"Thing to come out for them. However the washington post also notes that the cases of vaccinated individuals developing severe infections remain rare. And that's true and that proportionally unvaccinated people are still much more likely to be infected then vaccinated people are. That's also true so mostly. The findings presented in this case. Study are bad news for vaccinated folks and folks who have unvaccinated people in their household and that's scary so the question is how do we square those takeaways. With that headline finding five percent of the people in this outbreak were vaccinated number. Makes it look like it's worse to be vice. It does it does. Yeah but luckily that is not the case. Okay so about the cdc report so this was published by brown and colleagues in july twenty twenty one and it's describes a case study of particular cove in nineteen outbreak that happened in barnstable county massachusetts In like early to mid july of twenty twenty one and so barnstable county is home to province town and in province down they host these annual events like independence week and bear week. I had never heard of these. I don't know if you heard in the animal sense or no. No i'll tell you so from the down tourism website. They say bear week is the largest gathering of bears in the world. Tens of thousands come to province down during this still and tell them to go to bars. Eat honey to go to bar clarify parties bars and clubs throughout the town. You know what you're getting into when you attend. no we're not talking about actual bears. That's finally okay. Now it becomes clear. Yeah so these are large events. There attended primarily by gay men. And i only. I only mention this because these these events were connected to the outbreak and because this this does become very relevant to interpreting. Some of the statistics. Later in terms of the demographics in this outrage so there were four hundred sixty nine cases in this particular outbreak which was associated with multiple events and gatherings and of those four hundred sixty nine cases. Three hundred forty six or seventy four percent of the cases did occur and fully vaccinated people. Wow another another. Finding that they talked about was that In a sub sample of these cases in in one hundred thirty three cases they were able to perform genomic sequencing to determine which strains of ovid were represented in this outbreak and the vast majority of the sampled were the delta variant so it was like eighty nine percent of the sample tests were from delta. There was one additional case that was a delta descendant and other ten percent of cases. They said that the genomic sequencing was not successful. so it's entirely possible that all of these cases have been delta but at least ninety percent of them were so the the findings that are being played up in the news coverage of this or that the majority of infected were vaccinated and the majority infections were caused by delta and then the report goes on to say that there were five infected persons in this outbreak. Who were hospitalized. And four of those five were back stated that's another finding that could sound bad eighty percents. The people in this outbreak were vaccinated. Yeah should say no. Deaths had been reported from this outbreak at the time of the reports publication. And i still don't think there have been been. I could've miss something there and then another key finding that the report talks about is that They were able to analyze a metric from the pr tests. The copa tests called threshold cycles. In a sub sample of the cases so in so in one hundred and twenty seven of the pr tests from vaccinated people and eighty four of the pr tests from unvaccinated people and this metric These thresholds essentially indicate. How much virus is present in. The sample and the values on this metric looked very similar between the vaccinated and unvaccinated individuals. This is where claim comes from that the vaccination may not reduce your risk of spreading the delta variant right. It looks like the viral load was similar in both groups in additionally the the threshold or the cycle threshold values. They weren't similar between the two groups. They indicated very high viral load in the two groups which is more evidence supporting the the idea. The delta is like very transmissible. One key. Finding i think this actually segue nicely into that into that. Point is that They they gave. They reported the types of vaccines that were represented in the breakthrough. Vaccinations they got in this in this outbreak. So they essentially said like the percentage of the people that had pfizer earner and jj that developed a breakthrough infection and so their numbers were as follows. A forty six percent of the people in the outbreak had pfizer. Thirty eight percent of the people in the outbreak had moderna and sixteen percents of the people that had breakthrough infections had had johnson and johnson. And so like. I think yeah. I think that this is a this is an interesting thing to key in on because i think it does kind of it could be interpreted or misinterpreted in a way similarly to that seventy five percent right. So if i'm looking at that. And i'm seeing that like the majority of these breakthrough infections were with pfizer and madonna only sixteen percent. Were with j. j. I might go. Hey wait does that mean. J. and j. is much more protective against breakthrough infections. But even i can tell ya you gotta look at the base rate of people that are vaccinated with those right exactly right and they do provide that information so in the massachusetts population of vaccinated people fifty six percent of those vaccinated in massachusetts got visor pfizer versus the forty six percent of this outbreak. Thirty eight percent got madonna versus the thirty eight percent in this outbreak and only seven percent got j..

