Well, I talk about my mental health problems on all social media platforms specifically because I have. Known a school borderline personality disorder. which has quite a lot of negative connotations around it about like stereotypically will people alike with it and I wanted to show people you lot of different people that you wouldn't expect might have. and so that was ready wireless. So vocal on social media platforms and take talks a great. site to solve tell funny stories from your podcast, and because I've been mentally ill for quite a while. Most of like my funny entertaining stories are often around that. Yeah really love it over Steph. Sort of check out your account is really great end enough sort of how frank you're TIKTOK. Really interesting. I think it's one of those things that people my age of. Any like sued if anyone twenties is still kind of going on this thing, I would we actually do with it but basically frank chats with people. On Yeah She's always Kind of response if you had some people, do you have a find it difficult to just about your mental health them in that sort of way because I know that people combative. That can sort of get into into dams. That sort of stuff have you found it? When I was younger, actually my school didn't assembly about time that I had tried to kill myself. which was the first time I'd ever have people of coming into my DM's saying, Oh, I, heard this happened to you and you get a now with talking about it as well. You get a lot of people that will dm you and say, Oh, I'm ready struggling with my mental health and sometimes it's difficult because if you integrate place with your mental health, it's very difficult to help others and you can feel of responsibility when you have a load of twelve, thirteen, fourteen year olds mass you saying that they're ready L. and they need a little help and then getting it so it can be a lot to take but generally. I don't, mind if. People make. Negative commands all criticize me talking about it because I personally don't see a problem was talking about it. I'm very careful about what I say I didn't give people especially in regards to Anorexia or self. self-harm a lot of people when they talk about it can accidentally give tips. I'm very careful with that so I have. I'm like a Schule that what I'm saying is not going to negatively impact people. Some people think on the Internet you'RE GONNA end up offending someone. Yeah. Couse. Do you have sort of boundaries in place in terms of what should go online and instead of what you'd have? Generally, I try not to follow too many people with mental health issues I mean I. Obviously I follow a lot like to have variety or I think you can sometimes become too consumed and people can post a lot of. Quite upsetting sometimes because people can pass like I'm feeding ready down. Awful. So I sort of like to try and keep a variety to make sure that I didn't become too embroiled in just the negative stuff. This is all new to me to be honest Kazakhstan. The TIKTOK side of things new to Mina city of on Youtube another social platforms. But for me sort of twisters, the place that I. Got It to chat to be about mental health stuff generally, and I'm still getting to grips with TIKTOK. What was the community like on Tiktok in terms of mental health checks on a straight feels fairly supportive of. TIKTOK is very supportive on it's mostly. So of Gen, Z. Kids. And I think as a whole always really sports each other the thing about Tiktok that's quite different to a lot of other platforms is that every time you release a video, it can be seen by a whole new audience of people. If it gets picked up by the Algorithm, it will get to it can go out to hundreds and thousands of people. and so you're always interacting with a new audience. From tiktok in. Particular I've never had many negative comments to be honest. They've always been really quite leaned.

