"So i was able to bring in kyle and matty kyle has more than one limb affected with lymphoma dima but are brought in. Kyle's right hand and i brought in maddie's left leg and every therapist who attended was able to bandage toes fingers hands foot ankle risks. I mean all the way up k so up to the up to the elbow because they were half models of a pediatric limb. A real pediatric limb. It looks feels shaped like a pediatric limb. And not just an adult hand and we're playing make belief so that was something that was really cool to me to see people. you know. Have questions to say that they've treated a kid. You know what if or how can they improve in their treatment approach for their little that come in to them. That was just really fun to be able to answer those questions. And i would not have been able to do it without my team. Because there's a lot of questions and the timeframe for the conference was short will mean what you get in there. That goes by so quickly. You hardly have time to really like set up with any education. You really don't have enough time to get into the details. it is just. Hey let's go let me show you this and we have to move on but my team were all volunteers from camp. This last summer. I had at least cantu. Who is the voice of the uncle. Pt podcast. She's a pt dpt clt. Lanna out of fort worth. Texas amber prolly out of houston texas. She works at texas children's hospital and she is also pt. Dp clt lleida and karen roach out of la. She is an ot sale t- and she came to camp this summer with no pediatric experience in neither did a lease so it was really cool that over camp. They fell in love with pediatric treatment. They fell in love with our mission to educate other therapists and they were able to do rounds in the room. Help with bandaging supplies help with technique and answer some questions because there were a lot of people in the courses that needed a little bit of help with application. And how do i do this with the hand or the finger roll and where to put the foam and all these other questions so is really really great to have them there alongside me. I feel like. I could've used two or three more people and i also could have used two or three more hours heck two or three days to really be able to dive into pediatric lymphoma treatment and the protocols that we can apply brittany williams. Was there. I obviously can't go anywhere without my sidekick and partner for life brittany williams. She's the founder and ceo. Brian's feet foundation and they're in the exhibit hall. we had a booth four pediatric lymphoma alliance. Which is launching next week i. It is a resource for bandaging kits for pediatric lymphoma. A murray again like if you're a parent and you're trying to find some education resources for your clt who may not have pediatric experience on the soviet great opportunity to come and learn in meet me brittany and some of the other therapist. Yeah and i would also just add that if you are a therapist that currently only sees adult patients and you have any like even the slightest bit of interest in learning more about pediatric cases or or possibly treating them This would be a really good way to kind of stick your toe in the water so to speak half and get a little bit more hands on. Get that information I just recently had a family reach out that said. Hey we're looking for a therapist. We've called all the therapists in our area. Everyone is denying us because my child is six years old and they only see adults. And that's something that we're really trying to correct especially through violence foundation Because these parents need help just like the adults do and guess what eventually these kids do become adults and the most successful way to treat and manage lengthen. Ema is by implementing a treatment plan early so when these parents are reaching out. And they're asking for help in their getting denied it becomes really frustrating and they feel so alone so frustrated and they're just trying to reach out to get some help for their kids so as i said if you have you know even the slightest inkling of like oh. I'd like to learn a little bit or you know just interested in the slightest This would be they analyze. It would be a really good way to You know get those basic techniques and you know basic understanding of how to Sweatshop your treatment plan so that it is for kids. 'cause i've been working on my outline and we'll be covering different modifications or modification. Read your mind ways to modify an adult bandaging kit if that is something that you have to use or if you only have certain supplies that are available in your clinic. We're gonna go over how to modify a kit or going to go over how to modify your sequence for mld your hand placement for mld some techniques that you are you know commonly using on adults but you can modify them for a child how to measure in fit for compression garments. We're going to modify actual bandaging applications and things like that. So we're going to pack in as much as possible in this two hour timeframe but again stay tuned for more to come because we have more kind of related. It's hard to even stay indoors like seattle. You guys having one hundred and fifteen hundred twenty almost degree temperatures in. You're not used at so. Even your houses are prepared to cool. Maybe it's just standing in the shower sitting in the bathtub. Elevating your legs with like a cold towel on you probably totally naked so you can even cool off a little bit. That's going to help. My son tried one moved from texas to arkansas. We left arkansas to go to colorado for twelve days. We're back in arkansas. Trying to get moved in and settled. No internet no washer and dryer delivered yet because everything is backward. That's a story for another day and it's hot in. You already know this but if you need to get some relief if you have access to a body of water. Poll lake upon ended ocean a bathtub. Whatever it is and you can cool off. one foot of water is ten millimeters of mercury compression. And the deeper. You go the deeper. Your gradient compression is that you can't go in dive deep. Get in their way flat kick circles. Punch pull squat. Whatever your movements are that you can do do them to get that. Fluid moving and to cool off. Try to avoid the sun between the hours of ten. Am and four pm is that is when it is hottest and at its highest peak try a cooling garment something that has like cotton or moisture working in it. Something that is going to be a little bit more breathable but do not skip your compression. Do not skip bandaging or you're pumping or your mld self mld diaphragm breathing. Dry brushing whatever toll. You have in your toolbox. Don't skip it because that fluid is going to continue to build up. You're going to be out of compression and by the time you try to get back into said garment. It won't fit because you're fluid will have accumulated built up and it will be a road. You don't wanna follow because as you know getting back down into compression can sometimes be a pretty big challenge so please stick it out stay. Compressed stay in your garments stain. Your bandages whatever. You have to do for that lymphatic health. Please keep it going Hydrate guys. i hope you're having a great summer. It's been a crazy one and again. "I'll be happy to help. Answer this whenever you give us your opinion there. Sure okay. i think that's great. So unfortunately