Well, I talk about my mental health problems on all social media platforms specifically because I have. Known a school borderline personality disorder. which has quite a lot of negative connotations around it about like stereotypically will people alike with it and I wanted to show people you lot of different people that you wouldn't expect might have. and so that was ready wireless. When I was younger, actually my school didn't assembly about time that I had tried to kill myself. which was the first time I'd ever have people of coming into my DM's saying, Oh, I, heard this happened to you and you get a now with talking about it as well. You get a lot of people that will dm you and say, Oh, I'm ready struggling with my mental health and sometimes it's difficult because if you integrate place with your mental health, it's very difficult to help others and you can feel of responsibility when you have a load of twelve, thirteen, fourteen year olds mass you saying that they're ready L. and they need a little help and then getting it so it can be a lot to take but generally. I don't, mind if. People make. Negative commands all criticize me talking about it because I personally don't see a problem was talking about it. I'm very careful about what I say I didn't give people especially in regards to Anorexia or self. self-harm a lot of people when they talk about it can accidentally give tips. I'm very careful with that so I have. I'm like a Schule that what I'm saying is not going to negatively impact people. Some people think on the Internet you'RE GONNA end up offending someone. Yeah. Couse. Do you have sort of boundaries in place in terms of what should go online and instead of what you'd have? Generally, I try not to follow too many people with mental health issues I mean I. Obviously I follow a lot like to have variety or I think you can sometimes become too consumed and people can post a lot of. Quite upsetting sometimes because people can pass like I'm feeding ready down. Awful. So I sort of like to try and keep a variety to make sure that I didn't become too embroiled in just the negative stuff. This is all new to me to be honest Kazakhstan. The TIKTOK side of things new to Mina city of on Youtube another social platforms. But for me sort of twisters, the place that I. Got It to chat to be about mental health stuff generally, and I'm still getting to grips with TIKTOK. What was the community like on Tiktok in terms of mental health checks on a straight feels fairly supportive of. TIKTOK is very supportive on it's mostly. So of Gen, Z. Kids. And I think as a whole always really sports each other the thing about Tiktok that's quite different to a lot of other platforms is that every time you release a video, it can be seen by a whole new audience of people. If it gets picked up by the Algorithm, it will get to it can go out to hundreds and thousands of people. and so you're always interacting with a new audience. From tiktok in. Particular I've never had many negative comments to be honest. They've always been really quite leaned.

It can be a big SHRUB and I've seen that. But. I. Guess. What do you think is that just about it? Yeah, and and like what you said you know we think when we hear if you hear the phrase ground cover, you would think, oh, turf can walk on it but there really ain't no such things I mean there are so few things that can tolerate that I mean maybe creeping timing lawn, you could technically walk on but you know really almost none of them so So yeah. The Worker Shortage Mystery
"It is a tough time to be looking for work. We're hearing stories all over the country dozens and dozens of people competing for a single job opening there just aren't a lot of open positions right now unless you're chat Dolan, we have so many jobs open. You wouldn't believe it chat president of Midwest, staffing in Minneapolis they help companies hire workers. All over Minnesota and Chad says, he has a ton of open jobs. Let's see. So we place welders, we place machine, operation? People General Labor where how shipping and receiving we have an entry level HR role and administrative assistant role I e reading me a list of like what you're currently hiring for. Yes. Just some of the ones were curly hiring for. We have hundreds and hundreds of open job orders right now. Meaning Chad wants to hire hundreds and hundreds of people for full-time jobs with benefits. So Chad has basically the Oprah of jobs out there like you get a job, you get a jog, everybody gets a job except Chad says, he cannot find people to take these jobs. So I have been in this industry for over twenty years. Never in my career have I experienced a situation where we literally