"For fun Well i work hush go you do. That's what i wonder if you're having any fun. I you know. I interact with my family and my partner and that is the most gratifying thing. I'm very protective of my personal life. i do. I do exercise. So i really enjoyed going for walks in. It's so nice now that the weather is nice and in pennsylvania washington that we can god for great walks and and i have And i have a dog. I have a a soft coated wheaten terrier bailey and he's a little old man he's almost fifteen years old and so we walked slowly together. Because you said that you are very protective of your personal life. i am home. But you're also a historical figure in an icon. How difficult for you to to to balance those two truths about yourself. I have been able to balance it fine. So when people say that i'm an icon. You're gonna see me blush. I mean i find that concepts remarkable but but You know there's my my professional life there. Is you know my life in terms of stri would say a strong advocate for the lgbtq community And this is my personal life and my personal life is my personal is where i can't find you on instagram or anything like that. I am not on social media at all. And i had a lot of challenges last year on social media as we were advocating for the health of pennsylvania in the during covid nineteen There were many people who disagreed. There were many people who were very lgbt and specifically transphobic. And i found it easier and better just to come off social media you. I don't know if you know this about me or not but my first attempts to break into sports as nobody gay man wasn't great more Things are much better now but were certainly periods. When i was growing into this profession where i questioned. Why come out. And you know what..

"A you ever feel ghetto is a little bit and when you're working specifically for things just dealing with things that are important to tq community when you have all these other interests that you to sort of explore. Now i really don't. Because in the office of the assistant secretary of health there are many many divisions that are working on all of these issues issue. So you know. We are working on health equity We're working on on the opioid crisis and mental health issues We're working on on. They're going to have a new office specifically about climate change and health equity. Could you explain it sure. Climate change in hell absolute auto sold to connect. Well you know this is. This is an office that will concentrate on and on on environmental justice. You know those that are experiencing the health impacts from climate change are predominantly vulnerable communities african american communities lat next communities native american communities American indian native alaskan communities. And so if you look at the impact of climate change On on on on certain cities whether it's You know miami or new orleans or new york If you look at the impact of heat in the southwest and lack of water in the southwest at at the melting of the of the arctic in terms of alaskan native communities it is it is it is hitting some communities far more than others it is not a fair distribution and so there is a strong health equity components climate change. And we're gonna we're gonna have a specific office that that is part of a presidential executive order dealing with those health equity issues. So you can see. There's a lot on my plate. Absolutely you got your start in medicine pediatrics. Correct absolutely pediatrics. And then the sub specialty of adolescent medicine. What was it like for you again. Going back to the confirmation hearing being asked questions about the wellbeing of children recognizing taking care of children as how you got into this field to begin with absolutely so you know i appreciate it. Some questions and i had less appreciation for other questions. Well senator thank you for your interest in this question Transgender medicine is a very complex and nuanced field with robust research and Standards of care that have been developed. You know. I bring everything that i've ever done to this experience to this job. As the assistant secretary for health. "What do you remember about the confirmation process today and what elements or aspects of that entire process. Surprised you well you know. The process was interesting. It was Challenging i think it is for everyone to go through. The confirmation process I i think that it was particularly challenging. Because i'm an openly transgendered transgender women. When i remember most about the process is my gratitude to those at health and human services in the administration that worked to prepare me. Do you recall any moments in which you thought to yourself. Oh my god. I'm trying to make history here now. I was really trying to be very mindful in the moment. That's the best way to deal with with experiences like that so It didn't really hit me about a about the history-making part of it. I was just in the moment. You know answering the senator's questions and and being open and truthful and and working through that process replete tell me afterwards. You celebrated like blunt somewhere or something right. Well you know. There's a copay nineteen pandemic. So i went home and and with my partner my family and we had a very nice dinner Girl we got to celebrate with you. That's not the way we make history in our community. We gotta make sure. There's some confetti and some glitter right johnson. Today you mentioned earlier about the work that you did witnessing and obviously tending to people dealing with hiv aids during the early days of the crisis in new york The cdc recently released a study a report dealing with hiv aids With a lot of encouraging news of one particular does sort of raise my eyebrow and that was twenty. Three percent of the people who are eligible for prep are currently prescribed. Prep that means about seven. Percents are not what can be done to close that gap so very important point. You know i in the eighties. When i was taking care of patients who all passed away they all dot. If you had told me that we have a medicine that you could take once a day that if you were at risk would prevent hiv. If you had medicines that people with hiv take every day and that would keep them their viral load down and we keep them undetectable And and duly calls you and they would be a not able to transmit the disease dan I'm transmittable than i would have thought that those were a medical miracle and we have all of those things but we still have issues in terms of health equity. We have to reach communities that we are not reaching with these with with these medications with these protocols and so that is why health equity is such a critical part of of of my priorities at as the assistant secretary for health. And i know. It's one of the secretaries priorities. And the president's priorities we're going to be working across the administration and the department on health equity in particular regards to hiv to make sure that people have access to these These extremely powerful and and important medications the work you're doing and it's so fantastic to hear the work you're doing particularly when it comes to. Hiv aids is so dear and close to our community but obviously you are interested in a gun involved with medicine for a whole spectrum of reasons..

