"This is weekend edition from NPR news I'm Judy Garcia Navarro good morning and thanks for being with us today some forty thousand dead here in the United States in less than two months from covert nineteen some seven hundred and forty thousand people infected with corona virus the highest in the world still despite warnings from public health experts president trump is anxious to ease restrictions and restart this economy what does that mean for public safety we have NPR national political correspondent moralizing joining us now on the line good morning hello hi so president trump on Thursday introduced this phased plan for states to reopen but he didn't mention any dates this time as he had in the past so I guess the question is where does he actually stand on opening up this economy well he wants the economy open as fast as possible but his guidelines were vague enough that they leave most of the big decisions to the states to the governors and that means that he can take a backseat and you can blame the governors for anything that goes wrong including a second spike in infections but the president has really made a one hundred and eighty degree turn from saying in the beginning of the week that he had total authority over re opening the governor's couldn't do anything without his permission to now I'm the backup you fix it so I think politically he wants to take credit for the good stuff like people getting back to work not get blamed for anything bad like a second spike in infections and what we've learned is you know president trump is not a dictator he just sometimes likes to play one on television and to make things even more confusion confusing the president is aligning himself with people who are protesting against certain democratic governors for doing exactly what he told them to do in his lockdown guidelines that's right just one day after he released the guidelines for what he called a careful step at a time re opening he is now encouraging right wing protesters in St some of them are swing states with democratic governors like Michigan they're protesting against the restrictions the president is trying to follow his north star which is his base try to figure out where they are and make sure that he's with them at all costs he was asked about the protesters he said they seem to be protesters that like me a lot of them were wearing bags hats and holding trump signs it's possible he feels this is even more urgent now because his approval rating has slipped six points in the Gallup poll from the middle of March but the big question is when he tweets liberate Michigan or liberate Minnesota liberate from what Michigan for instance doesn't yet meet his own criteria for the phase one and phase two opening what R. the repercussions for tweets like these and rallying cries if any I mean what does this mean because it is unprecedented yeah well politically it has it's a double edged sword clearly the base likes it I've talked to Republican strategists who say they don't like it but beyond the politics just on a practical level it has implications because up until now the president has more or less sided with his public health science advisors but now instead not just creating political divisions he could be creating practical problems for Republican governors who are now under tremendous pressure from the president to open up fast what if they disagree with their local officials what if there's a second spike because they open too fast would it be possible to convince people to go back into self quarantine if the president is encouraging these protests that's pretty tough and just briefly speaking of testing where are we with that well everyone tells us C. O.'s governor's public health officials say we don't have the number of tests that we need we have about a hundred and fifty thousand a day that's a third of the number that we need to know who is sick who needs to still be in quarantine who can go back to work or maybe with a different kind of test is recovered and has some immunity to the virus I without that kind of information we can't really pinpoint a containment strategy and we're stuck doing this massive blunt instrument mitigation which is putting the whole chunks of the economy into a medically induced coma that's NPR national political correspondent Mara license thank you very much thank you lack of testing a struggle to find ventilators all that has hampered dealing with a pandemic here in United States but doctors in hard hit hospitals are now saying the problem is not enough dialysis equipment Fred mogul at W. I. that's cute kidney injury but the good news is he's responding to medication I don't think he needs dialysis at this point since he's making a little money for small farm recorded the exchange on his phone later he says this man is relatively lucky almost one third of coronavirus patients on ventilators also have kidney failure they are critically ill they are not doing well and every day a couple of patients of doctor Goldfarb says so far at the three hospitals where he works they have enough dialysis equipment and staff but three doctors who work elsewhere in the New York City region describe much more dire situations they say no one wants to call it rationing but that's exactly what's happening we only have nine machines or televisions and now we have over thirty patients that need them that's a doctor who manages and I see you at a hospital in queens we're not naming him because he fears losing his job for speaking freely so it becomes an issue of the resource goes still and those are very difficult decisions it's not clear exactly how the corona virus damages kidneys Dr Benjamin Humphries at Washington university hospital and St Louis says the virus could be infiltrating kidneys directly or it could be in in direct effect from COPD patients tendency to develop blood clots we don't have any other clues as to what differentiates patients that do develop kidney failure who are infected with covered with those that don't the widespread need for dialysis has come as a surprise research from China and didn't indicate major kidney problems particularly not among patients who don't already have other problems like diabetes but that's what American doctors are seeing now Dr Steven Fisher pain is with Northwell health new York's largest hospital network everybody is running into shortages at this point the shortages in just dialysis machines but also the unique fluids and filters that use and there also are not enough of the highly specialized dialysis nurses many of whom have gotten sick with Kobe nineteen themselves Fishman says hospitals need to get creative these are intensive care unit nurses who usually might take care of two patients now what's one nurse for for patients doctors and at least two hospitals in the city say they're coping with the shortage my taking patients off twenty four hour continuous dialysis instead they're having to patients Shera machine and each gets twelve hours and sometimes doctors and nursing supervisors have to choose which patients will simply go without dialysis based on their prospects of surviving again the doctor in queens where our hospital ethics councils there are R. Kelly if care teams but the kidney doctors the one deciding who's in the worst failure at a given time so he's a dialysis shortage actually killing patients in the ICU it's hard to say the virus is what's killing patients mostly by shutting down the longs doctors say it is possible to get the lungs working again but it's hard in these patients need all the help they can get from the kidneys more from the best mechanical substitutes available for NPR news I'm Fred mogul in New York experts believe the corona virus pandemic may have started at a food market in China I'm where wild animals can be bought and slaughtered on the spot now there are growing calls to ban these types of places worldwide NPR's Jackie Northam reports you can find so called wet markets across China do usually a jumble of stalls caring produce seafood some farm meat but the problem is sometimes these wet markets also Kerry illegal wildlife crammed into unsanitary cages bats stacked on top of pigs on top of pangolins on top of civet cats and other bodily fluids they're kind of flowing into each other you've got very stressed animals who said more viruses are not stressed as Ralph with a double when you were stressed to illness sh and all of viruses are mixing with each other Jennifer is a senior advisor for advocacy at the world wildlife fund she says those viruses can spill over to people turning the market into a giant Petri dish extra perfect recipe for an epidemic he is leading the world wildlife fund's efforts to close down these high risk wildlife markets her group is part of a growing chorus amongst politicians advocacy groups and government agencies and includes the top infectious disease expert in the US Anthony Fauci who made no bones about how he feels about the wet markets recently on fox news I think you should shut down those things right away I mean it just it boggles my mind how when we have so many diseases that emanate out of that unusual human animal interface that we don't just shut it down in the wake of the corona virus China's government banned selling wild animals at the wet markets he did the same thing after the two thousand and three sars outbreak but the band back then didn't last says Republican congressman Michael McCaul of Texas we didn't learn the lessons from the past and I think that's what we want to be looking at now is how can we stop this from happening ever again the question is whether a ban is enforceable because a lot of the trade in these wild animals is already illegal but still widespread McCall is among a bipartisan group of more than sixty congressmen calling on international bodies such as the World Health Organization to ban live wildlife markets we need to take a hard look at how can we make the W. H. O. more factors how can I have more authority to go in and stop the global pandemic the W. H. O. a unit of the United Nations doesn't have enforcement power all I can do is offer guidance this is what markets are an important source of food and jobs all over the world but the government should ensure food safety and rigorously enforce bans on the sale of wildlife.