have people to work, but they won't go back to work. I mean, we have clients who saved US five people a week. If you can find them for us, we can't find them and when it first started happening chat thought I know what's going on here? What's been challenging for us our our average pay rates it's all about the Benjamins, our average pay rates for candidates. Anywhere from fourteen to eighteen dollars an hour, we weren't able to pay them enough to put them back to work. So Chad, was doing the math back in April when government was paying out in extra six hundred dollars in unemployment as part of the cares act that Congress passed back in March and Chad found was that people could actually make more money off of their unemployment benefits than they would by taking one of his jobs and. It was not just Chad's Math pro-west that led him to the conclusion a situation where I called a candidate up I explained the job to them. It was. It's exactly what they've done in their past and literally laughed at me and hung up the phone they laughed at him. That's crazy. Chad thought as long as those extra benefits earn place getting people to take these jobs is going to be a problem but when that extra. Six hundred dollars went away the end of July Chad's problem didn't. So it was not the six hundred dollars. Chad started thinking about other possibilities like maybe it's the location like maybe there just aren't a lot of people looking for jobs in Minneapolis right now but that doesn't really seem to explain the mystery either because right across town from Chad Keith rose says he's been desperately looking for work. Keep is an audio visual technician he worked. At a company setting up for Conferences Expos back in March he was furloughed for two months and then another two months and another, and since the pandemic started I am over one hundred thirty applications and I've had only eighth interviews since then and I've had no job offers. So every day Keith wakes up and checks his job alerts he sends resumes and meets with his career counselors. He even has a spreadsheet on his computer where he logs. Job He's applied to he says, the competition is overwhelming and he's just not getting many calls back at times. It's kind of a bit of an emotional beatdown I have. Just been fighting this uphill battle and. Constantly getting the form. Thank you for your application, but you're not exactly what we're looking for. At this time, we encourage you to continue to look at our career site for any further opportunities you you might be qualified for Keith's family relies on his income. His wife is high risk for contracting corona virus and cannot go back to work Keith son just graduated from college and is living at home. He says, money is getting tight, but he says that is not the only reason that he wants to go back to work for me workers having the sense of purpose I want to be in a position where I'm making a contribution to something or to someone. You've got supply of Labor you've got Keith saying he desperately wants job and he's looking and looking and he can't find one and there are reports of this kind of situation happening all over the country right now. Institute my first question variety is there like some kind of worker shortage happening the answer to that is just an absolute no. Right now, there are quite literally more than eight million more unemployed people than job openings. So there's an absolute excess of workers who need jobs over and above job openings that are posted. But if I don't get one of those big for more inclined to wait like four or five rounds and then go to get a TJ Hopkinson plus Janis Smith's or an Eric Ibrox plus a rob Gronkowski. Go with Chris Herndon with any of those guys because I really liked Chris. Herndon this year go even later grabbing Herb Smith. Robert Tanya N- you know. Blake Darwin, there are so many. Dr. Rachel Dolan Discusses The Antipsychotic Drug Epidemic
"Welcome to the healthcare policy podcast on the host David Intra. Kosovo. With me today Dr Rachel Dolan the US House of Representatives ways and Means Committee majority staffer to discuss the majority staffs recently released report titled Under enforced and over prescribed. ANTIPSYCHOTIC drug epidemic ravaging America's nursing homes. Dr Dole and welcome to the program. I David thanks so much for having me. Please call me Rachel. While this'll be the last time Dr Dolan's bio is posted on, of course, the podcast website. In testimony before the House Energy and Commerce Committee two, thousand seven, the FDA's Dr David Graham stated quote. Unquote. Fifteen thousand adults elderly people in nursing homes are dying each year from the off label use