Coming from senator rand paul who did not ask dot singer question about the pandemic but instead used his time to focus on the genitalia of children's. Let's be a little more specific since you've aided. The question do support the government intervening to override the parents consent to give a child. Puberty blockers cross sex hormones and or amputation. Surgery abreast genitalia. I would imagine for someone who began her career in pediatrics. For someone who was pediatrics professor at penn state university for someone with children of her own the very idea that data levin will seek to put a child in harm's way is nonsensical that best prejudicial at worst but then again trying to break a glass ceiling they're always be people lining up to protect it for no other reason than to score political points and in today's political environment using a confirmation hearing to score points with the base is common practice no matter how dehumanizing the line of questioning is no matter how old entire the attacks our case in point save our children was to i organize push back against the tq equality and as you can tell from his name. "It's that the body just does not like to make inadequate immune response against hiv. Just doesn't and that's one of the perplexing aspects of hiv it integrates itself into the genome of the sells so quickly becomes very difficult to rid the body of it and one of the tenets vaccine. -nology is that in order to get a successful vaccine. You've got to have a disease in which the body actually makes a good immune response so when you get diseases like measles like polio Even though those are diseases that caused a considerable degree of mobility mortality at the end of the day. Most people recover from those diseases because the immune system is capable of suppressing ultimately. Ridding the body of whatever pathogen. You're dealing with the smallpox measles or polio. And then the body then has an immune response that stays with you that protects you against infection from the same pathogen so the body is giving you approve of proof of concept that it is capable of making a good immune response and that's the reason why we have successful vaccines against measles against smallpox polio. And now against covid nineteen. That's just not the case with hiv. Because for reasons that are still a bit perplexing. The body does not make a good response against hiv. So in order to get a vaccine. Which i think and i'm always cautiously optimistic that we will ultimately get a vaccine for hiv. We've got to do better than natural infection. When last i checked black people made forty two percent of the country's new hiv cases in two thousand eighteen alone. That black men are less likely to get care. Black women are about fourteen times more likely to be diagnosed with hiv than their white counterparts. So my question to you is when you hear those numbers. Does it feel like it's a crisis situation for you for the black when it comes to hiv. I'm talkin about that for a very long time. In fact i talk about the extraordinary disparities in health with not on the hiv but also with cope in nineteen. You're absolutely right. The numbers of very very disturbing in a thirteen percent of the population is african american. Forty four percent of the new infections are among african americans and of those sixty five percent or among men who have sex with men who are african american and of those seventy five percent are young african american men who have sex with men so there's a considerable disparity there you know. The reasons for that are multifaceted. Really it has a lot to do. I think with the stigma that still 'associated with hiv and being gay among african americans because if you look at the charts of the country and the counties will you will you see a considerable amount of residual infection even today after all of the things we have which prep and treatment as prevention the accessibility of something as effective as pre exposure prophylaxis seemingly one pill trodden every single day. That's not as accessible to african american men as to other members of the population so it is another example of the unfortunate and disturbing disparities in health in access to health access to medications access to health. Care and the stigma and discrimination against men who have sex with men particularly african american men who have sex with. Do you believe the biden administration is aware of this dynamic and is prepared to do something to address it..

There was, of course, they were always in a hurry. And we have four employees up to five at the most. And then we're just cranking it out as fast as we could do. I have it right that you've got A contract from the T s a out of l A X. I mean, those that's not a small customer. No, it's not. It makes you wonder how they found us. But they're the agents sit behind these podiums and they needed to be covered at the front and two sides of the podium and we ended up making something along the order of G 140 of those things plus covering their break room tables. So that was that was one of the biggest sales we've ever had, I'd say. Kind of funny that this giant government bureaucracy found little low. Y'all, you know? Well, I think we've done a pretty good job of staying on top of our Internet in terms of it being an easy find. Look at that's smart. That's smart. As you sort of got going. Did you have a decent supply chain? Were you able to get the stuff you needed to make what your customers wanted. There was a shortage and it reared its head by the end of April. We're starting to see that demand go up again, like dramatically just in the last two weeks as California is gearing up for full reopening. Hmm. You know, honest. Honestly, If you have this conversation with me three weeks ago, I would have said I don't know is this is never coming back. Wow. That's that's a little terrifying it is. You know, we didn't bother with the P P P loan the first time because we were so busy and I guess we figured well that that's for bowling alleys and restaurants and places literally taking a hit, But we had to go for the second time immediately. You can work on containment that involved the testing and contact tracing very hard to contain a virus that has spread this far. For example, Today we are reporting 11 over 11,000 new cases. In Pennsylvania. It's impossible to contact that many people, so we have to prioritize. We prioritize to congregate, setting such as nursing homes and other long term care facilities, Correctional institutions, schools, etcetera, Then we have mitigation.
