What words are you going to share here? Nothing. If if it was another Texas school out here, I would. I would talk to him. But my wife recently told me that when I make fun of other people's medical school, it is unprofessional. But it's so funny. The problem for me came when West Virginia joined the big 12 and I went to UT for undergraduate Yes s o now there's like a rival. What do I do? Do I I mean, of course, also go with you t. But it was a little tough because we saw the big 12 head to head competition. Yes, yes, well, the good news is neither of you guys are in any danger of winning a lot of games. So you know, I really have to worry about danger is going to the dangerous can't eat. Erica. I think a lot of people may not know the total breath of what a general surgery gets toe gets to do. You guys train in a way that allow you to treat problems from the top of the head to the bottom of the foot. But most people would probably know you guys is dealing with problems in the abdomen, right? Correct heartburn. Is a fat has been a fascinating disease just in our lifetime. How it's changed, right? Sure, when we were young, in's it was because we had less stress in our life. Just how that has changed the opinion of heartburn how dangerous it can be. All that stuff has just been like a remarkable change. We learned about surgeries in my residency that were Basically can now considered historical operations that we would you know, people get ulcers from excess acid, and you have to do an operation to stop their ulcer from bleeding or perforating. And then their stomach contents leaks out into their belly. And they're sick as a dog After that, that was just 10 years ago, and it was in a different Era. You know, now that people have acid blocking medicines. The ulcers are very easy to treat. There's billions of dollars pharmaceutical costs that just go too purely antacids and anti reflects medicines and histamine blockers. You know, if if you have had heartburn for a long time, you may be stuck in the old way of thinking about heartburn. You may still think that soldiers were brought on by stress stress. Or you may think that acid excess is what causes the ulcer, as opposed to bacteria. You know that working treat if you just stop eating spicy food or chocolate, or, you know no more jalapenos. You know your acid reflux will go away and I just think I also think that the fact that a lot of these medicines were available over the counter now made people bypassed doctors when it when it comes to that condition. And so they don't get like the information. And so one of things I love about doing this show for many listeners out there. That's the most time they're going to get with the doctor to discuss an issue that they may not have sought out for. Maybe so, give me your spiel. Yeah, I'm coming to you Tell me I'm having heartburn. Tell me what I would be feeling or doing to let you know that it's pretty bad harbor, So I always start with asking people. How long have you taking and Any medicine for acid reflux, Whether it's in an acid like Tom's, or whether it's Zantac or Pepcid, or you're getting over the counter Prilosec and if you are seeing a doctor that's actually prescribing that for you because you need a neck stronger level. So first things first. If you're taking the medicine, and it's working to control your heartburn, and it's a low dose, you're probably actually doing pretty good. The problem comes when you start to double your dose or take it twice a day or take one kind of medicine in the morning and a different kind of medicine at night. Then you know you go on vacation, you forget to bring it and it's the most miserable. Day of your life. You know, you're just trying to find the pharmacy so that person has now sort of graduated beyond your typical medical management case of heartburn. That's where surgery comes in. And that's why you know someone like me. A general surgeon can do Surgery to stop your acid reflux. The goal of surgery to stop at three flux is to number one stop it like we just said, Keep the acid from going up into your esophagus. But two is to stop having to take medication. There's some people that think they do what we do, whether they be m. You know, quote unquote melt men's health clinic or testosterone shot clinic or, you know, amends it institute like within, and Other practice, but really walking the walk with all the different things that we have functional medicine, a complete supplement line real focus on cardiovascular health nutrition as an important component of your overall treatment strategy. This goes well beyond just giving you drugs and surgery. Mm hmm. That's right. We take that holistic approach would take it a little further. We can actually get you into See somebody pretty soon. That's what that's been a very important like component or mantra of our practice. I was never proud of those guys. Who out there who said Oh, I have a six week or eight week waiting list. I was like, really, You think you're that important and good? That's here eight weeks. You should really figure something out. You know, so that people can see you quicker. Specially almost seems mean. I mean, do you think I wait eight weeks? See the doctor? No, I don't wait weeks, right. I call somebody That's right. I called somebody I need to see you right now. Three o'clock like Donna, I need to Dennis right now. And that's what I want People toe feel about with us. If you have a urologic complaint, if you have a painful