"The pandemic whether it was serious disease and illness and hospitalization or whether it was just getting access to the vaccine our community health centres and federally qualified. Health vendors have been really major players and reaching into those communities. Thank you so much for all that you've been doing in chicago As in the rest of the nation what needs to happen now is just continued effort to make sure that vaccines are accessible to these communities struggle to get access to the next in previously and then also to continue their efforts to engage with people who have questions and concerns about the vaccines themselves. We're vaccinating about five hundred over five hundred million people per day in the nation and while that number is much smaller than what it was. A few months ago it's still is five hundred million people that are getting vaccinated On a daily basis. I a weekly basis and that. That's truly remarkable so that work is happy. Those people are getting vaccinated because of these kinds of sustained efforts to earn the trust to answer the questions to address the concerns. That people have about the next scene so alternately we look at much higher rate of vaccination but it really does require the sustained effort of reshi in the community listening to them answering their questions having the messengers that they trust the most convey the message messages so that we can actually get people back from professor knows. You want to add to that. Yeah i'll just say. I'm so grateful that there are groups that are going out and doing the groundwork a grassroots approach to help people get vaccinated because i think we maybe wrongly assume that anyone who's not vaccinated as opposed to it but in part some people just haven't had the opportunity or the ability to to access back scenes because of the circumstances of their lives But i also want to stress that this is a fight for all of us. I mean the data showing Abilities of people to get back willingness. It's they're often influenced by people. They know friends and family members. So well it's important for healthcare workers and public health officials to Go out into the communities. Do the work. It's also important for all of us. Who are listening to you. Know talk to your loved ones and hear what their concerns are in an answer them Because that i think is going to have probably a lot more influence than You know some talking had that people don't have a personal relationship with You know i take your point very professor nozoe and at the same time that we get tons of of messages from people who are feeling frustrated. They say well they. They engage wholeheartedly in the long period of lockdown that we all went through. They've they did. they're masking. They washed their hands. They maintain their social distance. They got vaccinated as soon as they possibly could. And they look to those folks in the united states who as of yet been vaccinated for whatever reason and they just say like. We've done our part time for you to do your part and there's a stakes rising sense of frustration that you can even here. Let me just play this. In the voice of alabama's republican governor. Kay ivey she was recently asked about the surge of the delta variant among unvaccinated alabamians. And here's what she said. She was pretty frustrated. These folks jusin horrible lifestyle of self inflicted pain. What is it going to take to get people get shots in our tunnel. Now you kill folks supposed to have come. But it's time for the start blaming unvaccinated folk not the regular folks unvaccinated folks letting the snake alabama's republican governor. Kay ivey jim mongia. You're they're working on the ground in in laws in south los angeles like. How do you respond to that growing frustration. That i'm sure you've heard i understand it. Obviously i feel it It is incredibly frustrating. But i also think the mask mandate in los angeles for example. That's been reinstituted is a way to try to protect and take responsibility for those who are not vaccinated as well. I think it's easy to be angry on. It's a lot harder to be compassionate. And i think there's a tremendous amount of misinformation out there and people are afraid and again is it rational. No is it is it frustrating. Yes but i think the fundamental issue here is. We have to continue to stay engaged. We have to continue to educate we have to continue to talk to people over and over and over and over again about the benefits of vaccine in about the need to get vaccinated. And that's you know that's not going to happen by being frustrated with people necessarily a publicly But it certainly has to include patients and compassion. Dr morita We've got about two and a half minutes left here so many more questions coming in from listeners. Let let me just see if i can sneak in one or two of them. Sierra marie grosses a complication of relying on local vaccination rates and public health. Data in the summer is that many people are traveling. Sierra maria just dropped off a kid in a tourist heavy state to go camping. That state has a high rate of vaccination but seeing license plates from all over the country. Made sierra marie extra nervous doctrine rita. Should she be nervous. So let me just make a correction. I think i said that there were five hundred million doses of being mr per weekend. I've got my numbers wrong. It was five hundred thousand per day which orders of magnitude just wanna make. That clear thanksgiving opportunity do that. I think it's true that there is risk of people coming together From various places where back nation levels maybe difference and were transmission rates may be different but it does bring remind me or should help remind us that we do need to keep our guard up in terms of making sure that we're washing your hands. Well you know keeping this avoiding contact with people who are sick and then also making sure that there's provisions in place at the particular summer camp or play for people are congregating where testing can happen. If people become sick we're treatment is available making sure that there's a sense that there is Surveillance going on for disease and also vexing covers level so knowing when you take your children or when you're taking a trip to understand what beckton coverage levels are and these rates are in the those areas have been also understanding. What other practices are being put in place to make sure the majority of people are vaccinated or the majority or or an indoor places. People are wearing masks so that the there are extra