"That auction yesterday, the two year auction yielding the highest since 2006. Is the bond market more sure than the policy Maybe, market? yes. In our view, the central bankers, and then I especially talk about the ECB, is way too optimistic still in its latest projections about the economic growth. They expect actually a pickup in growth in the second half of this year, which seems a bit at odds with the more than 400 basis points in rate hikes, and the fact that that still needs to filter through to the real economy. So we expect much slower growth. Actually, we expect a mild recession and I can maybe call it stagnation. At the same time, the ECB also expects inflation to remain high for longer, while actually the key here is the labour market and we do see now early signs of the labour market turning, weakening, we see wage growth having reached its peak, coming down slightly, still strong, but it's coming down. So we do expect inflation to drop quite significantly before the end of year. the So that will set the path for actually rate cuts next year. Okay, but if you're talking starting to kick in in a more material way, the economy is starting to feel it, are we also going to start feeling it in a more material way in credits? Are we going to see defaults actually pick up in a way that they haven't yet, as some may have expected? Well, there are early signs, for instance, Eurostop publishes about currencies, and these see a continuous rise in the past couple of quarters. So I think definitely also with a lot of companies and households having benefited from government compensation schemes, that is likely to be much less next year when governments need to curb expenses. Dr burks and dr fauci. What did you take away from what they said yesterday. And what's your view of these incredibly stark numbers. We just saw good morning to everyone. I think that take away from yesterday was actually both an incredible dire warning about thanksgiving about the numbers that we can expect and then also a ray of sunshine. If we can actually do what we need to do. Dr fauci spoke about the extraordinary. And casey mentioned it. The extraordinary progress we've made with vaccines from operation warp speed as well as global cooperation. But a vaccine does not mean that you can actually just kind of read the sigh of relief and go about your merry ways. It really does mean is. Dr burke said yesterday that we have to do things today and change behaviors today in order to be able to have a light at the end of the tunnel and benefit from the vaccines and willie looking at those pictures. I can't help but shudder and fear that some of those people in line are actually doing testing so that they can go and celebrate thanksgiving. I i know that a lot of what my patients are saying. And and that is frankly a big concern your task force and the cdc again was clear yesterday. Small groups within your own household and do not even travel at this point for thanksgiving. That's about as serious as that warning could be so dr patel as you rightly. Say great news on the vaccine front but for most people in this country. That's at least what six months away to get that so we have six months. Let's say from here to there where anything can happen. If we don't have further mitigation efforts from your point of view based on these numbers that we're seeing the trajectory you say were on and that we are on should we go further nationally or at least state by state in terms of stay at home. Orders for example mask mandates. What would you like to see. Yes this is absolutely the conversation that i hope every governor again because we have no national strategy what we did not hear yesterday was kind of an announcement or a bold proclamation about what we can count on at the national level. Especially if you're going to go and celebrate thanksgiving with people who are not normally in your household. Here's something that i think is incredibly controversial. That happening were closing schools. Before we're making mold steps close or reduce indoor capacity of a restaurant than places where we know people are getting together. I would do the reverse. I would not penalize younger children where we have seen that. We have not been having as much of an outbreak as we have in other settings that people are moving towards indoors shops and restaurants and then i really hope that on a policy level that we can resurrect some stimulus funding because these businesses especially during a holiday season are dying on the vine and again. It's all unnecessary. We've had zero ability to offer small business owners an away to have reliable testing to even keep their employees safe. And that's what we've decided to do in this country. So that's what i want to ask you about in terms of testing dr patel's a former white house health and policy director. I know the biden team says on day one. The president plans to invoke the defense production act on things like testing these lines. That we're seeing Football fields of cars people waiting for hours and hours sometimes days to get a false result on testing is a massive almost the number one impediment to mitigation because as they're waiting in line. They're missing work if they have work. They're exposing themselves to people as they're waiting for these tests and these test results that sometimes take days to come and for those who are lucky enough. Maybe they can buy a test. But it's not like that for the rest of america. Who is in this situation right here. Which is so unacceptable. Had president trump invoked the deep on testing. Would we be in this situation right now. No your outrage. I can't tell you how much that outrageous felt by so many healthcare workers around the country. Here here's something to wrap our brains