i think there's still a lot of confusion surrounding the topic of exercise ltd. My patients and i think the reason for that is because Up until two thousand eleven. We really didn't have research. That really showed us. That exercise is indeed safe for breast cancer related patients but the american lymph dima framework project did a systematic review in two thousand and eleven where they looked at twenty one different citations and in that review they found that exercise does not put someone at a higher risk of developing lumps for dima or making it worse so that really has changed a lot of our thoughts and we've seen a huge paradigm shift Including with the national emphysema demon network who revised their position statement in two thousand and thirteen so relatively speaking. This topic of exercise is kind of new. It's a new way of thinking in the last ten years. So now we know that exercise is indeed something that should be a part of dima management and i really love. I love to tell people you know. Go back to doing what you did before your health crisis just take it slow and steady and get your physicians. Okay and i also think it's a really good idea to continue on with remedial exercises even when you do get back to your new normal. What do you think about that buddy. So i agree. One hundred percent the only thing that i would clarify. Maybe is that e need wearing compression. Your exercise which some people are like you know. Should i wear ryon mar garment. Should i not my arm. It you know even with swimming. Should i wear my garment. If i like to pool exercises or if i have open water that i couldn't get into wherever they live in all talk first about the swimming. Most people do where they're impressment in water but you don't have to. It does give you that extra layer but it also makes it a little bit more difficult to get off any kind of run. The risk of some chafing or a little bit of your attention because of the fabric but back in one of my interviews with cam allah. He gave us a little science lesson about buoyancy and handled pretty much for every foot of water. You're in that's twin millimeters of mercury. So if you are submerged into a pool of water flow And you're like six foot into the pool. You're getting like sixteen years of mercury pressure externally on your body from them buoyancy in the water and it's the end because it's deeper the wadham in more shallow towards the top. So that pressure is gradient. Much like your compression garment. It's probably a little bit harder to go and stick your arm down in the six foot in the pool and get concussion off without being you know upside down on your head. So he recommended for anyone. With upper extremity mma go to academy or out of similar sporting store. And get a little swimming novel not another word for it but will mouthpiece that you can breathe with some gobbles or something and you can kind of like go a little bit deeper in the water and you can get that pressure on your arm but when it comes to just general idea of exercises. Aerobic exercise biking weightlifting. Any of that stuff. Yoga do it. I just always say. Do it where you're compression into it and follow more likely the guidance of your lymph dima therapist out physician sometimes are hesitant. Either still somebody physicians out there who say like after breast cancer surgery and recovery not exercise. Don't lift over your head. Don't carry anything over ten pounds outdated practice. it's it's not supposed to be that way anymore. We have evidence in those studies that show that you go back to living a healthy and active lifestyle and really your lymphatic system. Will thrive in benefit frontman activity in x. Is so i completely agree with the position statement in those studies that were concluded by the. I'll left that summarizes that it's safe That to all of my patients said anyone who ever asks me about exercise. Go in slowly where you compression garment and just take your time to eat back into it but you can't get back to. I mean even intense exercise there are models. Or how models the powell. Yeah that you know they they show you how to get into that exercise and how to increase as you go so i mean. I think i've enough on that i agree. We should interest and encouraging that for patients. Who have one. For dima. I have a really awesome quote from catherine schultz. Who is one of the reasons that we have that fantastic evidence on through the trial about weightlifting. I love it. It's a great analogy. She says i believe cardiac. Rehab is a great analogy. Let's say you have damage to the heart muscle because of a heart attack. Will that those damaged cells are never coming back with the more you strengthen the rest of the heart through exercise the smaller to chance of a problem down the line. I loved that. So true in the lymphatic system works off of them muscle pump. It doesn't work off of your heart. Pump like blood circulatory system does so the more you're active in moving the more you're losing fluid out. I mean it. It's a hand in hand situation. Exercise is good for lymphoma patients. All right let's see. I think that's our summary. There the exercises good. "Have some thematic. Physiques and surgery is indicated. Well i should say that conservatively because the prophylactic the even this procedure is so new that even hell were calling it is somewhat controversial. Some surgeons don't like the name prophylactic because they feel that they are right in that. There's a ready lymphatic injury. Let's say for example nonstop william section than radiation following breast cancer. There is a ready lymphatic injury. Even if the patient is in symptomatic so they prefer to call it immediate lymphatic reconstruction the truth is it is immediate lymphatic reconstruction for a synthetic Injury so is it really prophylactic putting the naming controversy saw. Let's just say this preventive or immediate lymphatic reconstruction so far. I mean this is a new concept however the procedure itself is a new. It's actually based on our extensive experience and expertise that's been developed from third buick super microscope. Lv a and now. We're simply applying this technique earlier before the patient develops some dramatic aziz. So in the way it's a very relatively technically simple procedure Minimally invasive that can be done in less than an hour patient. Wake up having no pain at all and they would say. Have you done anything a deal editing. That's the point the so they're completely comfortable. And based on our data so far and also other groups other surgeons studies these so far or data are highly favourable. It's so favorable dad If a family member is unfortunately in this situation a definitely was highly recommend the this procedure what would disqualify a patient. And i know we're not talking about a specific surgical approach just yet but what is a hard pass or a complete. Contra indication of surgical intervention realized. We asked this question before i didn't answer. You know that's okay. We got off a good tangent. So we'll do a yes and this is very important. I meant to answer this question. And i got up on the attention so In general patient asked. Would i be a search candidate. Nearly every body is a surgical candidate for lymphatic reconstruction has said it's not a second line treatment. Anyone with length of dima should be offered the option of both in my opinion length.
