"Evening and the Monday night fifty fifty six degrees right now the temperature on Peachtree street I'm Cheryl Castro ninety five point five WSP Atlanta's news and talk this is the weekly check the staff member and today the weekly check up is stroke we're talking about cerebral vascular accidents were talking about strokes how to prevent them had a screen for them what's going to be done if god forbid you have symptoms developing with one what are the current what's the current incidents in management everything you can think of a stroke is gonna be covered today we've got doctor Michael Colleen doctor fighting a hobbit every health care live in the studio with me and guys I thought it would be great after that prior call re heard kind of the disaster side of having stroke symptoms finally convincing yourself you've got to get to an emergency room and then getting there the diagnosis not being made to how does it work in the best of situations today and when that patient walks in the door because time is of the essence and we'll get more into that what should be happening Sir one important thing to realize is when we say at that time is of the essence is that about two million neurons nerve cells in the brain are dying and minute based on estimates and so when we think about what are the things that are most and I'm gonna go back to that okay so so this is in the case of no blood flow rain when you are in the midst of having a stroke right limited Inc the boat the blood flow to the brain estimates are that about two million nerve cells in the brain are dying every single minute okay and how many how many millions of neurons to most people have so total number of cells is twenty two billion twenty billion yes missing two million per minute exactly at twenty two billion is for the average person not me right now I'm only kidding so we all have these twenty two billion and you start they start dropping off minute two million a pop so when we think about somebody having a stroke and why time is so important is many of us know if you walk into an emergency room and you may get a okay take a seat will be with you shortly that doesn't fly with stroke so what needs to happen is is the importance of when you think you're having a stroke it's about calling nine one one that ambulance is not only going to go and pick you up they're going to actually give us a call and memory hospital and tell us before they even arrive Hey we've got a possible stroke not now set standard for any ambulance picking up anywhere in the city if the ambulance is thinking that stroke is a possibility yes and so when they think that there is a potential stroke then they're going to actually go ahead and call ahead and say Hey guys we've got a stroke potentially come in right so I want to stop once more time because you talked about the acronym for people to remember and I think we're gonna repeat this at least ten times over the course of the show because if it's critical that they know the EMTs know that you may be having a stroke you have to be able to communicate what it is that's going wrong and the fast acronym seems like the best way to do that if you know what that is and you're experiencing that you know that it could be a stroke you can convey that information absolutely and fast itself but the face the arm this beach trouble actually covers about three quarters of presenting types of stroke there's actually more recently something that's been called be fast the B. E. represents sudden balance trouble side and I or vision difficult to get back to my son my wife straight at the ocular migraines it was the very first time that you had loss of vision in there I. N. right exactly and so and so ultimately be fast can cover about eighty to eighty five percent of presenting types of strokes so if the ambulance where it is is and the EMS crew realizes Hey we're picking up somebody with suspected stroke we're actually calling in and to the hospital and on our end in an Emory health care hospital what that means is we are activating a stroke alert and that stroke alert is basically activating physicians or neurologist it's activating radiologists it's activating our pharmacy team and its activating all the rest of the emergency department folks as well as part of this and basically are CT technologists and what happens is as soon as the patient actually comes in the door we're all there so we might have been somewhere else in the hospital but we were all there you're a physically coming to the E. R. waiting for the station to could be wielding that's what every minute counts we don't have time to be saying well let the patient calm spend a few minutes ten million neurons earn no worries and we would we let it go no world down there and so when you're an acute stroke patients coming in with a new symptoms the room is full and it's all because there is somebody who is drawing blood there is somebody who is examining the patient somebody's getting a history of the patient to understand what was going on when the symptom started and they will our goal is to get that patient to a scanner usually a CT scan of the head to see is this a bleeding kind a stroke or is there no evidence and are you following metrics on like from from crossing the threshold of the door to getting on the C. T. table what your target is and how often you hit that target those are just a small piece of all the metrics we track literally step by step by step that metric do we know that metric is so entering the door to actually getting on the this the scanning table yeah so so that the current national guidelines are trying to target basically a door to a C. T. acquisition time of basically being within twenty five minutes more recent targets are actually to bring that down to about fifteen minutes now