around. We were able to develop to at least two highly efficacious vaccines and cheaper than it is to get a pcr nasal swab which your head around that that is unacceptable that somehow we have decided that the ability to actually offer employers to open safely away to actually keep schools open in a way for americans to feel. Testing does not replace masks and prevention whatsoever but it certainly could be a critical piece in finding those asymptomatic carriers the majority of people we know who are either infected or are able to transmit the coronavirus right now if we had reliable cheap and easy testing. It doesn't even have to be perfect makeup. We could do something every three days. We know that that's probably the time period where we could catch the majority of cases and this is just a system failure on all levels that the fda is cdc initially relying on flawed testing in the beginning and then again no national vocation of the defense production. Act at no pressure to try to identify how we can get testing especially to the hardest hit communities black and brown communities where we still have really pathetic testing eighth and turnaround times stability and the and the needless loss of life here is mind. Why Shame Is A Bad Public Health Tool Especially In A Pandemic

"Believe me I get it. I'm frustrated and angry to. After all, it's been four months of this. We know the right things to do. And when you see someone wearing a mask or groups of people hanging out close together, it's easy to get mad, even if in all fairness. Once or twice. Open defiance at this Castle Rock Colorado restaurant large crowds, no social distancing, and there's some news coverage right now. That caters to this anger. You know what I'm talking about. Many Americans are out and about on this memorial day visiting newly reopened businesses seems from the unofficial kickoff to the summer showing many Americans not practicing social distancing measure. I'm telling you to wear a mask where a damn ask, but this Kinda thing anger public shaming the urge to yell at people who aren't doing the right things. That can be precisely the opposite of productive. Yeah, as the researcher I've been. Watching all this unfold through that Lens Julia Marcus is an epidemiologist and professor at the Harvard. Medical School, she said he's HIV prevention. And for scientists Julia, who work in HIV or sexual health or even substance abuse? They know that shame can be a huge barrier when it comes to public health, and in these first few months of the Cova pandemic I was watching this same pattern happen where you know, these kind of absolutist public health messages and moralistic undertones were potentially contributing to what became rampant shaming of people who were flouting public health guidelines or doing things that people felt. Felt were high risk, and when we shame people for their risky behavior in a way that distracts us from where risk is really happening, which is typically much less visible like in prisons and nursing homes and food, processing plants, and those don't inspire the same moral outrage. I think for two reasons one. They're not right in front of our faces, but also to we don't think of those as people having fun and a pandemic which I think people really upset. Matt rage, Julia says might feel good to act on in the moment, but it's not gonNA solve our biggest problems right now. I find that taking that rage home, and really screaming alone has been very helpful for me to. Do that as well or you know my rage these days first of all I would say that knows no bounds, but also. To be honest. My regions more directed at institutional failures than individual ones. To episode Julia Marcus on the role. Shame plays in public health crises. We talk masks. School reopenings in the long road ahead. I'm Maddie's defy, and this is shortwave daily science podcast from NPR. Julia Marcus has written a bunch of great pieces for the Atlantic about why. Shame is not helpful right now and how we can do things better. She's looked this when it comes to mask wearing social distancing and how we open college campuses, we talked about all those things, but the first thing to say here is that there is a fine line between public shaming and some positive forms of peer pressure. I, yeah I WANNA make a distinction here between social norms and shaming I. think social norms are very powerful and. That can be one of the best ways I think to change. Health behavior is like well. Everybody else is doing it so I'm going to do it because it's more like i. want to feel good when I go in the grocery store and I'm not gonNA. Feel great if I'm the only one not wearing a mask, so, but there's a difference between making people feel bad about their risky behavior and making people feel good about engaging and protective behaviors as a way of like becoming part of What the new social norm is Marie right? Right Okay Julius. You've written a bunch of great pieces for the Atlantic. Let's talk about your most recent one I. It's you know how to not open colleges this fall. You started out by describing an email that went out to students at Tulane University earlier this month July seventh. What what happened there? Yeah I mean I I I don't WanNa. Pick on two lane here. Becher, that was it just an example of some of the communications that were starting to see toward students who are on campus this summer and have been having some parties. And there was an email that we're not to students that really condemned stat behavior as disrespectful, indefensible, dangerous selfish, and made it very clear in bold all caps that hosting parties of more than fifteen people would result in suspension or expulsion from the university and that if students wanted the school to remain open, they needed