Professor Ruqaiijah Yearby Discusses Structural Racism in Health Care

"Welcome to the healthcare policy podcast on the host David Cosso. During this podcast rookie year be professor and member of the Center for Health Law. Studies at Saint Louis. University School of law. Joins me to discuss the effects of structural racism in healthcare, Professor Year be welcome to the program. Thank you for having me on professor year. Because bio is of course posted on the podcast website briefly on background listeners are likely well aware of the US has had a long history of persistent and substantial disparities in healthcare, access, delivery and outcomes. For example, the black to white infant metality ratio has never dropped below two to one. Over the past two decades, the healthcare research and quality has publishes disparities report. In it, a third of disparity measures have shown no improvement in nearly one in six have worsened. Or largely explains healthcare disparities or health inequities is structural, institutional or systemic racism, though frequently less overt structural racism, the fair to provide equal benefit to racial and ethnic minorities is embedded in healthcare, education, employment, environmental housing, transportation, and numerous other governmental policies as a result, structural or endemic racism, 'cause minority population, suffering far greater disease burden, and has results significantly higher mortality rates. As I've noted recently in previous PODCASTS, due to higher rates of on and under insurance that have led to higher rates of comber biddies covid nineteen related deaths among African Americans and Hispanics are far greater than among Non Hispanic whites. Can you explain this difference? Yes Oh structural racism is about the ways that our systems are structured particularly to advantage the dominant group in disadvantaged minorities. It also includes ways that organizations and institutions work together to create standards and policies that benefit them while harming minority and so we can see an example of this particularly in the healthcare sooner in the healthcare system when we look at access to healthcare, many predominantly African. American neighborhoods, predominantly immigrant neighborhoods do not have access to hospital care and that is so important doing Kobe nineteen, because that's where many people are receiving tests and treatment for covert nineteen and so the fact that we don't. Place hospitals based on need or need for health care rather we structure our system in a way that access to healthcare is based on ability to pay then benefits though who have jobs that have health insurance that can pay for health care, while it disadvantages racial and ethnic minorities who tend to work at low wage jobs, but do not have health insurance and cannot pay off for health care. Let me ask as a follow up question regarding critical race theory, somewhat similar fries actually possibly shocked that a week ago today the health affairs blog. BRIEFLY DISCUSS CRT in a post by Michelle Morrison and others. Can you give us a brief definition of what is critical race theory? for me. Critical race theory is about critiquing how. Has Been used as another means to harm minorities, particularly as I think about it anti-discrimination law when we look at historical articles about this we see they. antidiscrimination law has been set up to facilitate and support the existing social structures. When you look at anti-discrimination law in the area of employment you are looking to prove that an individual or that institution allowed policies that harmed individuals and. And so it never gets to the point where you're challenging the structures or the systems of employment, they can stay the same. We only look at individual perpetrators who have done some harm. So when we think about employment that is so relevant now in covert nineteen, because a lot of the wage, workers are being deemed as essential workers under covid nineteen, but they're not being provided with math. They don't have paid thickly. They do not Count under worker's COMP. They do not receive unemployment compensation because. Those are do not apply to them and so let me give you a specific example so the fair Labor. Standards, act. was passed back during the new deal time in nineteen thirty eight, but that was also the time of Jim Crow, and so it left out. Many workers domestic workers who include home care. Workers agricultural workers as well and so. What the? Standard acted was. Provide for a minimum wage overtime pay and limit the work week to forty hours, So. Most of these workers are not covered We're not covered by this actual act when in two thousand and fifteen. They did actually begin to be covered under fairly. Standard to act Then you see a shift again and the structure of employment, and so no longer are they considered employees companies shifting to independent contractors

"First off before we get all the way into today's episode. I just WANNA check on you for a minute. Are you doing? Are you hanging in there? This is a very very tough time for so many of you. Many of our listeners have already started to email me. Dm Me and and reach out to me and let me know that just struggling in a very real way not just not just the emotionally which is real not just mentally and that's real but you're struggling financially You are struggling physically to hold it all together. Many of you have lost your jobs. You've lost your income and we don't necessarily know where the end is going to be and don't understand what's next and what's going to be happening in the days and weeks ahead and it's part of why. I want to have today's episode episode number one ninety nine. I want to have it on a key. Point that I think is at the center of my life of my leadership of the book that I wrote that comes out in April and I will talk more about that next week as well. But when you don't have an agenda as a person as a community as a nation as as a group of people when the people you love and care about don't have an agenda don't have a plan or a strategy people will take advantage of you. Do you understand what I'm saying that as you are as you are having a hard time right now as you see that hard times are about to come your way as we