kidney stone, you're not waiting six days, right? You call us You tell us you were painful. Kidney stone will see you now, if you have not pain that's like unexpressed diagnosis to get in to see us the H O V lane directly into that's right. That's right. We don't let way. Don't let testicular pain. Wait. That's right. What was the phrase testicular pain doesn't see the sunshine. No. A trajectory. He said some no, no, he says, testes. Masses will never see the sun set. And that was because back in the old days when we were training his residents we were always afraid that that young 22 year old with testis cancer that came in the ER what would leave and he would run away. I don't know why we thought that but we were afraid they're gonna run away. So we never let him leave, like admitted in the hospital like we just like, you know. Give him cake and carrots so that they would stay caking carrots? I maybe I'm hungry. Wow. But it really raises a very important point, which is that the urologic approached. The young man is not the same as the older man like we are worried that the young guys going to run away and ignore problem that maybe something that's bigger, and I think that the approach to the young man Is has to be different. Over the course of my career. I've heard patient after patient. Young guys tell me that their symptoms were brushed off by previous doctors, and that's always makes me wonder like, what is that doctor try and teach. This person could not be concerned about their health. Don't know, or something like testicular pain or prostate pain. They're just given antibiotic after Anna bodyguard for antibiotic, and I wonder if it didn't work the first two times. Why is going to work the next two times? And so really these kinds of episodes throughout my career are what has prompted me to try to approach Young men with your logic problems little different, and I think we have a question to that degree we do. It's It's very intricate. So I'm going to kind of paraphrase Mrs. Hello. He's a listener of are shown, and I'm fascinated with him because he's so young, but a little bit of background. He goes into how healthy he is. He's gluten free. He's had acne and in school, he's he likes holistic care when he was in a really great detail to explain what a healthy young man he is, he said. I just want to know what the long term ramifications for that and it was a long night long weekend that I was like, Oh boy side of medical school. In all seriousness, it just seemed very fast. Eating to me and the disease course is quite complex. And I thought I could make a difference by helping people with this disease and knowing that ultimately, in many cases if the liver starts to fail, there's transplantation as a as a fix for that. And and I and I enjoyed that. So so next to urologist, Of course, I've always thought that hepatology ists were like vest. Smartest doctor right below your old thunder just just just under urologist. And what often fascinated me about hepatology was that you? Do a lot of transplant medicine, so it's not just chemistry balance. It's not just helping people with horrific end of liver disease kind of problems, like the cirrhosis causes fluid in the abdomen and various sees with bleeding vessels everywhere and bleeding out their throat and then living after liver transplants. No walk in the park, either. It's a huge incision. Somebody either has to donate part of their liver or somebody has to die to get a liver. And then you have to be an anti rejection. I mean, had liver disease sucks. Idiot. It's hard, though. People who spend tireless hours helping patients with liver disease and know that it's complicated, and they have a special calling to help those people so that they're just like anybody else. When we look at liver disease, we know well, at least of the physician. I know what a really sick liver patient looks like. But I don't know is what is a less sick liver patient. And this concept of fatty liver was something that I guess I heard more about when I was finishing training, But I'm out of it, and I don't know. I see so many patients diagnosed with fat. Oliver and I don't know what it is or how to fix it or why people should care. Well, it's really not that easy to identify out the box, so you really have to be discerning and looking. But the first thing I would say is look at your patients. If you see someone who's an obese, diabetic, Hispanic female, Maybe that's a that's a good fino type of patients that are likely to have patty liver, diabetes and in particular, they may or may not have abnormalities in their liver in science, But the fact that they have those metabolic syndrome disorders, it's likely that they could Fatty liver, just swimming in the background as well. And the way for you to identify that is you can start with just some liver enzymes if they're abnormal, and when I say abnormal, it may not be that the lab calls it abnormal and it's red on the on the lab profile. It's just the S T greater than 19 and a female or greater than 30 and a male that might be a trigger to look into liver disease. Also the platelet count the platelet kind kind of flies under the radar. But if you see a platelet count less than 150,000, that might be someone with advanced liver disease. And there is a technology which is relatively new. It's called a fibrosis Can it will measure the stiffness of the liver. So if you have a patient that you're suspecting, fatty liver disease in, especially who fits.