"You know that I used to love for its variegated sort of silvery and green leaves. kind of almost looks like an IV ish Gr-. vining but low prostrate thing lamb the astrum galley. Think a ground cover is a plant that increases in numbers over time but does not run away or spread too fast it's usually we'd suppressing that's what we hope and we have a couple of those. And you think of ground cover is something you can walk on, but there's not a whole lot of plants that will tolerate. Being walked on besides grass lawn but a ground cover is anything that could do what I suggested I. You know spread a bit and suppress weeds and it could be seven feet tall. It can be a big SHRUB and I've seen that. But. I. Guess. What do you think is that just about it? Yeah, and and like what you said you know we think when we hear if you hear the phrase ground cover, you would think, oh, turf can walk on it but there really ain't no such things I mean there are so few things that can tolerate that I mean maybe creeping timing lawn, you could technically walk on but you know really almost none of them so So yeah. So it can be any hide I completely agree it's it's it's maybe a living mulch is. Is the. cloudy a west of landscape designer of fido studios. She says, plants are the MULCH. You know that's one of her key phrases that we need to remember and I think in a way. Like I have a lot of masses of ground cover. Like Geranium Macrocosm, the big route Geranium. So, I find that he's unit, you can just edit it. "It is a tough time to be looking for work. We're hearing stories all over the country dozens and dozens of people competing for a single job opening there just aren't a lot of open positions right now unless you're chat Dolan, we have so many jobs open. You wouldn't believe it chat president of Midwest, staffing in Minneapolis they help companies hire workers. All over Minnesota and Chad says, he has a ton of open jobs. Let's see. So we place welders, we place machine, operation? People General Labor where how shipping and receiving we have an entry level HR role and administrative assistant role I e reading me a list of like what you're currently hiring for. Yes. Just some of the ones were curly hiring for. We have hundreds and hundreds of open job orders right now. Meaning Chad wants to hire hundreds and hundreds of people for full-time jobs with benefits. So Chad has basically the Oprah of jobs out there like you get a job, you get a jog, everybody gets a job except Chad says, he cannot find people to take these jobs. So I have been in this industry for over twenty years. Never in my career have I experienced a situation where we literally have people to work, but they won't go back to work. I mean, we have clients who saved US five people a week. If you can find them for us, we can't find them and when it first started happening chat thought I know what's going on here? What's been challenging for us our our average pay rates it's all about the Benjamins, our average pay rates for candidates. Anywhere from fourteen to eighteen dollars an hour, we weren't able to pay them enough to put them back to work. So Chad, was doing the math back in April when government was paying out in extra six hundred dollars in unemployment as part of the cares act that Congress passed back in March and Chad found was that people could actually make more money off of their unemployment benefits than they would by taking one of his jobs and. It was not just Chad's Math pro-west that led him to the conclusion a situation where I called a candidate up I explained the job to them. It was. It's exactly what they've done in their past and literally laughed at me and hung up the phone they laughed at him. That's crazy. Chad thought as long as those extra benefits earn place getting people to take these jobs is going to be a problem but when that extra. Six hundred dollars went away the end of July Chad's problem didn't. So it was not the six hundred dollars. Chad started thinking about other possibilities like maybe it's the location like maybe there just aren't a lot of people looking for jobs in Minneapolis right now but that doesn't really seem to explain the mystery either because right across town from Chad Keith rose says he's been desperately looking for work. At a company setting up for Conferences Expos back in March he was furloughed for two months and then another two months and another, and since the pandemic started I am over one hundred thirty applications and I've had only eighth interviews since then and I've had no job offers. So what is going on? I get this question all the time that tiny shareholder. Senior economist at the economic policy. Institute my first question variety is there like some kind of worker shortage happening the answer to that is just an absolute no. Right now, there are quite literally more than eight million more unemployed people than job openings. So there's an absolute excess of workers who need jobs over and above job openings that are posted. Firefighters overtaken by flames in California mountains
"Several firefighters were overrun by flames while trying to protect a fire station in the rugged mountains near the central California coast. Dr. Rachel Dolan Discusses The Antipsychotic Drug Epidemic
