And i think a lot of people listening who are about our age Probably have it too. So why don't you start by rewinding to a time for me it was late. Ninety s maybe a little later for you but take us back to how the parents of our generation approached the internet. Yeah well so my experience. And i think many people's experience it sounds like your experience was that there was a real Caution a real concern about the early internet particularly children and young adults using it on their own without supervision and you know without good judgment to recognize what could be dangerous concerns about the amount of time that we spent online. There were concerns about I think the the fear back then was like you'd be in an aol chat room and someone pretending to be your peer who's actually like a an adult pedophile is going to lure you out and kidnap you There were concerns about porn. I remember In my high school computer class. They they really drilled into us that yours needed to google before you went to a website because if you just typed in a url and maybe you misspelled it. All the pornographers bought up all the misspellings of the websites. That kids want to go to. So you misspelled disney dot com just like boom. It's going to be the worst thing ever. I never heard that one yeah. I don't think it's shoe but there was a lot of concern and in a sense. You know i think real concern that. There are a lot of bad things on the internet. Did we know At the time what. The real dangers of the rest of the internet would be. I don't think anyone back then anticipated. I mean maybe professionals but in in sort of like the common Understanding don't think anyone anticipated what social media would be like And how it would us and how it would really affect our thought patterns and and the way we consume information and the way we that information Anyone really guessed what that would be like. How did we learn. Exactly what The internet but yeah in particular social media was doing to our brains. And what do we know now about what's happening in our heads. Yeah so there's been there's been increased attention to this For some time. Now one Book i mentioned in my article with his A good read though. I wonder if he's going to put out an updated edition to address more of social media. Because it was it was published in two thousand eleven is about called the shallows what the internet is doing to our brains Written by a guy named nicholas carr and he focuses really on the effects of the medium. How it it changes how we think It makes it shortens our attention span at it. Interrupts our ability to concentrate our ability To just sort of contemplate or meditate with their own thoughts And you know he wrote that a almost a decade ago now You know before twitter was really a thing and the way it is now. Instagram didn't exist. Facebook was was very much like a place for just sort of meaningless little friendly interactions and so since then i think all of the trends that he identified early have only accelerated and The the introduction and increasing ubiquity of social media You know it really is designed to addict us in a sense. I don't think that's too strong a word. It's all very carefully designed by very smart people for return usage to get us picking up our phones five hundred times a day and that's not an exaggeration. Pick up our funds hundreds of times a day. We go to these sites dozens of times a day that that can't help but change how we think and what we think about who is most vulnerable to that kind of manipulation. I guess if you want to call it that yeah we're i think we're all vulnerable. That said i did write the article with a generational angle and did focus on older people My p- my parents are in the boomer generation. As our i think you know The parents of most of my peers. And what's what's different generational only Is that we do find that. Older people are more likely to share misinformation And that like to share it. Unwittingly and that age more than other demographic indicators like education or political perspectives Wealth things like that Age is the by far and away the the most determining factor of that likelihood and anecdotally. That is something that I've encountered again and again with Friends of mine with readers online both before this article came out and certainly after that this is a a a problem and again by no means exclusive to one generation. There are plenty of younger people who have very dysfunctional internet habits In which i very often would include myself but there is a an a sense in which older people can be more vulnerable because of the lack of Digital literacy despite the pure happenstance of how old they happen to be when this stuff came