We deal with the entire multitude of your logic issues for both men and women. But, you know, several years ago, I thought that kind of our take on men's health was somewhat unique. We have kind of a The whole body. Holistic approach with multiple different specialties on DH disciplines reflected in our practice, and I thought there's no way to communicate that I used. We used to go to primary care doctors and communicate that, But then they got too busy. They wouldn't want to see it. And so and so then you're like we got you. We got your cards Go away. And then, if the patients didn't know that they were going to get that kind of service double when they walked in. They were completely surprised that we would refer them for nutrition or biomechanical therapy or what within the clinic. And And so the radio show just comes a wonderful way to kind of prime. The community on what we do Prime are our competition on what we do way still are the second largest in town. You know what second best means. We're prettier on the weekend. That's right. Big part of what we do in our urology practice is the treatment for prostate cancer. And if you've been a listener of the show, you may be aware that we do a special kind of treatment. It's not widely available. It's called high intensity focused ultrasound or high food treatment for prostate cancer. It is almost Almost too good to be true, Donna, it really is because there's no side effects basically well, I can't say that I hardly any that that works. But I can say that we have patients. I would say about 70 to 80% of our patients that get this treatment for prostate cancer come from outside of Austin. It's not widely available. It is a one day procedure that is painless. That uses ultrasound energy to identify where cancer is and to a blade it using this ultrasound energy is like a It's like a magnifying glass that focuses the sun's rays on a point And that that that that ultrasound energy can kill the prostate cancer if you've been diagnosed with prostate cancer and your watchful waiting are you awaiting you know something about how to treat it, and you haven't been given the option of Hei Fu h f u. It's definitely something that we is probably our most popular reason for people to get a second opinion this year in our own shooting this week in our own literature, it was shown that just treating where the tumor is in your prostate called focal therapy, just treating where it is had Equivalent treatment outcomes to radical prostatectomy radical surgery, and I've been doing robotic surgery for almost 20 years. Now, this is this is not something that is new to me or new to our practice. But the idea that we can provide vocal therapy for just where your cancer is and give you lower side effects, equivalent treatment outcomes and much easier. Decision making is something that makes me really happy. That's right. We have some podcast about this perfectly from a while back, so it's great to refresh. That's right. You know, I think that now that we have been going on for so long are newer listeners may not be interested in going back. And I think we're a little afraid of them listening. We should take those down the first one. I think we sound like robots having the cover on the microphone. I helped out things sound. We have an amazing nutrition program as well. Volley Rivarola, Our nutritionist will put all of our prostate cancer and virtually all of our cancer patients on what we think is a very cancer specific nutrition program. It's really intended to try to give men opportunity to ask questions and to seek guidance on Those more sensitive topics that we may not feel comfortable going to the doctor for or have gotten food, somehow or in some fashion in the past, but are really unclear if that was the right way. There's a study done for sexual dysfunction that shows that men and women will bring up the issue, maybe once or twice with their doctor, But if they feel like they're blown off for Things are not done enough in the way that they feel most comfortable with. This patient is 75 years old, and he's been sexually inactive for many years recently, he said. I masturbated for the first time in years, and my ejaculate was brown. I tried it again a few days later and got the same result. What does Brown semen mean? That's a great question. Discoloration of the semen is in our experience, Mohr likely to be from an infection of the prostate gland. Now it could be an old infection that has resolved or inflammation has resolved or something active. Usually, I think of recurrent Bright red blood in the semen to be more worrisome thing of brownish or older kind of blood. If you've had a procedure done more likely like a prostate biopsy, or cist, Oscar P and have brown seamen. That's not surprising at all. I could conceive of having something even after maybe maybe a colon or rectal surgery, but I think that's pretty rare. For most people. It's just going to be an infection of the prostate or prostatitis. If it's just that you're only symptom is discoloration of the ejaculate. We generally don't do anything. You just clean the tubes out. That's your only symptom claim the tubes out between the two. That's right, Donna. It's exactly what it means. Saying those words. You have a teenager at home. Don't you know what these words? We have three teenagers at home? Yes, I do know, but I was just want to make sure that's right. Cleaning the tube. And so just getting rid of the ejaculate, I think is probably in your best interest. Your spouse or your partner is not likely to contract anything or become ill from that discoloration and that singer blood You think he's got a hand partner that the sometimes it's the most loving partner never let you down. You can. Unless you have arthritis. I guess I don't know if you have discoloration of your ejaculate, but you also have urinary symptoms like urgency. Frequency, Lower back pain fever in complete bladder emptying and that these symptoms have gotten worse. Then in that case, we almost always think that it's justified to do a on antibiotic course. In our office. If you're really worried about consistently discolored semen, we also do something called a seaman culture. It's easy as you just giving us a semen sample. Are we sending it for a routine tissue culture? So those are some options to think about with discolored semen? Interesting. Well, didn't ever want to talk about that. But thank you. We have another question. This gentleman's 50 years old. He had a prosthetic to me in 2008 with the DaVinci. Which is something you're feeling After peaky. I was able to get an erection and also used to pump for awhile. At some point I developed pero knees disease, which is our biggest topic. Correct. I'm 99% sure and have had issues getting an erection. When I do. It's just not a full one, and there's a large curvature. My Penis is also shrunk. And, yes, I am £20 overweight. I see ads for shockwave therapy and believe that I heard you all talk about it on this show that it does not work. Well, he wasn't listening to this show. But then caught a guest doctor saying that it does work. OK, I'm on testosterone shots weekly. My first count was 3 50 a year ago. Now it's 700. That's great. So just because of brevity. I won't really address the testosterone issue to really summarize what we're talking about. Here. You have a history of prostate cancer. You had your prostate removed and he developed post prostatectomy impotence, which is common that occurs in up to 70% in even in patients that have a nerve, sparing bacon still developed. Erectile dysfunction is one of the main things that people consider when they're talking about treatment. You're able to get an erection using the pump or by yourself, and that's awesome news. Would it be dangerous to start testosterone? Now he's in his sixties. The cancer was fairly aggressive, but now he's fine. So this is a great question. What it's getting at is, Is it safe? To use testosterone. If you have prostate cancer, are at risk for prostate cancer or have already had prostate cancer treatment. The short answer is that it's not dangerous. The longer answer is, there's ample evidence now to suggest that a normal testosterone level And feeling normal in your overall health is protective against cancer recurrence Better predicts whether you're going to get through treatment better and does not alter your prostate cancer risk whatsoever..