of antipsychotic medications. For an indication that the FDA knows the drug doesn't work the problem has been only FDA for years and years close quote. Legal the FDA does provide a black box warning label. Regarding off label use of these drugs, eleven years later, Human Rights Watch published a report titled They Want Docile. How. Nursing homes in the US overmedicated people with dementia. The report found in two thousand, sixteen, seventeen quote unquote massive use or abuse of Anti Psychotics, for example, Sarah. Quel. Doll and Rispler doll that have serious side effects including sudden cardiac death. The human rights report estimated in an average week over one hundred, seventy, nine, thousand, long-stay Nursing Home Facility patients who administered antipsychotic drugs. Without a diagnosis which the drugs are indicated or approved rover, polar disorder and schizophrenia in testimony the ways and means. Committee. Heard this past November Richard Mollet Executive Director of the Long Term Care Community coalition concluded quote the use of San Anti psychotics in skilled nursing facilities is so extensive that puts the US in violation of internal conventions and covenants on torture and cruel inhumane and degrading treatment or punishment. Close quote. This is my third related interview. In December twenty twelve I discussed the topic with Diana Zuckerman. And in February, eighteen high interviewed Hannah Lamb who authored the above mentioned human rights report. With me again to discuss the ways and means report just released titled Under enforced and over prescribed is Rachel Dolan the reports lead author. So that Rachel as background let's get right into this or immediate neatly into the specifics of the report. What did the report find regarding the extent to which? They're persists overuse or misuse of anti psychotics in skilled nursing. David. So the report showed what what you what we would expect from your introduction, which is the use of antipsychotic does persist in nursing homes across the country and it remains quite high and not of course, has implications for patient safety and and health We found in the fourth quarter of Twenty nineteen approximately twenty percent of all skilled nursing facility residents in the US. So that's about two, hundred, Ninety, eight, thousand, six, hundred, fifty people every week received some form of antipsychotic medication and most of that was without any psychosis diagnosis for which these drugs are indicated So specifically, we actually looked at trends and surveyor citations for unnecessary medication use in nursing home. So that's kind of the. Part of this study and what we found was a clear change in citation rates for these facilities between the change in administrations from the Obama Administration to trump administration So we found citations for antipsychotic misuse in sniffs increased by two hundred percent between twenty, fifteen, twenty seventeen but then declined by twenty two percent from two thousand, seventeen to twenty eighteen, and importantly a ten percent of citations associated with actual harm or immediate jeopardy to a residence health or safety. So those are some of the most severe citation surveyors ever capture resulted in no fine from twenty seventeen to twenty eighteen under the trump administration. So you know. I. Would say even though this study in particular couldn't determine causation we we did see a clear association between the Trump Administration's regulatory rollback campaign twenty, seventeen, twenty eighteen and a reduction in citations for these particular drugs. Okay thank you and we'll get into the trump administration's regulatory decisions in this regard in a minute let me just ask as a follow up or an aside question and I don't think I saw this new report. So you may not have these numbers top of mind but worth asking, can you give an approximation of the cost? To the Medicare program at least relative to the overuse I, mean, this is a massive amount of money in reimbursement for these medications. I don't remember offhand. Let's see I think in the in the actually in the report we got About one third of older adult Medicare part d enrolling with dementia who spent more than one hundred days in a nursing humber prescribed antipsychotic in two, thousand, twelve constituting roughly three, hundred, sixty, three, million part D plan payments that year and of course, there's also cost associated with hospitalizations for inappropriate use of these drugs So I would expect you know that that that is obviously very under an understatement understated estimate that does not capture the full realm of payments. So it's it's fairly substantial.

