You know, where did the popularity of it come from that so much so that patients are acting asking for it, and even the doctors, some doctors are actually prescribing it out. It's one of those things that is quickly spreading on social media. In a lot of Facebook groups, and on reddit, there's been a lot of attention to ivermectin. What physicians and researchers are most concerned about right now. Is people actually taking them? Veterinary formulation, which is often a dosage is far higher than are appropriate for human consumption. And in some cases, then they are overdosing. And they are calling poison control centers reporting all kinds of problems, including nausea, diarrhea. A lot of other issues, so the real risk here in particular is around people taking for veterinary formulation. But then again, it also, even the human formulation has not been approved by the FDA to treat Covid. So physicians are saying that people should not be seeking this out to treat and prevent covid right now, since there is no indication that it's been approved by the FDA for you, and the FDA even said something or I think the CDCR the FDA was even trying to Acknowledge it last week as well to your point and Mississippi's Health Department said that earlier this month 70% of their calls to their poison control center had come from people who ingested ever met him that they got from livestock supply stores. So people are going out and seeking the animal version of this That is exactly right. People are going out on their own and seeking it and not as at great risk to their house. So before we look at what to expect from side effects, you spoke to experts about the Biden administration choice about these booster shots. What are scientists perceptions about the booster shot and who should get it? Right. So there is a debate about that. And and right now, I mean, there are some groups that everyone agrees should get a booster shot, and that is definitely people who have compromised immune systems. I think some people think that particularly older adults, maybe people in nursing homes, you know, there might be some disagreement about what age exactly the cut off should be. But there is a bigger debate about whether boosters are going to be needed for all adults, and if so, when whether it's now or in a year or months down the line. You know, governmental agencies are reviewing these plans to authorize boosters and governmental agencies are looking at other studies in other countries. Um and Israel study, for instance, because they Received vaccinations earlier than many parts of the country, so they actually have at least a little bit of data right to show what happens when people receive another dose. The Biden administration that the FDA the CDCR reviewing all sorts of data, whether studies from the U. S, or, like you were saying, sort of the experience in Israel, and what the data suggests so far is that people might become more vulnerable to an infection months out from their vaccination. "Lot of questions about whether a booster is even necessary. And there there are the side effects. So many of us have experienced the first go around. What will a third shot actually bring? Through. Joseph has been reporting on this. He's with our partners at stat. The health and medicine publication Welcome. Thanks so much? Yes. So before we look at what to expect from side effects, you spoke to experts about the Biden administration choice about these booster shots. What are scientists perceptions about the booster shot and who should get it? Right. So there is a debate about that. And and right now, I mean, there are some groups that everyone agrees should get a booster shot, and that is definitely people who have compromised immune systems. I think some people think that particularly older adults, maybe people in nursing homes, you know, there might be some disagreement about what age exactly the cut off should be. But there is a bigger debate about whether boosters are going to be needed for all adults, and if so, when whether it's now or in a year or months down the line. You know, governmental agencies are reviewing these plans to authorize boosters and governmental agencies are looking at other studies in other countries. Um and Israel study, for instance, because they Received vaccinations earlier than many parts of the country. So they actually have at least a little bit of data right to show what happens when people receive another dose. The bite administration that the FDA the CDCR reviewing all sorts of data, whether studies from the U. S, or like you were saying, sort of the experience in Israel. And what the data suggests so far is that people might become more vulnerable to an infection months out from their vaccination. And there's a question about if we should be concerned about that, because basically what we really want from these vaccines is to protect us from severe covid hospitalization and death, and it appears they're still able to do that, like at the same rate, as they've done throughout the pandemic. At least part of the plan will be implemented. Dr Anthony Fauci on face the nation over the weekend, talking about the government's plan to offer Covid vaccine booster shots beginning September 20th. But there is lots of confusion over what the rollout of those booster shots will look like. Late last week, federal regulators said they may only be available to recommend a third dose for some fighter recipients by the administration. September deadline. So what about people who got the Moderna or J and J vaccines and who will get prioritized for those third shots? Joining me to talk about this are two people. A poor VOC. Mandeville Lee is New York Times Science and Global Health reporter. Welcome to the program. Thanks for having me. Also joining us Dr Peter Chin Hong professor of medicine at UCSF, and he specializes in infectious diseases. Welcome back So great to be back on, Madeline. Thank you. Great to have you back for for Let's begin with you. So as of now, are we on track to get these booster shots? Into arms beginning on September 20th. That's a multipart answers, so I think we are on track to get the booster doses out to some people. I think there's no question that the FDA will probably say it's good to have these booster doses for older people and immuno compromised people who are the ones for whom that data is clearest. But for the rest of us, it's not really clear what the data are showing yet, And so I think it's a lot will depend on these studies that the FDA and the CDCR apparently reviewing and what those data show. Okay. So explain for for us why there would be a scheduled date September 20th before the CDC and the FDA have reviewed all the data. Well, that's a very good question. Then one um and I think that there's the Biden administration has received a lot of criticism for making this announcement before the FDA and the CDC has had a chance to review. The administration has always said they'll follow the science. But in this case, the scientists at those agencies Have not fully looked at all of the information yet, So it's a little cart before