The past president of the American society for preventive cardiology one of the major risk factors that we can actually manage well is down the aisle cholesterol blood cholesterol if we lower LDL cholesterol we can dramatically lower the risk of heart attack stroke and even death so we hadn't Staten that are the mainstay of therapy that have been around for a few decades and then in the summer of two thousand fifteen a new drug class was approved by FDA to drugs because the PCS canine inhibitors they're injectable cholesterol lowering drugs that lower LDL sixty percent on top of the stand very very effective in the clearly lower the risk of stroke and heart attack clearly we are very excited about this we prescribe the drug and lo and behold most of our patients were denied by the players some of whom some of the patients who got the drug abandon the drug because of the high copay because there could be a responsible because we found over the last few years those patients who either didn't get the driver abandon the drug anti much higher risk of cardiovascular events and that twenty one percent higher we publish that in circulation so that's a problem many discussions were held and everybody pointed the finger at the pharmaceutical industry you know the the one who who innovate in creating these rather and so the day we lowered the price sixty percent a dramatic lowering and then I was in meetings were I was told you know that in the any lowering the price would be translated into patient and everybody would have access and what's happened is is the the opposite frankly and this is where it becomes extraordinarily complex only to try very hard to simplify this in and if it's his decision top area with so many moving parts with basically the drugs are cheered and and the here that need PCS can I never had been on was called the specialty here and let's look at Medicare now in the specialty here the patient's responsibility can be up to a third of the cost of the drug or the list price of the drugs I should say a third right hi impatient couldn't afford it now the drug prices lowered sixty percent so we are proud of that of the two remaining tears preferred or non preferred to drugs will be played in preferred status but over two thirds are actually non preferred non preferred status means he's responsible for up to fifty percent of the cost so even though the price was lowered it's still an unbearable car and that's for two thirds of the page into what's going to happen to these people is too you're not gonna get the dread the law because god will abandon the drug and he will definitely have a higher incidence of stroke and heart attack what we want is for these drugs the place in preferred status with a fixed co pays for the patient knows that he or she is responsible for let's see twenty five or thirty dollars a month not two hundred or two fifty okay and if that happens and that can happen by the way of enough people call their congressmen and senators and put pressure on them they can apply enough pressure to get the the the payers and to a lower court you change rather the status from non preferred to preferred and that's what we really want to happen in patients need to advocate for themselves the this is the time where people have to stand up into my doctor prescribed is I deserve it I needed and the call your congressman you call the insurance company clearly insurance commissioner of the state you're in call the commissioner and and have the commission's look into and I've had patients who've done that so you know be allowed to use your voice can.