You know, One of the things that we work on together is a very unique approach to helping men increase the girth of the Penis. It is not for us to really, really, really look deeply and question men's desires for why they wanted. Why why they want to have these things come in varieties, and some men would liketo overcome what maybe you know, Genetics gave them and get something that looks better and feels more comfortable. It certainly is a very personal choice. You know, we do talk a lot about people's motivations. Guys motivations for wanting to come do this, and I've kind of Put it down to the fact that you know, whatever your Penis sizes. There's probably related to how much money you have in the bank. No matter how much you have always wished you had a little bit more that you can never be too rich for too handsome for AA. Lot of men. This idea of genital enhancements not going to be a life. Threatening one but certainly can really impact their self esteem. How they feel when they're undressed and kind of you know how they perceive themselves when they're both sexually intimate, a CZ? Well, it's just kind of how they're being looked at by others, and this is true. I think that whether you want to admit it or not, there's a lot of information in the media that has to do with masculinity and Amos size and whether it's true or not, whether it has your their ability to impact being able TTO have satisfaction with their partners. So it is a great deal of focus. It's one of those things that previously has just not been talked about. Now People are talking about, especially thanks to the miracle of the Internet that the joke on the Internet was when you got lots of spam. You're being offered pills that give you penal enhancement kind of benefits, but There's no pill, right? There's nothing out there. You know, chemically, you're especially gonna buy over the counter. That's kind of be ableto help. Obviously, I'm sure you're right. Lots of Sallis and bagger prescriptions and yes, those can help. They're not going to help with, you know, length or girth, And you know, interestingly enough, A lot of men worry about what their Penis looks like when it's erect. But a lot of men worry about how earthy and long it looks when it's placid. I have so many men that kind of try to explain away things when they're getting a vasectomy. Other shores, not growers, You know, we offer something that both improves the girth and the appearance of the Penis both in the flats it as well as the erect state. Yes, yes, that definitely can. Certainly with enhancement does achieve Mohr of shore type of appearance versus the click Grow where the grand And so in the past some things that I haven't really worked our harvesting fat from other areas and injecting into the penal shaft. Also, just like laying in tissue like Allah, derm or some kind of false kind of tissue also hasn't been as successful because the body kind of reacts to that tissue, and you could end up with kind of Not the best cosmetic and functional outcome, right? And certainly there were much more invasive, more aggressive procedures requiring more anesthesia and the results are less outcome. I mean, we do fat grafting all the time and plastic surgery. Whether it's Brazilian butt lifts are two faces there. Some practices do pack grafting to the breast results that I've seen have not been particularly Good in terms of I would want to adopt Fat grafting is part of my practice. I think another difference between the Penis and other parts. The body is the fact that the Penis gets erect. So the underlying tissue in the Penis changes from soft to hard from soft heart. So if you put something over it to make it more girly It's going to change its appearance. You know, as you get an erection, and this is very true, and also you have to graft a significantly larger amount of fat. And then you think you're going to need because you may lose 50% arm or the fat that's crafted and patients can be very disappointed. But the fact that they start out thinking things were going pretty well. And then you lose have for more of what's been added. Women more and more are getting fillers fillers put into their lips put into the creases of their of their face. On other things to kind of overcome the effect that skin thinning or muscular, fat thinning in the face might have. And so is that same filler technological advancements that really helped propel the kind of enhancement that we do, right? Yes, that's correct. It's very similar to other products. But it's the next step forward and fillers. Maybe you could describe what makes up the filler and then kind of the process of how we put it in. It's basically a small amount of an ultrahigh purified. Medical grade silicon oil that's similar to a product that's used for ocular eyeball injections, which, if we can put it in your eyes, probably safe to put on your Penis also used maybe with another product called car boxing at the cellos, which has been used with Fillmore products for many, many years. Kind of like the vehicle that that it drives that silicone in right? Yes, that's correct. And the combination of the two is what sets up this stimulation are slight inflammatory process..