"Welcome to the healthcare policy podcast on the host David Intra. Kosovo. With me today Dr Rachel Dolan the US House of Representatives ways and Means Committee majority staffer to discuss the majority staffs recently released report titled Under enforced and over prescribed. ANTIPSYCHOTIC drug epidemic ravaging America's nursing homes. Dr Dole and welcome to the program. I David thanks so much for having me. Please call me Rachel. While this'll be the last time Dr Dolan's bio is posted on, of course, the podcast website. In testimony before the House Energy and Commerce Committee two, thousand seven, the FDA's Dr David Graham stated quote. Unquote. Fifteen thousand adults elderly people in nursing homes are dying each year from the off label use of antipsychotic medications. For an indication that the FDA knows the drug doesn't work the problem has been only FDA for years and years close quote. Legal the FDA does provide a black box warning label. Regarding off label use of these drugs, eleven years later, Human Rights Watch published a report titled They Want Docile. How. Nursing homes in the US overmedicated people with dementia. The report found in two thousand, sixteen, seventeen quote unquote massive use or abuse of Anti Psychotics, for example, Sarah. Quel. Doll and Rispler doll that have serious side effects including sudden cardiac death. The human rights report estimated in an average week over one hundred, seventy, nine, thousand, long-stay Nursing Home Facility patients who administered antipsychotic drugs. Without a diagnosis which the drugs are indicated or approved rover, polar disorder and schizophrenia in testimony the ways and means. Committee. Heard this past November Richard Mollet Executive Director of the Long Term Care Community coalition concluded quote the use of San Anti psychotics in skilled nursing facilities is so extensive that puts the US in violation of internal conventions and covenants on torture and cruel inhumane and degrading treatment or punishment. Close quote. This is my third related interview. In December twenty twelve I discussed the topic with Diana Zuckerman. And in February, eighteen high interviewed Hannah Lamb who authored the above mentioned human rights report. With me again to discuss the ways and means report just released titled Under enforced and over prescribed is Rachel Dolan the reports lead author. So that Rachel as background let's get right into this or immediate neatly into the specifics of the report. What did the report find regarding the extent to which? They're persists overuse or misuse of anti psychotics in skilled nursing. David. So the report showed what what you what we would expect from your introduction, which is the use of antipsychotic does persist in nursing homes across the country and it remains quite high and not of course, has implications for patient safety and and health We found in the fourth quarter of Twenty nineteen approximately twenty percent of all skilled nursing facility residents in the US. So that's about two, hundred, Ninety, eight, thousand, six, hundred, fifty people every week received some form of antipsychotic medication and most of that was without any psychosis diagnosis for which these drugs are indicated So specifically, we actually looked at trends and surveyor citations for unnecessary medication use in nursing home. So that's kind of the. Part of this study and what we found was a clear change in citation rates for these facilities between the change in administrations from the Obama Administration to trump administration So we found citations for antipsychotic misuse in sniffs increased by two hundred percent between twenty, fifteen, twenty seventeen but then declined by twenty two percent from two thousand, seventeen to twenty eighteen, and importantly a ten percent of citations associated with actual harm or immediate jeopardy to a residence health or safety. So those are some of the most severe citation surveyors ever capture resulted in no fine from twenty seventeen to twenty eighteen under the trump administration. So you know. I. Would say even though this study in particular couldn't determine causation we we did see a clear association between the Trump Administration's regulatory rollback campaign twenty, seventeen, twenty eighteen and a reduction in citations for these particular drugs. Okay thank you and we'll get into the trump administration's regulatory decisions in this regard in a minute let me just ask as a follow up or an aside question and I don't think I saw this new report. So you may not have these numbers top of mind but worth asking, can you give an approximation of the cost? To the Medicare program at least relative to the overuse I, mean, this is a massive amount of money in reimbursement for these medications. I don't remember offhand. Let's see I think in the in the actually in the report we got About one third of older adult Medicare part d enrolling with dementia who spent more than one hundred days in a nursing humber prescribed antipsychotic in two, thousand, twelve constituting roughly three, hundred, sixty, three, million part D plan payments that year and of course, there's also cost associated with hospitalizations for inappropriate use of these drugs So I would expect you know that that that is obviously very under an understatement understated estimate that does not capture the full realm of payments. So it's it's fairly substantial.