"It's a cash game lineup which has a higher floor or a GP lineup which has guys who can really pop off, but you also understand it could be a lineup that can finish last in a particular contest because you wanNA create some diversity in your lineup and you want to have a lineup that nobody else is going to have that can win the whole thing. I think the strategy there would be go to draftkings look for contests that are eagle either single entry or have like a three to five lineup maximum and enter those contests with a single lineup that you feel really good. About is probably the optimal strategy. If you don't want if you don't have the bankroll frankly to Max enter or multi enter multiple contest that's. Probably. The Best Strategy for you the most fun strategy for you other people will almost certainly disagree with me but I think that's a strategy that a lot of casual the offense players who want a chance to not just double up there twenty five or fifty dollar entry but also have a chance to win five figures that gives you that chance without going up against people who are entering hundred different lineups in the same context. What would you tell people how to pray you streaming the prayers through the archdiocese on on the Internet? We are John. We've got it livestream say Patrick's cathedral all of the Holy Week liturgies including Bob Sunday this morning and and next Sunday Easter Sunday and We're doing it on of course are are Catholic Serious Network One Sirius. Xm One twenty nine as well as Catholic faith networking. I'm happy to tell you John that the Pix is picked us up. They're going to do live the ten. Am mass both Palm Sunday and Easter Sunday from Saint Patrick's and I'll tell you this as well. I'm very grateful to my priests because almost all of them some are doing it already. The shut ins the homebound but now almost all of them are live streaming this Sunday. Mass is because jot as you hinted at our people are telling us who we Miss Church. We miss the Liturgy. We missed the SACRAMENTS. We need it more than ever so while it's never going to be as good as being there in person it's still mighty powerful for them at least to pray together Via Radio or TV people needed and we all need it. It's a time that is challenging to all of us. I'm wondering if there's a way to get people to to come in for for a minute and just go through a line. Twenty feet long and just pray and some of that John we got even though Were not open publicly for the mass and the sacraments most of our churches are open and people are coming in and they keep their distance. Like you like you mentioned. And they're praying Let me tell you like. It's a patrick. So the priests there will be on duty and they'll just be on the front steps and people come in. They might stop again at a distance and ask for blessing. Summer even going to confession. So we're trying to be creative and it's working and people appreciate it. I've got a number of my My Priests John. Saying that you know they. They're used to take a walk in the morning and our long walk in the city or whatever neighborhood. They're in usually they put on. Maybe a tracksuit or something. Now they're doing their collars so that people know that their priests and people are stopping them again keeping the distance but say father could. I have a blessing. Would you say a prayer over me and So some of this is working. There's there's Kinda we can't and I even with all the secularism and all the cynicism and doubt and all the fear that we got now that there's an innate reservoir of faith in people's hearts I hope I'm not naive in thinking People are tapping into that now and we might even see a revival after all this is dot is people said you know. We're turned him back to God. The Bible tells us that inside of every tragedy inside of every of every adversity and Lord knows we're in one. Now there's an invitation from the Lord to come back to him and I'm hoping that's happening. Have you seen that that people that were Didn't have the faith that they wo- a Sunday have come back they H- they have John as you and I were chatting before we went on the air. I don't think there's anybody that has not been personally affected by this. Thanks to God they may not have the virus but a family member somebody with whom they work they know has got it so. This is not only spreading physically. This is a moral and spiritual effect on our people and they're all feeling affected. I tell you the other day John. I had the burial of one of our priests who died from the virus now. He was in his mid eighties. His health was rather fragile but anyway he was retired but it was so sad. Here's a here's a guy that for his whole life it consoled people at funeral masses and at the grave there. I am literally by myself with a pet. That local pastor was off to the side. One of my exhilarating. Bishops was on the other side but at a distance there. I am with the casket in the open grave and I look around. And there's people there but they're all in their cars or their standing apart from one another at a distance. I'm glad they were. They should have done that. But I'm thinking you know these folks not only do they mourn the loss of their pastor. But they're not even able to benefit from the regular constellations that that the church is able to provide and I'm hearing that from people all over the place when their grandparents go when friends or neighbors go. Teams are better than bad teams. That's all true but more plays run more value simple but i want people to look at the list of teams from last year and the plays that they ran dan and understand why some of these teams might be worth investing in for fantasy last year the team that lead the n._f._l. In place