face. This corona virus pandemic as we face national unemployment levels that we've never seen in modern American history as as scientists suggest that huge percentage of Americans will likely get the corona virus in that it will hit certain communities way harder than others as we see nations like Italy and others have Ford five hundred people a day dying from the impact of Corona virus. If we don't have a plan for our family for our community for the Group of people that we most identify with and listen. I'm just going to be transparent if that's your ethnic group if that's your nationality if that's your economic group if it's working people if we don't have plans and strategies in place right now. I need you to understand that. Corporations and people in power are are developing their own strategies. And what's going to be happening today through the weekend and particularly next week? Is that the United States government from the White House from the House the Senate will see it also in court. Systems will see it. In governor's offices state legislatures city. Councils mayor's offices they are going to be making decisions. District Attorney's offices police departments. They are going to be making decisions on their agenda on their plan on their strategy. Moving forward and if we don't tell them what our preferences are what our needs are what our our requirements as part of that strategy are if we don't say those things out loud do not assume ever that they are going to just because they love you and care about. You are going to integrate your needs into their agenda. Do you hear what I'm saying right now. Our local state and federal government is about to make huge decisions on bailouts on cash infusions on policy shifts and changes. And if we aren't telling them exactly what they need to do with that if we aren't saying specifically that you need policy a B C and D in there. They're not GONNA put it in there. Here's here's what I know in. It's what you know in. This crosses all party lines. This crosses nationality ethnicity faith religion age. Most of our government officials do not act day in and day out with the best interest of every day Americans in mind. They don't I don't believe it. They often act with the best interest of their donors in mind with their backers in mind but rarely do government officials act with the best interest of the masses in mind in great part particularly when we start talking about members of the House and Senate and of the executive branch they have not been apart of the everyday class of Americans in so long that they don't even understand often the needs and struggles of everyday people and the only way they are going to include our priorities in their bailouts in their legislation in their packages. The only way they're going to include us in their agenda is if we demanded. And here's here's what? I have to teach even Lynn. We demand that they include our priorities in their agenda even then. We only get a fraction of our priorities. I wish you could see my hands right now but I have. My hands stretched out far to the left into the right. You have to give somebody a huge number of priorities with the reality that they are only going to adopt some at. My hands are getting smaller and smaller and smaller. They are only going to adopt some of those priorities. But if you don't give them any priorities if you don't give them an agenda at all you get nothing. Do you hear me if you don't say listen. You need to include these nineteen things in whatever legislation you pass if you don't say those nineteen things you you're not gonna get anything because what they'll do is they'll they'll squish those nineteen down and they might only take three of your policy points but if you only give them one or none you'll get nothing it's the it's the beauty of having a big vision. A big agenda. Do you understand what I'm saying? It's the beauty of asking for lot. Knowing that the the the the meat grinder the the sausage factory of of our government will grind that thing down to something much less than what you were originally fighting for. So if you're fighting for a big agenda it's GonNa get squished down to something much smaller but if you're fighting for little biddy agenda it's still going to squish down normally to nothing and if you're not fighting for anything at all that's exactly what you're going to get

"And they also don't talk about that sometimes outweighs, you know, like, the the benefits outweigh the side effects in a lot of cases. So yeah, that that biddy ill the response to that made me feel like so much better about my decision. Also, like, I stayed on this Olafur few months after that. Because I felt like I'm making I was making the right choice. But I certainly point it was like, well, this isn't the. Option for me. So I might as well, try something that could not have that side effects. I think a low of on medication including myself, obviously think they have to put up with a certain number of side effects because it's like, well, this is literally preventing me from feeling awful. So I'll put up with not being able to sleep. We'll putting on weight once the kind of balance of that been like fee. Do you have any advice on when the side effects are too much? And when you do need to think about changing meds. I think you should always be having a big discussion with your psychiatrists when you're starting any medication. And you know, if you're if you have a good one, they're constantly, you know, while you're transitioning they're checking in there, making sure it's good like, I think that you you sort of know, I also think that there's been improvement so medications there is combinations of medications. And so it's not something where like you're gonna try one thing. And then you'll be fixed. I mean, sometimes that happens, and that's great. But. It is a journey in like you. It's your body and you have to decide like what you're comfortable with. So there was so many options out, the absolutely there's so many now I didn't realize when I don't I thought it was just Prozac, and that's it, but you kinda actually try different options in find one that works better fee, totally different different dosage, and then like, you know, these like these additions like Wellbutrin really balances nicely with like an SSI and link I was onto thing else at one point from tex-. Like, there's a lot of things that they can do. And also, there's you know, other things other than medication that you can do exercise is so important sleeping while is so important, you know, potentially like meditation mindfulness like it's it's not like a one stop shop. Like, you have to do a bunch of things for you. Personally, you doing a of quite intense Wirkstoffe with port casting with YouTube. How you the audio mental health in the midst of? Actually by mental health is the healthiest one, I'm busy. It's I really kind of spiral out of control when I don't have purpose when I don't have anything to do. So for me right now as like a bunch of my projects like winding up like I'm terrified of like August. 