You could get a job after five years. You could knew that you'd be able to have enough money for deposit of a house and then you would be able to get mortgage you know how to family. That's not going to be the case for our generation because the nature of technology is deflationary. Because i as technology moving forward more and more people are job because we're not going to need that many people who don't have unique amazing skills so what we need the cut this book although yes it tells you how to Have a better career in the future. But the truth is the steps. The things that i talk about in the book they are not for everybody. This is not the kind of thing you know. It's not a book that's like you know emmanuel for how to get the job like how to write a better seavy. It's not that kind of thing you know. You need to come a unique worker unique person. You need to develop maltese skills. You need to be multi skilled and you need to be highly highly skilled in your soft skills or you know what i call human skills just emotional intelligence contextual creativity. Mindfulness and critical thinking. And those things take time to develop. So if you are you know have twenty four hours. Eight hours of sleep. You need that right. I used to sacrifice sleep. And i realized you know from reading a lot in it and thinking a lot about where i'm going to go and i was like what i'm doing is not sustainable. I need to get enough sleep. Okay so if i'm gonna get enough sleep. Then i have sixteen hours to work right and out of those sixteen hours I definitely need to give an hour for exercise. Okay i can listen to an audio book while i'm on treadmill and i'm doing my workout. I need maybe a bad of fourteen minnesota. So for general kind of things like say a meditation and things like that twenty minutes to shower so it about fourteen hours who work fourteen hours to work right so what. I'm suggesting in the book. If you are going to be in that top ten percents. This book is written for people who want to be in the top ten. Set if you're going to be the top temps then you need to work for fourteen hours. I'm not talking about work as in. Sit down and like you know just like not. Talking argument repetitive things. When i say works. Let's say out of that. Fourteen hours four to six hours of paraguay should be dedicated to research and development. Learning new skills when i say working over. I'm sitting here practicing linear algebra. I'm working as part of work. I think about newtonian and i aside. You know like dot. Org beh- using so many. I think so. Many people. And i know that there is a group of people who would say oh. I'm going to just reflect on your unique training path and we were just discussing before we started recording. Here that there's going to be a probably a lot of interest. In pursuing some sort of combined die batali cardiovascular education for court in the future. But would you mind just telling us how you got interested in really devoting yourself to cardio metabolic disease and diabetes in general. Absolutely am so first of all i. I'm super thrilled to be here. And i i'm so excited to be talking to the nerds here tonight so i. I think it's phenomenal. What you guys are doing and again. I very much appreciate the opportunity here so so i was. I'm kind of bridge between endocrinology in cardiology back in germany. I did my thesis. Actually in endocrinology lie protein metabolism. And that kind of got me interested in cardiology. I went to pursue cardiology training back in germany and then research fellowship at ucla. I was supposed to go back after that to germany. But i decided to stay and then obviously being a foreign Repeated training and i was always torn between the endocrine in preventive cardiology aspect than truecar ideologies. So i ultimately decided. I'm just gonna do both so. I did endocrinology fellowship and cardiologist fellowship by the university of kentucky. I had a phenomenal time. There and kind of still even now much of what we do in cardiology is really prevention. And that much of it. This endocrinology so i think these sub specialties really are complementary in or very closely together but let me say on behalf of everyone here at the clinic. How glad we are that. You decided not to go back and stayed here. Join us over here as a faculty because you've just added tremendous value to the program and you've been such an incredible resource for all of us. We are very excited to dive into cardio dive tallahassee which begs the question dr boomer we are cardio nerds why should cardiologists focus on diabetes. I mean why not. Just refer are diabetic patients to endocrinologist or leave the to their primary care doctors. Yeah i think. I think that's kind of the common theme and that is the current care. We leave it to others in cardiology. Now i think that is sub optimal. I think we as cardiologists we see these patients all the time i mean when is ever a patient. In the cath. Lab that presents with an semi or stemming that doesn't have diabetes so we do see the far spectrum of this disease of cardiovascular complications that arise of having ama- diagnosis of diabetes. So i think we have to be involved in managing this. I think to a minimum. We should referred patients to primary care or make sure that the diabetes is. What managed or console endocrinology now as as we all know as physicians quite frequently. Not even that happens so and there's good evidence for that if you look at registries just about six percent of diabetic patients with cardiovascular disease actually get appropriate care for their diabetes and cardiovascular conditions as you know that vitas says associated with the two to four fold increase in risk it is a cardiovascular risk equivalent termed many many years ago seventy percent of our acute coronary syndrome patients have diabetes. So you you could argue. Yes we leave it to primary care but or the endocrinologist but keep in mind that endocrinologists currently see about five percent of the patient population with diabetes.