Be Okay and no more than a shot of Tom Hardy's planes silently gliding over the beaches of dunkirk knowing his job has been done. dunkirk is also uniquely Christopher Nolan Movie One where his signature use of time has never melded together better and more cohesively than it does in the final twenty minutes of this film while Dunkirk will understandably never be a wholly enjoyable or easy rewatch. It's one that gets richer and richer with repeat viewing. Now, Adam Mention Nolan signature use of time, and that is one aspect of dunkirk among others that certainly connects to his entire body of work. But there are other aspects that Mark Dunkirk is unique. It is only film that is based on historical events, which means it is inherently rooted in the past and yet I'd argue it is only film that truly feels present tense meeting there are no. Dead parents or wives or forsaken children or other tragic events haunting the air's barrier played by Tom. Hardy the Moles Army private Tommy fiene whitehead or for the most part the sees Mr Dawson played by Mark Rylance like all Nolan protagonist there's plenty of personal sacrifices on display but no guilt no sense of futility about toning past actions just the seemingly futile need to survive the current event that they. Are Tragically experiencing here's something else that argues unique about dunkirk as we every other commentator on Dolan's work is discussed in some form or fashion for all those dead parents and wives and foresaken children. There's nothing so emotional or borderline sentimental in any of his previous films as the climax of inception and yes, mileage may vary there which led to interstellar and not only mcconaghy's crushing breakdown watching twenty three. And if so is that what establishes Dunkirk as his best its balance of coldness the sober portrayal of the hell of war of life in the misguided choices, some men will make when facing death with warmth. In the compassionate inspiring choices some men will make to maintain a way of life. So a lot of subtleties in that question and you're onto something so you're not crazy I think I would distinguish. The word blatantly, I. Don't think this is blatantly emotional or moving, but I do think you're onto something in the fact that it is maybe his most heartwarming in where it ends up and let me kind of try to parse those. You're right as long as the Nolan Canon has convulsing McConnell. Interstellar is going to be his most blatantly emotional I mean you can't have a scene like that without registering that on it surface. That's his most emotional movie. But I will also say that I find Dunkirk to be his most emotionally affecting movie maybe even more. So than inception, which we about how I did find that very moving especially in the character of Mal. So yet dunkirk is really a unique thing even as it's working within the strands and trends that we have been tracing in Nolan's other films, I. Think the heartwarming aspect is probably a part of that. You've nailed it there in that it brings us to a place that. Is One of Constellation and I think it's earned because I think up until that point we have just been suffering alongside all of these other characters in so many ways and we've also been recognizing as the movie does that this is You know a an account of a military failure, a huge defeat and so for the movie to kind of offer, some sort of solace at the end of that I think is. He was held down in Minneapolis ten years ago held down with a knee to his neck. For four minutes. Obviously the same officer Derek John It was not involved in this interesting incident needs to be said. He passed away. Same town same exacting. Four minutes passed away. Internal Affairs investigated. Police officers were cleared. As far as I know, there's no cell phone video. David Cornelius Smith this incident back in two thousand ten. We didn't join it because it was like a bunch of lip syncs and stuff like that and I- scarred myself from ever lip synching again when I was fourteen We talked about this in the last episode but war sweater and I stayed home And I skipped the date to make a lip sync mine but the lip sync energy. I mean it's still there and it's fun and that's fun for the kids on talk but I feel like there's a lot of stuff on there that actually genuinely makes me laugh so it kind of caught my attention and brought me to the APP and I'm having so much fun with it so I think that's where everybody is during the quarantine. Stuff like that. It's because I'm so used to having somebody around me and I just I get a little bit nervous but I get like extra boredom alone and too little nervous for him. I guess so I kinda crawl back inside of my show. It's easy for me to be myself when I have my twin around and when I don't I don't feel regular I don't feel like myself. You know what I mean. Maybe we would have gone to different colleges and then being forced to live different lifestyles without each other. But we're college age right now. So the reason reason that I made this resolution is because I don't want us to really kind of develop a dependent relationship to the point where it's irreversible