and so our goal within Emory hospitals is actually we don't want to just be there we want to be at the scanner within five to ten minutes we want to know is it a bleeding stroke or not based on the CT scan and then we start tracking metrics like does this patient qualify for the clot busting medicine T. PA if they qualify for the clot busting medicine we want our patients to get it within thirty minutes of arrival no that's a that's a that's a steep no no what which is hard because you can't get to on the CD table in twenty five and get to be a five minutes later exactly yeah do the procedure that exactly and so and so what we are doing as well as other leading institutions of the country is saying how can we bring that down how can we bring that down every step of the way so that we're treating patients as early as possible to get better and better outcome so we know that the earlier you treat the better the outcome of the patient get Michael into an end and Bruce we track metrics am funny specifically on on many different things but going back to having the team ready before the patient even hits the door you know we often time we'll have an operating room where an NGO sweet emptied ready to receive that patient if it is one of those twenty or thirty percent of patients who are having a stroke that might benefit from a perch procedure to pull that clawed out of the ordinary and to bring to you bring up a really interesting point there could you take twenty to thirty percent so it's never gonna be a hundred percent because that means if everyone who walks in actually has a stroke means you've missed tons of other stroke patients who'd eating catch so you're gonna overshoot you're gonna be doing this because it's a rule out you're gonna have a list in differential diagnosis of all the things that could be happening and the one that's most critical because you can remediate if you act quickly as a strip right and so it all is about that rule out process and so is that where that number should be is that the goal is that you get thirty percent should be higher lower so we have published data on this where we showed that for example at Emory University hospital that of all the stroke alerts about sixty percent of them end up being a final diagnosis of stroke obviously you don't wanna call wolf for anything and everything right but at the same time we actually found that at about two thirds of cases being stroke was actually a good target to be knowing that yes in some cases these people wouldn't have stroke we would realize that and then we can Paul basically take a step back once we did that but we still need the imaging we still needed the history taking we still need to evaluate do they deserve to have the club busting medicine or the interventional and for those listening to me you know things you may be more familiar with so a breast biopsy after an abnormal mammogram so it turns out that only about fifteen percent of abnormal mammograms that go to breast biopsy actually result in a diagnosis of cancer that means eighty five didn't need to be biopsied if you think of it that way but on the other hand you don't want to miss one it is okay if you do it a biopsy in a needle removal of tissue with you know that doesn't lead to the diagnosis of cancer but the critical thing is not to miss a cancer I think in this case the critical thing is not to miss the stroke so so Michael there's all these metro X. being followed in so there's constant pressure on the team to perform and to kind of keep looking at operations and systems on how to improve it even I guessed when you come to like physical design absolutely where we're constantly trying to get the the time to definitive therapy down we would like to have you know the time from a patient hitting every healthcare or any facility to being able to provide definitive therapy to be less than sixty minutes or less than ninety minutes the the twenty to thirty percent I mentioned earlier it's important to point out that only about twenty to thirty percent of strokes or due to what we call large vessel occlusion or an inclusion of a large blood vessel in the brain and and those are the ones where we can have the greatest impact with modern medicine in modern techniques to actually physically pulled those clocks out of the arteries so it almost I don't know where to go but I I'm because I'm thinking about hemorrhagic verses you never symbolic but but what what but let's hold that I can get back to this path so we've got this patient hitting the door and the goal is going to be that in in in the future a future state of not even now fifteen minutes they're honest a table for see in a CT scan sweet thirty minutes then there into the treatment algorithm whereby they might be getting to be a clot busting they might be getting and the vascular procedure or they might be getting something else which could even be I guess a surgical procedure if it turns out that it's a hemorrhagic stroke and not a blood clot that has to be dislodged from the vessel and and so at with this kind of a change what has been the outcome differences it can be like translate that into the the outcomes from stroke that you Michael is talking about you know twenty thirty years ago you grandma went into a dark room and you know you you in two days you would know how bad it was so a couple of things with respect to that so one is is the outcomes are no longer about are you showing up within four and a half hours of the onset of your stroke symptoms which was the time frame the maximum time frame that you could give the clot busting medicine now with interventional procedures that doctor calling his team do you can actually treat selected patients out to twenty four hours from the onset of their symptoms so those patients who might.
