataracts and then if you're living in some village somewhere, you just go blind and then you're blind person. So the fact that not only could he use that scalpel and even without even really hurting me because sometimes you have to surgeon has to hurt you. I had a hernia operation three or four years ago and it took about two weeks to get healed from it. Yeah. And I was in pain for a while. This was so precise that it was my eyeball and the next day I'm up like having an online Dharma gathering. So here's an interesting thing because we're talking metaphors here. So cataract is often used as a metaphor for delusion, right? And so we have the scalpel of prajna the cataract. And what I want to get to is, obviously, we've talked about what's being removed. So the cataract is like this. Sort of unhelpful confusion, conceptual construction that we cut through with the sort of project. But what's the positive way? What are the insights that so what do you so you've cut away the counteract and you're not seeing blurry anymore? You're not seeing confused. We've got rid of confusion. But what is it that you see with clarity now? What defines a positive aspect of pragma? So first up best on? Yeah. Eyebrows. Eyebrows. I can see your eyebrows really clearly. Okay. You know, and the detail in your face. It's something I'm getting used to, but I've been for 30 or 40 years wearing glasses. And with that occlusion removed, you may see things in much greater detail with greater precision..

He figured the pain medicine at least kit him home. I'm very lucky. Both my parents are still extremely independent. Decision i have a friend driving him to the hospital and back and he can't be there so he wants health karma holy crap. Yeah i'm thinking about the to bill you've got your damn line. What possibly could be the reason for a circumcision. At ninety two doctors who listened to the show. I'd love to know. Yeah one of them's gonna tell us. Isn't that just amazing. It wasn't until earlier this year that the federal government actually started requiring that hospitals post their prices so that consumers would know the real charges for tests and labs and procedures will hospitals didn't like the proposals because it would level the playing field allowing people and insurance. Companies and employers to compare services on cost so the hospitals have done what hospitals often do they tried to throw up smoke screens and blocked the policy. Will it turns out that this is one one part of Obamacare that the trump administration actually likes so the policy was enforced starting earlier this year hospitals must be delighted because this price transparency policy has. Has turned into a joke. I recently had my medical students. Look at the U._C.. Davis and U._C._l._a. price postings what are called the charge masters will my students universally agreed to price listings. Things are completely unreadable unsearchable and unintelligible the web pages are encrypted in medical billing abbreviations that make no sense to a doctor or nurse. Let alone consumer. Different hospitals use different terms so there's no way a person can compare one hospital to another the posted charges do not include the price of the doctor so it's a little like buying a refrigerator and not getting being the total price but being told the price list for component pieces for the condenser for the door engines for the ice maker and the like. What are we supposed to do with that information? A patient scheduled safe for outpatient hernia.