to be personally responsible. I'm in their behavior and When a university says, we will hold you accountable for having a party, and actually there will be dire swift punishment when inevitably there is an outbreak at a party. Students are going to be terrified to disclose that they were there. And students have now said this at the University of Connecticut were interviewed and surveyed about what kind of thing is going to work for them what their concerns are about the fall. And they universally said we. We are early close to universally said we're really afraid of how infection and risky behavior are going to be stigmatized such that we outbreaks will not be able to be controlled, so there needs to be appropriate consequences for putting your community at risk, and I would never say otherwise but that needs to be balanced against the need for public health efforts to be separate from discipline. And we've already seen contact tracing start to break down outside of campuses, because people are afraid to talk about having been at event that that they know is something they should not have been doing yeah. So. You know kind of following that thread. The part of this pandemic that's been hardest for a lot of people is is social distancing in in several of your pieces you wrote about how a lot of the advice especially in the beginning was almost like an abstinence based approach like stay home. See Nobody which absolutely made sense kind of at the. The beginning, but tell me about why. That approach doesn't necessarily make sense for the long-term well asking people to abstain from all social contact indefinitely or until we've scaled up. An effective vaccine is just not going to be a sustainable public health strategy, and I think now our messaging has evolved a bit especially as there's been an accumulation of evidence around. The risk is highest like what's settings or higher risk, in which ones are lower risk, but I think we continue to still have a tendency toward absolutist messaging and I think that our goal should be to two inch. People tour to a place where they are living their lives in a way that addresses all aspects of their health, while trying to keep tr- risk of transmission low, and so one way that that could play out is encouraging outdoor activities, especially in spacious areas, opening up more outdoor space for people, and there's been a tendency to close beaches and close parks where people gather, but. But I actually think doing the opposite on could could be helpful, but the essential point is. We can't stay in our homes forever and many people couldn't stay in their homes for the last few months because they were working sure, but it's clear from other areas of public health that asking people to abstain from something that they fundamentally need or strongly desire is not an effective public health strategies, so we have to find ways of making our messaging more nuanced, that allows people to get what they need to be able to live sustainably while keeping the risk of transmission low until you there. There are examples of nuanced messaging from others accessible public health campaigns. Right I. Mean You work on HIV? Can you give me an example of that? Yeah, so we you know we don't tell people don't have sex. Because that's the best way to not get HIV, we may save the safest thing you can do to avoid HIV transmission is not have sex, but we understand that many people are going to have sex, and that it's a you know a part of a healthy life, and so here are some safer ways to have sex, both in terms of certain sexual acts in in terms. Terms of protection different ways you can protect yourself and you know becomes a more nuanced message, but it's much more sustainable for people and realistic and the long term, and it also acknowledges people's basic human needs right, and there's also this idea that talking about ways to reduce risk encourages people to take those risks, even though from a public health standpoint. We know that isn't true. So I'm wondering Julia like. Why do people hold onto this concern? Like what is this really about yeah I, mean this is definitely not new. It comes up a lot. I think especially around drug, use and sex. And I think the reason it especially comes up in those settings is that those are behaviors that we have a lot of moral judgments about particularly in this country, and there's this kind of moral outrage that happens when we think about people engaging in risky, which is often pleasurable, behavior, sex, drug use, and these days going to the beach like. it's kind of playing out in this new way now with social contact and partying and people having a good time in a pandemic, which it's actually a public health win when we find ways to support people in enjoying their lives, and and getting their basic social or sexual needs, met while remaining a safest possible, and you've made the point that we've. We've already seen this play out with the corona virus, public health officials, hesitating to give people detailed ways to protect themselves instead of avoiding risk altogether, I mean I remember. We reported early on in this pandemic when Dr Burks of the White House Coronavirus Task Force said. We don't want people to get this artificial sense of protection because they're behind a mask. This lack of consistent messaging is one of the reason that a lot of people still aren't convinced that masks are helpful, so you know. Julia, how do public health officials effectively reach? Those people yeah I mean I. Think in general we always see some resistance to any new public health intervention, condoms, and you know pre exposure prophylaxis for each V. I mean every intervention that comes out. There's resistance. There's challenges with implementation. There are moral