protections that are in place based on this myth for people coming from various places We've got under a minute to go so professor another. Do you think we're gonna start seeing more of that in other places if vaccination rates don't go up significantly that we're going to see it You know i think There's probably going to be a a camera to do more And because people's concerns about not wanting any level of illness after being vaccinated. "This point i'm meghna chakrabarti and today. We are talking about so-called breakthrough cases of cova. That's when folks come down with a case of covered even after being fully vaccinated. We're trying to get the facts about how to best understand breakthrough cases and how they inform the ongoing larger public health response as the united states continues to try to get the covert pandemic under control. They were vaccinated in january but the attended a small wedding for their daughter this month and were among the fifteen people at that wedding. Who came down with cova. We just kinda let our guard down a little bit stopped wearing our mass as much just because you're vaccinated doesn't mean you're not still going to get sick. I mean we are proof of that. As far as we know it could be lessening are symptoms. You know i mean it could have been much worse. Had we not vaccine well joining us. Now is dr julie morita. She's with us from chicago. She's executive vice. President of the robert wood johnson foundation former member of president biden's transition covert advisory panel. That was during the presidential transition and she led the chicago department of public health burden. Nearly two decades dr morita. Welcome back to on point. Thanks for having me also with us. Today is professor jennifer nozoe. She's an epidemiologist at johns hopkins. University's corona virus research center. Excuse me resource center and senior scholar at the hopkins center for health security. professor nozoe. welcome back to you. So dr morita. Let me start with you. You heard travis's story in the first part of this show there. I'm just wondering sort of what lessons or you draw from what he said right. So it's interesting to hear. Travis is experiencing unfortunate for him that he got the sec. But i think the key thing in my mind was that he didn't hospital as he did not require ventilation and he didn't die. And and that's a critical success from the vaccination perspective because vaccine helped him to fight that that's infection so that's critical the other point that i think he made up. That was really important. Is this idea of turning. Up the dial entering down the dial based on other factors and versus just an on and off switch. And i think that does make sense and and i think what we have to do. Make sure that we're looking at local disease activity. Local transmission rates and local vaccination levels mean you decide what kind of public health measures need to be put in place and need to be acted upon so mask wearing social distancing capacity limits. Really make sense when you still have a lot of high. A lot of disease activity happenings community. Andrew have low vaccine covers levels but in areas where there are high that's nation levels and there's not that much disease happening. These kinds of measures may not be as necessary so we the data and the science that we have available to us early on in the pandemic made clear. The math social distancing capacity limits in indoor spaces really could make a difference and when you have a lot of disease in your have a low of poor coverage levels from vaccine perspective. Those kinds of measures really need to be considered and to be anti entity activated again. Yeah so we'll talk in more detail about Again those the dials were going. Keep using draft choices analogy here a little later in the show but for the next couple of minutes what had actually also liked to do with both of you here is sort of gets a medical facts on the table about you. Know what we know. And don't get no about these so-called breakthrough cases so professor nestle. Let me just ask you quickly when say breakthrough cases of cova. D- what do we mean. Or what should we mean. Actually or well i mean i think we have to keep our eye on. What the the big picture is you know. We should be tracking breakthrough Cases in order to understand of the vaccines are working and really the best way to do that is to focus on serious illness in part. Because that's really our overarching goal in using these vaccines if the virus that causes kobe. Nineteen we're never capable of putting people in the hospital or killing them. We probably would never heard about it you. Most people wouldn't have heard about it so really. My overarching goal and using the vaccine is just to take that off the table. Tame virus sodas no longer capable of doing that and so this is why see tracks. I'm hospitalizations among people who are fully vaccinated as an indicator of if the vaccines are doing what they're supposed to now that's an apl who are fully vaccinated. We'll still test positive and people who are fully vaccinated may still go on to develop some symptoms of cove. Nineteen that i think should be expected so far best we know and again this is not something that's track nationally but best we know from surveys. Another such things as that. It's not that common compared to the number of people who've been vaccinated But part of the reason why. I think we need to keep that in mind as as a as a real possibility in part because of how vaccines work you know if they're not a force field they don't just sound the alarm as soon as you get near the virus What they do is they train your immune system. So that when you are exposed to the virus and the virus infects your cells. Your body should hopefully respond quickly to try to limit that infection and in some cases is able to do that before you develop any symptoms at all but unfortunately in some cases as with travis Not not quickly enough to prevent the the symptoms from setting in. Okay so dr morita. Let me turn back to you. There's what about the question of if you have been vaccinated but for whatever reason you feel like you need to get tested and you test positive for co vid. But you're ace dramatic. Should we be considering those breakthrough cases and part b. of that question. Is you still still tested positive. Do we know if those folks are able to transmit than cova to other people. Because i think that's probably what there's a pretty big underlying concern. Their parents worrying about transmitting it to their kids..