'cause like my my deadlines will be done by then and don't really have the next thing. And so that's like what's the scariest thing to me? And I'm trying to just like not focus on that. And realize that like things change all the time and opportunity to present itself after the open to different opportunities. And also that you know, I have worked really hard this year. And if I take a month off that's not the end of the world, which is something I could never have done a couple of years ago. I relate to that quite hot. I'm just terrible at taking any kind of holiday 'cause I know the entire time. I'll be no feeling great. All of a sudden, Andrew luck cannot get touch during a game is unbelievable in Chris Ballard GM and Frank Reich. The head coach have done a phenomenal job at changing the culture in that locker room. And when I say that even in training camp, I went in and I watched them, and I talk to the offense of line Antarctica the team after practice, and I remember going in yesterday, and we walk in the locker room pre-game, and I told told Chris and Frank this feels like home this is the locker room that I'm used to when you walk in. There's an expectation. They're going to win games. There's an expectation that their offense and defensive lines are going to are going to run roughshod and give them a chance to win games. And I'm so proud of of the. Culture that has been created in such a short window. It's Ballard singing and Frank's first, but the culture that's being developed and the belief by these men that they're going to go somewhere. They may not be this year. You know, they may have some pieces they gotta fix, but I'm telling you there is a belief in this locker room amongst these guys did impact a football team not for just one season. But for a collection of seasons and not only did Andrew luck. Not get set. He didn't get touched yesterday against a line in a defense. Totally ret Tom Brady and the patriots the week before. I mean, you said that they make these cones around looking for food. I mean, the damage I guess is the all the tunnels they dig tunnels and cosmetic damage. I mean, it looks ugly. You know, it's unsightly. So these little mounds they make good size. Yeah. Yeah. They're they're pretty good size will plug some pictures here on the break, and I'll show you. But you could see him just taken over some yards. But they are treatable. I mean, it's it's it's not all lost. But interesting interesting little creatures. So now, do the moles gophers and stuff. Do they eat them? The mole crickets, you know, bowls. Moles will actually I know the grubs worms. I know for a fact Aimo Kirk it's I wouldn't see why not gophers will.

"So can I share an experience I had I had I was having a big reading and I really prepped for it big time. So Emily had sent me some awesome handmade bath salts that she had created site took a bath in these amazing bath salts, and then I journal the little bit at my table. I like lit some candles, maybe let some incense. It was special. But also I have to mention this is like four in the afternoon. So a good two hours before my husband and kiddo would be home. So it wasn't like all moody. And there's some areas where you just can't be too precious about it. I would have loved for the full moon to have already been out. So it was a full moon reading as during the eclipse full moon was having an eclipse. And so I go into my reading a meditating on pulling my cards, and the news came editing on the cards in just started pouring and lightning and thundering, and all. All this rain started coming down. There was no weather in the forecast. And by the time I was like done meditating and coming out of it and really journaling on the cards. The sun was out and birds were chirping. I mean, it was like a fool cycle, and I was like, okay, this is real. That is also has been chemo me like, did you see that rain is like, yeah, I made that happen. That's what happens when a right meditate say there's no, you go anything going on here at all at all right. I think that I think that that is an import. I think you're making an important point here though, and that is unsullied level doing big picture. Tarot readings like that. I think there has to be some preparation. You have to like really center yourself. Find your like a place where you can really get in touch with your intuition in order to do some of those big picture things, but not all tarot has to be all Basso and incense like it can be as easy as just pulling a car before he get on a phone call with friends or whatever it may be. And I think I think quite often people get stuck in one of those where they're either going to be super flippant about it and not really understand what all it can do for you, or they're going to be way too into the ritual side. Not understand how everyday tarot can really help you stay connected as well. I going to remember like how to choline the directions and all of these kinds of things. I question actually, speaking of the thunderstorm that happened, it was almost like this wink of synchronicity, and for. Me synchronicity always feels like a little wink from the universe that allows me to actually have faith that there is something maybe bigger going on whether or not it's true that's side. The point for me feel special and it feels like I said, like a little wink. And so I love pulling a card, especially from using an an deck with animals in it, and then seeing where those animals begin to show up..