concerns you know. This is all kind of par for the course, but I think what's new here and a bit different is not necessarily just the polarization which we do, see an Ciaran things like vaccines, but the politicization. Politicization I don't think there has been I can't think of an example where a sitting president has flouted public health recommendations and I think that that has created kind of a politicized around masks. That wouldn't have necessarily been there and so how do we overcome that? And how do we reach people I think again it comes back to hearing people's concerns, acknowledging them, and then working to overcome those barriers in our messaging and I. Think there are some good examples of that there have been a couple of great mask campaigns that have come out of California acknowledging that people dislike wearing them and acknowledging the reasons why people dislike wearing them. And I would guess that they are more effective in reaching certain populations than campaigns that that are more focused on this. Just wear ask. It's really easy kind of messaging. Yeah and don't you care about your community and don't you want to not kill people and That kind of messaging is like early days of AIDS. Messaging around condoms that I think was not as successful as the messaging that really focused on what the barriers were, and how people could overcome them. Yeah, yeah, with all this stuff that we've been talking about colleges masks. You know keeping safe distance. It's pretty tough because the stakes feel so high like this is really a nasty virus, and when we see people, you know not doing the right things, the instinct there to shame them to get mad for a lot of us at first instinct and I. I guess it's just that we need to take some patients to push past them. Yeah, I mean I, think it's really. Valid to feel angry about what's happening right now, and for people who are not necessarily taking care of themselves or their community and putting other people at risk. It's very frustrating to see, but I think especially for public health professionals. It's on us to do the work to avoid the shaming and the anger and the moralizing in our messaging. Because we've learned that that doesn't work in other areas of health and really try to take the time to craft messaging. That is going to be more effective. Julia Marcus. "The United States about to hit forty thousand new corona virus cases today. The first time that we've had that many one day, the death toll in the United States closing in on one hundred twenty five thousand people, but if all you did was listened to the president and vice president today, you would be absolutely stunned here what I just said because this is what they said. Today? All fifty states and territories across this country are are opening up safely and responsibly. Well, that's incorrect. It's factually incorrect. They're not I mean eleven. States are currently on pause. Some of them are actually backtracking and closing some things down on plans to reopen Texas and Florida specifically. They are rolling back reopening plans governors of the other nine states. They are not moving forward with the next phase of reopening. So that was incorrect, and then the vice president went on to make this claim. The truth is we did slow the spread. We flattened the curve. So he says we flattened curve, so let me just show you America's curve. So you see, the surge right and then in in March and then you see the plateau. And then you see. The jump there at the end where we are now getting to fifty forty thousand cases today. That was a plateau, and it's on its way back up and I want you to compare the graph to these other graphs. These are countries around the world right? They have that surge at the beginning the UK Germany France Italy Japan South Korea European countries measured in thousands of cases Asian in hundreds, but look what happens at the bottom. They go all the way back down and stay that way. Ours never went down right, and now it's going back up currently with the same steepness of slope as it did at the beginning. And yet the vice president was taking a victory lap during today's briefing and he made sure to credit person number one. Under the leadership of President, trump is the president's made clear credit I believe to our president of the president made that decision. So, where was the president during this briefing while he was somewhere in the vicinity, but he was on twitter, talking about confederate statues, tweeting this image of people suspected of trying to vandalize statue of Andrew Jackson and what he was writing was that many people in custody with many others being sought for vandalisation of federal property in Lafayette Park Ten year prison sentences. It was hours later that the president did finally say something about the pandemic. That's amazing raging right now in the United States like nowhere else here he s. We have work to do that. We'll get it done. while. That's an understatement it. We are learning tonight that people traveling from the United States most likely will not be allowed to even enter the European Union. Banned, because the United States has gotten the virus under control Kaitlan Collins is out front. She's live outside. The White House Tonight Caitlyn. No briefings for eight weeks the White House. Felt like they had to do something here, but then they came out and said everything's opening you safely and responsibly opening fifty states. When that's just you know factually untrue. Yeah. It wasn't the message that some people were expecting the vice president to project from that briefing today after Wednesday's coronavirus task force briefing, and after you're seeing these numbers from this week including yesterday setting a record high of cases per