the horse, um but hopefully at least for the older people, and they may not compromise people because the data is clear that won't be so controversial. But they have gotten a lot of pushback for, you know, speaking a little too soon about booster shots for the rest of us. This is one of those cases where Really? I think for the first time since the Trump administration, we're seeing some conflict between the scientific community at large and the Biden administration in terms of what they think is appropriate. Right because two top regulators are leaving the FDA in part based on this right over over this particular issue, That's right. That's right. One of them is leaving in October and the other in November, and they both told other people that They don't think that boosters are really necessary for the average person, and, um, it's said that that's one of the reasons they're leaving is because the Biden administration push this forward. Without waiting for the scientists to make a decision. Okay, Peter, you have presumably looked at the science. What do you think? Is there science backing the need for a third shot? Well, I think that's a complicated answer as well. Madeline. So the science is that everyone cites this Israeli study where in the Over 60 year olds the data the vaccine efficacy for serious disease Now goes from about 91%. If you received the vaccine within four months about 86% if you received that Within six months after and if you look at the age group of 40 to 59 year olds that drop in efficacy only goes from 98% to 94% again. Speaking to approval is point. Maybe a little bit clearer in the older population, although you would say that. Let's be proactive rather than reactive and wait for a lot of people to flood hospitals. And I think it's reasonable as a clinician to think about really protecting that older population, but for the rest of us, I mean, if you look at the data Every isn't much of a case that we need to make for the rest of the population to get a boost the shop What about immuno compromised people immune compromise, of course, But then we're not necessarily talking about the boosts Our first of all, we're trying to rule out that third shot that additional shot and even with that group There's no guarantee that that group will even get in on the body response. If you look at the studies of the people who didn't get an antibody response as an in your compromise individual only about 30 to 50% of them will get a response after a third shot. So I think for that group, you know, some of us have been thinking about other strategies like maybe antibody therapy as a bridge to vaccination. Maybe after they got gotten over there, Uh, time of most immune suppression, But the answer is less clear. But certainly I think that's the population of interests. A lot of clinicians. Mm. Well, so that's interesting. So after six months if you are in the 40 to 59 age group, according to this Israeli study, you are still 94% protected. That's incredible. And would you assume that it would also hold for people younger than 40? Oh, yes, Most definitely, um, you know, I think the data is clear from the beginning of the pandemic that it's very much aged related in terms of serious disease and infection, an infection and above the age of 50. Every decade. You go about the age of 50 you add an additional risk of mortality and serious disease. Now does this address the variance? Or was this just for the original version of Covid? You know this At the time when the Israeli study was done, it does account for some of the variant, sick leading, including Delta, but again if there is some new super variant that comes on board That is vaccine invasive. Um, you know, you could imagine a situation although I hit an shudder to think about that future where everyone would have to be re boxing it again. In fact, the Israelis, I think, over the weekend Started, um mumbling about maybe having 1/4 those which again sent a lot of people into Disbelief and a little bit of disarray. Because most of the world is still waiting to get one shot. Yes, exactly. Approval. What's being of variants? We're now hearing about another one. A new one called the Mu Variant spelled M You What do we know about that one? So the mu variant has some of the same mutations that were in the one that was circulating in South Africa and in Brazil, Beta and gamma and those variants were noted for being able to evade the vaccine response. A bit more than Delta. Want to be careful. They're not say that they are resistant vaccines or anything like that. They still responded, but less well than Delta and certainly less well than the original version of the virus that we saw. So that's partly why scientists are watching it. However, it was really mostly in South America right now in Colombia and a few other countries, and it's not really something that we in the U..

That comes amid a national vaccination campaign in which Children as young as 12 are eligible to get shots, as well as a general decline in covid hospitalizations and deaths. The CDC is not advising schools to require shots for teachers and vaccine eligible kids. However, again, the CDC saying vaccinated teachers and students do not need to wear masks inside of school buildings. Well, some recent developments may have many people raising questions about the fast spreading delta variant of coronavirus and whether a booster shot is needed. Detection fighter is about to seek US authorization for a third shot of its vaccine after an Israeli study showed effectiveness of that shot wanes over time. Joining us live now as Washington Post science reporter Joel Achenbach, Joel, the FDA and the CDCR saying fully vaccinated people for now, don't need a booster. Is this the case where it's possibly everyone is right. Uh, we're hearing from federal health officials and from Pfizer and from other studies, could everybody be right here? Well, clearly, there's a little bit of a of a spat going on between Fizer and the federal agencies because Pfizer yesterday put out a statement saying, we think That you'll need a booster 6 to 12 months after your fully vaccinated and I think the federal agencies or like hold on, you know, this has not been established. Scientifically. We're going to make the call here, not Pfizer. Advisor hasn't shared all of its data from its own clinical trials. And, you know, it does have a A, um you know, a financial interest in and selling more, You know, booster shots. So this is an interesting moment. I mean, the scientifically we know that over time. Your antibodies start to wane against, uh, you know anything out there. The delta is a problem because it already has a little bit of slippery, You know, vaccine of Asian mutations, so we probably will need a booster at some point, But when is the question Yeah, That is the question. What do we know about Johnson and Johnson's one shot vaccine against Delta? There's a lot of talk about the RNA shots from Pfizer and Moderna. What do we know about J and J's one dose vaccine? There's good news on the vaccination front in both northern and Southern California in Sacramento County.