How Do You Distribute A Vaccine To Suspicious Citizens?

"Corona virus vaccine to millions of Americans who may be suspicious of authority or on the fringes of society. Especially when those people are among those most at risk of covert 19. Dr. Julie Morita has been thinking and writing about these problems for months as Chicago's former health commissioner, and now she'll have an opportunity to shape these policies on a national level as part of President elect Biden's covert response, team Doctor Marina Welcome to all things considered. Thanks for having me, Ari. Well, let's start with this question of how you build confidence in a vaccine when there is so much suspicion and disinformation circulating. What do you think needs to be done right now, even before vaccine is widely available to the public. I think it's really important for people to have ah, sense of what's happening and what's been done so far so very clear and concise and consistent messaging about the vaccine development process. Venture manufacturing process. The approval process is So that people have a sense that we're not doing this blindly that The vaccine development that's happened with the co of vaccine has been fast, but it's built on decades and decades of experience in making vaccines and distributing vaccines as well. When you're Chicago's chief medical officer, you were involved in distribution of the H one n one vaccine around the city. What did that experience teach you about how to address hesitation, especially And marginalized communities. And are there lessons there that you think the incoming by the administration can apply nationally? I think we learned a lot of lessons through the H one n one pandemic in 2009. We were working frantically to get vaccine providers lined up to actually make the vaccine available, and we also hosted vaccine clinics and communities that are typically underserved. The weather physician accesses a little bit more challenging. And what we found was that these mass immunization clinics that we hosted in some of our communities of color. The turnout for the vaccines was lower. And so at that point we started reaching out to community organizations, Faith based organizations to have them help us understand what was going on in the community. Why weren't they turning out for the vaccine? Why weren't they getting vaccinated? It would have helped US organizations that are already in touch with members of that community. Right. So we were we had organization that we were already partnering with. We reached out to them and ask them to engage with us again, and it worked to some extent, but without we didn't get the success that we were hoping for, and so building on that experience in non pandemic times, the Health Department and our partners worked established relationships on ongoing time. Between pandemic so that we would actually have these relationships and could really rely on the information from those community groups to inform our planning for future pandemics. The current health department is actually tapping into in relying on those relationships now to really help to get the information to the communities and understand what their questions and their concerns are. Interesting, But let me ask. There are real questions about vaccine safety. I mean yesterday, the CDC Immunization Advisory Committee recommended to give the first round of vaccines to health care workers and residents of long term care facilities. But there has not been a lot of research into the effectiveness of the vaccine and the safety of the vaccine in the nursing home population. How can experts be sure that it is safe for them? So the information is being evaluated, but it will be evaluated by FDA to look at the safety and efficacy to understand who the study populations included. Information that I've seen so far suggested the vaccines how are safe and effective in a wide range of age groups, a wide range of races and ethnicity and someone that information's fully evaluated by the FDA is advisory committee. Next week. We'll have additional information to really inform the recommendations with CDC or the C I. P did yesterday was really make recommendations in anticipation of these results. But they can be refined based on what the FDA and their advisory committee actually determined. Now the CDC is reputation has taken a hit during this pandemic. Guidance on testing and school re openings has sometimes been influenced by political pressure. So how do you think a new administration can restore confidence in the agency? So I think one of these like when I think about the vaccines situation currently, the key thing to keep in mind is that the E C. I P is not. It's the independent group of physicians, scientists, public health officials who come together and really review the data River to review the facts, review The evidence and the makeup, eventually Advisory Committee that's the advisory committee and they make recommendations to CDC. And then CDC that makes recommendations and buy a large those recommendations of the pipe makes the CDC usually fit makes us well. And so we'd be looking for after the recommendations are made by a CFP. What a CDC actually say is their alignment. Is there agreement with what A C I P s saying as well. Now, as you know, distribution of the vaccine is going to be handled by the states. And a couple weeks ago, I spoke with the head of Maine's Center for Disease Control and Prevention, Dr Nirav Shah, and he told me his state does not have enough money to build the infrastructure required to make sure everybody who needs it gets the vaccine. Here's what he said. We at the state level are ready to receive the baton to have that baton pass to us to begin the vaccination process, but without proper funding. It'll be like putting up a tent poles without having the tent. And he said, This is true of many other states, too. So Dr Marie to do you think the federal government is ready to provide the money that is going to be necessary? I think it's important to provide the adequate resources to public health What I've seen in the past in public health emergencies during my 10 20 years, the Chicago Department of Public Health Was that there would be large Pulis's of funding that would come to public health agencies of the state and local level during a crisis and then shortly after the crisis, But then the funding would actually go away. And so public health infrastructure really has suffered over time, and there is a need There's a strong immunization system that's in place, but it has to be enhanced that it has to be strengthened so that actually, people can actually get the vaccine in an equitable fashion.