per game was the baltimore baltimore ravens. They averaged about seventy one place per game well. If you're somebody who loves their backfield for obvious reasons they essentially ran a service academy the off offense when when jackson took over last year they ran the ball more than anybody well. They also ran at a high pace. The clock wasn't really stopping seventy eighty one place per game for the baltimore ravens. I expect them to be at a at a high pace again and if you love that backfield i've been drafting mark ingram a whole lot this offseason in very very recently. I've started diving in with multiple shares of justice hill well. You might think well. That's counterintuitive. If you really like mark ingram and you're you don't you're not a big fan of handcuffing which i'm really not early in the season and you're and you're drafting justice civil. Why are you driving both of those guys because ross an offense that runs mm seventy one place per game can support multiple running backs in multiple ways. They ran a tana place more plays more fantasy opportunities. The new england patriots were second in the n._f._l. So i think even if you're buying into the recent camp report that sony michelle is going to get the ball a lot well. They can support michelle n james whiten that backfield the rams that sixty six place per game. There's more targets moore carries to go around on the flipside. You look at teams but that didn't run a whole lot of plays last year. Miami and arizona have new coaching staff so you kind of want to throw them out and see what might happen there but tennessee fifty fifty nine plays per game that raises alarm bells for me because there's not as many targets to go around for corey davis not as many targets for dion lewis the number two running going back <hes> not as many carries perhaps for derrick henry. The chargers ran under sixty plays per game. They're coaching staff is back. They don't have melvin gordon so i. I want you to understand that if you're drafting a second receiver or a second running back from a team you wanna make sure at least understand that certain teams are we're going to run ten to fifteen plays per game more than other teams and those ten to fifteen plays per game could be the key between winning or losing that drafted for you yeah. That's a really good point and that's one that i i don't remember seeing three to five years ago. Go joe that seemed like a more recent thing that people have realized the value in just how many snaps these teams get off and it's as applying uh right it's simple and if you run more snaps it's it's more plays. We see snap rate ross. Oh he played sixty five percent of the snaps but but if you play sixty five percent of one hundred snaps that's way more valuable than playing sixty five percent a fifty snaps. No it's a great point <hes>. Do you have any other joe. Well yeah ross and i once again. This is a simple one focus on good teams with good offenses that kind of plays off the previous one. I know there's there was a a kind of a blow back last year and i think one of the things that the public in fantasy would always rebel against was ranking patriots running back hi. Oh we you never know what they're going to do. The bill belichick uses five running backs but there's always somebody who produces because the offense is good and i think one are those teams. I'm looking at this year. Ross is philadelphia when you look at the eagles and you say oh. I don't want to draft miles sanders because they brought in jordan howard and they always have running back by committee but ross. If the offense is scoring points. Somebody is going to produce so. I think it's lazy analysis to say you know what this is this. This team is running back situations situations of mass. I'm just going to avoid it. I think you want to get guys in messy. Situations who are on good teams. If you want to avoid a messy situation like like with the titans and you don't know what to do with their pass catching group or you don't know what to do with the buffalo bills running back group. I understand that a mediocre to a bad team but i'm i'm always going to invest in messy situations with good teams because there's value there because if these situations ross weren't nessie than these guys would be league top picks. Imagine that you weren't howard wasn't in philadelphia. Where do you think miles sanders is getting drafted right now. I mean it second round third round. He might be going right next to josh jacobs with oakland and mile sanders is almost certainly going to be on a better team with a better offense then then <hes> josh jacobs this so i want to attack messy situations with good teams and avoid them with bad teams. You get a discount for messy situations with a good team. Well i think the market usually they kind of understand messy. Situations with bad teams a little bit more not all messy situations are created equal. Wow that's interesting way to look got it yet. The eagles are an interesting one joe just because doing their their t._v. Games in the preseason. They just have a lot of good players man. I mean they you you know they've got to legit titans. They've got four really good wide receivers. They've got at least two running backs six that will play a lot and i'm not convinced that they won't play three or four runny mags. I mean i don't know how much they're gonna use sproles. I even thought last thursday against the ravens corey clement looked pretty good so i don't know i mean i. I think those are ones where people typically i see on twitter. Her fantasy analysts will say you don't want running back by committees. We want volume volume volume. We want guys. You're going to get a lot of touches. How do you know