day since the month of April, of course, which is the last time that they had these coronavirus briefings now you would that they would have. have taken more questions. The vice president instead of Aaron only took a handful of them, but before he started taking questions, you saw him arguing saying that he doesn't want the American people to think that because of these new surging infections that the United States where it was two months ago, even though the case numbers per day or mirroring that or beating it and. And of course, the vice president went on to talk about what precautions Americans should be taking. He ticked off a list of things that the CDC has recommended. But Aaron he notably left out wearing a mask when a reporter asked about the fact, that mask have become this political issue. He then only told people that they should be following local or state guidance ignoring the. The fact that the CDC a Federal Agency of course, has recommended that people wear masks when they are out in public, and within the vicinity of other people. He also defended those rallies that he and the president have been holding and encouraging their supporters to come to where thousands of people are put indoors with very little social distancing errand by saying that it was people's. People's right to the first amendment characterizing it more as a personal decision than something that they're organizing and urging people to come in to attend, but I do want to note to striking things that came out of that briefing today was the vice president offering a pretty rosy assessment of these numbers, and then Dr Falcon getting up there and having a very sobering warning. What's to? To come and personal responsibility and this, but also Aaron the vice president saying he believed it's an arguable that the reason that there are more cases in the united. States because there's more testing Dr. burks got up and she was going through these slides and one of the ones she pointed to was Texas where it showed in May as they were increasing testing their positive. Positive test rates were going down in the last two and a half weeks they continue to increase testing, but now those positive test rates Aaron are going up, and that completely refutes with the president and the vice president have been telling people and governors for the last several weeks. All right, thank you very much. CAITLIN absolutely makes the crucial point. Right the percent test rate. Rate is what they're looking at twenty percent of them. Positive that that's that's the relevant point. It's not the absolute number and Caitlyn I. Appreciate Your Time, so let me go now to Dr. Sanjay Gupta and Dr Jonathan Reiner. Saying that almost euphemistically I thought there was going to be an acknowledgement that look. We have some problems. Significant ones in here is our plan to address them. Here's what we're GonNa do we're convening the coronavirus task force to to address this? We didn't see that at all. I mean there's the last task force briefing. So some of this is increased testing in areas where we know are the highest risk whether it's nursing homes whether it's areas where people work and sleep and stay together transport together and really getting proactive testing out there to find cases before there's community spread. I think you know from the Arkansas case. A lot of that was associated by a social gathering. And that's why we really made it clear that during this reopening social gatherings should not be more than ten people even if they're outside because you still need to maintain that social distancing but but exactly as you said Freddie that people can go out to beaches. This weekend is as long as they stayed distance. But when you look at images of these large crowds at beaches on Saturday at outdoor restaurants water parks not keeping social distance. Does this still make you confident that reopening beaches and parks was the right call? I think it's our job as public health officials every day to be informing the public that what puts them at risk. And we've made it clear that there's a symptomatic spread and that means that people are spreading the virus unknowingly and this is unusual in the case of respiratory diseases in many cases. So you don't know who's infected and so we really want to be clear all the time. Social distancing is absolutely critical. And if you can't social distance and you're outside you must wear a mask. These are items that are really critical to protect individuals. We've learned a lot about this virus but we now need to translate that learning into real change behavior that stays with us so we can continue to drive down the number of cases but I guess that's my point you're not seeing it across the country. You're not seeing those speeches and I think that's our job to continue to communicate and I think we have to communicate through different venues making sure that our generation sees our millennials can help us get that message out there of how to be together socially yet distant and I think there is a way to do that. Americans are amazingly innovative. And I think we really just need to have better. Continuous communication on how important that is and then highlighting these issues that. Come up like in Arkansas with this pool party. This is why it's really important that you maintain those distances and again to speak to those who the vulnerabilities Komar both in phase one and phase two of openings. We had asked you to continue to shelter in place because we know that those co morbidity put you out of greater risk for more significant disease and so those two pieces need to continue to happen where people who have commodities are continuing to shelter while those who don't are going out but for maintaining social distances.