"People's first impression might be. This took place on the beach. What so do you have any reason to believe. That delta spreads outdoors more than other variants based on this though. Well we know that the temporary is the most contagious of the lot. But there's nothing to suggest that if you are walking outside and somewhat around. You has kobe. Somehow you're going to get covert from that casual interaction. I mean no one is recommending outdoor masking for just everyday use. I do think that we need to be very careful about indoor poorly ventilated studies especially ones in which people are engaging in for long periods of time and again i just i really don't want us to draw the wrong from what happened in barnstable county in the province town outbreak because five by all accounts there were individuals for spending as you were saying brian. A lot of time indoors in cramped settings And this is we're not talking about casual interactions win in people going shopping in a well-ventilated grocery store i don't want people who are vaccinated to now be panicking and thinking. Oh my goodness. I now need to hunker down and not see anyone again. I really want the lesson to be that. We need to be thinking about our own risk here so the way that i would process this is think about your own medical risk. In the medical risk of those your household if healthy everybody else your household is also vaccinated. The risk to you is still very low. Because even if you have a breakthrough infection. You're not going to get very sick. Chances are on the other hand. If you live at home with someone who is unvaccinated or immuno-compromised. You should take additional precautions. And so for example for me. Because i have two young children. My husband and i will mask in indoor crowded settings will. We're in the grocery store when we're we're we're at church But we're not going to be basking outdoors if we have people coming to visit us. We're happy to visitors who are fully vaccinated but if the visitor told us that they were just at a at a rave or something with thousands and thousands of people we might want them to quarantine get tested. Prior.

"Is that correct. Yeah i think we've looked at different numbers a different time points so I would say that you know i. It's definitely high case numbers Per hundred thousand but it's also potentially the rate of acceleration of increasing cases that we're seeing region as well And so probably it will be you know up to jurisdictions we're still thinking about what that firm number would be I would say that the high number that we've been talking about in bristol barnstable county potentially Depending on what the governor thinks of may qualify as sitting that description so the two of you are really it. Sounds like now turning to the governor to see what step he's going to take next in terms of public health recommendations here in the commonwealth. Is that fair dr boucher. Yeah that's the way that's the way. The system works is that the cdc provides guidance and then at the states and provide the actual policy that we need to follow. I think for your listeners and for all of us as citizens you know it's important that we pay attention to this information and there's been emerging information about the delta burien that is concerning and again. I'm going to be like your broken record today. Keeps just reinforcing the fact that vaccines are the best tool. We have to protect ourselves and our families And you're there the reason why This isn't a lot lot worse right now. And dr pierre cases eight times higher than lows in june. I know that still the overall numbers are low compared to the peaks earlier this year and last winter or positive positivity rate is above one point six percent though four times higher than at the beginning of july. What would you if you were advising the governor. What would you tell him to do right now. Well i think that The delta variance is clearly forced to be reckoned. So addition to thinking about how we can continue to support vaccination efforts in our entire commonwealth. But especially in those particularly under vaccinated spots that remain vulnerable Especially as we move into cooler weather having more indoor teaching and congregation I would recommend really taking a critical look at the activities that we've seen in our coming off and elsewhere contributing to spread of cove nineteen And also it's really difficult to feel like we're backtracking on the progress that we've made and we're really weary. Of course it's not done with us and so we really do need to take a hard look and make sure that we are protecting our communities ourselves families and particularly the most vulnerable including those who are unvaccinated but Children and those who are not eligible for vaccination yet as well But i my colleague and saying that vaccination really is one of the most important methods that we have at this point We still need to think as we're getting the vaccination rates up and higher hopefully What other layers of infection prevention we've used in the past still work and can work for us moving forward one eight hundred four three eight to five five to.

This is just one big mass for all of us. Older people. Nashville Mayor John Cooper stating the obvious, as most of you have heard the vaccine distribution is taking longer. Then expected across the country. States in the deep South have dismal inoculation rates less than 2% of their populations. In some cases, we've also heard from Cape Qatar's There's Ah large Senior population on the Cape. They have been frustrated and confused about who's first in line to get the vaccine. So a remedy is coming. Barnstable County launching by the end of the weekend, a new hotline for Cape Qatar's so you can call in and say, when do I get my vaccine? So that's coming and we will provide that number for you as soon as we have it. The Biden administration is hoping to put 100 million shots into the arms of Americans in his 1st 100 days in office. Now we've been talking about this third vaccine, this one from Johnson and Johnson. They're still in trials. But couple of amazing things about this first, it doesn't have to be stored at those extremely cold temps. This is former FDA commissioner Dr Scott got lead. This could be popular because it could be stored in a doctor's office. It's more easily so as you push vaccines out into the primary care setting away from these mass distribution sites. This is gonna be an attractive vaccine to using those kinds of more Maura steer settings. Second, the Johnson and Johnson vaccine looks like you only need one shot rather Than two. But again, it would be the third to be distributed in the United States, along with Visor and mode, Erna They have to wrap up there getting close all of the clinical trials and then of course, it will go to the FDA through that whole regulatory approval process. As we saw with the first two vaccines coming up, it is the weekend. It's a holiday weekend. We've got some rain. More on that rainstorm coming up after this. Listening to WBZ during.