and that we're not gonna be able to grow in our own. Because I think as the older we get the more normal it is for us to have our own interests and have earned relationships outside of you know pretty much every single friend that we have is each other's friends and we've never had a friend that wasn't the other person's friend or you know that we are closer with or anything like that. It's kind of all shared and I think we should have our own kind of not of a religious our own possessions like our own items that we you know we can have that are that are personal to us and then this is La okay. So you're not driving. But that was said you're not with my money to now all right our money. Okay but relationships. We've never done well. We've never shared a girlfriend grayson specify. Okay we've obviously never shared a girlfriend. He's in but yeah okay. But we've had we've had relationships where I kind of like talking about romantic relationships route. French romantic relationships. We've had relationships where you know. Ethan had been in a relationship with someone you know both really liked each other cared about each other. But then they get into say an argument as most normal people do relationships and I immediately go into full Defense Miller. Because that's my brother. You know what I mean but events moment I mean I would kind of sort of take on the emotion that maybe would be feeling if he was getting you know in an argument with someone that he cared about. And that's that's not good for me and my mental health. That's not good for Ethan in the development of his personal relationship that he's forming you know what I mean it just like and then I'll be easily. Why do you let someone talk to you like that? But it's an eye. That is not my place to say. I should not be meddling in his relationship. I should let him figure it out and grow on his own. So who the fuck are you to say? I think it's good natural progression to I mean it's like that with your parents like you know it's like when you move out when I moved out for my dad I thought maybe it was gonNA make us further apart but it didn't honestly appreciate it more when you really around him like that and I think that that can go for any relationships so so. It's good that we just bought a house together. That's also true. That resolution is GONNA work out. Well no it's great. It's great and we need to be in that because obviously do twenty. We're not getting married and having families anytime soon. If we did it would be too soon. I think that'd be premature decision for us to make yeah. I think that's a decision that I'm going to have to hold you back from making no I. I think I understand it. Here's the thing I just told you that I do. I think I will get married. I and I think kids I do. I think racing. We'll have kids before we get married. Not necessarily I think that maybe in my marriage I won't be ready for kids as soon as Grayson would be so maybe if I got married two years before gracing Graceland will get married and have kids right away Williams competition. Obviously I want my kid to your so we can give you wait. Willie's I want our kids to between age because hey if they're not twins that would be have twin cousins. That would be fun because it would for me so we are to live life and grow up without a personnel was my own age that compete with and and do fun things with and beat the shit out of you know you didn't ever did so ever. I was camper the week like every week. A number. Well, first of all, I was a good camper. The youngest of eight kids. So that's really good camper. But I think our highlight activity were those tie-dyed t-shirts and that said FBI day camp. I mean that was top notch for Fairfield beach club. Yeah. Okay. She didn't she didn't. She is your older sister. She didn't use you. Or like, I would think that you would specifically not get camper. The because she wouldn't wanna show favorite. No, Liz, and Julie were nice to me. They were other siblings. That were not remain nameless. Jimmy ticky Brendan. All right. I I will say that the seems preternatural -ly entrepreneurial to me like I mean, I may have had eliminate stand. I'm not even sure that may just be like a false memory in my brain. I was mainly. I just like to watch TV when I was a kid. What gives what do you think made Liz such an entrepreneur at a young age? Well, I think first of all our dad worked with his family. He owned a family business. So that was pretty normal that you would just work with your siblings. And then her parents really forced to get jobs very early on. There were no expensive tours to Europe. Now just a lot of like labor, and we figured out pretty early on Dolan's like we can do chores at home and not get paid. Or we could do chores for other people and get paid. So we all were babysitters, and dishwashers waitresses and lifeguards and housepainter. Yeah. Yeah. Well, chores for other people anything Bandra Cy I imagined. It's like, you know, coal miners, electrical workers, Connecticut. Rica's? No. Now, do you think changed from then until now I young entrepreneur to honestly, I don't think is changed at all..