in in any of those situations like zakar threat. He can't possibly catch as many balls this year right. I mean you take the under on record setting performances. I've been saying all off-season season ross. The kansas city chiefs could've added prime jerry rice to the offense they had last year and literally nothing else changed and i would take the under on fifty touchdown passes for patrick mahomes. It's just it's the smart bet to say. Regression is coming from record setting performances so if you're drafting soccer's expecting him to catch one hundred twelve balls again i i'd say hey you're gonna be foolish but then you have to just look at what's going to be realistic okay. If zach ertz loses twenty five percent of his of his targets from last year any any still catches seventy five passes how many tight ends we're going to catch seventy five passes george kendall maybe you maybe travis kelsey. He's probably gonna do it. I think you could say evan. Ingram could do it and maybe one or two others but that's still a hell of a season for zachert so you wanna take the under on record setting performances but you don't wanna you don't wanna just instantly fade somebody for a record setting performance or a great performance if their price has adjusted so like exactly it's if you if people were expecting him to do what he did last year again he'd be right up there with traffic kelsey. He's not some say he's still a little bit too expensive but taking taking the under on record setting performances and fading them entirely that i mean i think that's a different story ross yeah. I think i think that makes sense any other one last tip. You have anything else that we need to know before. People have their drafts the next couple of days or next weekend of course you and i will start next week with giving your thoughts and breakdown of pretty much every fantasy relevant guy in in each game starting starting next week on tuesdays and wednesdays going to be fantastic if you can't already tell joe knows his stuff and he definitely brings things the energy and the juice. You've got a very good delivery joe way about it's fun. I really appreciate that ross. One of the things i would like to say just to give a a tip here is understand especially for some running backs win loss splits and i i'm going to bring this up in reference reference to marlin mac marlin max numbers in his career and it can be noisy here because a lot of these losses came in two thousand seventeen with the the old coaching coaching staff but marlin max numbers winds are significantly better than they are in losses and last year in in colds situations where they were trailing naim hines played significantly more snaps than marlin mac and i'm gonna give an example from last year for why i think it's important to understand the volatility of some running backs adrian peterson with the redskins last year ran for over one thousand yards ross. I would think any any football fan looks at two hundred and fifty carries for forty yards and seven touchdowns as a pretty damn good season for running back wouldn't you wouldn't you say good season. Yes however you look at it from a fantasy perspective and things start to get a little bit messier for adrian peterson because six hundred eighty of those one thousand forty a two rushing yards came in their seven wins three hundred sixty two of them came in their nine losses and for a running back like a marlin mac an adrian peterson a derrick henry on the lesser side. Maybe a sony michelle because the patriots are so good and they aren't going to be behind all that much. It's important to recognize that these guys aren't going to catch a whole lot of passes are going to be very very hard to play and to support in a game in which the team that they play on is a betting underdog so a team like washington a team like the colts now that andrew luck has retired. I'm expecting to see situations nations in which they're six seven eight point underdogs same with tennessee. I would avoid playing guys like that when they're heavy underdogs because that's gonna be a winning bet more often than not and that's why in fantasy not just do. We look for past catches because we call them. Calorie rich catches calorie alary rich touches where a lot of fantasy points but if the guy can't catch the ball and isn't gonna be out there when the team is losing then you're going to get a goose egg and that's that's a really important stats look at and that's something more to focus on when we're in season but running backs who don't catch the ball from bad teams those are almost always waist fades on drafting excellent work joe. I love it. Your tips right on at a lot of people asked year while you're number one speaking of of number one. There's other ways become number one like joe dolan. You go ahead and you use p._f. So these are the kind of things that have happened to the New York Knicks very few people think about coming there. If you wanna come there, you're going to have to overpay just to get somebody to come there. And it's never because Steve mills it's never because of it's not gonna cause be because Scott Perry. It wasn't even because of Thomas when he was there or Mike Woodson or anybody else it is religiously it consistently because the presence of James Dolan gives the impression that this team will always be on a fast track to nowhere because they are too many things get in the way, no basketball ever be. He has been described by multiple people who have sat through meetings with him as this is how it was described to me the dumbest guy in the room. Who thinks he's the smartest guy in the room. And that guys actually in control because he is the son of a billionaire who built up the business.
